    A study on prescribing pattern and drug utilization among depressed patients in a tertiary care teaching hospital

    Background: Drug utilization is defined as the marketting, distribution, prescribing and use of drugs in society. Various factors like quality, quantity, and cost of drugs plays an important role in selection of drug therapy. Hence we wanted to observe that drug utilization and prescribing pattern of antidepressants.Methods: A prospective observational study were conducted in a psychiatric department at RMMCH for a period of 6 months from November 2019 to April 2020. Sixty patients were enrolled and analysed to study the pattern and the world health organisation (WHO) prescribing indicators.Results: From 60 prescriptions, the most widely prescribed antidepressant drug escitalopram 21 (42%) as a combination drug citalinforte 30 (50%), linotril 21 (42%) lorazepam 12 (20%), haloperidol 6 (10%) and quitipin 3 (5%). Most commonly, depression was seen in age group between 31-40 years and males were more to depression than females. Most drugs were prescribed under brand names (55%) than generic drugs (45%). Monotherapy was the preferred drug pattern for the mild depression followed by dual therapy and triple therapy for moderate and severe depression respectively. The number of drugs which were prescribed from the WHO and national list of essential medicine (NLEM) was haloperidol (10%) and escitalopram (85%) respectively.Conclusions: Escitalopram was the commonly prescribed drug for depression followed by lorazepam, linotril and haloperidol. Monotherapy was most preferred, because of increased medication adherence, low ADRs, minimum cost and lesser drug interactions compared to other therapies

    Analgesic Activity of Sebastiania chamaelea (L.) Muell. Arg.

    Background: This study intended to experimentally evaluate the analgesic activity of Sebastiania chamaelea (L.) Muell. Arg. which is widely used by folklore and traditional healers in pain relief. The drug is commonly known as ‘Bhumi Eranda’ among the locals.Methods: The plant material of Sebastiania chamaelea (L.) Muell. Arg. was collected from the fields of Sri Sri College of Ayurvedic Science and Research, Bangalore and preserved as per the standard method. The toxicity studies carried out earlier has proved that, the drug was non-toxic up to 3000 mg/kg body weight. The effective doses were derived as 300 mg/kg (lower) and 600 mg/kg (higher) body weight and the experimental study was conducted. Analgesic screening models used for the study are – Eddy’s Hot plate and Tail immersion models. The study was carried out in 4 groups i.e. Control, Standard, Lower dose Kashaya of Sebastiania chamaelea (L.) Muell. Arg. and Higher dose Kashaya of Sebastiania chamaelea (L.) Muell. Arg. in each model. The activity was compared with a standard reference drug, Tramadol and Diclofenac.Results: The results were analyzed by using one-way analysis of variance (ANOVA) test followed by Dunnett test to detect the significance of differences between each group and control.Conclusion: The study helped to conclude, Sebastiania chamaelea (L.) Muell. Arg. as an ideal analgesic and supported the traditional claim

    Hemophilic patient for emergency spinal decompression

    Hemophilia is mostly an inherited genetic disorder, caused by mutations in the clotting factor gene. With the available treatment options, life expectancy of a hemophilic patient is usually of that of the general population. Hence, it is not uncommon that they present for surgical procedures. However, hemophilic patients for the major surgical procedure are always a real challenge for the perioperative physician. We have recently encountered one such patient who was hospitalized with acute paraplegia due to a mass lesion of spine and successfully managed with the recovery of motor power. His pre-operative Factor VIII level was 0. Desmopressin nasal spray has a limited role in severe hemophilic. Our main concern was effective replacement therapy and maintenance of desired Factor VIII levels not only during surgery but also in the immediate post-operative period

    Prevalence of Hepatitis B surface antigen (HBsAg) among visitors of Shashemene General Hospital voluntary counseling and testing center

