    Theoretical evaluation of engine auxiliary inlet design for supersonic V/STOL aircraft

    A higher order panel method is used to evaluate the potential flow of a 2-D supersonic V/STOL inlet. A nonsymmetric analytical inlet model is developed to closely match a wind tunnel model tested at NASA-Lewis. The analytical inlet used is analyzed for flow characteristics around the lower cowl lip and auxiliary inlets. The results for this analysis are obtained for the output of a computer program produced by the McDonnell Douglas Corp. This program is based on the Hess Panel Method which determines source strengths of panel distributed over a 3-D body. The analytical model was designed for the implementation of a drooped/translated cowl lip and auxiliary inlets as flow improvement concepts. A 40 or 70 deg droop lip can be incorporated on the inlet to determine if these geometry changes result in flow improvements which may reduce the propensity for flow separation on the interior portion of the lip. Auxiliary inlets are used to decrease the mass flow over the inlet lip; thus, the peak flow velocity is reduced at the lip which also lessens the likelihood of flow separation on the interior portion of the lip. A 2, 4, and 6 in. translated lip can be used to also decrease mass flow over the inlet lower lip in the same manner

    High degree of efficacy in the treatment of cyclic vomiting syndrome with combined co-enzyme Q10, L-carnitine and amitriptyline, a case series

    <p>Abstract</p> <p>Background</p> <p>Cyclic vomiting syndrome (CVS), defined by recurrent stereotypical episodes of nausea and vomiting, is a relatively-common disabling and historically difficult-to-treat condition associated with migraine headache and mitochondrial dysfunction. Limited data suggests that the anti-migraine therapies amitriptyline and cyproheptadine, and the mitochondrial-targeted cofactors co-enzyme Q10 and L-carnitine, have efficacy in episode prophylaxis.</p> <p>Methods</p> <p>A retrospective chart review of 42 patients seen by one clinician that met established CVS diagnostic criteria revealed 30 cases with available outcome data. Participants were treated on a loose protocol consisting of fasting avoidance, co-enzyme Q10 and L-carnitine, with the addition of amitriptyline (or cyproheptadine in those < 5 years) in refractory cases. Blood level monitoring of the therapeutic agents featured prominently in management.</p> <p>Results</p> <p>Vomiting episodes resolved in 23 cases, and improved by > 75% and > 50% in three and one additional case respectively. Among the three treatment failures, two could not tolerate amitriptyline (as was also the case in the child with only > 50% efficacy) and one had multiple congenital gastrointestinal anomalies. Excluding the latter case, substantial efficacy (> 75% response) was 26/29 at the start of treatment, and 26/26 in those able to tolerate the regiment, including high dosages of amitriptyline.</p> <p>Conclusion</p> <p>Our data suggest that a protocol consisting of mitochondrial-targeted cofactors (co-enzyme Q10 and L-carnitine) plus amitriptyline (or possibly cyproheptadine in preschoolers) coupled with blood level monitoring is highly effective in the prevention of vomiting episodes.</p

    The use of TeleMedicine in the Treatment of Pediatric Obesity: Feasibility and Acceptability

METHODS: Seventeen families were randomly assigned to one of two conditions (physician visit, TeleMedicine). Measures included feasibility, satisfaction, and intervention outcome measures such as BMI percentile, and nutrition and activity behaviors. Measures were completed at baseline, post-treatment, and at one-year follow-up. RESULTS: Analyses indicate that both feasibility and satisfaction data regarding the TeleMedicine intervention were positive. Intervention outcome indicates no change in BMI percentile or nutrition and activity behaviors for either treatment group. CONCLUSIONS: A behavioral family-based weight loss intervention delivered via TeleMedicine was well received by both parents and providers. Due to the small sample size, null findings regarding intervention outcome should be interpreted with caution. Future research should focus on methods to increase the impact of this intervention on key outcome variables

    Treatment of cyclic vomiting syndrome with co-enzyme Q10 and amitriptyline, a retrospective study

    <p>Abstract</p> <p>Background</p> <p>Cyclic vomiting syndrome (CVS), which is defined by recurrent stereotypical episodes of nausea and vomiting, is a relatively-common disabling condition that is associated with migraine headache and mitochondrial dysfunction. Co-enzyme Q10 (Co-Q) is a nutritional supplement that has demonstrated efficacy in pediatric and adult migraine. It is increasingly used in CVS despite the complete lack of studies to demonstrate its value in treatment</p> <p>Methods</p> <p>Using an Internet-based survey filled out by subjects with CVS or their parents, the efficacy, tolerability and subject satisfaction in CVS prophylaxis were queried. Subjects taking Co-Q (22 subjects) were compared against those taking amitriptyline (162 subjects), which is the general standard-of-care.</p> <p>Results</p> <p>Subjects/parents reported similar levels of efficacy for a variety of episode parameters (frequency, duration, number of emesis, nausea severity). There was a 50% reduction in at least one of those four parameters in 72% of subjects treated with amitriptyline and 68% of subjects treated Co-Q. However, while no side effects were reported on Co-Q, 50% of subjects on amitriptyline reported side effects (P = 5 × 10<sup>-7</sup>), resulting in 21% discontinuing treatment (P = 0.007). Subjects/parents considered the benefits to outweigh the risks of treatment in 47% of cases on amitriptyline and 77% of cases on Co-Q (P = 0.008).</p> <p>Conclusion</p> <p>Our data suggest that the natural food supplement Co-Q is potentially efficacious and tolerable in the treatment of CVS, and should be considered as an option in CVS prophylaxis. Our data would likely be helpful in the design of a double-blind clinical trial.</p

