    People in the study area, the surrounding of Borkena plain, suffer from a complex interplay of environmental degradation, population growth, ethnic and religious conflicts, and related social tensions. But the most depressing problem in their life situation is that they can not find a way out of the downward spiral of resource scarcity and conflict. They are not given any chance to get involved themselves in actively searching of solutions specific to their problems. The advises they can get from the experts and authorities are not taking into account the complexity of their social situation, and consequently can not solve their problems

    Gossypiboma: A Case Series and Literature Review

    BACKGROUND: Gossypiboma (retained surgical sponge) is a rare medical event. It could cause a serious complication that can threaten patients’ life. Its diagnosis is usually difficult because the clinical symptoms are nonspecific and the imaging findings are often inconclusive.CASE PRESENTATIONS: We present two cases, a 32 years old woman who passed a retained surgical sponge via rectum 5 months after cesarean section and a 30 years old lady presented with an acute abdomen that later found to have localized right lower quadrant abscess with a retained surgical sponge.CONCLUSION: The most important approach to reduce the incidence of gossypiboma is prevention. At the end of the surgery, a correct count is always the gold standard safeguard against it. Although errors are not to be completely avoided, continuous CPD and strict adherence to rules of the operating room will reduce its incidence to a minimum

    Weathering of the Ethiopian volcanic province: a new weathering index to characterize and compare soils

    © Walter de Gruyter Berlin/Boston 2015.Soil formation occurs through numerous physical and chemical weathering processes acting to alter the parent rock on the Earths surface. Samples of surface soils were collected over a range of elevations (2000-3600 m) from profiles directly overlying basaltic to more felsic parent rocks, over a region in NW Ethiopia. The soils were investigated to determine their chemical composition and X-ray diffraction was used to identify and quantify individual mineral phases. The data set was analyzed using non-parametric statistics (Spearmans Rank and Mann-Whitney U tests) to compare the soils forming over the two parent rocks. Principal component analysis (PCA) was used to identify the mineral alteration assemblage and formation during pedogenesis. The extent of alteration was quantified using several chemical weathering indices (Chemical Index of Alteration = CIA; Chemical Index of Weathering = CIW), including an index calculated by multivariate analyses of the soil chemical composition data (weathering W index). Further to this we devised and tested a new weathering index (Wmin) using multivariate analysis of the soil mineralogy, to estimate the extent of weathering and physico-chemical proprieties of the parent rock from which the soil formed. The soils present a fair to advanced stage of alteration, with abundant iron (Fe) oxides (up to 40 wt%) and phyllosilicates (up to 57 wt%), including kaolinite-smectite (K-S) mixed-layer phases. The K-S was composed of either 30-50% kaolinite or 94-98% kaolinite layers. Discrete kaolinite was also present. The bimodal K-S mineralogical composition is likely due to two precursor phases: feldspar for the kaolinite-rich K-S and volcanic glass for the smectite-rich K-S. K-S with intermediate composition (50-94% kaolinite) was rare, due to its instability. Statistical analysis showed significant differences between the chemical compositions of the soils developed on the two different parent volcanic compositions. The soils overlying the more felsic parent rocks were less altered than those overlying the flood basalt. When comparing the weathering indices calculated in this study, we conclude that while the CIA and CIW may be more readily determined, the W and Wmin indices can elucidate information on the composition of the original rock from which they formed. The W index is more sensitive to certain variables when compared with the newly derived mineralogical Wmin index; however the Wmin index takes into account mineral phases within the sample, which provides a more detailed interpretation of weathering rates than chemistry alone. In addition the Wmin index correlated with meteorological variables, such as elevation (and consequently temperature and precipitation), known to influence the degree of pedogenesis. The Wmin index can be used to enhance our understanding of the processes that occur during weathering processes to supplement information gained from traditional chemical weathering indices

    Pattern of Vascular Diseases at Tikur Anbessa Specialized Hospital, Addis Ababa, Ethiopia

