    In Ecuador, there are a variety of activities that help the country's productive matrix, among them the bodywork production activities that have the greatest boom in the province of Tungurahua, so they always look for integral logistics that includes planning, organization and control of all the activities related to the reception of raw materials, transfer and storage of products, to satisfy the needs and requirements that the demand requires, in a fast, efficient way and with the lowest possible costs. Objectives: Identify how logistics influences to achieve a 4.0 supply chain, analyze the impact of quality on the competitiveness of the bodybuilding market. Methodology: The research design turns based on a quantitative approach to more than a descriptive method. The population was made up of 27 auto body companies from the province of Tungurahua, this information is mentioned based on the latest information from 2017, the questionnaire was made up of 14 items, the questionnaire to have the validity required for this research was previously approved by professionals on issues related to this research. Contributions: This article seeks to identify the origin of the different production processes through which a product goes through, as well as its subsequent logistics with its delivery to the final consumer, all this to the application of traceability. In addition to the recognition of the bodybuilding companies in the province of Tungurahua that implement the ISO 9001En Ecuador existen una variedad de actividades que ayudan a la matriz productiva del país entre ellas las actividades de producción de carrocerías que tiene mayor auge en  la provincia de Tungurahua,  por lo que siempre buscan una logística integral que incluya planificación, organización y control de todas las actividades conexas con la recepción de materia prima, traslado y almacenamiento de productos, para satisfacer las necesidades y los requerimientos que la demanda requiere, de una manera rápida, eficiente y con los menores costos posibles. Objetivos: Identificar cómo influye la logística para alcanzar a obtener una cadena de suministros 4.0, analizar el impacto de la calidad en la competitividad del mercado carrocero. Metodología: El diseño de la investigación gira en base a un enfoque cuantitativo a más de un método descriptivo. La población estuvo conformada por 27 empresas carroceras de la provincia de Tungurahua, esta información se la menciona en base a la última información del año 2017, el cuestionario estuvo conformado por 14 ítems, el cuestionario para que tenga la validez requerida para esta investigación fue previamente aprobado por profesionales en temas relacionados con esta investigación. Aportes: El presente artículo busca identificar el origen de los diferentes procesos productivos por los cuales atraviesa un producto, así como su posterior logística con su entrega al consumidor final todo esto a la aplicación de la trazabilidad. Además del reconocimiento a las empresas carroceras de la provincia de Tungurahua que implementan la certificación de la norma ISO 9001 para crear técnicas estandarizadas y de esta manera mejorar su calidad competitiva y los atributos de los productos que ofertan en el mercado

    Comparación del Aprendizaje Basado en Preguntas y el Aprendizaje Basado en Proyectos en el rendimiento académico y la comprensión conceptual de estudiantes de Octavo semestre de Ingeniería Ambiental

      The research presents a detailed study that analyzes how two different pedagogical approaches influence the academic performance and conceptual understanding of university students of Environmental Engineering in their eighth semester. The study individually evaluated the academic performance of students in two groups: those who participated in Question-Based Learning activities; and those who engaged in Project Based Learning. Subsequently, the results obtained in written and practical tests were compared. The results obtained showed that the question-based approach proved to be more effective in improving academic performance in conceptual tests, suggesting greater theoretical understanding and the ability to apply abstract concepts to specific problems. On the other hand, the project-based approach showed better results in practical tests, indicating better application of the knowledge acquired in real situations and the ability to solve problems practically. These findings highlight the importance of considering different educational approaches and their effects on specific areas of learning. Furthermore, they highlight the need for balanced teaching that integrates both theory and practice to provide students with a well-rounded education and prepare them to meet the challenges of the field of Environmental Engineering effectively.La investigación presenta un estudio detallado que analiza cómo dos enfoques pedagógicos diferentes influyen en el desempeño académico y la comprensión conceptual de los estudiantes universitarios de Ingeniería Ambiental en su octavo semestre. El estudio evaluó individualmente el rendimiento académico de los estudiantes en dos grupos: aquellos que participaron en actividades de Aprendizaje Basado en Preguntas; y aquellos que se involucraron en el Aprendizaje Basado en Proyectos. Posteriormente, se compararon los resultados obtenidos en pruebas escritas y prácticas. Los resultados obtenidos mostraron que el enfoque basado en preguntas demostró ser más efectivo en la mejora del rendimiento académico en pruebas conceptuales, lo que sugiere una mayor comprensión teórica y la capacidad de aplicar conceptos abstractos a problemas específicos. Por otro lado, el enfoque basado en proyectos mostró mejores resultados en las pruebas prácticas, lo que indica una mejor aplicación de los conocimientos adquiridos en situaciones reales y la capacidad para resolver problemas de manera práctica. Estos hallazgos resaltan la importancia de considerar los diferentes enfoques educativos y sus efectos en áreas específicas del aprendizaje. Además, destacan la necesidad de una enseñanza equilibrada que integre tanto la teoría como la práctica para proporcionar a los estudiantes una educación completa y prepararlos para enfrentar los desafíos del campo de la Ingeniería Ambiental de manera efectiva. &nbsp

    Adolescent and young adult preferences for financial incentives to support adherence to antiretroviral therapy in Kenya: A mixed methods study

