    Vertical Raindrop Size Distribution in Central Spain: A Case Study

    A precipitation event that took place on 12 October 2008 in Madrid, Spain, is analyzed in detail. Three different devices were used to characterize the precipitation: a disdrometer, a rain gauge, and a Micro Rain Radar (MRR). These instruments determine precipitation intensity indirectly, based on measuring different parameters in different sampling points in the atmosphere. A comparative study was carried out based on the data provided by each of these devices, revealing that the disdrometer and the rain gauge measure similar precipitation intensity values, whereas the MRR measures different rain fall volumes. The distributions of drop sizes show that the mean diameter of the particles varied considerably depending on the altitude considered. The level at which saturation occurs in the atmosphere is decisive in the distribution of drop sizes between 2,700 m and 3,000 m. As time passes, the maximum precipitation intensities are registered at a lower height and are less intense. The maximum precipitation intensities occurred at altitudes above 1,000 m, while the maximum fall speeds are typically found at altitudes below 700 m

    Vaccination adjuvated against hepatitis B in Spanish National Healthcare System (SNS) workers typed as non-responders to conventional vaccines

    [EN] Trial Design: An interventional, phase 4, single group assignment, without masking (open label), preventive clinical trial was carried out in health workers with biological risk in their tasks, who have been filed as non-responders to conventional vaccination against Hepatitis B. Methods: 67 health workers with biological risk in their tasks, who have been filed as non-responders to conventional vaccination against Hepatitis B, were enrolled in the Clinical Trial. All participants were from 18 years up to 64 years old. Inclusion Criteria: NHS workers -including university students doing their internships in health centres dependent on the National Health System (inclusion of students is regulated and limited by specific instructions on labour prevention in each autonomous community)- classified as non-responders. The criteria defining them as non-responders to the conventional hepatitis B vaccine is anti HBsAb titers < 10 mUI/ml following the application of six doses of conventional vaccine at 20 lg doses (two complete guidelines). The objective of this study was to provide Health workersstaff with an additional protection tool against hepatitis B infection, and to evaluate the efficacy of the adjuvanted vaccine in healthy non-responders to conventional hepatitis B vaccine. The primary outcome was the measurement of antibody antiHBs before the first Fendrix dose and a month after the administration of each dose. Other outcome was collection of adverse effects during administration and all those that could be related to the vaccine and that occur within 30 days after each dose. In this study, only one group was assigned. There was no randomization or masking. Results: The participants were recruited between April 13, 2018 and October 31, 2019. 67 participants were enrolled in the Clinical Trial and included the analyses. The primary immunisation consists of 4 separate 0.5 ml doses of Fendrix , administered at the following schedule: 1 month, 2 months and 6 months from the date of the first dose. Once the positivity was reached in any of the doses, the participant finished the study and was not given the following doses. 68.66% (46 out 67) had a positive response to first dose of Fendrix. 57.14% (12 out 21) had a positive response to second dose of Fendrix . 22.22% (2 out 9) had a positive response to third dose of Fendrix and 42.96% (3 out 7) had a positive response to last dose of Fendrix. Overall, 94.02% (64 out 67) of participants had a positive response to Fendrix . No serious adverse event occurred. Conclusions: The use of Fendrix , is a viable vaccine alternative for NHS workers classified as ‘‘nonresponders”. Revaccination of healthy non-responders with Fendrix, resulted in very high proportions of responders without adverse events. Trial registration: The trial was registered in the Spanish National Trial Register (REEC), ClinicalTrials.gov and inclusion has been stopped (identifier NCT03410953; EudraCT-number 2016-004991-23).     Effect of viral storm in patients admitted to intensive care units with severe COVID-19 in Spain: a multicentre, prospective, cohort study

