    La American College Sports Medicine reco¬mienda alternar el ejercicio aeróbico con un trabajo muscular de fuerza-resistencia en un marco psicosocial saludable. Esta prescripción general puede extenderse a pacientes con cán¬cer. En este trabajo se presenta una guía de ejercicio físico de fuerza-resistencia adaptada al cáncer de próstata. Se incluyen las caracte¬rísticas y contraindicaciones del ejercicio de fuerza permitiendo su adaptación en los casos de fatiga, náuseas, citopenias, metástasis y de¬presión. Los elementos básicos de estudio son la fatiga y la calidad de vida (CdV). Así mismo se evalúan los cambios en la fuerza muscu¬lar y la composición corporal. Se consideran también varios factores relacionados con la adherencia al programa. El concepto multidi¬mensional y subjetivo de CdV se evalúa con el cuestionario FACT-P (Functional Assessment of Cancer Therapy-Prostate). El resultado prin¬cipal de un programa de estas características, durante y después del tratamiento de cáncer de próstata, es una mejora general de la calidad de vida. Observamos una mejora significativa de la subescala física del cuestionario, princi¬palmente con la disminución del síntoma de fatiga. La conclusión principal de la aplicación del programa es el resultado de mejora de la CdV mediado indirectamente con la disminución de la fatiga.The American College of Sports Medicine recommends aerobic exercise combined with a complete muscular workout in a healthy psychosocial environment. This general pres¬cription can be applied to cancer patients. This program offers a physical exercise guide of strength and endurance adapted to pros¬tate cancer patients. The program considers the characteristics and contraindications of strength building exercise allowing for adap¬tations in the face of fatigue, nausea, cytope¬nias, metastasis and depression. The basic ele¬ments of the study are fatigue and quality of life (QuL); changes in the muscular strength and body composition are also evaluated. On the other hand, other factors are taken into consideration such as adherence to the pro¬gram. We have evaluated the subjective and multidimensional meaning of QoL with the FACT-P questionnaire (Functional Assessment of Cancer Therapy-Prostrate). The principal re¬sult of this program during and after prostate cancer treatment is a general improvement in the quality of life (QoL). We have observed a significant improvement in the physical subs¬cale of the questionnaire, (principally) with a decrease in fatigue, the final conclusion (of the applications) of this program is the result of the improvement in the QoL indirectly media¬ted with the decrease in fatigue

    Cáncer de próstata y calidad de vida: análisis del cambio en la respuesta a partir de una triangulación entre métodos

    Introduction. The aim of this paper is to describe the changes in the quality of life (QoL) in a group of men with prostate cancer. The three mechanisms involved in the response shift correspond to the reconceptualization, reprioritization and recalibration. Material and methods. Proposed methodology is based on sequential triangulation between methods. To study the same empirical unit it combines two strategies of research: quantitative and qualitative. Results. We identified a significant decrease in QoL between the pretest and posttest period. The recalibration mechanism with the Thentest confirms that the QoL is low in all the periods evaluated. Conclusions. The results confirm that QoL in prostate cancer is low in all the periods evaluated. Coping with the disease differs between the subgroup of young men and the subgroup of older men.Introducción. El objetivo de este trabajo es describir el cambio en la Calidad de Vida (CdV) en un grupo de hombres con cáncer de próstata. Los mecanismos implicados en el cambio en la respuesta de la CdV corresponden al de reconceptualización, repriorización y recalibración. Material y método. La propuesta metodológica se basa en la triangulación secuencial entre métodos, en que para el estudio de una misma unidad empírica se combinan dos estrategias de investigación, una cuantitativa y otra cualitativa. Resultados. Se identifica una disminución significativa de la CdV entre el periodo pretest y postest. A partir del mecanismo de recalibración el thentest confirma que la CdV en el momento inicial de la enfermedad es baja. Conclusiones. La CdV del hombre con cáncer de próstata es baja en todos los periodos evaluados. El afrontamiento a la enfermedad difiere entre el subgrupo de hombres jóvenes y el subgrupo de hombres mayores

    Beneficios de un programa de ejercicio de fuerza para la mejora de la calidad de vida del hombre con cáncer de próstata

