    Crimean-Congo haemorrhagic fever virus in Kazakhstan (1948-2013)

    AbstractCrimean-Congo haemorrhagic fever (CCHF) is a pathogenic and often fatal arboviral disease with a distribution spanning large areas of Africa, Europe and Asia. The causative agent is a negative-sense single-stranded RNA virus classified within the Nairovirus genus of the Bunyaviridae family.Cases of CCHF have been officially recorded in Kazakhstan since the disease was first officially reported in modern medicine. Serological surveillance of human and animal populations provide evidence that the virus was perpetually circulating in a local enzoonotic cycle involving mammals, ticks and humans in the southern regions of the country. Most cases of human disease were associated with agricultural professions such as farming, shepherding and fruit-picking; the typical route of infection was via tick-bite although several cases of contact transmission associated with caring for sick patients have been documented.In total, 704 confirmed human cases of CCHF have been registered in Kazakhstan from 1948-2013 with an overall case fatality rate of 14.8% for cases with a documented outcome.The southern regions of Kazakhstan should be considered endemic for CCHF with cases reported from these territories on an annual basis. Modern diagnostic technologies allow for rapid clinical diagnosis and for surveillance studies to monitor for potential expansion in known risk areas

    Collaboration and modelling – tools for integration in the Motueka catchment, New Zealand

    A conceptual model of integrated catchment management (ICM) is presented in which ICM is defined as a process to achieve both ecosystem resilience and community resilience. It requires not only biophysical knowledge developed by hydrologists and other environmental scientists, but an active partnership with catchment communities and stakeholders to break the ‘paradigm lock’ described by the UNESCO-HELP programme.This paper reports observations from ICM research in the Motueka HELP demonstration basin in the upper South Island of New Zealand. The Motueka occupies 2 170 km2 of land yet the river effects are felt on the seabed more than 50 km2 offshore, so the true ‘catchment’ is larger. A hydrologically temperate mountainous catchment with horticultural, agricultural, plantation forestry and conservation land uses, the Motueka also hosts an internationally recognised brown trout fishery. Land and water management issues driving ICM research include water allocation conflicts between instream and irrigation water uses, impacts on water quality of runoff from intensifying land uses, catchment impacts on coastal productivity and aquaculture, and how to manage catchment processes in an integrated way that addresses cumulative effects of development.Collaboration with catchment stakeholders can be viewed as having two primary purposes:• Building knowledge and commitment of resource users towards sustainable resource management (collaborative learning)• Stakeholder involvement in resource management itself (governance).Examples are presented of a Collaborative Learning Group on Sediment learning of their differing perspectives on fine sediment impacts, and a Catchment Landcare Group working with scientists to improve water quality in their river. Success factors for water user committees making decisions about water resource management include creating opportunities to communicate and build trust, share scientific knowledge on the issue, and willingness to compromise. Functioning catchment groups have potential to take on delegated governance responsibility for meeting agreed water quality and other community goals.Finally a scenario modelling framework IDEAS (Integrated Dynamic Environmental Assessment System) is presented, in which environmental indicators such as nutrient fluxes are simulated alongside socio-economic indicators such as job numbers and catchment GDP for a range of land and marine use options.Keywords: integrated catchment management (ICM), resilience, HELP, UNESCO, water governance, Landcare, scenario modelling, collaborative learning, water allocation, water user committees, catchment groups, watershed managemen

    Developing Feature Types and Related Catalogues for the Marine Community - Lessons from the MOTIIVE project.

    MOTIIVE (Marine Overlays on Topography for annex II Valuation and Exploitation) is a project funded as a Specific Support Action (SSA) under the European Commission Framework Programme 6 (FP6) Aeronautics and Space Programme. The project started in September 2005 and finished in October 2007. The objective of MOTIIVE was to examine the methodology and cost benefit of using non-proprietary data standards. Specifically it considered the harmonisation requirements between the INSPIRE data component ‘elevation’ (terrestrial, bathymetric and coastal) and INSPIRE marine thematic data for ‘sea regions’, ‘oceanic spatial features’ and ‘coastal zone management areas’. This was examined in context of the requirements for interoperable information systems as required to realise the objectives of GMES for ‘global services’. The work draws particular conclusions on the realisation of Feature Types (ISO 19109) and Feature Type Catalogues (ISO 19110) in this respect.     Black gold: trustworthiness in artistic research (seen from the sidelines of arts and health)

