    A case of Non-compaction of Left Ventricle Coexistent with Juvenile Nephronophthisis. Is this Another Presentation of Ciliopathy?

    Nephronophthisis is a chronic tubulo-interstitial nephritis which can progress to end-stage renal disease. Juvenile nephronophthisis is the most common type of nephronophthisis, which accounts for 5-10% of the cases of pediatric end stage renal diseases. Left ventricular non-compaction (LVNC), a rare form of cardiomyopathy, is the result of intrauterine arrest of compaction of the endomyocardial morphogenesis. Clinical manifestations of LVNC range from asymptomatic child to a progressive deterioration in the cardiac function, congestive heart failure, arrhythmias, systemic thromboembolism and sudden cardiac death. This report presents a case of juvenile nephronophthisis with LVNC. A 15-year-old boy was referred to our nephrology outpatient clinic with a 6-month history of non-specific complaints such as lethargy, anorexia, polydipsia, polyuria, and pallor. Abdominal sonography showed a generalized increase in the parenchymal echo of kidneys. Renal biopsy was performed for him which showed nephronophthisis. Echocardiography was done and revealed LVNC. He was discharged with training for careful follow-up. Our reported case had nephronophthisis and LVNC. To the best of our knowledge, there is no report of this combination in the literature. This suggests that LVNC may be another presentation of cilia involvement. The clinical coexistence of LVNC and nephronophthisis could guide us to better localize and discover the underlying genetic mutations and the role of ciliopathies in various human diseases; Therefore, further research with a special focus on potential common derangement of cilia and protein products in these diseases is recommended. Keywords: Juvenile nephronophthisis; Isolated Noncompaction of the Ventricular Myocardium; Ciliary Motility Disorder

    Vectorial characterization of Bloch surface wave via one-dimensional photonic-atomic structure

    Use of hot atomic vapor as a new tool for tracing the complex nature of light has become a knowledge-based topic in recent years. In this paper, we examine the polarization ellipse of the Bloch surface wave (BSW) through the effect of a magnetic field on the coupling of these surface waves in BSW-hot atomic vapor cell. For this purpose, we fabricate a one-dimensional photonic crystal-based Bloch wave atom cell, where under different configurations of magnetic field, polarization ellipse of Bloch surface waves has been recorded experimentally. Our results indicate that by applying the magnetic field in different directions, Faraday and Voigt, the characteristics of electromagnetically induced transparency (EIT-like) of hybrid system change. We have used these changes to redefine the geometry of Voigt and Faraday for evanescent waves, as well as to measure the ratio of the components of the elliptical polarized electric field. These characterizations can open new insight into the miniaturized atomic field in high quality and low volumetric areas

    Talent management is a system for identifying, upbringing, promoting, and preserving talented persons to optimize the capability of an organization to come up with optimal business results. Nowadays, the developing talent management is one of the most important business factors, which is considered as one of the key components to maintain a sustainable competitive advantage in organizations. the purpose of this study was to identify the components of talent management in the custom organization so that its implementation will be expedited. Considering various advantages of Fuzzy Delphi, we opted this method in conducting the study (The most important advantage is its greater accuracy than classical Delphi). Twenty persons were chosen as experts through a snowball sampling method. At the first step, 139 components were proposed based on previously reported through interviews, suggested to experts that, after four phases of deactivation, 54 indicators and components that were more relevant to the experts remained. In this study, we used a modified Three-branch model of Dr Mirzai Ahranjani (2005) (Of course, we also added a perceptual/cognitive branch that our model has a circular layer of four) to present a talent management model. The results of the research show that the behavioral/functional components in the final analytical model have the largest share.La gestión del talento es un sistema para identificar, educar, promover y preservar a las personas con talento para optimizar la capacidad de una organización y obtener resultados empresariales óptimos. Hoy en día, el desarrollo de la gestión del talento es uno de los factores empresariales más importantes, que se considera como un componente clave para mantener una ventaja competitiva sostenible en las organizaciones. El propósito de este estudio fue identificar los componentes de la gestión del talento en la organización personalizada para que su implementación se agilice. Teniendo en cuenta las diversas ventajas de Fuzzy Delphi, optamos por este método para realizar el estudio (la ventaja más importante es su mayor precisión que la clásica Delphi). Veinte personas fueron elegidas como expertos a través de un método de muestreo de bola de nieve. En el primer paso, se propusieron 139 componentes basados en entrevistas anteriores, y se sugirió a los expertos que, después de cuatro fases de desactivación, quedaban 54 indicadores y componentes que eran más relevantes para los expertos. En este estudio, utilizamos un modelo modificado de tres ramas del Dr. Mirzai Ahranjani (2005) (Por supuesto, también agregamos una rama perceptiva / cognitiva que nuestro modelo tiene una capa circular de cuatro) para presentar un modelo de gestión del talento. O objetivo deste estudo foi identificar os componentes do gerenciamento de talentos na organização personalizada para que sua implementação seja acelerada. Considerando várias vantagens do Fuzzy Delphi, optamos por este método na condução do estudo (a vantagem mais importante é a sua maior precisão do que o Delphi clássico). Vinte pessoas foram escolhidas como especialistas por meio de um método de amostragem de bolas de neve.     Investigating the safety atmosphere and its associated factors in a construction firm in 2011

