    Investigating library operational issues being faced by medical university librarians in Islamabad.

    Medical libraries are continual sources of knowledge and information. Without this knowledge and information, there will be no researcher, teacher, and doctor. Libraries cannot share knowledge with knowledge seekers alone. In this scenario, the role of the librarian becomes very influential. A librarian is an information services provider. The providence of these services is dependent on the facilities which are required by librarians. Realizing the importance of this issue the researcher has carried out research to investigate issues faced by librarians while operating medical library services. This study was related to quantitative research methodology; therefore, the researcher adopted quantitative research methodology for this research work. The presented issue in this study was solved through collecting data, with the help of using different techniques associated with data collection techniques, description of collected data, analyzing collected data and then finding the solution to the problem after a detailed analysis of data. In this study, the university libraries located in Islamabad were the population of the study to collect data regarding library operational issues in medical university libraries in Islamabad. The findings of the study show that medical librarians are facing numerous issues. These issues are impending medical librarians to operate library services in effective and appropriate ways. So, this research work suggests that the budget of libraries should be increased, medical librarians should be trained to meet their challenges, and medical resources and digital resources ought to be augmented in the respective universities in Islamabad. The aforementioned strategies can be useful for medical libraries and can enable librarians to operate their library services in more influential and effective ways in terms of information providers

    Developing of Institutional Repository in Public Sector University Libraries of Punjab, Pakistan

    The target of this analysis to discover the current status of open source institutional repository software’s usage in the public sector university libraries of Punjab, Pakistan. Survey method was used by the researchers and a survey was adopted for information gathering. The target population of the investigation was library professionals of public sector universities in Punjab. Data was gathered from a sample of 135 university librarians using a convenience sampling approach. The analyses explored in which majority of library professionals have incorporated or are currently in the process of incorporating institutional repositories (IR) into their respective libraries which shows that library professionals are well aware about the importance of development/need of IR in this digital age. Further, this study revealed that most of the professionals preferred to use open source software i.e. D-Space for development of IR. Moreover, library professionals are of the view that IR software is a vital tool to organize the digital content/material. Further, IR software provides easy access to the users and enables them to provide efficient and effective services to their users. Furthermore, this study highlights that majority of participants were preferred to use those software which have the facility of customization, E-mail based sharing of books. Library professionals are satisfied with that IR software which is freely available, provide format for conversion of different digital files, and publishing digital contents. The study also highlights barriers which faced by majority of respondents i.e. selection of proper software while developing the Institutional Repository. The respondents also ranked lack of budget is another major barrier that forbade the development of IR followed by the selection of scanner technology, selection of material for digitization, institutional repository knowledge. The investigators will explore additional avenues for additional investigation through the lens of institutional repositories (IR)

    Use of Electronic Resources among Users of Medical College Libraries in Multan Division, Pakistan

    The rationale of this report was to evaluate the current use of electronic resources among users of medical college libraries in Multan division. Moreover, mean variation about medical e-resources in respect of awareness, satisfaction, utilization and barriers with respondents’ gender trait were also explored. This study used a questionnaire to conduct a cross-sectional survey research design. The survey questionnaire was send to the target population of medical college libraries users in Multan division of Pakistan and response rate was 88 per cent. The findings of the study shows that users of the medical libraries were slightly aware about the medical e-resources and majority of the participants were used medical e-resources for education, learning and to update knowledge purposes. In addition, Medical e-resources that are MEDLINE, PubMed, Springer Link, Science Direct, Black-Well Synergy and ProQuest Database were utilized rarely by the respondents. Users are partially satisfied with medical e-resources of libraries. Lack of training/ orientation, low speed of internet, energy crisis/load shedding, lack of printing facility, non-availability of full text access to the most of Journals, lack of awareness, information overload and inadequate IT infrastructure were the major problems faced to medical colleges library users while using e-resources. There is no statistically significant difference in the mean scores of awareness, utilization, satisfaction and barriers about the medical e-resources with respect to male and female. The findings allow library professionals to be more effective, proactive, and successful in achieving digitize library services and resources. This study also made a significant contribution to the existing literature on medical e-resources and services. The findings could contribute in the promotion of digital library services and products, as well as virtual culture, in Pakistani medical college libraries, particularly in the Multan Division

    Skills of ICTs among Library Professionals of South Punjab

    The study aimed to explore the skills, knowledge, abilities, and competencies of library professionals working in the academic libraries of public sector universities and private sector universities located in south Punjab. Library professionals working in the libraries of public sector universities and private sector universities were the population of the study. Convenience sampling technique was used to meet the objectives of the study. Quantitative research technique followed by the questionnaire as a data collection tool was used to accomplish the research. The findings of the study revealed that ICT is an essential part of the libraries and librarians are there to perform library operations to facilitate the library users of their respective libraries. It is the basic necessity of the present age to learn new skills, abilities, and competencies related to ICTs to survive in the profession of librarianship. Training courses about ICT are an effort to enhance the skills of library professionals. Library associations can play an important role in uplifting the trainings regarding ICTs

    Dynamic Wireless Information and Power Transfer Scheme for Nano-Empowered Vehicular Networks

