    Predictors of Severe Acute Malnutrition among Children Aged 6 to 59 Months Attended out Patient Therapeutic Program Center in Kavre District of Nepal - A Case Control Study

    Background: Severe acute malnutrition is an excessive loss of weight due to the acute shortage of food or illness. It is one of the major public health problems in developing countries including Nepal. According to multiple indicator cluster survey (MICS) 2014, 2.6% severely malnourished in Nepal and 4.4% are severely malnourished in Kavre district. However, there are limited studies about predictors of severe acute malnutrition in Nepal. Thus, this study was aimed to identify the predictors of severe acute malnutrition in Kavre district of Nepal.Methods: Health facility based matched case control study was conducted among 210 (70 cases and 140 controls) children aged 6-59 months from November 2015 to April 2016. Data was collected through face to face interview with mother of eligible children using structured questionnaires. Multivariate analysis was applied to estimate adjusted odds ratio along with 95% confidence interval.Results: Children with severe acute malnutrition were 11.32 times more likely than control to have recurrent diarrhea in past six months (95% CI=4.64-28.21). Similarly, severe acute malnutrition was associated with female sex (AOR=2.44, 95% CI=1.88-6.78), fathers occupation daily labor (AOR=4.69, 95% CI=1.17-13.76) and agriculture (AOR=6.850, 95%CI=3.81-12.93), improper exclusive breast feeding (AOR=6.646, 95%CI=2.11-20.90), not feeding colostrum (AOR=3.89, 95% CI=2.88-11.21), severe food insecurity access (AOR=3.55, 95% CI=1.85-9.77) and monthly income less than average level (AOR=8.214, 95% CI=1.43-22.16).Conclusion: Severe acute malnutrition was independently associated with sex of child, occupation of father, monthly household income, not feeding colostrum, improper exclusive breast feeding, severe household food insecurity access and recurrent diarrhea

    Does Patient Counselling Correspond to Medication Adherence in Chronic Diseases? - A Scoping Review

    Background: Quality care is essential to minimize the complication or consequences of chronic diseases. The main objective of this scoping review was to identify the role of patient counseling on medication adherence in chronic diseases. Methods:Scoping review framework, as described by Arksey and O’Malley was used to map out the literature. Databases were searched for published studies from 1 to 30 August 2020 which described medication adherence, factors contributing and role of patient counseling in medication adherence. Results: Total 21 primary studies, including 29% were randomized controlled trials designs and 76% of studies were conducted in the hospital setting. The medication non-adherence in the chronic disease was between 12-33% in reviewed studies. Conclusion: Most of the studies identified patient counseling as a major contributing factor for drug adherence in terms of chronic diseases. Similarly, pharmacist’s communication with patient is also described as important factor for medication non-adherence in reviewed articles. Thus, efforts must be broadened to improve counseling services within the health system to overcome chronic diseases through addressing drug non-adherence. Practical Implication: The findings of this review would be relevant for all health professionals, medical persons, and pharmacists to sensitize them about the importance of counseling in terms of overcoming chronic diseases

    Factors Influencing Dietary Diversity of Pregnant Women Attending Antenatal Care in Western Regional Hospital, Nepal: A Cross-sectional Study

    Introduction: Diet during pregnancy is important to fulfill the nutritional demand of physiological changes as well as to create an environment for fetal development. In Nepal, meal diversity scores of mothers and children are low. The situation of dietary diversity among pregnant women was unknown in the Western Region of Nepal. Thus, this study was conducted to generate evidence regarding the status and determinants of dietary diversity among pregnant women. Methods: It was a cross-sectional study. Systematic random sampling was done to select 282 pregnant women of third trimester attending antenatal care in Western Regional Hospital, Nepal. The semi-structured questionnaires, 24-hour recall tool, and Household Food Insecurity Access Scale were used to collect information from participants. Univariate, bivariate and multivariate analysis was done to assess the status, association as well as strength of association between study variables respectively.   Results: The mean (±SD) Women's Dietary Diversity Score was 4.96 (±1.42). Pregnant women having education level < SLC compared to ≥ SLC were 74.7% less likely to have high dietary diversity to lowest dietary diversity (AOR: 0.253, CI: 0.103 – 0.620, p=0.003). Similarly, pregnant women having an unpaid occupation of husbands compared to paid were 74.5% less likely to have high dietary diversity to lowest dietary diversity (AOR: 0.255, CI: 0.074 – 0.876, p=0.030). Conclusions: Consumption of medium dietary diversity was predominant among pregnant women. The education of pregnant women and the occupation of her husband were the two significantly associated factors with dietary diversity.     Morphological and Functional Correlates in Goldmann-Favre Syndrome: A Case Series

    The purpose of this study is to describe the correlation of findings between results from spectral domain optical coherence tomography (SD-OCT) and microperimetry in a case series regarding patients with Goldmann-Favre syndrome. Goldmann-Favre syndrome is a rare autosomal recessive hereditary vitreo-retinal degeneration that impacts the functionality of vision in subjects. Three men with this condition were assessed and subjected to microperimetry and SD-OCT. Two of the men were brothers. This study finds that the retinoschisis and macular cystoid changes noted in the SD-OCT matched the scotomas revealed by the microperimetry.     Extensive soot compaction by cloud processing from laboratory and field observations

    Soot particles form during combustion of carbonaceous materials and impact climate and air quality. When freshly emitted, they are typically fractal-like aggregates. After atmospheric aging, they can act as cloud condensation nuclei, and water condensation or evaporation restructure them to more compact aggregates, affecting their optical, aerodynamic, and surface properties. Here we survey the morphology of ambient soot particles from various locations and different environmental and aging conditions. We used electron microscopy and show extensive soot compaction after cloud processing. We further performed laboratory experiments to simulate atmospheric cloud processing under controlled conditions. We find that soot particles sampled after evaporating the cloud droplets, are significantly more compact than freshly emitted and interstitial soot, confirming that cloud processing, not just exposure to high humidity, compacts soot. Our findings have implications for how the radiative, surface, and aerodynamic properties, and the fate of soot particles are represented in numerical models

    The platelet receptor CLEC-2 blocks neutrophil mediated hepatic recovery in acetaminophen induced acute liver failure

    Acetaminophen (APAP) is the main cause of acute liver failure in the West. Specific efficacious therapies for acute liver failure (ALF) are limited and time-dependent. The mechanisms that drive irreversible acute liver failure remain poorly characterized. Here we report that the recently discovered platelet receptor CLEC-2 (C-type lectin-like receptor) perpetuates and worsens liver damage after toxic liver injury. Our data demonstrate that blocking platelet CLEC-2 signalling enhances liver recovery from acute toxic liver injuries (APAP and carbon tetrachloride) by increasing tumour necrosis factor-α (TNF-α) production which then enhances reparative hepatic neutrophil recruitment. We provide data from humans and mice demonstrating that platelet CLEC-2 influences the hepatic sterile inflammatory response and that this can be manipulated for therapeutic benefit in acute liver injury. Since CLEC-2 mediated platelet activation is independent of major haemostatic pathways, blocking this pathway represents a coagulopathy-sparing, specific and novel therapy in acute liver failure
