COVID-19-Associated Invasive Pulmonary Aspergillosis in the Intensive Care Unit: A Case Series in a Portuguese Hospital
Invasive pulmonary aspergillosis (IPA) has become a recognizable complication in
coronavirus disease 2019 (COVID-19) patients admitted to intensive care units (ICUs). Alveolar
damage in the context of acute respiratory distress syndrome (ARDS) appears to be the culprit in
facilitating fungal invasion in COVID-19 patients, leading to a COVID-19-associated pulmonary
aspergillosis (CAPA) phenomenon. From November 2020 to 15 February 2021, 248 COVID-19
patients were admitted to our ICUs, of whom ten patients (4% incidence) were classified as either
probable (six) or possible (four) CAPA cases. Seven patients had positive cultural results: Aspergillus
fumigatus sensu stricto (five), A. terreus sensu stricto (one), and A. welwitschiae (one). Five patients
had positive bronchoalveolar lavage (BAL) and galactomannan (GM), and two patients had both
positive cultural and GM criteria. All but two patients received voriconazole. Mortality rate was
30%. Strict interpretation of classic IPA definition would have resulted in eight overlooked CAPA
cases. Broader diagnostic criteria are essential in this context, even though differentiation between
Aspergillus colonization and invasive disease might be more challenging. Herein, we aim to raise
awareness of CAPA in view of its potential detrimental outcome, emphasizing the relevance of a
low threshold for screening and early antifungal treatment in ARDS patients.info:eu-repo/semantics/publishedVersio
Long-term TNT and DNT contamination: 1-D modeling of natural attenuation in the vadose zone: case study, Portugal
The vadose zone of a trinitrotoluene (TNT) and dinitrotoluene (DNT) contaminated site was investigated to assess the mobility of those explosives under natural conditions. Located in the left margin of the River Tejo Basin, Portugal, the site is located on unconsolidated sediments. Wastewaters associated with the 50-year explosives production were disposed in excavated ponds, from where water would infiltrate and pollute the unsaturated and saturated parts of the local aquifers. Two boreholes were drilled to 9 m depth in such a former waste pond to investigate the contaminant's fate in the vadose zone. Sediment samples were taken every 1-2 m for analysis of the polynitroaromatics (p-NACs) and organic volatile compounds, pH, organic carbon content, cation exchange capacity and grain size analysis. The main contaminant was TNT representing >70 % of the total p-NACs concentration that peaked approximately 7 mg/kg in one borehole, even if the median in both boreholes was of similar to 1 mg/kg. DNT was 4-30 % of the total p-NACs and nitrotoluene (NT), up to 5 %. No other (volatile) organic compound was detected. The predominance of TNT as the main contaminant implies that any natural mass reduction has been inefficient to clean the site. Several 1-D model simulations of p-NACs cleaning of the vadose zone under natural conditions indicated that the most probable scenario of combined advection and partitioning will only remove TNT after 10's of years, whereas DNT and NT will hardly be removed. Such low concentrations and long times for the p-NACs removal, suggest that by now those compounds have been washed-out to a level below standard limits
Epidemiology of Superficial Fungal Infections in Portugal: 3-year review (2014-2016)
Resultados: Foram estudados 2375 isolamentos, pertencentes a 2319 doentes.
Developing number sense in Preschool and 1st grade through games and children's literature
crianças do pré-escolar e do 1.º ano do Ciclo do Ensino Básico (CEB), recorrendo a jogos e literatura
infantil.
concretamente a contagem e correspondência termo a termo. No pré-escolar, foram implementadas
atividades lúdicas que envolviam a contagem e correspondência termo a termo utilizando as narrativas
e jogos.
A literatura serviu
The main purpose of this research is to enhance the development of number sense in preschool
and 1st grade children using games and children's literature.
Number sense refers to the understanding of fundamental mathematical concepts, such as
quantity, order, and number patterns. In this specific context, games and children's literature are used
as a pedagogical tool to promote meaningful and playful learning of mathematical concepts. The primary
focus was the development of number sense, more specifically counting and term-to-term
correspondence. In preschool, playful activities involving these same skills were implemented using
narratives and games.
In the 1st grade, the focus was on developing mental calculus. The concept of "friends of 10"
was worked on, by exploring combinations of numbers that add up to 10. The "ten frame" strategy was
also implemented, which eased up the identification of patterns and contributed to the visual recognition
of numbers, besides helping to understand the positional value.
Counting, term-to-term correspondence, friends of 10 and the 10-frame were worked in an
integrated way, providing a playful and meaningful learning environment for the children. All in all, the
activities developed during the pre-school and 1st grade internship demonstrated that the combination of
games, hands-on activities, and children's literature is an effective approach to developing number sense
and mental calculation. Literature served as an additional resource to consolidate mathematical concepts
and stimulate students' interest and active participation