    Rigour plays a central role in contemporary research culture. But how appropriate a concept is it to think, perform, and make judgements with on what is trustworthy and excellent in artistic research and its neighbouring field of arts and health? The historical meanings of rigour suggest severity and rigidity: straight lines, austere habits, privations. As a word, rigour has a mixed ancestry – French, Latin, Middle English. Some of its earliest uses coincide with a feudal system of government in Europe, with rigge [verb] meaning to plough a straight line in a narrow strip, and rig [verb] to provide a straight ridge to a house. Rig [noun] a derivation of ridge, was used in England five hundred years ago of human and animal backbones, perhaps reflecting everyday physical burdens. Rigours [noun] conveyed the meting out of un-cautioned punishments and cruelty. While the temperament of rigour might be appropriate for research that follows pre-set norms and standards of repeatability, its use to judge what is trustworthy in artistic research is questionable. Though artistic researchers need to understand the rigour concept, by contrast, artistic research as a kind of ‘thinking through making’ (Ravetz, 2011, 159; Ingold, 2013, 6), places value on improvisation, chance encounter, unforeseen admixture and the in- and outward- folding of process, affect and material. Once it is accepted that poiesis is part of the research process (Ingold, 2013; Haraway, 2016), it becomes apparent that artistic research cannot easily accommodate straight backed rigour

    The earliest thymic T cell progenitors sustain B cell and myeloid lineage potential

    The stepwise commitment from hematopoietic stem cells in the bone marrow to T lymphocyte-restricted progenitors in the thymus represents a paradigm for understanding the requirement for distinct extrinsic cues during different stages of lineage restriction from multipotent to lineage-restricted progenitors. However, the commitment stage at which progenitors migrate from the bone marrow to the thymus remains unclear. Here we provide functional and molecular evidence at the single-cell level that the earliest progenitors in the neonatal thymus had combined granulocyte-monocyte, T lymphocyte and B lymphocyte lineage potential but not megakaryocyte-erythroid lineage potential. These potentials were identical to those of candidate thymus-seeding progenitors in the bone marrow, which were closely related at the molecular level. Our findings establish the distinct lineage-restriction stage at which the T cell lineage-commitment process transits from the bone marrow to the remote thymus. © 2012 Nature America, Inc.     The Perfect Finance Minister: Whom to Appoint as Finance Minister to Balance the Budget?

    The role and influence of the finance minister within the cabinet are discussed with increasing prominence in the recent theoretical literature on the political economy of budget deficits. It is generally assumed that the spending ministers can raise their reputation purely with new or more extensive expenditure programs, whereas solely the finance minister is interested to balance the budget. Using a dynamic panel model to study the development of public deficits in the German states between 1960 and 2009, we identify several personal characteristics of the finance ministers that significantly influence budgetary performance. Namely her professional background seems to affect budget deficits. During times of fiscal stress, our results can guide prime ministers in the nominating of finance ministers in order to assure sound budgeting

    Ten-year mortality, disease progression, and treatment-related side effects in men with localised prostate cancer from the ProtecT randomised controlled trial according to treatment received

    Background The ProtecT trial reported intention-to-treat analysis of men with localised prostate cancer randomly allocated to active monitoring (AM), radical prostatectomy, and external beam radiotherapy. Objective To report outcomes according to treatment received in men in randomised and treatment choice cohorts. Design, setting, and participants This study focuses on secondary care. Men with clinically localised prostate cancer at one of nine UK centres were invited to participate in the treatment trial comparing AM, radical prostatectomy, and radiotherapy. Intervention Two cohorts included 1643 men who agreed to be randomised and 997 who declined randomisation and chose treatment. Outcome measurements and statistical analysis Analysis was carried out to assess mortality, metastasis and progression and health-related quality of life impacts on urinary, bowel, and sexual function using patient-reported outcome measures. Analysis was based on comparisons between groups defined by treatment received for both randomised and treatment choice cohorts in turn, with pooled estimates of intervention effect obtained using meta-analysis. Differences were estimated with adjustment for known prognostic factors using propensity scores. Results and limitations According to treatment received, more men receiving AM died of PCa (AM 1.85%, surgery 0.67%, radiotherapy 0.73%), whilst this difference remained consistent with chance in the randomised cohort (p = 0.08); stronger evidence was found in the exploratory analyses (randomised plus choice cohort) when AM was compared with the combined radical treatment group (p = 0.003). There was also strong evidence that metastasis (AM 5.6%, surgery 2.4%, radiotherapy 2.7%) and disease progression (AM 20.35%, surgery 5.87%, radiotherapy 6.62%) were more common in the AM group. Compared with AM, there were higher risks of sexual dysfunction (95% at 6 mo) and urinary incontinence (55% at 6 mo) after surgery, and of sexual dysfunction (88% at 6 mo) and bowel dysfunction (5% at 6 mo) after radiotherapy. The key limitations are the potential for bias when comparing groups defined by treatment received and changes in the protocol for AM during the lengthy follow-up required in trials of screen-detected PCa. Conclusions Analyses according to treatment received showed increased rates of disease-related events and lower rates of patient-reported harms in men managed by AM compared with men managed by radical treatment, and stronger evidence of greater PCa mortality in the AM group. Patient summary More than 95 out of every 100 men with low or intermediate risk localised prostate cancer do not die of prostate cancer within 10 yr, irrespective of whether treatment is by means of monitoring, surgery, or radiotherapy. Side effects on sexual and bladder function are better after active monitoring, but the risks of spreading of prostate cancer are more common