    BACKGROUND: Vascular diseases are evolving fast in sub-Saharan Africa, but its management is challenged by lack of expertise and infrastructure. In the light of the prevailing challenge, this study was done to see the pattern of vascular disease and its treatment in a tertiary referral hospital.METHODS: A prospective cross-sectional study was done over a period of one year (February 9, 2016 to February 8, 2017) at Tikur Anbessa specialized hospital (TASH) at Addis Ababa, Ethiopia.RESULTS: A total of 386 patients were seen at the surgical OPD. Of these, 78(20.2%) were admitted and operate on. The male to female ratio was 1.3:1. The mean age affected was 39 +/- 10 (Range 12-91 years). On the other hand, 132 (34.2%) patients came with PAD. Of them, 46(34.8%) presented with either frank Gangrene or pre-gangrene stage. The rest 86(65.1%) had claudication pain. The other diseases seen are Varicose Vein, 100(25.9%), Carotid body tumors, 60(15.4%), Aneurysmal diseases, 36(9.1%), Vascular malformations, 34(8.7%), and Vascular injuries, 22(5.6%). During the study period, 28(35.9%) PAD, 22(28.2%) trauma patients, 8(10.2%) Chemodectomas, 8(10.2%) aneurysms, 6(7.7%) hemangiomas, 4(5.1%) varicose vein and 2(2.6%) AV fistula patients were operated.CONCLUSION: The pattern of vascular disease in Ethiopia is becoming a challenge. The gap created due to limited vascular surgeons, poor infrastructure and absent supply chain system has significantly compromised the number and type of operated-on patients. These challenges result in preventable morbidity and mortality

    Observation of resonant interactions among surface gravity waves

    We experimentally study resonant interactions of oblique surface gravity waves in a large basin. Our results strongly extend previous experimental results performed mainly for perpendicular or collinear wave trains. We generate two oblique waves crossing at an acute angle, while we control their frequency ratio, steepnesses and directions. These mother waves mutually interact and give birth to a resonant wave whose properties (growth rate, resonant response curve and phase locking) are fully characterized. All our experimental results are found in good quantitative agreement with four-wave interaction theory with no fitting parameter. Off-resonance experiments are also reported and the relevant theoretical analysis is conducted and validated.Comment: 11 pages, 7 figure

    Changes in response to integrated interventions in the value chain of vegetables in Atsbi-Womberta District of Tigray Region, Northern Ethiopia

    With irrigated vegetables development, interventions on the uses of improved inputs such as water lifting devices; varieties; on-farm water, nutrient and pest management, and access to credit and market information were introduced in Atsbi-Womberta district, Ethiopia. Besides, skill and uptake capacity of vegetable growers, extension service providers and vegetable traders were improved accordingly. The response to the integrated interventions in the valuechain of vegetables indicated that the total annual income increased from less than 16,733 in 2000 to more than 3.0 million USD in 2008. Simultaneously, beneficiaries increased by 82% while irrigated vegetable coverage by 87%. These successful changes in income attributed to improved capacity of actors to manage irrigated vegetables effectively in response to emerging opportunities and challenges including shifts in irrigated crop choices in reply to nutrient mining, pest load and market demand. Hence, the introduction of highland pulses in rotation with vegetable successfully breaks the pest load and increase soil fertility while simultaneously generating high income. Moreover, the presence of attractive market for vegetables and alternative crops triggers the expansion of water harvesting and utilization, increased crop diversification and sharpened the choice of marketable crops to optimize income

    Shifting towards market-oriented irrigated crops development as an approach to improve the income of farmers: Evidence from northern Ethiopia

    Rainfed crop production in the semi-arid areas is associated with high risk of rainfall variability which occasionally leads to complete crop failure. Most of the farmers in Atsbi-Womberta district of Eastern Tigray region, northern Ethiopia, have been classified as food insecure. To improve farmers’ livelihoods in the district, interventions targeted at resource conservation and better use of conserved water for market-oriented irrigated crops development have been promoted. This paper analyses the process and outcome of shifting towards market-oriented irrigated crops development and compares variation in income between beneficiary and nonbeneficiary households. The transformation processes are based on market-oriented diversification into high value irrigated crops, based on a value chain approach, and demand driven, participatory and knowledge based extension. The paper also assesses the response of smallholder farmers to emerging opportunities and challenges, and the spectrum of transformation processes and strategies followed to bring changes in smallholder farmers’ income. The changes associated with the interventions were systematically captured using household level surveys, specialized studies and key informant interviews. Results show that farmers gradually shifted to non-cereal, more market-oriented irrigated crops, in which the share of market-oriented irrigated crops increased from 27% of the total irrigated land in 2004 to 89% in 2009. Similarly, the total irrigated area increased by about fourfold and the number of beneficiaries by about threefold due to improved use of conserved surface and groundwater, and investment in farmer skills and water lifting devices. Moreover, crop diversification and intensification of households increased in response to emerging opportunities and challenges. Annual cropping intensity shifted from mono-crop cereal to 2–3 high value diversified crop farming by some farmers. On aggregate, average income of beneficiary households increased by about ninefold compared to non-beneficiary households. In the transformation, there have been continuous and interconnected processes and strategies in improving the effectiveness of adopting and using new skills and knowledge in harvesting and productive use of water, and responsive capacity of the extension system and partners. The lessons imply that investment in resources conservation could be enhanced if linked with short-term income generating activities through linkages with markets
