High Prevalence of Non-Typhoid Salmonella Bacteraemia Among Febrile HIV Adult Patients Admitted at a Tertiary Hospital, North-Western Tanzania.
Bacterial blood stream infections constitute a significant public-health problem and it is an important cause of morbidity and mortality in HIV infected patients. Little is known in developing countries regarding salmonella bacteraemia among HIV patients. The purpose of this study was to determine the bacterial pathogens causing blood stream infection among febrile adults attending in a tertiary hospital North-Western, Tanzania. A prospective cross-sectional study involving 346 consecutive, febrile adult patients admitted at Bugando Medical Centre was conducted. Demographic and other data were collected using standardized questionnaires. Blood culture was done followed by susceptibility testing using disc diffusion method. HIV testing was also performed as per Tanzania national algorithm and total white blood cell counts and CD4+ counts determined. Of 346 febrile adult patients 33 (9.5%) had blood stream infections. The common isolates were Salmonella spp 13(39.4%), Escherichia coli 8 (24.2%), Streptococcus pneumonia 5(15.2%), Staphylococcus aureus 4(12.1%), Citrobacter spp 1(3%), Streptococcus pyogenes 1(3%) and Klebsiella pneumonia 1(3%). A total of 156 (45.1%) patients were HIV infected; of whom 12/156 (7.6%) were infected by non-typhoid Salmonella spp compared to 1/190 (0.5%) of non-HIV infected patients (RRR 11.2, p=0.029) infected with Salmonella typhi. HIV infected patients with bacteraemia had significantly lower CD4+ count than those without bacteraemia (median 28 vs. 88 cells/ml, p=0.01). Patients with salmonella bacteraemia had significantly lower median of WBC than those with non-salmonella as well as those without bacteraemia (median, 3.6 vs. 17.5 vs. 9.8x109, p=0.0001). All Salmonella spp were sensitive to ceftriaxone and imipenem, while being 84%, 69.2%, 38% and 8% resistant to chloramphenicol, ampicillin, sulphamethaxazole/trimethoprim and ciprofloxacin respectively. Predictors of mortality were HIV infection (OR 2.3, p=0.006), Glasgow coma score of less than 15 (OR 3.4, p=0.0001) and night sweats (OR 2.4, p=0.014). Non-typhoid Salmonella spp that are highly resistant to common antibiotics are predominant cause of bacterial blood stream infection among HIV patients attending Bugando Medical Centre. Continuous surveillance and intervention strategies should be put in place to monitor and manage cases of bloodstream infections in HIV-positive patients in Mwanza, Tanzania

A novel cellular pathway of antigen presentation and CD4 T cell activation in vivo
Dendritic cell activation of CD4 T cells in the lymph node draining a site of infection or vaccination is widely considered the central event in initiating adaptive immunity. The accepted dogma is that this occurs by stimulating local activation and antigen acquisition by dendritic cells, with subsequent lymph node migration, however the generalizability of this mechanism is unclear. Here we show that in some circumstances antigen can bypass the injection site inflammatory response, draining freely and rapidly to the lymph nodes where it interacts with subcapsular sinus (SCS) macrophages resulting in their death. Debris from these dying SCS macrophages is internalized by monocytes recruited from the circulation. This coordinated response leads to antigen presentation by monocytes and interactions with naïve CD4 T cells that can drive the initiation of T cell and B cell responses. These studies demonstrate an entirely novel pathway leading to initiation of adaptive immune responses in vivo
Survival and cause-specific mortality of female eastern wild turkeys in two frequently-burned longleaf pine savannas
Longleaf pine savannas have declined throughout the southeastern United States due to land-use change. Fortunately, natural resource professionals are currently restoring these ecologically and economically important savannas. Although efforts are underway to restore longleaf pine savannas, little information exists on female eastern wild turkey Meleagris gallopavo silvestris population dynamics in these systems. Therefore, we evaluated survival and cause-specific mortality of female eastern wild turkeys in two longleaf pine savannas in southwestern Georgia. We radio-marked 126 female wild turkeys during 2010–2013 and monitored their survival; 66 (52.4%) radio-marked females died during the study. We estimated causes of death for 37 mortality events with predation serving as the leading known cause of mortality, with 35.1% of mortalities attributed to mesocarnivore predation (e.g., bobcat Lynx rufus, coyote Canis latrans, and gray fox Urocyon cinereoargenteus) and 18.9% to great-horned owl Bubo virginianus predation. One female (2.7%) was hit by a vehicle. Seasonal survival estimates varied from a high during fall (Ŝ = 0.94; 95% CI: 0.86–1.00) to a low during spring (Ŝ = 0.76; 95% CI: 0.68–0.87). Survival of incubating females was 0.82 (95% CI: 0.71–0.93) and survival of nonincubating females was 0.67 (95% CI: 0.52–0.87). Annual survival was 0.55 (95% CI: 0.44–0.67). To ensure sustainable wild turkey populations in longleaf pine savannas, we suggest managers monitor relationships between survival and population productivity
