    The effect of changing position and early ambulation after cardiac catheterization on patients' outcomes: A single-blind randomized controlled trial

    Background: Cardiac catheterization is the gold standard diagnostic test for coronary heart diseases. In order to minimize the post-procedure complications, patients are restricted to prolonged bed rest that is always accompanied by fatigue and discomfort. Objective: The aim of this study was to assess the effect of changing position and early ambulation on the level of comfort, satisfaction, and fatigue and on the amount of bleeding and hematoma after cardiac catheterization. Participants: A sample of 70 patients, who had undergone a non-emergency 6-French cardiac catheterization via the femoral artery from September to November, 2006. Methods: In a single-blind randomized controlled trial, each patient was randomly assigned to either the control or experimental group. The patients' position in the experimental group was intermittently changed during the first 6 h after catheterization. Seven hours after the procedure, they were allowed to be ambulated and to undertake their self care activities. A pillow was placed under the patients' bodies. Patients in the control group were managed as routine; they were restricted to a 10-24 h bed rest bed rest in supine position with the affected leg straight and immobilized and a sand bag on the puncture site for at least 8 h. The levels of comfort, satisfaction and fatigue, and the amount of bleeding and hematoma were measured at regular intervals after the procedure. Results: The patients in the experimental group had significantly higher comfort and satisfaction and lower fatigue levels than the control group at 3, 6, 8 h and the next morning after catheterization (P 0.05). Conclusion: The results of this study showed that the levels of comfort, satisfaction and fatigue after catheterization are related to the duration of bed rest and patients' position in bed. Changing patients' position accompanied by early ambulation after cardiac catheterization are associated with increasing comfort and satisfaction levels and decreasing the level of fatigue without increasing the amount of bleeding and hematoma. © 2009 Elsevier Ltd.     Seasonal Variation in Exposure Level of Types A and B Ultraviolet Radiation: An Environmental Skin Carcinogen

    Background: The main source of ultraviolet radiation (UVR) is the sun, affecting organs such as the skin, eyes, and immune system. According to American Conference of Governmental Industrial Hygienist (ACGIH) reports, the amount of UVR reaching the Earth’s surface is increasing yearly and is responsible for an increase in solar radiation‑related diseases. Aims: To investigate the amount of UVR reaching the Earth’s surface and understand the risk of UVR on disease among outdoor laborers in one of the central provinces of Iran.Materials and Methods: Arak city was divided into two geographic areas, and the weekly measurement of UVR was done in three locations) asphalt, grass and rooftop). To measure UVR, Hanger UV spectrometer, standard deviation (SD8‑A), and SD8‑B detectors were used. Amounts of UVR for a consecutive year and varying weather conditions were measured. Finally, values obtained were compared to ACGIH standards. Results: The minimum and maximum levels of UV type A radiation occurred in April 1.27 (0.724) W/m2 and September 7.147 (4.128) W/m2, these figures for UV type B were in March–April 0.005 (0.003) and September 0.083 (0.077). The maximum UVR is received between 11 and 15 o’clock.Conclusions: In the central cities of Iran, the minimum and maximum UV type A and B is received in March–April and in September, respectively.     Efficient design and evaluation of countermeasures against fault attacks using formal verification

    This paper presents a formal verification framework and tool that evaluates the robustness of software countermeasures against fault-injection attacks. By modeling reference assembly code and its protected variant as automata, the framework can generate a set of equations for an SMT solver, the solutions of which represent possible attack paths. Using the tool we developed, we evaluated the robustness of state-of-the-art countermeasures against fault injection attacks. Based on insights gathered from this evaluation, we analyze any remaining weaknesses and propose applications of these countermeasures that are more robust

    The Development of an Empirical Model for Estimation of the Sensitivity to Heat Stress in the Outdoor Workers at Risk

