    Depression and alcohol use among the Dutch residential home elderly: Is there a shared vulnerability?

    The purpose of this article is to investigate whether data from an older population sample would support the co-occurrence between depression and (problematic) alcohol use found in the general population and in clinical samples. Additionally, important predictors concerning these phenomena are identified in this population, by interviewing 156 inhabitants of five residential homes (mean age 84 years), using several questionnaires. The results showed that there is no link present between depression and alcohol use in this very old, mostly female population. Our results found a relation between the personality traits extraversion and openness to experience with both depression as well as alcohol use. Neuroticism was only related to depressive symptoms. Chronic diseases was related to non-alcohol use and parental problem drinking was found to be a risk factor for late life problem drinking. Future studies should aim at developing screening instruments for alcohol use in this population and, because of the importance of the personality traits, aim at developing or adapting of psychotherapeutic interventions fit for this population.     Onion Fusarium Basal Rot Disease Control by Arbuscular Mycorrhizal Fungi and Trichoderma harzianum

    Soilborne pathogens reduce 60% of the yield of onion crops. A common fungal pathogen causing wilt disease and severe losses is Fusarium basal rot (FBR). In this study, the combination of Arbuscular Mycorrhizal Fungi (AMF) with Trichoderma harzianum was investigated against FBR. Onion samples were collected from the Ankara–Polatlı region. Among the isolates, isolate S6 was identified as F. oxysporum f. sp. cepae (FOC) using morphological and molecular methods and pathogenicity tests. Different combinations of AMF (Funneliformis mosseae pure strain and the commercial AMF) and T. harzianum were inoculated on susceptible onion cultivars (Seç, Gence, and Şampiyon). The effects of the treatments on FOC biocontrol were studied under growth chamber conditions. The results showed that Şampiyon was the most resistant, while Gence was the most susceptible to basal rot disease. Different colonization rates (8.91–24%), spore densities (16.4–50.4 spore/10 g soil), and the extent to which a plant needs mycorrhizal conditions to grow to its maximum potential (i.e., mycorrhizal dependencies—18.3–51.9%) were recorded by treatment. Both single and combined applications of AMF and Trichoderma applications suppressed FOC. Suppressive effects were more pronounced when the F. mosseae pure strain was used alone (when F. mosseae was used, disease severity decreased from 90 to 68%, p < 0.05). The F. mosseae pure strain also showed the best plant growth promotion and phosphorus content release.     Genomic and Phenotypic Characterization of Clostridium botulinum Isolates from an Infant Botulism Case Suggests Adaptation Signatures to the Gut

    In early life, the immature human gut microbiota is prone to colonization by pathogens that are usually outcompeted by mature microbiota in the adult gut. Colonization and neurotoxin production by a vegetative Clostridium botulinum culture in the gut of an infant can lead to flaccid paralysis, resulting in a clinical outcome known as infant botulism, a potentially life-threatening condition. Beside host factors, little is known of the ecology, colonization, and adaptation of C. botulinum to the gut environment. In our previous report, an infant with intestinal botulism was shown to be colonized by neurotoxigenic C. botulinum culture for 7 months. In an effort to gain ecological and evolutionary insights into this unusually long gut colonization by C. botulinum, we analyzed and compared the genomes of C. botulinum isolates recovered from the infant feces during the course of intoxication and isolates from the infant household dust. A number of observed mutations and genomic alterations pinpointed at phenotypic traits that may have promoted colonization and adaptation to the gut environment and to the host. These traits include motility, quorum-sensing, sporulation, and carbohydrate metabolism. We provide novel perspectives and suggest a tentative model of the pathogenesis of C. botulinum in infant botulism. IMPORTANCE While the clinical aspects of infant botulism and the mode of action of BoNT have been thoroughly investigated, little is known on the pathogenesis and adaptive mechanisms of C. botulinum in the gut. Here, we provide for the first time a comprehensive view on the genomic dynamics and plasticity of C. botulinum over time in a case of infant botulism. The genomic and phenotypic analysis of C. botulinum isolates collected during the disease course offers an unprecedented view of C. botulinum ecology, evolution, and pathogenesis and may be instrumental in developing novel strategies for prevention and treatment of toxicoinfectious botulism. While the clinical aspects of infant botulism and the mode of action of BoNT have been thoroughly investigated, little is known on the pathogenesis and adaptive mechanisms of C. botulinum in the gut.     Vitality, Language Use, and Life Satisfaction : A Study of Bilingual Hungarian Adolescents Living in Romania