    Background: The contribution of the virus to the pathogenesis of severe COVID-19 is still unclear. We aimed to evaluate associations between viral RNA load in plasma and host response, complications, and deaths in critically ill patients with COVID-19. Methods: We did a prospective cohort study across 23 hospitals in Spain. We included patients aged 18 years or older with laboratory-confirmed SARS-CoV-2 infection who were admitted to an intensive care unit between March 16, 2020, and Feb 27, 2021. RNA of the SARS-CoV-2 nucleocapsid region 1 (N1) was quantified in plasma samples collected from patients in the first 48 h following admission, using digital PCR. Patients were grouped on the basis of N1 quantity: VIR-N1-Zero ([removed]2747 N1 copies per mL). The primary outcome was all-cause death within 90 days after admission. We evaluated odds ratios (ORs) for the primary outcome between groups using a logistic regression analysis. Findings: 1068 patients met the inclusion criteria, of whom 117 had insufficient plasma samples and 115 had key information missing. 836 patients were included in the analysis, of whom 403 (48%) were in the VIR-N1-Low group, 283 (34%) were in the VIR-N1-Storm group, and 150 (18%) were in the VIR-N1-Zero group. Overall, patients in the VIR-N1-Storm group had the most severe disease: 266 (94%) of 283 patients received invasive mechanical ventilation (IMV), 116 (41%) developed acute kidney injury, 180 (65%) had secondary infections, and 148 (52%) died within 90 days. Patients in the VIR-N1-Zero group had the least severe disease: 81 (54%) of 150 received IMV, 34 (23%) developed acute kidney injury, 47 (32%) had secondary infections, and 26 (17%) died within 90 days (OR for death 0·30, 95% CI 0·16–0·55; p<0·0001, compared with the VIR-N1-Storm group). 106 (26%) of 403 patients in the VIR-N1-Low group died within 90 days (OR for death 0·39, 95% CI 0·26–0·57; p[removed]11 página

    Prodromal symptoms and the duration of untreated psychosis in first episode of psychosis patients: what differences are there between early vs. adult onset and between schizophrenia vs. bipolar disorder?

    To assess the role of age (early onset psychosis-EOP &lt; 18 years vs. adult onset psychosis-AOP) and diagnosis (schizophrenia spectrum disorders-SSD vs. bipolar disorders-BD) on the duration of untreated psychosis (DUP) and prodromal symptoms in a sample of patients with a first episode of psychosis. 331 patients with a first episode of psychosis (7–35 years old) were recruited and 174 (52.6%) diagnosed with SSD or BD at one-year follow-up through a multicenter longitudinal study. The Symptom Onset in Schizophrenia (SOS) inventory, the Positive and Negative Syndrome Scale and the structured clinical interviews for DSM-IV diagnoses were administered. Generalized linear models compared the main effects and group interaction. 273 AOP (25.2 ± 5.1 years; 66.5% male) and 58 EOP patients (15.5 ± 1.8 years; 70.7% male) were included. EOP patients had significantly more prodromal symptoms with a higher frequency of trouble with thinking, avolition and hallucinations than AOP patients, and significantly different median DUP (91 [33–177] vs. 58 [21–140] days; Z = − 2.006, p = 0.045). This was also significantly longer in SSD vs. BD patients (90 [31–155] vs. 30 [7–66] days; Z = − 2.916, p = 0.004) who, moreover had different profiles of prodromal symptoms. When assessing the interaction between age at onset (EOP/AOP) and type of diagnosis (SSD/BD), avolition was significantly higher (Wald statistic = 3.945; p = 0.047), in AOP patients with SSD compared to AOP BD patients (p = 0.004). Awareness of differences in length of DUP and prodromal symptoms in EOP vs. AOP and SSD vs. BD patients could help improve the early detection of psychosis among minors

    Clonal chromosomal mosaicism and loss of chromosome Y in elderly men increase vulnerability for SARS-CoV-2

    The pandemic caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2, COVID-19) had an estimated overall case fatality ratio of 1.38% (pre-vaccination), being 53% higher in males and increasing exponentially with age. Among 9578 individuals diagnosed with COVID-19 in the SCOURGE study, we found 133 cases (1.42%) with detectable clonal mosaicism for chromosome alterations (mCA) and 226 males (5.08%) with acquired loss of chromosome Y (LOY). Individuals with clonal mosaic events (mCA and/or LOY) showed a 54% increase in the risk of COVID-19 lethality. LOY is associated with transcriptomic biomarkers of immune dysfunction, pro-coagulation activity and cardiovascular risk. Interferon-induced genes involved in the initial immune response to SARS-CoV-2 are also down-regulated in LOY. Thus, mCA and LOY underlie at least part of the sex-biased severity and mortality of COVID-19 in aging patients. Given its potential therapeutic and prognostic relevance, evaluation of clonal mosaicism should be implemented as biomarker of COVID-19 severity in elderly people.     Influence of clinical and neurocognitive factors in psychosocial functioning after a first episode non-affective psychosis: differences between males and females

