Evaluation Criteria of the Applications for Public Funding and Their Role in Increasing Innovative Potential of Enterprises: The Case of the Opolskie Voivodeship Regional Operational Programme 2014–2020
The aim of the study is to assess the innovative potential of the selected companies in Opolskie Voivodeship through comparing and contrasting meeting by them criteria used for the evaluation of projects of the Opolskie Voivodeship Regional Operational Programme (RPO WO) 2014–2020. The study process is focused around the following research questions: (1) What criteria are used for evaluation of projects in theOpolskie Voivodeship Regional Operational Programme (RPO WO) 2014–2020? (2) How has the innovation level of project outcomes i.e. products/ services/ technologies changed? (3) What is the level of cooperation with the R&D sector in the evaluated companies? (4) Are there any changes observed in regard to developing by companies their in-house R&D capabilities?
Guidelines from the Polish Surgical Society and Polish Society of Oncological Surgery Concerning Quality Assurance for Centres Performing Cytoreductive Procedures and HIPEC Procedures in the Treatment of Primary and Secondary Peritoneal Tumours
Surgical treatment of patients with peritoneal metastases in combination with Hyperthermic intraperitoneal Chemotherapy (HIPEC) and systemic treatments is applied with increasing frequency and, with correct patient qualification, allows for obtaining 5-year survival at a level of 32–52%. The conditions necessary for positive results of such treatment include the high experience of a given centre, its appropriate infrastructure, and appropriate patient qualification for the procedure. As a result of the debate connected with the need to evaluate treatment quality and results, at the request of the Peritoneal Cancer Section of the Polish Society of Oncological Surgery, the conditions for quality assurance were worked out and a Quality Assurance Commission was set up for the centres performing cytoreductive procedures and HIPEC procedures in the treatment of primary and secondary peritoneal tumours
Soft tissue sarcoma - diagnosis and treatment
W ciągu ostatnich kilkunastu lat osiągnięto znaczący postęp w leczeniu mięsaków tkanek miękkich - nie
tylko ogniska pierwotnego, ale również wznów miejscowych i przerzutów. Podstawową metodą jest leczenie
chirurgiczne, a wprowadzenie zasady skojarzonego postępowania, czyli standardowego łączenia
chirurgii z radioterapią i niekiedy chemioterapią oraz rehabilitacji w wyspecjalizowanych ośrodkach znamiennie
zwiększyło szansę na ograniczenie zasięgu operacji, całkowite wyleczenie chorego lub uzyskanie
długoletniego przeżycia. Współczesne leczenie skojarzone oraz stosowanie metod rekonstrukcyjnych
pozwala u większości chorych na mięsaki tkanek miękkich o lokalizacji kończynowych na oszczędzenie
kończyny. W porównaniu z latami 60.-70., kiedy amputację wykonywano u około 50% chorych,
obecnie w ośrodkach referencyjnych odsetek ten nie przekracza 10%. Ogólnie obserwuje się powolny,
ale stały wzrost odsetka chorych na mięsaki tkanek miękkich z wieloletnimi przeżyciami. Wskaźnik
5-letnich przeżyć całkowitych chorych na mięsaki tkanek miękkich zlokalizowane w obrębie kończyn wynosi
obecnie 55-78%. W przypadku rozpoznania przerzutów długość okresu przeżycia jest jednak nadal
zła i wynosi średnio około 1 roku.
Korzystnych wyników miejscowych leczenia mięsaków można spodziewać się tylko w przypadku planowego
(tzn. po wcześniejszej biopsji) całkowitego wycięcia mięsaka w mikroskopowo wolnych od nowotworu
granicach (tzw. resekcja R0). Po przeprowadzeniu prawidłowego postępowania diagnostycznego
u większości chorych poddawanych radykalnej operacji konieczne jest zastosowanie uzupełniającej radioterapii,
wielotygodniowej rehabilitacji i kontynuowanie badań kontrolnych w tym samym ośrodku przez
minimum 5 lat.
Postęp wiąże się z wprowadzeniem do praktyki klinicznej leków o celowanym działaniu związanym
z molekularnymi lub genetycznymi zaburzeniami w komórkach, które uczestniczą w etiopatogenezie tych
nowotworów. Ze względu na rzadkie występowanie mięsaków oraz konieczność skojarzonego postępowania
najważniejsze jest prowadzenie diagnostyki i terapii już od początku w ramach wielodyscyplinarnego
zespołu w ośrodku onkologicznym o odpowiednim doświadczeniu i zapleczu diagnostycznym.Significant progress in the treatment of soft tissue sarcoma (STS), both primary tumor and local recurrences/
metastatic disease, has been achieved in recent years. Surgery is essential modality, but the use of
combined treatment (standard combination of surgery with adjuvant radiotherapy, chemotherapy in selected
cases and perioperative rehabilitation) in highly-experienced centers increased possibility of cure
and limitations of extent of local surgery. Current combined therapy together with the use of reconstructive
methods allows for limb-sparing surgery in majority of soft tissue sarcoma patients (amputation in 10% of
cases as compared to approximately 50% in the 1960-70s). The slow, but constant, increase of rate of soft
tissue sarcoma patients with long-term survival has been observed. Contemporary 5-year overall survival
rate in patients with extremity soft tissue sarcomas is 55-78%. In case of diagnosis of metastatic disease
the prognosis is still poor (survival of approximately 1 year).
Good results of local therapy may be expected only after planned (e.g., after preoperative biopsy - tru-cut
or incisional) radical surgical excision of primary tumor with pathologically negative margins (R0 resection).
Following appropriate diagnostic check-up, adjuvant radiotherapy is necessary in the majority of
patients treated with radical surgery need, as well as long-term rehabilitation and follow-up examinations
in treating center are needed for at least 5 years.