    This study examined the relationship between objective and subjective vitality, in-group language use, and life satisfaction among two groups of bilingual Hungarians adolescents living in Romania: a low objective vitality group from Cluj-Napoca/Kolozsvar, where Hungarians are the demographic minority, and a high objective vitality group from Sfantu Gheorghe/Sepsiszentgyorgy, where Hungarians are the demographic majority. Consistent with predictions, the high objective vitality group reported higher subjective Hungarian vitality, lower subjective Romanian vitality, more frequent use of the Hungarian language, and higher life satisfaction, compared with the low objective vitality group. The effects of objective vitality on language use were partially mediated by subjective Romanian (but not Hungarian) vitality. Conversely, the effects of objective vitality on life satisfaction were fully mediated by subjective Hungarian (but not Romanian) vitality.Peer reviewe

    Client's Experiences of Online Counseling: Satisfaction and Therapeutic Alliance

    Online counseling has been increasingly becoming widespread. Nevertheless, the number of studies investigating counselees' online counseling experience in-depth is scarce, especially in Turkey, despite the proliferation of the service. Therefore, this phenomenological study aims to understand the experiences of participants who received online counseling regarding satisfaction and therapeutic alliance. For this purpose, semi-structured interviews were conducted with six participants. Thematic analysis was applied to explore their satisfaction with online counseling and therapeutic alliance. The results showed that three themes emerged for satisfaction with online counseling; improvements in the symptoms, the flexibility of the online counseling, and limitations of online counseling. Thematic analysis of therapeutic alliance revealed the themes; goals, tasks, and bond. As a result, the participants reported high satisfaction with the online sessions, and they were able to establish a therapeutic alliance with their counselor in setting goals, seeking solutions to problems, and bonding.     Performance in adults of the EULAR/PRINTO/PRES (Ankara 2008) classification criteria for IgA vasculitis

    Objective: To examine the performance in adults of the European Alliance of Associations for Rheumatology (EULAR)/Pediatric Rheumatology European Society (PReS)-endorsed Ankara 2008 classification criteria for IgA vasculitis (IgAV). Methods: The EULAR/PReS/Ankara 2008 classification criteria for IgAV were applied to patients enrolled in an international observational cohort which included patients with IgAV and comparators with other forms of small-vessel and medium-vessel vasculitis. After the initial assessment of the performance of the criteria, possible revisions to increase the performance were tested. The revised criteria were then assessed in an independent validation cohort within a multicentre Turkish vasculitis registry. Results: The dataset consisted of 178 IgAV cases and 1705 comparators. The Ankara 2008 criteria require skin involvement plus one of the following four criteria: abdominal pain, a biopsy showing IgA deposition, arthritis or arthralgia, or renal involvement (any haematuria and/or proteinuria). The specificity of the criteria improved when a positive test for anti-neutrophil cytoplasmic autoantibody or blood cryoglobulins was considered an exclusion criterion. The revised criteria had a sensitivity of 76.4% (95% CI 69.8% to 82.2%) and a specificity of 94.5% (95.0% CI 93.4% to 95.1%). In the validation set, the sensitivity and specificity of the revised criteria were 97.8% (95% CI 94.0% to 99.0%) and 85.0% (95.0% CI 78.0% to 90.0%), respectively. Conclusion: The revised EULAR/PReS-endorsed Ankara 2008 IgAV classification criteria perform well in adults with IgAV and are appropriate for use in clinical research

    Considerations for the use of plasma cytokeratin 18 as a biomarker in pancreatic cancer

    Background:Enzyme-linked immunoassays of full-length (M65) and/or caspase-cleaved (M30) cytokeratin 18 (CK18) released from epithelial cells undergoing necrosis and/or apoptosis, respectively, may have prognostic or predictive biomarker utility in a range of solid tumour types. Characterisation of baseline levels of circulating full length and cleaved CK18 specifically in patients with pancreatic cancer.Methods:Plasma samples from 103 patients with pancreatic cancer stored at 80 °C were assayed for M65 and M30 levels. The median (inter-quartile range (IQR)) duration of plasma storage was 34 (23-57) months. Patients with metastatic disease (n19) were found to have greater median (IQR) M65 levels (1145 (739-1698) U l 1) compared with the locally advanced (n20; 748 (406-1150) U l 1) and resected (n64; 612 (331-987) U l 1) patients (P0.002). Elevated M65 levels were associated with poorer overall survival on univariate (P0.001) but not multivariate (P0.202) analysis. M65 concentrations also exhibited significant associations with concurrent serum-bilirubin levels (P0.001) and the duration of plasma storage (P0.001).Conclusions:Baseline plasma CK18 levels in pancreatic cancer are affected by the presence of obstructive jaundice and prolonged plasma storage. Clinical biomarker studies utilising serial CK18 levels are warranted in pancreatic cancer, provided consideration is given to these potentially confounding factors. © 2010 Cancer Research UK