    INTRODUCTION: To develop a patient-centred financial incentive delivery strategy to improve antiretroviral treatment adherence in adolescents and young adults (AYA) living with HIV in Kisumu, Kenya, we conducted a mixed methods study exploring preferences. METHODS: A discrete choice experiment (DCE) and focus group discussion (FGD) were conducted simultaneously to identify preferences for five incentive delivery strategy features: value, eligibility, recipient, format and disbursement frequency. We used consecutive sampling to recruit AYA (14-24 years) living with HIV attending three health facilities in Kisumu, Kenya. We calculated mean preferences, willingness to trade, latent class membership and predictors of latent class membership. The FGD explored preferred incentive features, and, after deductive and inductive coding, qualitative findings were triangulated with DCE results. RESULTS: Two hundred and seven AYA living with HIV (46% 14-17 years, 54% 18-24 years; 33% male sex, 89% viral load \u3c50 copies/ml) were recruited to the study (28 October-16 November 2020). Two distinct preference phenotypes emerged from the DCE analysis (N = 199), 44.8% of the population fell into an immediate reward group, who wanted higher value cash or mobile money distributed at each clinic visit, and 55.2% fell into a moderate spender group, who were willing to accept lower value incentives in the form of cash or shopping vouchers, and accrued payments. The immediate reward group were willing to trade up to 200 Kenyan Shillings (KSH)-approximately 2 US dollars (USD)-of their 500 KSH (∼5 USD) incentive to get monthly as opposed to accrued yearly payments. The strongest predictor of latent class membership was age (RR 1.45; 95% CI: 1.08-1.95; p = 0.006). Qualitative data highlighted the unique needs of those attending boarding school and confirmed an overwhelming preference for cash incentives which appeared to provide the greatest versatility for use. CONCLUSIONS: Providing small financial incentives as cash was well-aligned with AYA preferences in this setting. AYA should additionally be offered a choice of other incentive delivery features (such as mobile money, recipient and disbursement frequency) to optimally align with the specific needs of their age group and life stage

    Effects of amantadine on circulating neurotransmitters in healthy subjects

    Considering that glutamatergic axons innervate the C1(Ad) medullary nuclei, which are responsible for the excitation of the peripheral adrenal glands, we decided to investigate catecholamines (noradrenaline, adrenaline and dopamine) plus indolamines (plasma serotonin and platelet serotonin) at the blood level, before and after a small oral dose of amantadine, a selective NMDA antagonist. We found that the drug provoked a selective enhancement of noradrenaline plus a minimization of adrenaline, dopamine, plasma serotonin and platelet serotonin circulating levels. Significant enhancement of diastolic blood pressure plus reduction of systolic blood pressure and heart rate paralleled the circulating parameter changes. The above findings allow us to postulate that the drug was able to enhance the peripheral neural sympathetic activity. Minimization of both adrenal sympathetic and parasympathetic activities was also registered after the amantadine challenge. The above findings supported the postulation that this drug should be a powerful therapeutic tool for treating diseases affected by adrenal sympathetic hyperactivity

    Drug sensitivity testing on patient-derived sarcoma cells predicts patient response to treatment and identifies c-Sarc inhibitors as active drugs for translocation sarcomas

    BACKGROUND: Heterogeneity and low incidence comprise the biggest challenge in sarcoma diagnosis and treatment. Chemotherapy, although efficient for some sarcoma subtypes, generally results in poor clinical responses and is mostly recommended for advanced disease. Specific genomic aberrations have been identified in some sarcoma subtypes but few of them can be targeted with approved drugs. METHODS: We cultured and characterised patient-derived sarcoma cells and evaluated their sensitivity to 525 anti-cancer agents including both approved and non-approved drugs. In total, 14 sarcomas and 5 healthy mesenchymal primary cell cultures were studied. The sarcoma biopsies and derived cells were characterised by gene panel sequencing, cancer driver gene expression and by detecting specific fusion oncoproteins in situ in sarcomas with translocations. RESULTS: Soft tissue sarcoma cultures were established from patient biopsies with a success rate of 58%. The genomic profile and drug sensitivity testing on these samples helped to identify targeted inhibitors active on sarcomas. The cSrc inhibitor Dasatinib was identified as an active drug in sarcomas carrying chromosomal translocations. The drug sensitivity of the patient sarcoma cells ex vivo correlated with the response to the former treatment of the patient. CONCLUSIONS: Our results show that patient-derived sarcoma cells cultured in vitro are relevant and practical models for genotypic and phenotypic screens aiming to identify efficient drugs to treat sarcoma patients with poor treatment options.Peer reviewe

    A novel Ruminococcus gnavus clade enriched in inflammatory bowel disease patients

    Background: Inflammatory bowel disease (IBD) is characterized by chronic inflammation of the gastrointestinal tract that is associated with changes in the gut microbiome. Here, we sought to identify strain-specific functional correlates with IBD outcomes. Methods: We performed metagenomic sequencing of monthly stool samples from 20 IBD patients and 12 controls (266 total samples). These were taxonomically profiled with MetaPhlAn2 and functionally profiled using HUMAnN2. Differentially abundant species were identified using MaAsLin and strain-specific pangenome haplotypes were analyzed using PanPhlAn. Results: We found a significantly higher abundance in patients of facultative anaerobes that can tolerate the increased oxidative stress of the IBD gut. We also detected dramatic, yet transient, blooms of Ruminococcus gnavus in IBD patients, often co-occurring with increased disease activity. We identified two distinct clades of R. gnavus strains, one of which is enriched in IBD patients. To study functional differences between these two clades, we augmented the R. gnavus pangenome by sequencing nine isolates from IBD patients. We identified 199 IBD-specific, strain-specific genes involved in oxidative stress responses, adhesion, iron-acquisition, and mucus utilization, potentially conferring an adaptive advantage for this R. gnavus clade in the IBD gut. Conclusions: This study adds further evidence to the hypothesis that increased oxidative stress may be a major factor shaping the dysbiosis of the microbiome observed in IBD and suggests that R. gnavus may be an important member of the altered gut community in IBD.
