    Methodological aspects of assessing regional publication activity and citations: the case of the RF central federal District Universities

    The article focuses on calculations to assess contribution made by University publication activity and citations indicators to the regional integral indicator of publication activity and citations for Voronezh Region, Ivanovo Region, and Ryazan Region. It shows that relative indicators are more significant in calculations of this kind therefore regions with a large number of Universities have no advantage. The article offers a triangular matrix of ratios expressing how much one regional integral indicator of publication activity and citations exceeds the other, which was calculated for the RF Central Federal District regions. Ratios range up to 3.47.     В сучасних інерціальних навігаційних системах широко застосовуються датчики з пружними підвісами. У загальному випадку пружні підвіси мають вигляд просторових конструкцій, а вибір їх оптимальних параметрів вимагає багаторазових розрахунків і, в першу чергу, характеристик жорсткості. Розглянуто можливість поєднання методу вузлової конденсації та пакету пакеті прикладних програм скінченно-елементного аналізу. Це дає змогу розраховувати характеристики складних механічних конструкцій у аналітичному вигляді, заощаджувати час розрахунків та машинний ресурс. Можливість поєднання двох методів розглянуто на прикладі розрахунку жорсткістних характеристик пружного підвісу камертонного мікромеханічного гіроскопу. Для порівняння проведено моделювання у пакеті прикладних програм скінченно-елементного аналізу. Результати співпали з достатньою точністю. Таким чином доведено можливість інтеграції методу вузлової конденсації та пакету скінченно-елементного аналізу.The modern intertial navigation devices often utilize elastic suspension based sensitive elements. In general elastic suspension has a complicated structure, and its optimization requires numerous calculations for various parameters, such as stiffness characteristics. The major contribution to the solution of the above optimization problem is brought by highly effective nodal condensation method. This method however is often overcomplicated by difficulties in the analytical determinization of stiffness matrixes of the elastic elements. Furthermore, it is even impossible to perform exact calculations in some cases. On countrary to the nodal condensation method, the finite elements analysis tools provide inexact solutions to the optimization problems, and thus can be applied for elastic elements of even complicated structure. The disadvantages of the finite elements method is in large computational resourses required for the calculations, and in the absence of the preprocessing analytical structural analysis that might simplify the following calculations. Hence the research on the possible integration of the nodal condensation method into the finite-elements analysis tools is of the primary importance for the handling of the complicated designs. In this work, we investigate the results of the aformentioned integration for the stiffness characteristics analysis of the micromechanical tuning-fork gyroscopes. More precisely, stiffness characteristics and the computation of the loads on the individual elastic elements are proposed to compute using the equations of the nodal condensation method. After the individual characteristics are calculated, the finite-elements method is used to compute the optimal stiffness characteristics for the elastic element. Combination of the nodal condensation method and the finite elements method, was applied for the calculation of the stiffness characteristics of the micromechanical tuning-fork gyroscope. It showed better performance compare to their separate application, thus proving its effectiveness. Analysis of the results obtained in the conducted investigation provides a conclusion of the principal superiority of the proposed method. It allows to improve the precision of the obtained optimal solution, and to reduce the computational effort. In summary, proposed method gives an opportunity of utilizing strong sides of both nodal condensation method and finite elements method for the analysis of the application of different loads on the individual elements of the mechanical design.В современных инерциальных навигационных системах широко применяются датчики с упругими подвесами. В общем случае упругие подвесы имеют вид пространственных конструкций, а выбор их оптимальных параметров требует многоразовых расчетов и, в первую очередь, характеристик жесткости. Рассмотрена возможность объединения метода узловой конденсации и пакета прикладных программ конечно-элементного анализа. Это даёт возможность рассчитывать характеристики сложных механических конструкций в аналитическом виде, сэкономить время расчета и машинный ресурс. Возможность объединения двух методов рассмотрена на примере расчета жесткостных характеристик упругого подвеса камертонного микромеханического гироскопа. Для сравнения проведено моделирование в пакете конечно-элементного анализа. Результаты совпали с достаточной точностью. Таким образом доказана возможность интеграции метода узловой конденсации и пакета прикладных программ конечно-элементного анализа

    Molecular epidemiology and prevalence of mutations conferring rifampicin and isoniazid resistance in Mycobacterium tuberculosis strains from the southern Ukraine

    Understanding the molecular epidemiology of tuberculosis (TB) and mutations in genes associated with drug resistance may contribute to the development of appropriate interventions to improve tuberculosis control. A structured questionnaire was used to collect basic epidemiological data from 589 patients with radiologically confirmed TB in the Odessa and Nikolaev regions of the Ukraine in 2003–2004. A non-commercial reverse hybridisation assay and DNA sequencing were used to detect mutations associated with rifampicin and isoniazid resistance. Genotyping was performed using multilocus variable number tandem repeat (VNTR) typing and spoligotyping. Mutations conferring rifampicin and isoniazid resistance were detected in 32.9% and 44.0%, respectively, of 225 Mycobacterium tuberculosis isolates from individual consecutive patients. Mutations in codon 531 and codon 315 of the rpoB and katG genes, respectively, were predominant among drug-resistant isolates. Multidrug (MDR) resistance rates were significantly higher among former prison inmates compared with non-prisoners (54.8% vs. 27.3%; RR 2.01; 95% CI 1.35–2.97) and the prevalence of mutations was higher in Beijing strains sharing the VNTR signature 223325173533424 than in other Beijing strains (71.4% vs. 45.7%; RR 1.74; 95% CI 1.17–2.57), suggesting that this group may be responsible for rapid transmission of MDR TB in the southern Ukraine