    A role for left temporal pole in the retrieval of words for unique entities

    r r Abstract: Both lesion and functional imaging studies have implicated sectors of high-order association cortices of the left temporal lobe in the retrieval of words for objects belonging to varied conceptual categories. In particular, the cortices located in the left temporal pole have been associated with naming unique persons from faces. Because this neuroanatomical-behavioral association might be related to either the specificity of the task (retrieving a name at unique level) or to the possible preferential processing of faces by anterior temporal cortices, we performed a PET imaging experiment to test the hypothesis that the effect is related to the specificity of the word retrieval task. Normal subjects were asked to name at unique level entities from two conceptual categories: famous landmarks and famous faces. In support of the hypothesis, naming entities in both categories was associated with increases in activity in the left temporal pole. No main effect of category (faces vs. landmarks/buildings) or interaction of task and category was found in the left temporal pole. Retrieving names for unique persons and for names for unique landmarks activate the same brain region. These findings are consistent with the notion that activity in the left temporal pole is linked to the level of specificity of word retrieval rather than the conceptual class to which the stimulus belongs.     Using participant hedonic ratings of food images to construct data driven food groupings

    Theorists posit that food reward is a powerful determinant of intake, yet little is known regarding how individuals’ hedonic ratings of a variety of foods interrelate and how hedonic ratings correspond to habitual dietary intake. Participant ratings of food appeal of 104 food images were collected while participants were in a fed state (n = 129). Self-reported frequency of intake of the food items, perceived hunger, body mass index (BMI), and dietary restraint were also assessed. Principal components analysis (PCA) was employed to analyze hedonic ratings of the foods, to identify component structures and to reduce the number of variables. The resulting component structures comprised 63 images loading on seven components including Energy-Dense Main Courses, Light Main Courses and Seafood as well as components more analogous to traditional food groups (e.g., Fruits, Grains, Desserts, Meats). However, vegetables were not represented in a unique, independent component. All components were positively correlated with reported intake of the food items (r’s = .26–.52, p < .05), except for the Light Main Course component (r = .10). BMI showed a small positive relation with aggregated food appeal ratings (r = .19; p < .05), which was largely driven by the relations between BMI and appeal ratings for Energy-Dense Main Courses (r = .24; p < .01) and Desserts (r = .27; p < .01). Dietary restraint showed a small significant negative relation to Energy-Dense Main Courses (r = −.21; p < .05), and Meats (r = −.18; p < .05). The present investigation provides novel evidence that how individuals’ hedonic ratings of foods aggregate into food components and how these component ratings relate to dietary intake. The notable absence of a vegetable component suggests that individuals’ liking for vegetables is highly variable and, from an empirical standpoint, not related to how they respond hedonically to other food categories

    Confirmatory factor analysis of the Child Feeding Questionnaire among low-income African American families of preschool children

    This study examined the factor structure for three of the Child Feeding Questionnaire (CFQ) subscales, a widely used measure of parental feeding practices, among 296 low-income parents of African American preschool children. Confirmatory factor analysis showed an overall poor fit among CFQ subscales; Restriction, Pressure to Eat, and Concern about Child Weight, (χ2, (df = 87 = 300.249, CFI = 1.00, NNFI = 1.07, RMSEA = .091). Additionally, Cronbach’s Alpha coefficients for 2 of the three subscales were below acceptable recommendations (Restriction = 0.69; Pressure to Eat = 0.58). These results suggest further psychometric clarification is needed to understand commonly reported feeding practice constructs among low-income African American mothers of preschool aged children

    Symptoms of somatization as a rapid screening tool for mitochondrial dysfunction in depression

    <p>Abstract</p> <p>Aims</p> <p>Somatic symptomatology is common in depression, and is often attributed to the Freudian-inspired concept of "somatization". While the same somatic symptoms and depression are common in mitochondrial disease, in cases with concurrent mood symptoms the diagnosis of a mitochondrial disorder and related therapy are typically delayed for many years. A short screening tool that can identify patients with depression at high risk for having underlying mitochondrial dysfunction is presented.</p> <p>Methods</p> <p>Six items of the Karolinska Scales of Personality (KSP) were found to differentiate among 21 chronically-depressed Swedish subjects with low versus normal muscle ATP production rates. A screening tool consisting of the six KSP questions was validated in the relatives of American genetics clinic patients, including in 24 matrilineal relatives in families with maternally inherited mitochondrial disease and in 30 control relatives.</p> <p>Results</p> <p>Among the depressed Swedish patients, the screening tool was positive in 13/14 with low and 1/7 with normal mitochondrial function (P = 0.0003). Applied to the American relatives of patients, the screening tool was positive in 13/24 matrilineal relatives and in 1/30 control relatives (P = 2 × 10<sup>-5</sup>).</p> <p>Conclusion</p> <p>Our preliminary data suggest that a small number of specific somatic-related questions can be constructed into a valid screening tool for cases at high risk for having a component of energy metabolism in their pathogenesis.</p