    Background: Workers who work in hot environments may be at risk for heat stress. Exposure to heat can result in occupational illnesses, including heat stroke, heat cramps, and heat exhaustion. The risk of exposure to heat depends on individual, environmental, and occupational risk factors. Individual risk factors may decrease the individual’s tolerance to heat stress. Sensitivity as an intrinsic factor may predispose a person to heat stress. Aim: This study was aimed to determine the criteria for sensitivity parameter, specify their weights using the fuzzy Delphi-analytical hierarchy, and finally providing a model to estimate sensitivity. The significant of the study is presenting a model to estimate the sensitivity to heat stress. Materials and Methods: The expert’s opinions were used to extract the criteria in Delphi method. After determining the weight of each criterion, Fuzzy analytic hierarchy Process (FAHP), by mathematical principles matrix and triangular fuzzy numbers, was applied for the prioritization of criteria. Results: According to experts’ viewpoints and considering some exclusion, 10 of 36 criteria were selected. Among 10 selected criteria, age had the highest percentage of responses (90% (27/30)) and its relative weight was 0.063. After age, the highest percentages of response were assigned to the factors of preexisting disease (66.6% (20/30)), body mass index (56.6% (17/30)), work experience (53.3% (16/30)), and clothing (40% (16/30)), respectively. Other effective criteria on sensitivity were metabolic rate, daily water consumption, smoking habits, drugs that interfere with the thermoregulatory processes, and exposure to other harmful agents. Conclusions: Eventually, based on the criteria, a model for estimation of the workers’ sensitivity to heat stress was presented for the first time, by which the sensitivity is estimated in percent.Keywords: Heat stress, Sensitivity, Personal factors, Fuzzy AH

    Domestic violence and suicide attempt among married women: A case‐control study

    The aim of this study was to investigate the impact of domestic violence‐related factors on suicide attempt in married women. Suicide is a global public health concern that poses significant burden on individuals, families and communities. There is limited research on factors predicting suicide attempt in women. A retrospective case‐control design was adopted. Using a convenience sampling method, 610 participants, admitted to a teaching referral hospital in Northwest of XXX, were recruited to the study and assigned to case or control groups based on whether or not they had attempted suicide. The participants in two groups were matched in the terms of important demographic characteristics. Domestic violence‐related factors were considered as independent variables and suicide attempt as dependent variable. Descriptive statistics, simple and multivariate logistic regression analysis were used to analyze the data. Odd ratios (OR) of domestic violence related factors were compared between the groups. We used STROBE checklist as an EQUATOR in this study. The mean age of participants in the case and control groups was 28.4 years and 29.45 years, respectively. The infidelity was the strongest predictor of suicide attempt in women (OR 44.57, 95%CI 6.08‐326. 63, p<0.001), followed by being threatened to physical assault by husband (OR 37.01, 95%CI 11.54‐118.67, p<0.001), jealousy of husband (OR 23.46, 95%CI 11.63‐47.30, p<0.001), and previous attempts to divorce (OR 16.55, 95%CI 5.91‐46.31, p<0.001). Suicide attempt was significantly lower in women who reported a sense of peace in life or lived with their mother or father‐in‐law (p<0.001). To reduce the risk of suicide in women, violence against women should be condemned and appropriate prevention measures be taken by health professionals. Recognizing risk, assessment and referral of victims of domestic violence should be an integral part of health care systems.N/

    Comparison of clinical, para-clinical and laboratory findings in survived and deceased patients with COVID-19: diagnostic role of inflammatory indications in determining the severity of illness