    BackgroundDeficits in psychosocial functioning are present in the early stages of psychosis. Several factors, such as premorbid adjustment, neurocognitive performance, and cognitive reserve (CR), potentially influence functionality. Sex differences are observed in individuals with psychosis in multiple domains. Nonetheless, few studies have explored the predictive factors of poor functioning according to sex in first-episode psychosis (FEP). This study aimed to explore sex differences, examine changes, and identify predictors of functioning according to sex after onset.Materials and methodsThe initial sample comprised 588 individuals. However, only adults with non-affective FEP (n = 247, 161 males and 86 females) and healthy controls (n = 224, 142 males and 82 females) were included. A comprehensive assessment including functional, neuropsychological, and clinical scales was performed at baseline and at 2-year follow-up. A linear regression model was used to determine the predictors of functioning at 2-year follow-up.ResultsFEP improved their functionality at follow-up (67.4% of both males and females). In males, longer duration of untreated psychosis (β = 0.328, p = 0.003) and worse premorbid adjustment (β = 0.256, p = 0.023) were associated with impaired functioning at 2-year follow-up, while in females processing speed (β = 0.403, p = 0.003), executive function (β = 0.299, p = 0.020) and CR (β = −0.307, p = 0.012) were significantly associated with functioning.ConclusionOur data indicate that predictors of functioning at 2-year follow-up in the FEP group differ according to sex. Therefore, treatment and preventative efforts may be adjusted taking sex into account. Males may benefit from functional remediation at early stages. Conversely, in females, early interventions centered on CR enhancement and cognitive rehabilitation may be recommended

    Mechanical durability and combustion characteristics of pellets from biomass blends

    Biofuel pellets were prepared from biomass (pine, chestnut and eucalyptus sawdust, cellulose residue, coffee husks and grape waste) and from blends of biomass with two coals (bituminous and semianthracite). Their mechanical properties and combustion behaviour were studied by means of an abrasion index and thermogravimetric analysis (TGA), respectively, in order to select the best raw materials available in the area of study for pellet production. Chestnut and pine sawdust pellets exhibited the highest durability, whereas grape waste and coffee husks pellets were the least durable. Blends of pine sawdust with 10–30% chestnut sawdust were the best for pellet production. Blends of cellulose residue and coals (<20%) with chestnut and pine sawdusts did not decrease pellet durability. The biomass/biomass blends presented combustion profiles similar to those of the individual raw materials.     Se desarrolla un proyecto de innovación educativa que pretende mejorar el clima de convivencia en el centro a través del reconocimiento de las situaciones de conflicto desde la reflexión serena sobre sus causas, buscando soluciones con decisiones negociadas. Se trata de mejorar las necesidades educativas de la población inmigrante emergente y de las minorías. Desde el Plan de Acción Tutorial se potenció la detección de problemas y necesidades, el desarrollo de habilidades sociales y la educación en valores. La metodología es activa u participativa, con el fin de reforzar la motivación individual y grupal, prestando especial atención a que la actividad y la reflexión caminen juntas. Desde el área de Lengua se analizaron las manifestaciones del lenguaje violento y se utilizaron textos literarios para reflexionar sobre los valores y contravalores con el fin de que el alumnado creara los suyos. Se difundió información sobre el proyecto mediante la creación de carteles y la decoración de las aulas y otras dependencias del centro. Reinstalaron buzones de sugerencias con el fin de ofrecer una vía más de participación. Desde el proyecto se realizaron gran variedad de actividades: se realizaron charlas coloquios, se analizaron textos periodísticos, se visualizaron películas, se realizaron debates, se escribieron diarios, cónicas y cartas, se elaboraron encuestas y entrevistas, se diseñaron proyectos de empresas, se investigo sobre la explotación infantil, se fabricaron marcos de madrera, y se organizaron fiestas y exposiciones entre otras actividades.Castilla y LeónConsejería de Educación.     El Programa PALE (programa de apoyo al aprendizaje de lenguas extranjeras), cofinanciado por el Ministerio de Educación y Ciencia y las Comunidades Autónomas, pretende dar respuesta, desde el Sistema Educativo, a las necesidades derivadas de la mejora de la enseñanza y el aprendizaje de lenguas extranjeras en una sociedad como la actual, donde las lenguas se consideran conocimientos básicos que precisa la ciudadanía europea para su formación, para mejorar sus posibilidades de empleo, para la promoción del intercambio cultural y para la realización persona. El material incluido en este Cd-Rom ha sido realizado en la tercera fase del curso PALE 2008 para Secundaria del CPR de Oviedo, tras la formación recibida en Asturias y en el extranjero, mediante la constitución de un grupo de trabajo.
