    Drug-related mutational patterns in hepatitis B virus (HBV) reverse transcriptase proteins from Iranian treatment-Naïve chronic HBV patients

    Background: Immunomodulators and Nucleotide analogues have been used globally for the dealing of chronic hepatitis B virus (HBV) infection. However, the development of drug resistance is a major limitation to their long-term effectiveness. Objectives: The aim of this study was to characterize the hepatitis B virus reverse transcriptase (RT) protein variations among Iranian chronic HBV carriers who did not receive any antiviral treatments. Materials and Methods: Hepatitis B virus partial RT genes from 325 chronic in active carrier patients were amplified and directly sequenced. Nucleotide/amino acid substitutions were identified compared to the sequences obtained from the database. Results: All strains belonging to genotype D.365 amino-acid substitutions were found. Mutations related to lamivudine, adefovir, telbivudine, and entecavir occurred in (YMDD) 4% (n = 13), (SVQ) 17.23% (n = 56), (M204I/V + L180M) 2.45% (n = 8) and (M204I) 2.76% (n = 9) of patients, respectively. Conclusions: RT mutants do occur naturally and could be found in HBV carriers who have never received antiviral therapy. However, mutations related to drug resistance in Iranian treatment-naïve chronic HBV patients were found to be higher than other studies published formerly. Chronic HBV patients should be monitored closely prior the commencement of therapy to achieve the best regimen option. © 2013, KOWSAR Corp

    Assessment of Trinidad community stakeholder perspectives on the use of yeast interfering RNA-baited ovitraps for biorational control of Aedes mosquitoes

    Dengue, Zika, chikungunya and yellow fever viruses continue to be a major public health burden. Aedes mosquitoes, the primary vectors responsible for transmitting these viral pathogens, continue to flourish due to local challenges in vector control management. Yeast interfering RNA-baited larval lethal ovitraps are being developed as a novel biorational control tool for Aedes mosquitoes. This intervention circumvents increasing issues with insecticide resistance and poses no known threat to non-target organisms. In an effort to create public awareness of this alternative vector control strategy, gain stakeholder feedback regarding product design and acceptance of the new intervention, and build capacity for its potential integration into existing mosquito control programs, this investigation pursued community stakeholder engagement activities, which were undertaken in Trinidad and Tobago. Three forms of assessment, including paper surveys, community forums, and household interviews, were used with the goal of evaluating local community stakeholders' knowledge of mosquitoes, vector control practices, and perceptions of the new technology. These activities facilitated evaluation of the hypothesis that the ovitraps would be broadly accepted by community stakeholders as a means of biorational control for Aedes mosquitoes. A comparison of the types of stakeholder input communicated through use of the three assessment tools highlighted the utility and merit of using each tool for assessing new global health interventions. Most study participants reported a general willingness to purchase an ovitrap on condition that it would be affordable and safe for human health and the environment. Stakeholders provided valuable input on product design, distribution, and operation. A need for educational campaigns that provide a mechanism for educating stakeholders about vector ecology and management was highlighted. The results of the investigation, which are likely applicable to many other Caribbean nations and other countries with heavy arboviral disease burdens, were supportive of supplementation of existing vector control strategies through the use of the yeast RNAi-based ovitraps

    The prevalence and genotype distribution of adenovirus respiratory infection among Iranian Hajj pilgrims in 2015

    Aim: Respiratory viruses are the most common cause of hospitalization of pilgrims during Hajj. This study aimed to detect adenovirus genotypes among Iranian pilgrims with acute respiratory tract infection in Hajj season of 2015. Materials & methods: In this cross-sectional study, throat swab samples of 338 Iranian pilgrims with flu-like symptoms were screened for adenovirus by polymerase chain reaction, and virus genotypes were determined using sequencing method. Results: Seventy-seven cases were positive for adenovirus (22.78). The adenovirus type 6 was the most frequently detected virus followed by type 5, 1, 2 and 57. Conclusion: The prevalence of adenovirus respiratory infection among Iranian pilgrims is considerable. Preparation and administration of an effective adenovirus vaccine is recommended. © 2020 Future Medicine Ltd

    Impact of human rhinoviruses on gene expression in pediatric patients with severe acute respiratory infection