    Background: Since December 2019, when a cluster of pneumonia cases due to SARS-CoV-2 initially emerged in Wuhan city and then rapidly spread throughout the world, the necessity for data concerning the clinical and para-clinical features of Iranian patients with COVID-19 was highlighted. Therefore, we aimed to compare the clinical, para-clinical and laboratory evidences of deceased patients with survival group. Methods: We extracted data regarding 233 patients with laboratory-confirmed COVID-19 from Buali Hospital in Iran; clinical/para-clinical and inflammatory indexes data were collected and analyzed. The data of laboratory examinations and chest CT findings were compared between deceased and survived patients. Results: The mean age of the patients was 49.8 years and 64 of our patients were male. The acute respiratory distress syndrome occurred in 64 patients, 52 who were admitted to the ICU, which all of them underwent invasive mechanical ventilation, and 28 who died. Lymphopenia (79), neutrophilia (79), and thrombocytopenia (21) were the most frequently observed laboratory findings of the deceased group on admission. Most patients (68) had a high systematic immune-inflammation (SII) index of > 500 and increased C-reactive protein level (88). Levels of inflammatory indexes such as neutrophil to lymphocyte ratio (NLR), platelet to lymphocyte ratio (PLR) and SII were documented to be significantly elevated in the deceased group when compared with the patients who survived (P < 0.0001, P < 0.001, P < 0.0001, respectively). The most commonly presented symptoms were fever (70) and cough (63) on admission. Headache was uncommon (11). Ground-glass opacity with consolidation (mixed) was the most common radiologic finding on chest CT (51). No radiographic or CT abnormality was found in 15 of 204 patients (7). Conclusion: Small fraction of patients with COVID-19 may present without fever and abnormal radiologic findings. Elevated NLR, PLR and SII can be considered as prognostic and risk stratifying factor of severe form of disease. © 2020, The Author(s)

    Predatory efficiency of Hippodamia variegata (Col.: Coccinellidae) on common pistachio psylla, Agonoscena pistaciae (Hem.: Aphalaridae), under laboratory conditions

    Common pistachio psylla, Agonoscena pistaciae Burckhardt & Lauterer, is the most economically important species among the psyllid pests of pistachio in Iran. Hence, the possibility of using nonchemical control methods against A. pistaciae and the existence of its natural enemies need to be investigated. The spotted amber ladybird, Hippodamia variegata (Goeze), is one of the most effective predators in pistachio orchards. It is a highly polyphagous coccinellid that preys mainly on aphid and psyllid pests of pistachio trees. This study is intended to improve the knowledge about the predation capacity of A. pistaciae. The experiment was conducted under laboratory conditions of 25oC and 30oC, 55 ± 5 RH and 16: 8 L: D. Logistic regression was used to determine the type of functional response and nonlinear regression calculated the parameters of the rate of searching efficiency (a) and handling time (Th). The functional response on different density of prey at both temperatures was type II. The attack rate and handling time were 0.088 ± 0.012 h-1 and 0.0647 ± 0.011 h at 25ºC and 0.195 ± 0.031 h-1 and 0.0698 ± 0.056 h at 30ºC, respectively. The density and temperature affected the predation rate significantly. The data suggests that the H. variegata is a viable biological agent against A. pistaciae

    Animal based low carbohydrate diet is associated with increased risk of type 2 diabetes in Tehranian adults

    Background: To investigate the association of low carbohydrate diet (LCD) score with the risk of type 2 diabetes among adults. Methods: This cohort study was conducted on 4356 healthy participants aged � 19 years old, who were followed-up for a mean duration of 3 years within the framework of the Tehran Lipid and Glucose Study. LCD score was calculated using a food frequency questionnaire according to intake of carbohydrate, protein, and fat at baseline. Diabetes was defined according to the criteria of the American Diabetes Association. Multivariable logistic regression models, adjusted for potential confounders, were used to estimate risk of diabetes across quartiles of LCD score. Results: Mean ± SD age of the study participants (44.4 men) was 40.5 ± 13.0 years. The median (25-75 interquartile range) of LCD score was 17.0 (12.0-21.0) and after a 3 year follow-up period, 123 (2.8) incident cases of diabetes were ascertained. After adjustment for confounding variables, including age, sex, smoking status, physical activity, total calorie intake, saturated fatty acid, waist circumference, educational level, and family history of diabetes, the multivariable-adjusted ORs (95 CIs) of type 2 diabetes, comparing the highest with the lowest quartiles, were 2.16 (1.16-4.04) for total LCD score (P-value = 0.015), 1.81 (1.06-3.11) for animal-based LCD score (P-value = 0.029), and 1.47 (0.85-2.52) for plant-based LCD score (P-value = 0.160). Conclusion: Our findings suggest that a higher adherence to LCD, mostly with higher intakes of protein and fat from animal-source foods, can increase the incidence of diabetes; however, a plant-based low-carbohydrate dietary pattern is not significantly associated with risk of type 2 diabetes. © 2020 The Author(s)