    Objective: to analyze trends in the incidence and prevalence of major rheumatic diseases (RDs) among the adult population of Russia in 2013 to 2017.Material and methods. The trends in the incidence and prevalence of major RDs were analyzed among the adult population of Russia over 5 years (in 2013 to 2017) according to the statistical reports of the Ministry of Health of Russia, which are based on the data of accounting form No. 12 “Information on the number of diseases recorded in patients living in the region served by a health care facility”. The analysis was carried out in eight Federal Districts (FDs).Results and discussion. Over 5 years, there was a slight increase in the number of patients (by 21,677) with musculoskeletal diseases (MSDs) among the adult population of Russia and a small (0.64%) decrease in their prevalence. Noninflammatory osteoarticular diseases, such as osteopathy, chondropathy, deforming dorsopathies, and arthropathies (74%), were leading among MSDs. The incidence of osteoarthritis (OA) was highest: more than 4 million people, which is a quarter of all RDs. The increase in the number of patients with OA was observed annually, but was insignificant for 5 years – 190.400 people; the prevalence of OA increased by 3.7%. The largest number of patients with OA was registered in the Central (1.1 million) and Volga (0.9 million) FDs.The incidence of rheumatoid arthritis (RA) increased insignificantly in the Central, Northwestern, Southern, Ural, and Siberian FDs and decreased in the Volga and Far Eastern FDs. There was a substantial (21%) decrease in the prevalence of RA in the North Caucasian FD. There was a significant increase in both the incidence and prevalence of spondylopathy (SP) in almost all FDs, except for the Volga FD. In contrast, these indicators for reactive arthritis (ReA) had decreased in five FDs by 2017 and slightly increased in three FDs. These indicators fluctuated in almost all FDs for 5 years.The incidence and prevalence of psoriatic arthritis (PsA) showed a annual increase particular in the North Caucasian FD by 29%. The incidence of PsA slightly decreased only in the Siberian FD. The number of patients with systemic connective tissue diseases (SCTD) increased insignificantly in all FDs, with the exception of the North Caucasian FD, in which the prevalence of SCTD reduced dramatically (by 42%). The incidence of osteoporosis (OP) fluctuated during 5 years; by 2017, there had been its rise in five FDs, especially in the Northwestern FD. The prevalence of OP was found to increase significantly (by 33.5%) in the North Caucasian FD.Evaluation of the age composition of patients showed that OA and OP were more common in the older age group; and ReA; SP and SCTD were in the younger age one. There were no differences in the age composition of patients with RA.Conclusion. The analysis of the statistical incidence and prevalence rates of RDs in the adult population of Russia suggests that there is an increase in the number of patients with this pathology throughout the country.Цель исследования – анализ динамики заболеваемости основными ревматическими заболеваниями (РЗ) и их распространенности среди взрослого населения России с 2013 по 2017 г.Материал и методы. Динамика заболеваемости основными РЗ и их распространенности среди взрослого населения России проанализирована за 5 лет (с 2013 по 2017 г.) по статистическим отчетам Минздрава России, которые основаны на данных учетной формы №12 «Сведения о числе заболеваний, зарегистрированных у пациентов, проживающих в районе обслуживания медицинской организации». Анализ проведен по восьми федеральным округам (ФО).Результаты и обсуждение. За 5 лет наблюдались незначительный (на 21 677) прирост числа больных с болезнями костно-мышечной системы (БКМС) среди взрослого населения России и небольшое снижение их распространенности (0,64%). Среди БКМС лидировали невоспалительные заболевания костно-суставной системы – остеопатии, хондропатии, деформирующие дорсопатии, артропатии (74%). Самой высокой была заболеваемость остеоартритом (ОА) – более 4 млн человек, что составляет четверть всех РЗ. Прирост больных ОА наблюдался ежегодно, но был незначительным за 5 лет – 190,4 тыс., распространенность ОА увеличилась на 3,7%. Самое большое число больных ОА зарегистрировано в Центральном (1,1 млн) и Приволжском (0,9 млн) ФО.Заболеваемость ревматоидным артритом (РА) несущественно увеличилась в Центральном, Северо-Западном, Южном, Уральском, Сибирском ФО и снизилась в Приволжском и Дальневосточном ФО. Существенное уменьшение распространенности РА (на 21%) произошло в Северо-Кавказском ФО. Отмечено значительное возрастание как заболеваемости спондилопатиями (СП), так и их распространенности практически во всех ФО, кроме Приволжского. Эти показатели для реактивного артрита (РеА) к 2017 г., напротив, уменьшились в пяти ФО и несколько повысились в трех ФО. Данные показатели колебались практически во всех ФО в течение 5 лет.Выявлен ежегодный прирост заболеваемости псориатическим артритом (ПсА) и его распространенности, особенно в Северо-Кавказском ФО (на 29%). Незначительное снижение заболеваемости ПсА имело место только в Сибирском ФО. Число больных с системными заболеваниями соединительной ткани (СЗСТ) несущественно увеличилось во всех ФО, за исключением Северо-Кавказского ФО, в котором распространенность СЗСТ резко снизилась – на 42%. Заболеваемость остеопорозом (ОП) колебалась на протяжении 5 лет, к 2017 г. отмечено ее повышение в пяти ФО, особенно в Северо-Западном ФО. Значительное увеличение распространенности ОП (на 33,5%) выявлено в Северо-Кавказском ФО.Оценка возрастного состава пациентов показала, что ОА, ОП чаще встречаются в старшей возрастной группе, а РеА, СП, СЗСТ – у пациентов более молодого возраста. Различий в возрастном составе больных РА не отмечено.Выводы. Анализ статистических показателей заболеваемости РЗ и их распространенности у взрослого населения России свидетельствует о нарастании в целом по стране числа больных с этой патологией
