Contribution des phosphodiestĂ©rases 3 et 4 au maintien de lâhomĂ©ostasie calcique et Ă la prĂ©vention des arythmies ventriculaires dans le cardiomyocyte adulte
The ÎČ-adrenergic pathway (ÎČ-AR)/cAMP is crucial for the adaptation of the cardiac function upon stress. In heart failure (HF), this signaling pathway is disrupted and a significant proportion of patients dies of cardiac arrhythmias. Classically, the inotropic and lusitropic effects of cAMP are attributed to the phosphorylation by the cAMP-dependent protein kinase (PKA) of the key proteins of the excitation-contraction coupling (ECC). cAMP also activates the exchange factor Epac, which is involved in cardiac hypertrophy and controls intracellular Ca2+ homeostasis. Epac activates CaMKII, another kinase modulated by Ca2+ and calmodulin which phosphorylates the same key proteins of the ECC, and is involved in arrhythmogenesis.Phosphodiesterases (PDEs) type 3 and 4 are crucial enzyme to degrade cAMP and to control Ca2+ homeostasis, thus ECC. PDE3 inhibitors are potent cardiotonic drugs but their use is limited by their pro-arrhythmic effects. Furthermore, the invalidation of genes encoding PDE4 results in ventricular arrhythmias. My work allowed characterizing the perturbations of Ca2+ homeostasis which lead to arrhythmias when PDE3 and PDE4 activities are decreased. My results show that PDE inhibitors exert inotropic effects via PKA, but evoke pro-arrhythmic Ca2+ waves via both PKA and CaMKII, the latter being activated in part via Epac. Altogether, these results suggest the potential use of CaMKII inhibitors as adjuncts to PDEs inhibitors to limit their deleterious effects, a hypothesis I also tested in a porcine model closer to the patient.La voie ÎČ-adrĂ©nergique (ÎČ-AR)/AMPc est cruciale pour lâadaptation de la fonction cardiaque. Real-world study of the efficacy and safety of belantamab mafodotin (GSK2857916) in relapsed or refractory multiple myeloma based on data from the nominative ATU in France: the IFM 2020-04 study
Belantamab mafodotin (BM) is an anti-BCMA antibody-drug conjugate (GSK2857916) that represents an alternative option in multiple myeloma. We sought to assess the efficacy and safety of BM in a real-world setting in patients who benefited from an early access program. We conducted an observational, retrospective, multicenter study. Eligibility criteria were treatment of relapsed or refractory multiple myeloma (RRMM) in monotherapy in adult patients who have received at least three lines of therapy previously, including at least one immunomodulatory agent (IMiD), a proteasome inhibitor (PI) and an anti-CD38 monoclonal antibody, and whose disease progressed during the last treatment period. The primary endpoint of the study is to assess the overall survival (OS). Between November 2019 and December 2020, 106 patients were treated with BM; 97 were eligible for the efficacy evaluation and 104 for safety. The median age was 66 (range, 37â82) years. High-risk cytogenetics were identified in 40.9% of patients. Fifty-five (56.7%) patients were triple-class refractory and 11 (11.3%) were penta-class refractory. The median number of prior lines of treatment was five (range, 3â12). The median number of BM cycles administered was three (range, 1â22). The overall response rate at best response was 38.1% (37/97). The median OS was 9.3 months (95% confidence interval [CI]: 5.9-15.3), and median progression-free survival was 3.5 months (95% CI: 1.9-4.7). The median duration of response was 9 months (range, 4.65-10.4). Treatment was delayed for 55 (52.9%) patients including 36.5% for treatment-related toxicity. Ophthalmic adverse events, mainly grade â€2, were the most common toxicity (48%). The occurrence of keratopathy was 37.5%. Overall, our data are concordant with the results from DREAMM-2 in terms of efficacy and safety on a non-biased population

Le VIH/SIDA est la crise sanitaire la plus grave à laquelle le monde est aujourd'hui confronté. En 2 décennies, la pandémie a provoqué prÚs de 30 millions de décÚs, particuliÚrement dans les pays en voie de développement qui abritent 95% des personnes vivant avec le VIH. AprÚs une présentation du VIH/SIDA incluant l'épidémiologie mondiale de la maladie, la physiologie du virus et les thérapeutiques actuelles, la situation du Bénin face à cette épidémie est abordée à travers une expérience personnelle d'un stage à l'hÎpital de Malanville. Il figurera dans cette partie une présentation de données épidémiologiques et les déterminants de la propagation du virus dans ce pays africain. Développement de l'oncogériatrie (prise en charge officinale d'une personne ùgée atteinte d'un cancer du sein traitée par la capécitabine (XelodaŸ))
Suite à l'évolution démographique et la place prépondérante du cancer dans les pathologies actuelles, l'oncogériatrie a pour mission d'améliorer la prise en charge des personnes ùgées atteintes d'un cancer. En France, c'est sous l'impulsion du plan cancer 2003-2007 que l'INCa a commencé à développer l'oncogériatrie. De plus, avec l'augmentation des traitements disponibles à l officine, le pharmacien d'officine doit réguliÚrement prendre en charge ces personnes. Incomplete Assembly of the Dystrophin-Associated Protein Complex in 2D and 3D-Cultured Human Induced Pluripotent Stem Cell-Derived Cardiomyocytes
International audienceHuman induced pluripotent stem cells derived cardiomyocytes (hiPSC-CM) are increasingly used to study genetic diseases on a human background. However, the lack of a fully mature adult cardiomyocyte phenotype of hiPSC-CM may be limiting the scope of these studies. Muscular dystrophies and concomitant cardiomyopathies result from mutations in genes encoding proteins of the dystrophin-associated protein complex (DAPC), which is a multi-protein membrane-spanning complex. We examined the expression of DAPC components in hiPSC-CM, which underwent maturation in 2D and 3D culture protocols. The results were compared with human adult cardiac tissue and isolated cardiomyocytes. We found that similarly to adult cardiomyocytes, hiPSC-CM express dystrophin, in line with previous studies on Duchenneâs disease. ÎČ-dystroglycan was also expressed, but, contrary to findings in adult cardiomyocytes, none of the sarcoglycans nor α-dystroglycan were, despite the presence of their mRNA. In conclusion, despite the robust expression of dystrophin, the absence of several other DAPC protein components cautions for reliance on commonly used protocols for hiPSC-CM maturation for functional assessment of the complete DAPC
