    Une cause inhabituelle de dysphagie haute: schwannome de la l oge hyo-thyro-épiglottique

    Les schwannomes sont des tumeurs mésenchymateuses, bien encapsulées des nerfs périphériques, de croissance lente. La localisation laryngée est exceptionnelle, nous présentons un cas non décrit dans la littérature de shwannome localisé au niveau de la loge hyothyroépiglottique. Une patiente de 50 ans, qui présentait une sensation de corps étranger depuis 4 ans, évoluant vers une dysphagie haute associée à une voix rauque. La laryngoscopie directe en suspension objectivait une masse supraglottique sous muqueuse régulière, réduisant la lumière pharyngo-laryngée. La TDM et l'IRM concluaient à une tumeur bénigne de la loge hyo-thyro-épiglottique. Une exérèse chirurgicale par voie externe a été réalisée et une trachéotomie première était nécessaire. L'examen histologique avec une étude immunohistochimique confirmait un schwannome bénin. Les suites post opératoires étaient simples. Aucune récidive n'était objectivée après 2 ans de recul

    Prevalence of stress in Casablanca medical students: a cross-sectional study

    Introduction: recently, an important literature data has reported that medical students experience stress more than students in other disciplines. In contrast, there is a significant shortage of the stress impact on the academic performance. The primary purpose of our study was to determine the prevalence of stress among Casablanca Medical students and to investigate if there is an association between stress and academic skills. Methods: a total of 275 participants studying at Casablanca Medical School were included. The study was conducted using a self-administered, anonymous questionnaire, which included four subscales on academic skills perception (Academic competence, Test competence, Time management and Strategic study habits) and a Test Anxiety scale to assess the degree of stress related to exams among medical students. Results: the overall findings showed that 52.7% of respondents were stressed by examinations, and the highest stress prevalence was among the fifth-year medical students. Measures of comparative stress degrees between male and female students did not show any statistical significant differences (p=0.34). Correlation analysis revealed negative association between stress and academic competence (-0.394), test competence (-0.426), time management (-0.240), strategic study (-0.183) respectively (p<0.001). Conclusion: medical educators and psychologists have to increase clinical awareness of stress among medical students, by establishing strategies for stress management

    Working Memory in Overweight Boys during Physical Education Classes

    This study examined the effect of small-sided football games (SSFG) and small-sided Ultimate Frisbee games (SSUFG) on working memory, response times, and feeling mood of boys with overweight. Twenty-eight boys (age 12.23 ± 1.58 years) participated in two trials during physical education lessons (20-min SSFG and 20-min SSUFG) in a counterbalanced, randomized crossover design. The response times and feeling mood were measured for all participants post-exercise through the Sternberg paradigm and feeling scale. For the response times, the paired samples t-test revealed a significantly better reaction time after SSUFG at the “One item level” of the Sternberg paradigm (p = 0.014, Hedges’ g = 0.27, small effect) and (p = 0.010, Hedges’ g = 0.74, medium effect), at “Three item level” (p = 0.000106, Hedges’ g = 1.88, very large effect). The SSFG also showed vigor at the “Five item level” (p = 0.047, Hedges’ g = 0.61, medium effect). For the feeling mood, the feeling score was significantly higher after the SSUFG session than the SSFG session and the increase in feeling scores observed after switching from SSFG to SSUFG was significantly different (p < 0.001) from the decrease observed in feeling scores after switching from SSUFG to SSFG. Therefore, the results of the study allow the teacher to introduce new sports and reflect on the motor tasks he or she proposes for boys with overweight during physical education classes

    Training and fitness variability in elite youth soccer:perspectives from a difficulty prediction model

    Research within sport science disciplines seeks to enhance performance via the combination of factors that influences the team’s periodization. The current study aimed to investigate the variations in training load (TL), and the consequential changes in fitness variables, based on the use of match difficulty prediction model (MDP), level of opposition (LOP), days between matches, and match location during 12 weeks in the competitive period I. Seventeen elite soccer players (age = 17.57 ± 0.49 years; body height 1.79 ± 0.05 m; body weight 72.21 ± 6.96 kg), have completed a Yo-Yo intermittent recovery test, a running-based anaerobic sprint test, a soccer-specific repeated sprint ability, and a vertical jump test to identify changes in players fitness. TL was determined by multiplying the RPE of the session by its duration in minutes (s-RPE). Training monotony, strain, and acute:chronic workload ratio (ACWR) were also assessed. A simple regression model was conducted and the highest variances explained (R2) were used. The LOP score explained most of the variance in ACWR (r= 0.606, R2=0.37). TL declined significantly when compared the match-day by the first three days and the last three days of the week. No significant difference was found in s-RPE between the high and low MDP factor. Strong negative correlations were reported between ACWR and LOP (r=-0.714, p<.01). In addition, we found a significant improvement in repeated sprint ability, aerobic and anaerobic fitness variables between pre- and post-test in fatigue index (d=1.104), best testing time, ideal time, total time and mean-best (d=0.518-0.550), and aerobic and anaerobic fitness variables (p<.05), respectively. The MDP could facilitate the training prescription as well as the distribution of training intensities with high specificity, providing a long-term youth player’s development and allowing teams to maintain optimal fitness leading into more difficult matches