    Cancer of prostate is the most frequent cancer in men in many industrialized countries. Considering the percentage of relative survival 5 years (76.5%) and the state of morbidity generate by treatments, the quality of life of the older man with cancer of prostate is a priority objective in the public health intervention. The evaluation of the impact generated by symptoms has been determined by the frequency and the number of associated symptoms to illness and treatment. In our pinion it would be more appropriated to carry out the analysis objectifying also the impact generated in older men every day life activity. This article has a double objective. First, to evaluate the benefits of an adapted force exercise program for the illness and prostate cancer treatment. Second, to identify the most prominent symptoms of prostate cancer from the perspective of the patient and the impact that generate to their Quality of Life. The methodological proposal is based on the methodological sequential triangulation among methods, (quantitative and qualitative). Results show a significant improvement of the Quality of Life of the patients, indirectly mediated by the improvement of incontinence and pain symptoms. Also, an improvement of the capacity of the force and resistance is observed, more evident in lower extremities. It remains scientifically demonstrated the efficacy in the improvement of the Quality of Life of the adult of a physical exercise of force program adapted to the symptoms generated by the illness and the prostate cancer treatment.El cáncer de próstata es el cáncer más frecuente en los hombres en muchos países industrializados. Considerando el porcentaje de supervivencia relativa a los 5 años (76,5%) y el estado de morbilidad que generan los tratamientos vigentes, el concepto de Calidad de Vida (CdV) del hombre mayor con cáncer de próstata pasa a ser un objetivo prioritario en la intervención sociosanitaria. Clásicamente la evaluación del impacto que generan los síntomas se ha determinado por la frecuencia y el número de síntomas asociados a la enfermedad y al tratamiento. A nuestro parecer, lo más apropiado es realizar el análisis objetivando además el impacto que generan en la actividad de vida diaria de los afectados. Este artículo tiene un doble objetivo. Primero, evaluar los beneficios de un programa de ejercicio de fuerza adaptado a la enfermedad y al tratamiento de cáncer de próstata. Segundo, identificar los síntomas más relevantes del cáncer de próstata desde la perspectiva del paciente y el impacto que generan a la CdV del hombre mayor. La propuesta metodológica se basa en la triangulación metodológica entre métodos secuencial (cuantitativo y cualitativo). En los resultados se observa una mejora significativa de la CdV del enfermo, mediado indirectamente por la mejora del síntoma de incontinencia y el dolor. Así mismo se observa una mejora de la capacidad de la fuerza y resistencia muscular más evidente en las extremidades inferiores. Queda científicamente demostrada la eficacia de un programa de ejercicio físico de fuerza adaptado a los síntomas que genera la enfermedad y el tratamiento de cáncer de próstata en la mejora de la CdV de la persona mayor

    Effectiveness of a multimodal exercise rehabilitation program on walking capacity and functionality after a stroke

    The aim of this study was to determine the effectiveness of a 12-week multimodal exercise rehabilitation program on walking speed, walking ability and activities of daily living (ADLs) among people who had suffered a stroke. Thirty-one stroke survivors who had completed a conventional rehabilitation program voluntarily participated in the study. Twenty-six participants completed the multimodal exercise rehabilitation program (2 days/wk, 1 hr/session). Physical outcome measures were: walking speed (10-m walking test), walking ability (6-min walking test and functional ambulation classification) and ADLs (Barthel Index). The program consisted on: aerobic exercise; task oriented exercises; balance and postural tonic activities; and stretching. Participants also followed a program of progressive ambulation at home. They were evaluated at baseline, postintervention and at the end of a 6-month follow-up period. After the intervention there were significant improvements in all outcomes measures that were maintained 6 months later. Comfortable and fast walking speed increased an average of 0.16 and 0.40 m/sec, respectively. The walking distance in the 6-min walking test increased an average of 59.8 m. At the end of the intervention, participants had achieved independent ambulation both indoors and outdoors. In ADLs, 40% were independent at baseline vs. 64% at the end of the intervention. Our study demonstrates that a multimodal exercise rehabilitation program adapted to stroke survivors has benefits on walking speed, walking ability and independence in ADL

Clàssicament, l’avaluació de l’impacte que generen els símptomes s’ha determinat per la freqüència i el nombre de símptomes associats a la malaltia i al tractament.     The Quality of Life (QoL) is a multidimensional, dynamic and modular concept. QoL is influenced by psychological mechanisms related to the adaptation disease. The aim of this paper is to describe the research line on QoL and chronic disease. The specific objective is to determine the mechanisms involved in the change of QoL in a group of elderly people with cancer. Method. The study is prospective quasi-experimental. With a sample of 130 participants with cancer. Results. The rehabilitation program improves muscular fitness, cardiovascular efficiency, (at rest and effort) and reduces the continued decline related to treatment. We have observed a significant increase in QoL in the period evaluated. This result confirms that QoL at baseline of the disease is low. Adherence to the program achieved is 93%. Conclusions. We confirm the effectiveness of a rehabilitation program tailored to the symptoms generated by the disease and cancer treatment in improving QoL of older people. The program is a therapeutic option to preserve the QoL of the patient in the continuum of the disease. Now we are studying the timing where the implementation of the program is most effective