The progress is due to the introductionof targeted therapy acting on molecular or genetic cellular disturbances
detected during studies on etiopathogenetic mechanisms of sarcoma subtypes. In view of rarity of
sarcomas and necessity of multidisciplinary therapy, the crucial issue is that management of these tumors
should be hold in experienced oncological sarcoma centers
Recommendations for diagnostics and therapy of gastrointestinal stromal tumors (GIST) in 2008
Nowotwory podścieliskowe przewodu pokarmowego (gastrointestinal stromal tumors – GIST) są najczęstszymi nowotworami
pochodzenia mezenchymalnego w obrębie przewodu pokarmowego. Nowotwory te, dzięki postępom w diagnostyce
patologiczno-molekularnej, powszechnie rozpoznawane są dopiero od kilku lat. W pracy przedstawiono zalecenia dotyczące
diagnostyki i terapii tych nowotworów, opracowane na podstawie danych naukowych oraz doświadczenia ekspertów,
które są powszechnie akceptowane i warte rekomendacji. Nadekspresja błonowego receptora KIT, będąca następstwem
mutacji protoonkogenu KIT, jest wysoce specyficzna dla GIST, i możliwa do wykrycia metodami immunohistochemicznymi
(CD117) w preparatach histopatologicznych z guza, stanowiąc najważniejsze kryterium w jego diagnostyce mikroskopowej
i wskazaniach do leczenia drobnocząsteczkowymi inhibitorami kinaz tyrozynowych. W każdym przypadku zalecane jest
przesłanie materiału do badań molekularnych (w celu analizy mutacji KIT i PDGFRA). Radykalne leczenie operacyjne jest nadal najskuteczniejszą metodą leczenia pierwotnych GIST, jednakże nowotwór ten cechuje duży potencjał nawrotowości.
W przypadkach zmian nieoperacyjnych/rozsianych leczeniem z wyboru jest zastosowanie inhibitora kinaz tyrozynowych –
imatinibu, leku, który stanowi pierwsze efektywne leczenie systemowe w zaawansowanym GIST CD117(+). Zalecana dawka
początkowa powinna wynosić 400 mg raz dziennie (800 mg dziennie w przypadku mutacji w eksonie 9 KIT). Monitorowanie
leczenia musi opierać się na powtarzanym badaniu tomografii komputerowej jamy brzusznej, z uwzględnieniem zmian
wielkości i gęstości. W przypadku progresji zaleca się zwiększenie dawki imatinibu do 800 mg na dobę, a w przypadku
braku skuteczności zastosowanie sunitynibu w dawce początkowej 50 mg dziennie. Obecnie trwają badania kliniczne nad
ustaleniem roli leczenia chirurgicznego w skojarzeniu z imatinibem oraz skuteczności innych leków celowanych molekularnie
w przypadku występowania oporności w czasie leczenia imatinibem. Dostępne dane odnośnie leczenia uzupełniającego
wskazują na poprawę przeżyć wolnych od nawrotu, ale wpływ tej strategii na przeżycia całkowite i optymalna długość czasu
leczenia są nadal przedmiotem badań. Przedstawione zasady postępowania diagnostyczno-terapeutycznego powinny być
praktycznie wprowadzane przez lekarzy zajmujących się chorymi na GIST w Polsce. Zaleca się rejestrowanie przypadków
chorych na GIST w ramach narodowego Rejestru Klinicznego ([email protected]) oraz standardowe leczenie chorych
w wielodyscyplinarnych zespołach, mających doświadczenie w terapii GIST i włączanie nowych przypadków GIST do

in the understanding of the molecular mechanisms of GIST pathogenesis have resulted for last years in the emerging of
GIST as a distinct sarcoma entity. The paper describes guidelines for diagnostics and therapy of these tumors based on
scientific basis and experts’ experience, which are commonly accepted and worth to recommend. Overexpression of KIT
receptor, as a consequence of mutation of protooncogene KIT, is highly specific for GIST and enable for detection by
immunohistochemistry staining (CD117) in tumor specimens. It is the most important criterion in microscopic diagnostics
and indications for treatment with small-molecule tyrosine kinas inhibitors. Sending material for molecular analysis is
strongly recommended (for KIT and PDGFRA genotyping). Radical surgery is still the mainstay treatment for primary,
localized, resectable GISTs, although high percentages of the patients after potentially curative operations develop recurrent
or metastatic disease. In inoperable/metastatic lesions the treatment of choice is tyrosine kinase inhibitor – imatinib mesylate
– the first effective systemic therapy in advanced CD117(+) GIST. Recommended initial dose should be 400 mg daily
(800 mg for exon 9 KIT mutants). Monitoring of the therapy should be based on serial computed tomography imaging of
abdominal cavity with the assessment of changes of tumor size and density. In case of disease progression the increase of
imatinib dose to 800 mg daily is recommended and if further progression exists – sunitinib in the initial dose 50 mg daily
should be introduced. Clinical trials evaluating the role of surgery in combination of imatinib and the efficacy of other
molecular targeted drugs in resistant cases are ongoing. Existing data indicate beneficial role of adjuvant imatinib therapy
in terms of relapse-free survival, but the impact of this policy on overall survival, optimal duration of adjuvant therapy and
criteria for its implementation are currently investigated. Presented recommendations for diagnostics and therapy of GIST
should be practically implemented by physicians involved in management of GIST patients in Poland. The including GIST
cases in national Clinical Registry ([email protected]) and standard treatment of patients in multidisciplinary team with
expertise in GIST therapy, as well as enrollment of new cases to prospective clinical trials, are recommended
