    Cremophor-Induced Lupus Erythematosus-Like Reaction with Taxol Administration: A Case Report and Review of the Literature

    We report the first case of Cremophor EL-induced cutaneous lupus erythematosus-like reaction in a 40-year-old female undergoing treatment for breast cancer. There have been four reported cases of paclitaxel- and four cases of docetaxel-induced cutaneous lupus reactions in the published literature [Dasanu and Alexandrescu: South Med J 2008;101:1161–1162; Adachi and Horikawa: J Dermatol 2007;34:473–476; Lortholary et al: Presse Med 2007;36:1207–1208; Chen et al: J Rheumatol 2004;31:818–820]. Our patient developed findings of a cutaneous lupus-like reaction with administration of paclitaxel which was subsequently discontinued. She was re-challenged with albumin-bound paclitaxel which has no Cremophor EL compound in its formulation. This administration of albumin-bound paclitaxel did not induce further reaction. She did not develop a cutaneous lupus erythematosus-like reaction with three other subsequent administrations of albumin-bound paclitaxel. The diagnosis of lupus-like reaction in our patient was made based on the development of a malar butterfly rash sparing the nasolabial folds, the appearance of this rash in context of recently receiving treatments with paclitaxel, resolution of the rash after discontinuing the paclitaxel, and the presence of autoimmune antibodies in the patient's serum which resolved with discontinuation of the paclitaxel. This is the first case demonstrating that the cause of the cutaneous lupus erythematosus-like reaction is not likely due to the taxane component of paclitaxel but the chemical composition of Cremophor EL. If the chemotherapeutic agent was causing the reaction then the same reaction should be seen by albumin-bound paclitaxel. We propose that previously reported lupus reactions may actually be due to Cremophor EL, which consists of polyoxyethylated castor oil, and not the chemotherapeutic agent itself

    Integrative Medicine in a Preventive Medicine Residency: A Program for the Urban Underserved

    The Preventive Medicine Residency Program collaborated with the Department of Family Medicine\u27s Program for Integrative Medicine and Health Disparities at Boston Medical Center to create a new rotation for preventive medicine residents starting in autumn 2012. Residents participated in integrative medicine group visits and consults, completed an online curriculum in dietary supplements, and participated in seminars all in the context of an urban safety net hospital. This collaboration was made possible by a federal Health Resources and Services Administration grant for integrative medicine in preventive medicine residencies and helped meet a need of the program to increase residents\u27 exposure to clinical preventive medicine and integrative health clinical skills and principles. The collaboration has resulted in a required rotation for all residents that continues after the grant period and has fostered additional collaborations related to integrative medicine across the programs

    Propagation of Large Uncertainty Sets in Orbital Dynamics by Automatic Domain Splitting

    Current approaches to uncertainty propagation in astrodynamics mainly refer to linearized models or Monte Carlo simulations. Naive linear methods fail in nonlinear dynamics, whereas Monte Carlo simulations tend to be computationally intensive. Differential algebra has already proven to be an efficient compromise by replacing thousands of pointwise integrations of Monte Carlo runs with the fast evaluation of the arbitrary order Taylor expansion of the flow of the dynamics. However, the current implementation of the DA-based high-order uncertainty propagator fails when the non-linearities of the dynamics prohibit good convergence of the Taylor expansion in one or more directions. We solve this issue by introducing automatic domain splitting. During propagation, the polynomial expansion of the current state is split into two polynomials whenever its truncation error reaches a predefined threshold. The resulting set of polynomials accurately tracks uncertainties, even in highly nonlinear dynamics. The method is tested on the propagation of (99942) Apophis post-encounter motion

    Reconstruction of nuclear charged fragment trajectories from a large gap sweeper magnet

    A new method to reconstruct charged fragment four-momentum vectors from measured trajectories behind an open, large gap, magnetic dispersion element (a sweeper magnet) has been developed. In addition to the position and angle behind the magnet it includes the position measurement in the dispersive direction at the target. The method improves the energy and angle resolution of the reconstruction significantly for experiments with fast rare isotopes, where the beam size at the target position is large.Comment: 13 pages, 5 figures, accepted for publication in Nucl.     Field reconstruction in large aperture quadrupole magnets

    A technique to interpolate complex three-dimensional field distributions such as those produced by large magnets is presented. It is based on a modified charge density method where the elementary sources of the magnetic field are image charges with Gaussian shape placed on a three-dimensional surface. The strengths of the charges are found as the solution of a best-fit problem, whose special features are discussed in detail. The method is tested against the measured field of the MAGNEX large acceptance quadrupole, showing a high level of accuracy together with an effective compensation of the effect of the experimental errors present in the data. In addition the model field is in general analytical and Maxwellian. As a consequence, the reliability of the presented technique to the challenging problem of trajectory reconstruction in modern large acceptance spectrometers is demonstrated

    Lagrangian Reachabililty

    We introduce LRT, a new Lagrangian-based ReachTube computation algorithm that conservatively approximates the set of reachable states of a nonlinear dynamical system. LRT makes use of the Cauchy-Green stretching factor (SF), which is derived from an over-approximation of the gradient of the solution flows. The SF measures the discrepancy between two states propagated by the system solution from two initial states lying in a well-defined region, thereby allowing LRT to compute a reachtube with a ball-overestimate in a metric where the computed enclosure is as tight as possible. To evaluate its performance, we implemented a prototype of LRT in C++/Matlab, and ran it on a set of well-established benchmarks. Our results show that LRT compares very favorably with respect to the CAPD and Flow* tools.Comment: Accepted to CAV 201

    Memory for pitch in congenital amusia: Beyond a fine-grained pitch discrimination problem

    Congenital amusia is a disorder that affects the perception and production of music. While amusia has been associated with deficits in pitch discrimination, several reports suggest that memory deficits also play a role. The present study investigated short-term memory span for pitch-based and verbal information in 14 individuals with amusia and matched controls. Analogous adaptive-tracking procedures were used to generate tone and digit spans using stimuli that exceeded psychophysically measured pitch perception thresholds. Individuals with amusia had significantly smaller tone spans, whereas their digits spans were a similar size to those of controls. An automated operation span task was used to determine working memory capacity. Working memory deficits were seen in only a small subgroup of individuals with amusia. These findings support the existence of a pitch-specific component within short-term memory and suggest that congenital amusia is more than a disorder of fine-grained pitch discrimination
