    The socio-demographic, clinical and forensic medical investigation of suicide attempts over 18 years old presented to a training and research hospital’s emergency department: Izmir example

    *Parlak, İsmail ( Aksaray, Yazar )Suicide and attempted suicide are among the causes of serious morbidity and mortality and are an important public health problem today. In this study, it was aimed to evaluate socio-demographic, clinical and forensic medical aspects of suicide attempters admitted to our hospital, to identify some risk factors. Thus, we emphasize the importance of suicide and drawing attention to taking measures to prevent suicides. The necessary permissions were obtained for the study. The records of 710 patients who applied to the Emergency Department of Izmir Bozyaka Training and Research Hospital between 01 January 2014 and 31 December 2015 were retrospectively reviewed. 54.9% of the patients were female and 45.1% were male. Their ages ranged from 18 to 87 years and the mean age was 32.13 ± 11.52 years. It was determined that they preferred to drink drugs most frequently (n = 665, 93.7%) and then die with firearm (n = 30, 4.2%). Four (0.6%) cases died after all interventions. There is much scientific research on suicide attempts. However, there is insufficient research on how to approach suicidal cases when emergency department workers encounter suicide attempts. In order to prevent suicide attempts and deaths which are a serious public health problem, we think that risk factors should be determined, and protective measures should be taken

    Bu çalışmada, konut satın alınması kararının verilmesinde çok kriterli karar verme yaklaşımlarından Gri İlişkisel Analiz kullanılmıştır. Bu yöntem, veri miktarının yetersiz olduğu hallerde özellikle tercih edilmektedir. Bu yöntemle seçenekler arasında kriterlerin eşit ağırlıkta olması ve farklı değerler taşıması hallerine göre iki ayrı hesaplama ve sıralama yapılmıştır. Çalışmada altı farklı kriter esas alınmış ve dokuz farklı alternatif arasından seçimler yapılmıştır. Yöntemin benzer problemlerin çözümünde yalnız başına yada başka yöntemlerle birlikte kullanılabilmesi mümkün bulunmaktadır

    Yüklenici inşaat firmalarının planlama ve risk yönetimi algılamalarının belirlenmesi, eksiklik ve gerekliliklerin anlaşılması, bugünkü durumun fotoğrafının çekilmesi ve bu veriler yardımıyla ileriye yönelik projeksiyonların yapılabilmesi, tedbirlerin alınabilmesi bu çalışmanın ana amaçlarını oluşturmaktadır. Bu amaçlar doğrultusunda yurtiçi ve yurtdışında faaliyet gösteren inşaat firmalarına risk algılama ve tutumlarının değerlendirildiği bir anket uygulanmıştır. Edinilen bulgulara göre firmalarımız geç tamamlanma ve kalite düşüklüğü problemlerini az, maliyet artışı sıkıntısını ise çok sıklıkta yaşamaktadırlar. Tasarım riskleri orta, çevresel riskler düşük, finansal/ekonomik riskler en fazla, politik ve yasal riskler orta ve fazla sıklıkta yaşanmaktadır. Belirsizliklerin ölçülebilir hale getirilmesi, modern planlama tekniklerinin kullanılması ve ciddi sözleşme yönetimi uygulamalarının yapılması gerektiği düşünülmektedir

    Kat karşılığı inşaat yapımı günümüz inşaat sektöründe oldukça yaygındır. Müteahhit ve arsa sahibi arasında imzalanan kat karşılığı inşaat sözleşmesi ile tarafların yükümlülükleri ve hakları belirtilir. Bu çalışmada üç adet kat karşılığı inşaat sözleşmesinin; sözleşme sorumluluklarında risklerin dağılımı, risk grupları ve önem derecelerinin belirlenmesi ve İngiliz Local Government Task Force-Standart İnşaat Sözleşmeleri Klavuzu: Sözleşme Anahtar Özellikleri ve Bileşenleri Tabloları kullanılarak risk analizleri yapılmıştır. Tarafların sorumluluğunda olan işler ile bunları gerçekleştirememeleri durumunda ortaya çıkacak riskler tespit edilmiştir. Yapılan risk analizleri sonucunda müteahhitin arsa sahibine nazaran daha çok sorumluluğu olduğu ve sorumluluklarını yerine getirememesi durumunda daha çok risk altında olduğu saptanmıştır. Bundan dolayı müteahhitlik yapacak olan firmanın bir projenin sorumluluğunu yüklenmeden önce işin sözleşmesinin risk analizlerini yaparak ortaya çıkan sonuçlara göre karar vermesi daha doğru olacaktır

    Diagnosis of comorbid migraine without aura in patients with idiopathic/genetic epilepsy based on the gray zone approach to the International Classification of Headache Disorders 3 criteria

    BackgroundMigraine without aura (MwoA) is a very frequent and remarkable comorbidity in patients with idiopathic/genetic epilepsy (I/GE). Frequently in clinical practice, diagnosis of MwoA may be challenging despite the guidance of current diagnostic criteria of the International Classification of Headache Disorders 3 (ICHD-3). In this study, we aimed to disclose the diagnostic gaps in the diagnosis of comorbid MwoA, using a zone concept, in patients with I/GEs with headaches who were diagnosed by an experienced headache expert.MethodsIn this multicenter study including 809 consecutive patients with a diagnosis of I/GE with or without headache, 163 patients who were diagnosed by an experienced headache expert as having a comorbid MwoA were reevaluated. Eligible patients were divided into three subgroups, namely, full diagnosis, zone I, and zone II according to their status of fulfilling the ICHD-3 criteria. A Classification and Regression Tree (CART) analysis was performed to bring out the meaningful predictors when evaluating patients with I/GEs for MwoA comorbidity, using the variables that were significant in the univariate analysis.ResultsLonger headache duration (<4 h) followed by throbbing pain, higher visual analog scale (VAS) scores, increase of pain by physical activity, nausea/vomiting, and photophobia and/or phonophobia are the main distinguishing clinical characteristics of comorbid MwoA in patients with I/GE, for being classified in the full diagnosis group. Despite being not a part of the main ICHD-3 criteria, the presence of associated symptoms mainly osmophobia and also vertigo/dizziness had the distinguishing capability of being classified into zone subgroups. The most common epilepsy syndromes fulfilling full diagnosis criteria (n = 62) in the CART analysis were 48.39% Juvenile myoclonic epilepsy followed by 25.81% epilepsy with generalized tonic-clonic seizures alone.ConclusionLonger headache duration, throbbing pain, increase of pain by physical activity, photophobia and/or phonophobia, presence of vertigo/dizziness, osmophobia, and higher VAS scores are the main supportive associated factors when applying the ICHD-3 criteria for the comorbid MwoA diagnosis in patients with I/GEs. Evaluating these characteristics could be helpful to close the diagnostic gaps in everyday clinical practice and fasten the diagnostic process of comorbid MwoA in patients with I/GEs

It creates a hierarchy using the goal, decision criteria and decision alternatives, and sorts the various alternatives according to their relative importance. The aim in AHP is to choose the most suitable and important alternative, by making an arrangement from the most important to the least. In this study, a real life material selection application in a hotel building is described by using AHP method. Wall materials such as brick blocks, pumice concrete block, sand autoclaved aerated concrete blocks are decision alternatives and mechanical properties, physical properties, ease of application and costs of these materials are the decision factors. The analysis was performed based on the opinion of an expert and the most suitable alternative is selected. Also, it is concluded that the order related to the alternatives is reliable for the decision markers. Thus, a decision supporting method for a construction company using AHP applications is developed