    <p>Abstract</p> <p>Background</p> <p>Hepatitis B virus (HBV) infection is significant health problem, as it can lead to chronic hepatitis, liver cirrhosis, and hepatic carcinoma. Due to shared routes of transmission, HBV and human immunodeficiency virus (HIV) co-infection is common and is an emerging concern in the clinical management of patients because of increased mortality, accelerated hepatic disease progression, and the frequent hepatotoxicity caused by anti-retroviral therapy. The aim of this study was to determine the prevalence of Hepatitis B surface antigen (HBsAg) and its risk factors, among individuals visiting Shashemene General Hospital VCT center.</p> <p>Findings</p> <p>Institution based cross-sectional study was performed from November 3, 2008 to December 29, 2008 and 384 voluntary counseling and testing (VCT) clients were investigated. Data on socio demographic and HBV risk factors was collected using structured questionnaires. Blood samples were collected and screened for hepatitis B surface antigen (HBsAg) and HIV by commercially available rapid test kits. The prevalence of HBsAg in this study group was 5.7%. Fourteen percent of HIV positive subjects (8/57) and 4.3% (14/327) of HIV negative subjects were positive for HBsAg. Significantly high prevalence of HBsAg was observed among individuals who had history of invasive procedures, like tooth extraction, abortion and ear piercing; history of hospital admission, history of unsafe inject and HIV positives.</p> <p>Conclusions</p> <p>Although HBsAg prevalence is much higher among subjects who are HIV positive (14.0% versus 4.3%), the prevalence of HBsAg in HIV negative subjects is high enough to warrant a recommendation to screen all clients at VCT centers irrespective of HIV status.</p

    Sodium lauryl ether sulfate (SLES) degradation by nitrate-reducing bacteria

    The online version of this article (doi:10.1007/s00253-017-8212-x) contains supplementary material, which is available to authorized users.The surfactant sodium lauryl ether sulfate (SLES) is widely used in the composition of detergents and frequently ends up in wastewater treatment plants (WWTPs). While aerobic SLES degradation is well studied, little is known about the fate of this compound in anoxic environments, such as denitrification tanks of WWTPs, nor about the bacteria involved in the anoxic biodegradation. Here, we used SLES as sole carbon and energy source, at concentrations ranging from 50 to 1000 mg L1, to enrich and isolate nitrate-reducing bacteria from activated sludge of a WWTP with the anaerobic-anoxic-oxic (A2/O) concept. In the 50 mg L1 enrichment, Comamonas (50%), Pseudomonas (24%), and Alicycliphilus (12%) were present at higher relative abundance, while Pseudomonas (53%) became dominant in the 1000 mg L1 enrichment. Aeromonas hydrophila strain S7, Pseudomonas stutzeri strain S8, and Pseudomonas nitroreducens strain S11 were isolated from the enriched cultures. Under denitrifying conditions, strains S8 and S11 degraded 500 mg L1 SLES in less than 1 day, while strain S7 required more than 6 days. Strains S8 and S11 also showed a remarkable resistance to SLES, being able to grow and reduce nitrate with SLES concentrations up to 40 g L1. Strain S11 turned out to be the best anoxic SLES degrader, degrading up to 41% of 500 mg L1. The comparison between SLES anoxic and oxic degradation by strain S11 revealed differences in SLES cleavage, degradation, and sulfate accumulation; both ester and ether cleavage were probably employed in SLES anoxic degradation by strain S11.This research was supported by the Spanish Ministry of Education and Science (contract project CTQ2007-64324 and 447 CONSOLIDER-CSD 2007-00055).     The Myosin Va Head Domain Binds to the Neurofilament-L Rod and Modulates Endoplasmic Reticulum (ER) Content and Distribution within Axons

    The neurofilament light subunit (NF-L) binds to myosin Va (Myo Va) in neurons but the sites of interaction and functional significance are not clear. We show by deletion analysis that motor domain of Myo Va binds to the NF-L rod domain that forms the NF backbone. Loss of NF-L and Myo Va binding from axons significantly reduces the axonal content of ER, and redistributes ER to the periphery of axon. Our data are consistent with a novel function for NFs as a scaffold in axons for maintaining the content and proper distribution of vesicular organelles, mediated in part by Myo Va. Based on observations that the Myo Va motor domain binds to intermediate filament (IF) proteins of several classes, Myo Va interactions with IFs may serve similar roles in organizing organelle topography in different cell types
