For the last two or three decades the world has witnessed important changes in world production in terms of unbundling, rise of outsourcing, offshoring, vertical specialization, and new sourcing strategy of multinational enterprises. Although these changes have not completely changed the pattern of trade, the trade of intermediate goods instead of final goods is a rising phenomenon. This phenomenon, being called as “the fragmentation of production processes and the international dispersion of tasks and activities” by researchers, has been leading to the borderless production systems.     This paper summarizes the development process of a wordnet for Turkish as part of the Balkanet project. After discussing the basic method-ological issues that had to be resolved during the course of the project, the paper presents the basic steps of the construction process in chronological order. Two applications using Turkish wordnet are summarized and links to resources for wordnet builders are provided at the end of the paper

    A Matrix-Based Heuristic Algorithm for Extracting Multiword Expressions from a Corpus

    This paper describes an algorithm for automatically extracting multiword expressions (MWEs) from a corpus. The algorithm is node-based, ie extracts MWEs that contain the item specified by the user, using a fixed window-size around the node. The main idea is to detect the frequency anomalies that occur at the starting and ending points of an ngram that constitutes a MWE. This is achieved by locally comparing matrices of observed frequencies to matrices of expected frequencies, and determining, for each individual input, one or more sub-sequences that have the highest probability of being a MWE. Top-performing sub-sequences are then combined in a score-aggregation and ranking stage, thus producing a single list of score-ranked MWE candidates, without having to indiscriminately generate all possible sub-sequences of the input strings. The knowledge-poor and computationally efficient algorithm attempts to solve certain recurring problems in MWE extraction, such as the inability to deal with MWEs of arbitrary length, the repetitive counting of nested ngrams, and excessive sensitivity to frequency. Evaluation results show that the best-performing version generates top-50 precision values between 0.71 and 0.88 on Turkish and English data, and performs better than the baseline method even at n= 1000

    Objective:Otoacariasis is the presence of ticks and mites in the ear canals of humans or animals, and particularly common in rural areas. This study aimed to present the clinical characteristics of patients that presented with ticks in their ear canal.Methods:The study was conducted with a total of 425 patients with 527 ticks in their ear canal at the Muş Malazgirt State Hospital Ear-Nose-Throat Clinic between June 2019 and June 2020. The removed ticks were examined at the parasitology laboratory of Van Yüzüncü Yıl University.Results:Of the 425 cases included in the study, 72% (n=306) were female and 28% (n=119) were male (mean age 40±20, minimum-maximum: 4 months–81 years). A total of 527 ticks were removed in the one-year period. Three-hundred-and-fifty-one patients had adult or nymph ticks, and 74 patients had a larval form of the tick. Of the patients with adult or nymph tick, foreign body sensation was the dominant symptom in 68.7% (n=242), whereas pain was the dominant symptom in 62% (n=46) of those with larval tick. In the comparison between groups, foreign body sensation was statistically significantly higher in the adult tick group, and pain was higher in the larval tick group (p<0.001). There were no systemic diseases related to the ticks in any of the cases.Conclusion:Ticks in the ear is endemic in Eastern Anatolia and poses a public health problem. Tick infestations could be minimized with various precautions and educating the general public on preventive methods. Our study is the largest series in the literature on cases with ear ticks.

    Elective cancer surgery in COVID-19-free surgical pathways during the SARS-CoV-2 pandemic: An international, multicenter, comparative cohort study

    PURPOSE As cancer surgery restarts after the first COVID-19 wave, health care providers urgently require data to determine where elective surgery is best performed. This study aimed to determine whether COVID-19–free surgical pathways were associated with lower postoperative pulmonary complication rates compared with hospitals with no defined pathway. PATIENTS AND METHODS This international, multicenter cohort study included patients who underwent elective surgery for 10 solid cancer types without preoperative suspicion of SARS-CoV-2. Participating hospitals included patients from local emergence of SARS-CoV-2 until April 19, 2020. At the time of surgery, hospitals were defined as having a COVID-19–free surgical pathway (complete segregation of the operating theater, critical care, and inpatient ward areas) or no defined pathway (incomplete or no segregation, areas shared with patients with COVID-19). The primary outcome was 30-day postoperative pulmonary complications (pneumonia, acute respiratory distress syndrome, unexpected ventilation). RESULTS Of 9,171 patients from 447 hospitals in 55 countries, 2,481 were operated on in COVID-19–free surgical pathways. Patients who underwent surgery within COVID-19–free surgical pathways were younger with fewer comorbidities than those in hospitals with no defined pathway but with similar proportions of major surgery. After adjustment, pulmonary complication rates were lower with COVID-19–free surgical pathways (2.2% v 4.9%; adjusted odds ratio [aOR], 0.62; 95% CI, 0.44 to 0.86). This was consistent in sensitivity analyses for low-risk patients (American Society of Anesthesiologists grade 1/2), propensity score–matched models, and patients with negative SARS-CoV-2 preoperative tests. The postoperative SARS-CoV-2 infection rate was also lower in COVID-19–free surgical pathways (2.1% v 3.6%; aOR, 0.53; 95% CI, 0.36 to 0.76). CONCLUSION Within available resources, dedicated COVID-19–free surgical pathways should be established to provide safe elective cancer surgery during current and before future SARS-CoV-2 outbreaks