    Background and Aims: Safety performance in construction industries is a major concern. Occupational injuries and fatalities in these industries are so frequent indicating high rate of accidents. Improper comprehension and safety culture of workers may lead to unsafe behaviors which ultimately result in accidents. This work was conducted to investigate the safety climate and its associated factors in a construction firm.Materials and Methods: A validated questionnaire was used to determine the safety atmosphere and its relevant factors in two sites of a construction firm. According to MORGAN Table, 347 workers of a construction firm were randomly selected. The relations among safety atmosphere factors, as well as demographic characteristics were determined. Spearman’s rho test was used to analysis the data.Results: A meaningful relationship among the safety atmosphere factors was observed in both building workshops according to the Spearman’s rho test. The results also showed that management commitment factor was most associated with stress factors of production. There were significant relationships between age and some safety atmosphere factors such as safety training, production pressure, encouraging raising safety, and rule breaking (p<0.05). Job category had significant relationships with some factors including productionpressure, encouraging raising safety, and safety committees (p<0.05).Conclusion: Safety atmosphere and its associated factors are effective tools to determine the safety condition in the studied construction firm.Key words: Safety climate, Safety climate factors, Accident, Demographical features

    Experimental Study of Discharge Coefficient of Combination Labyrinth Weirs

    Labyrinth weirs are one of the kind of nonlinear crest weirs causing the increase of discharge capacity for specified height of water level in canals and dams. In this study, the effects of dentate and orifice labyrinth weirs on increasing the discharge capacity were investigated. Experiments were conducted in a laboratory flume with length of 12 m and width of 0.8 m. The nine experimental models with 15 cm height were used. The results showed that for L/W=2 and H/P=0.2, the discharge coefficient of the orifice-dentate labyrinth weir and the dentate labyrinth weir are 75.6% and 17.5%, more than the simple labyrinth weir, respectively. However, dent and orifice may decrease their efficiency in high heads and the discharge coefficient will be close to simple labyrinth weir. The reason for these changes was increasing the flow interference in downstream of labyrinth weir between dents, orifices and weir with increasing discharge. Also, the result showed the efficiency of orifice and dentate on labyrinth weirs will decrease with increasing the weir magnification (L/W) and they lose their effect

    Ocular chemical burns are common and serious ocular emergencies that require immediate and intensive evaluation and care. The victims of such incidents are usually young, and therefore loss of vision and disfigurement could dramatically affect their lives. The clinical course can be divided into immediate, acute, early, and late reparative phases. The degree of limbal, corneal, and conjunctival involvement at the time of injury is critically associated with prognosis. The treatment starts with simple but vision saving steps and is continued with complicated surgical procedures later in the course of the disease. The goal of treatment is to restore the normal ocular surface anatomy and function. Limbal stem cell transplantation, amniotic membrane transplantation, and ultimately keratoprosthesis may be indicated depending on the patients’ needs

    Evaluation of Social Support for Parents of Children with Congenital Heart Disease Referred to Imam Khomeini Clinic at Hamadan City, 2021

    Background and Objectives: Congenital heart disease is one of the most common congenital anomalies in children. The purpose of this study was the evaluation of social support in parents of children with congenital heart disease. Materials and Methods: This was a descriptive correlational study. The study population included 200 parents of children with congenital heart disease who referred to Imam Khomeini Clinic in Hamedan in 2020 via available sampling method and using the demographic information questionnaire, social support questionnaire Zimat. Data was analyzed using descriptive and inferential statistics. Results: The mean and standard deviation of the total score of social support 41.4 ± 5.61. The highest social support 15.71 ± 2.23 was related to the family dimension and the lowest social support 12.17 ± 2.39 was related to the dimension of friends. The results of this study showed that the average total score of social support in terms of all demographic variables of parents except maternal occupation has a statistically significant difference (P < 0.05). Conclusions: Social support is tool that improves the quality of life in parents of children with congenital heart disease, so it is recommended to develop a comprehensive care plan for these patients alongside, increasing their parent's social support

    Comparison of relationship between religious beliefs and psychological well-being, with a high burnout and low

    For downloading the full-text of this article please click here.Background and Objectives: Nowadays, tension and occupational burnout is a common and serious problem among people, particularly who are working in the human services jobs. Religious beliefs are attitudes towards the reality of mythological, supernatural or spiritual aspects of a religion. Subjective well-being is requires an understanding of the challenges of life. The aim of this study was to compare the relationship between religious beliefs and psychological well-being among employees of university of medical sciences with high and low burnout.Materials and Methods: In this correlational study, the statistical population were all 280 employees in ILAM University of Medical Sciences (n=280). Considering the size of the population and using Morgan table, 160 people were choosen by simple random sampling so that  80 persons with low and 80 persons with high occupational burnout were screened using Questionnaire of Burnout Maslach and Jackson. Then individuals completed psychological well-being Reef and religious beliefs questionnaires (Khodayarifard). The research data was analyzed using the statistical software SPSS and Pearson correlation coefficient, t-test and Z scale.Results: Data analysis using Fisher's Z-test showed that the relationship between religious beliefs and psychological well-being and its components, in the total score of psychological well-being (3.08) and components of reception (2.31), environmental control (3), purpose (3.24) and relationships with others (2.8) was significant between people with high and low occupational burnout.Conclusion: Details of the research showed that the relationship between religious beliefs with a total score of psychological well-being and component acceptance, environmental mastery, purpose and relationships with others is higher in persons with low occupational burnout in comparison with high occupational burnout people.
