At present there is a strong case for using new technologies, such as tele-nursing, to provide individualised and integral care to patients who live with a chronic process. The aim is to identify the main characteristics of tele-nursing and the factors that facilitate and hinder its implantation. Systematic review of the following databases: PubMed, Cochrane Library, Cinhal, Psycinfo, Cuiden, Dialnet and Scielo during the 2008-2019 period. The 34 articles selected identified virtual support, telephone monitoring and electronic devices as the main characteristics of tele-nursing; the patient鈥檚 profile (not young, with a low socio-economic and educational level) and the resistance of the nursing professionals were the barriers identified; while the new technological age was identified as a factor facilitating its implementation. The provision of greater training for nursing professionals to make them aware of the main characteristics of tele-nursing is essential for its acceptance and integration and consequent implantation in daily clinical practice

Finalmente, aquellos t茅cnicos con contratosThe aim of this study was to assess the level of job satisfaction of paddle tennis and swimming sports instructors by sex, age, length of service, type of contract and weekly working hours. To that end the Job Satisfaction Questionnaire for Sports Instructors (CSLTD in the Spanish initialism) was administered to 125 paddle tennis and swimming instructors (78 men and 47 women) aged between 18 and 53 (M = 28.78; DT = 8.94). The results showed average job satisfaction values for all participants. The highest scores were in the category of job security and promotion, while the lowest rated dimension was pay. In addition, job satisfaction was significantly higher in women compared to men and in paddle tennis instructors versus swimming instructors. Finally, instructors on temporary contracts and who worked significantly longer hours had lower levels of job satisfaction.Actividad F铆sica y DeporteAdministraci贸n y Direcci贸n de Empresa

Evaluation of criteria for clinical control in a prospective, international, multicenter study of patients with COPD

BACKGROUND: The concept of clinical control in COPD has been developed to help in treatment decisions, but it requires validation in prospective studies. METHOD: This international, multicenter, prospective study aimed to validate the concept of control in COPD [control = stability (no exacerbations or impairment in CAT scores) + low impact (low level of symptoms)]. Data from the screening visit was used to: investigate the level of control, compare characteristics of patients according to the control status, and perform a sensitivity analysis of the levels of control using either clinical criteria or questionnaires (COPD Assessment Test -CAT- or Clinical COPD Questionnaire -CCQ-). RESULTS: A total of 314 patients were analysed, mean age was 68.5 years and mean FEV1 was 52.6% of predicted. According to the prespecified criteria 21% of patients were classified as controlled, all of them with mild/moderate COPD (Body mass index, Obstruction, Dyspnea and Exacerbations, -BODEx-index <5). A high level of dyspnea, a high CAT score or an exacerbation in the previous 3 months were found, using univariate analysis, to be the main reasons for patients not being classified as controlled. Multivariate analysis showed that female sex, chronic bronchitis and having exacerbations in the previous year were associated with uncontrolled COPD. Changing the severity cut off of BODEx from 5 to 3 did not change significantly the percentage of patients fulfilling the criteria of control. CONCLUSIONS: The proposed criteria of control were only fulfilled by 21% of patients. The suggested cut offs and their predictive value for poor outcomes need to be refined in prospective studies

What pulmonologists think about the asthma鈥揅OPD overlap syndrome

    Get PDF
Background: Some patients with COPD may share characteristics of asthma; this is the so-called asthma鈥揅OPD overlap syndrome (ACOS). There are no universally accepted criteria for ACOS, and most treatments for asthma and COPD have not been adequately tested in this population. Materials and methods: We performed a survey among pulmonology specialists in asthma and COPD aimed at collecting their opinions about ACOS and their attitudes in regard to some case scenarios of ACOS patients. The participants answered a structured questionnaire and attended a face-to-face meeting with the Metaplan methodology to discuss different aspects of ACOS. Results: A total of 26 pulmonologists with a mean age of 49.7 years participated in the survey (13 specialists in asthma and 13 in COPD). Among these, 84.6% recognized the existence of ACOS and stated that a mean of 12.6% of their patients might have this syndrome. In addition, 80.8% agreed that the diagnostic criteria for ACOS are not yet well defined. The most frequently mentioned characteristics of ACOS were a history of asthma (88.5%), significant smoking exposure (73.1%), and postbronchodilator forced expiratory volume in 1 second/forced vital capacity ,0.7 (69.2%). The most accepted diagnostic criteria were eosinophilia in sputum (80.8%), a very positive bronchodilator test (69.2%), and a history of asthma before 40 years of age (65.4%). Up to 96.2% agreed that first-line treatment for ACOS was the combination of a long-acting 尾2-agonist and inhaled steroid, with a long-acting antimuscarinic agent (triple therapy) for severe ACOS. Conclusion: Most Spanish specialists in asthma and COPD agree that ACOS exists, but the diagnostic criteria are not yet well defined. A previous history of asthma, smoking, and not fully reversible airflow limitation are considered the main characteristics of ACOS, with the most accepted first-line treatment being long-acting 尾2-agonist/inhaled corticosteroids