    Human rhinovirus (HRV) is one of the most common viruses, causing mild to severe respiratory tract infections in children and adults. Moreover, it can lead to patients� hospitalization. Nowadays, evaluation of gene expression alterations in host cells due to viral respiratory infections considered essential to understand the viral effects on cells. Objective: In this study, we aimed to find important differentially expressed genes (DEGs) related to rhinitis and asthma exacerbation stimulated with Poly (I: C) and then to validate their expression in clinical samples of children how were less than 5 years old, hospitalized with severe acute respiratory infection (SARI) due to HRV infection in comparison with healthy cases. Methods: Eight candidate genes involved in immunity, viral defense, inflammation, P53 pathway, and viral release processes were selected based on the analysis of a gene expression data set (GSE51392) and gene enrichment analysis. Then quantitative real-time PCR on cDNAs was performed for selected genes. The results were analyzed by Livak method and visualized by GraphPad prism software (8.4.3). Result: CXCL10, CMPK2, RSAD2, SERPINA3, TNFAIP6, CXCL14, IVNS1AB, and ZMAT3 were selected based on the enrichment and topological analysis of the constructed protein-protein interaction (PPI) network. Laboratory validation by real-time PCR showed CXCL10, CMPK2, RSAD2, SERPINA3, and TNFAIP6 (belonged to immunity, inflammatory responses and viral defense) were up-regulated, whereas CXCL14 (related to immunity) and IVNS1AB, ZMAT3 (associated to Influenza and P53 pathway) were down-regulated. Conclusion: Our results showed, that in children less than 5 years old affected by HRV and hospitalized with SARI, the inflammatory responses, antiviral defense, and type 1 interferon-signaling pathway have significantly affected by viral infection. © 202

    Dietary intake, physical activities and nutritional status of adolescent girls in an urban population of Bangladesh

    In Bangladesh, under-nutrition is a common health problem, but for socio-cultural background, it is most predominant among the female population starting from their early life to motherhood. For the adolescent girls, there has been no such study though they will be the future mothers. Therefore, this study is designed to address the lifestyle and nutrition of the Bangladeshi female adolescents. The study was conducted purposively in Dhaka selecting randomly 15 of 95 City corporation wards of Dhaka City. All adolescent girls aged 10–18 years were considered eligible participants of an urban population of Bangladesh. The study included socio-demographic information, clinical examination, dietary intake, physical activities and body mass index (BMI = weight in kg / height in m. sq.). Overall, 352 adolescent girls volunteered. Socio-economically, 51% of them had monthly family income ³ 20,000 BDT and 11.4% had <10,000 BDT. Of the participants, 14.8% had BMI <18.5, 80.7% had 18.5 – 24.9, and 4.6% had ³ 25. BMI was found not to have significant association with physical activities. No clinical signs of vitamin A deficiency were observed. On clinical examination 75% of the participants were found healthy, 15.9% had anemia and 5.7% had diarrhea. Compared with the national dietary intake, the cereal intake was lower but protein containing foods like pulse and nuts, meat, egg, fish, milk and milk products were found very much close to the national intake. On the average, 95 % of calorie, 93.5 % of protein and 96.5 % of fat requirement were met. For micronutrient requirement, very low intake was observed with calcium (62 %) and iron (63 %). In conclusion, the participants consumed rice daily with frequent consumption of vegetables. Although the study subjects were mostly from higher class of urban dwellers their dietary intake was found not healthy as evidenced by daily rice intake and very low intake of fruits, calcium and iron indicating lack of awareness regarding food habit. Further study is needed to confirm the study findings and to initiate health education on diet among the Bangladeshi adolescent girls. Ibrahim Med. Coll. J. 2010; 4(2): 78-8

    Death in hospital following ICU discharge : insights from the LUNG SAFE study

Methods: The Large observational study to UNderstand the Global impact of Severe Acute respiratory FailurE study was an international, multicenter, prospective cohort study of patients with severe respiratory failure, conducted across 459 ICUs from 50 countries globally. This study aimed to understand the frequency and factors associated with death in hospital in patients who survived their ICU stay. We examined outcomes in the subpopulation discharged with no limitations of life sustaining treatments ('treatment limitations'), and the subpopulations with treatment limitations. Results: 2186 (94%) patients with no treatment limitations discharged from ICU survived, while 142 (6%) died in hospital. 118 (61%) of patients with treatment limitations survived while 77 (39%) patients died in hospital. Patients without treatment limitations that died in hospital after ICU discharge were older, more likely to have COPD, immunocompromise or chronic renal failure, less likely to have trauma as a risk factor for ARDS. Patients that died post ICU discharge were less likely to receive neuromuscular blockade, or to receive any adjunctive measure, and had a higher pre- ICU discharge non-pulmonary SOFA score. A similar pattern was seen in patients with treatment limitations that died in hospital following ICU discharge. Conclusions: A significant proportion of patients die in hospital following discharge from ICU, with higher mortality in patients with limitations of life-sustaining treatments in place. Non-survivors had higher systemic illness severity scores at ICU discharge than survivors. Trial Registration: ClinicalTrials.gov NCT02010073