    Les duplications de l'appareil digestif sont les malformations congénitales rares qui peuvent toucher tout  l'appareil digestive depuis la bouche jusqu' à l'anus. Certaines duplications sont asymptomatiques et sont diagnostiqués dans la plupart des cas pendant l'enfance. La prise en charge de la duplication gastrique est essentiellement chirurgicale. Le traitement de choix est l'exérèse complète de la duplication gastrique. Les auteurs rapportent un cas inhabituel de duplication gastrique complètement reséquée par laparoscopie. A notre connaissance, ceci est le premier cas d'une duplication gastrique traitée avec succès par laparoscopie dans la littérature Tunisienne.     Oil palm fruit bunch grading system using red, green and blue digital number

    This research deals with the ripeness grading of oil palm fruit bunches. The current practice in the oil palm mills is to grade the oil palm bunches manually using human graders. This method is subjective and subject to disputes. In this research, we developed an automated grading system for oil palm bunches using the RGB color model. This grading system was developed to distinguish between the three different categories of oil palm fruit bunches. The maturity or color ripening index was based on different color intensity. Our grading system employs a computer and camera to analyze and interpret images equivalent to the human eye and brain. The colors namely Red, Green and Blue (RGB) of the palm oil fruit bunch were investigated using this grading system. The computer program developed and used the mean color intensity to differentiate between the different color and ripeness of the fruits such as oil palm FFB. The program results showed that the ripeness of fruit bunch could be differentiated between different categories of fruit bunches based on RGB intensity

    A scoping review of the application of motor learning principles to optimize myoelectric prosthetic hand control.

    Although prosthetic hand rejection rates remain high, evidence suggests that effective training plays a major role in device acceptance. Receiving training early in the rehabilitation process also enhances functional prosthetic use, decreases the likelihood of developing an overreliance on the intact limb, and reduces amputation-related pain. Despite these obvious benefits, there is a current lack of evidence regarding the most effective training techniques to facilitate myoelectric prosthetic hand control, and it remains unknown whether training is effective in facilitating the acquisition and transfer of prosthetic skill. In this scoping review, we introduced and summarized key motor learning principles related to attentional focus, implicit motor learning, training eye-hand coordination, practice variability, motor imagery, and action observation, and virtual training and biofeedback. We then reviewed the existing literature that has applied these principles for training prosthetic hand control before outlining future avenues for further research. The importance of optimizing early and appropriate training cannot be overlooked. While the intuition and experience of clinicians holds enormous value, evidence-based guidelines based on well-established motor learning principles will also be crucial for training effective prosthetic hand control. While it is clear that more research is needed to form the basis of such guidelines, it is hoped that this review highlights the potential avenues for this work

    A sophisticated Drowsiness Detection System via Deep Transfer Learning for real time scenarios

    Driver drowsiness is one of the leading causes of road accidents resulting in serious physical injuries, fatalities, and substantial economic losses. A sophisticated Driver Drowsiness Detection (DDD) system can alert the driver in case of abnormal behavior and avoid catastrophes. Several studies have already addressed driver drowsiness through behavioral measures and facial features. In this paper, we propose a hybrid real-time DDD system based on the Eyes Closure Ratio and Mouth Opening Ratio using simple camera and deep learning techniques. This system seeks to model the driver's behavior in order to alert him/her in case of drowsiness states to avoid potential accidents. The main contribution of the proposed approach is to build a reliable system able to avoid false detected drowsiness situations and to alert only the real ones. To this end, our research procedure is divided into two processes. The offline process performs a classification module using pretrained Convolutional Neural Networks (CNNs) to detect the drowsiness of the driver. In the online process, we calculate the percentage of the eyes' closure and yawning frequency of the driver online from real-time video using the Chebyshev distance instead of the classic Euclidean distance. The accurate drowsiness state of the driver is evaluated with the aid of the pretrained CNNs based on an ensemble learning paradigm. In order to improve models' performances, we applied data augmentation techniques for the generated dataset. The accuracies achieved are 97 % for the VGG16 model, 96% for VGG19 model and 98% for ResNet50 model. This system can assess the driver's dynamics with a precision rate of 98%