    In this paper, we investigate the wireless power transfer and energy-efficiency (EE) optimization problem for nano-centric vehicular networks operating over the terahertz band. The inbody nano-sensors harvest energy from a power station via radio-frequency signal and then use the harvested energy to transmit data to the sink node. By considering the properties of terahertz band (i.e., sensitivity to distance and frequency over the communication path), we adopt the Brownian motion model to develop a time-variant terahertz channel model and to describe the mobility of the nano-sensors. Thus, based on the channel model and energy resources, we further develop a long-term EE optimization problem. The EE optimization is further converted into a series of energy-efficient resource allocation problems over the time slots via equivalent transformation method. The resource allocation problem for each timeslot, which is formulated as a mixed integer nonlinear programming (MINLP), is solved based on the particle swarm optimization (PSO) method. In addition, a dynamic PSO-based EE optimization (DPEEO) algorithm is developed to obtain the sub-optimal solution for the EE optimization problem. By exploiting the special structure of the reformulated problem, an improved DPEEO algorithm, is presented which can handle the problem’s constraints quite well, decreases the research space, and greatly reduces the length of the convergence time. Simulation results validate the theoretical analysis of our system

    Effect of mobile phone text message reminders on routine immunization uptake in Pakistan: randomized controlled trial

    Background: Improved routine immunization (RI) coverage is recommended as the priority public health strategy to decrease vaccine-preventable diseases and eradicate polio in Pakistan and worldwide.Objective: The objective of this study was to ascertain whether customized, automated, one-way text messaging (short message service, SMS) reminders delivered to caregivers via mobile phones when a child is due for an RI visit can improve vaccination uptake and timelines in Pakistan.Methods: This was a randomized controlled trial, conducted in an urban squatter settlement area of Karachi, Pakistan. Infants less than 2 weeks of age with at least one family member who had a valid mobile phone connection and was comfortable receiving and reading SMS text messages were included. Participants were randomized to the intervention (standard care + one-way SMS reminder) or control (standard care) groups. The primary outcome was to compare the proportion of children immunized up to date at 18 weeks of age. Vaccine given at 6, 10, and 14 weeks schedule includes DPT-Hep-B-Hib vaccine (ie, diphtheria, pertussis, and tetanus; hepatitis B; and Haemophilus influenza type b) and oral poliovirus vaccine (OPV). Data were analyzed using chi-square tests of independence and tested for both per protocol (PP) and intention-to-treat (ITT) analyses.Results: Out of those approached, 84.3% (300/356) of the participants were eligible for enrollment and 94.1% (318/338) of the participants had a working mobile phone. Only children in the PP analyses, who received an SMS reminder for vaccine uptake at 6 weeks visit, showed a statistically significant difference (96.0%, 86/90 vs 86.4%, 102/118; P=.03).The immunization coverage was consistently higher in the intervention group according to ITT analyses at the 6 weeks scheduled visit (76.0% vs 71.3%, P=.36). The 10 weeks scheduled visit (58.7% vs 52.7%, P=.30) and the 14 weeks scheduled visit (31.3% vs 26.0%, P=.31), however, were not statistically significant.Conclusions: Automated simple one-way SMS reminders in local languages might be feasible for improving routine vaccination coverage. Whether one-way SMS reminders alone can have a strong impact on parental attitudes and behavior for improvement of RI coverage and timeliness needs to be further evaluated by better-powered studies and by comparing different types and content of text messages in low-and middle-income countries (LMICs)

    An Efficient Channel Access Scheme for Vehicular Ad Hoc Networks

    Vehicular Ad Hoc Networks (VANETs) are getting more popularity due to the potential Intelligent Transport Systems (ITS) technology. It provides many efficient network services such as safety warnings (collision warning), entertainment (video and voice), maps based guidance, and emergency information. VANETs most commonly use Road Side Units (RSUs) and Vehicle-to-Vehicle (V2V) referred to as Vehicle-to-Infrastructure (V2I) mode for data accessing. IEEE 802.11p standard which was originally designed for Wireless Local Area Networks (WLANs) is modified to address such type of communication. However, IEEE 802.11p uses Distributed Coordination Function (DCF) for communication between wireless nodes. Therefore, it does not perform well for high mobility networks such as VANETs. Moreover, in RSU mode timely provision of data/services under high density of vehicles is challenging. In this paper, we propose a RSU-based efficient channel access scheme for VANETs under high traffic and mobility. In the proposed scheme, the contention window is dynamically varied according to the times (deadlines) the vehicles are going to leave the RSU range. The vehicles with shorter time deadlines are served first and vice versa. Simulation is performed by using the Network Simulator (NS-3) v. 3.6. The simulation results show that the proposed scheme performs better in terms of throughput, backoff rate, RSU response time, and fairness

    Impact of 10-valent pneumococcal conjugate vaccine (PCV10) on nasopharyngeal carriage in children 2 years of age: Data from a four-year time series cross-sectional study from Pakistan

    The dataset described in this paper was collected for a time-series cross-sectional study exploring the impact of 10-valent Pneumococcal Conjugate Vaccine (PCV10) on nasopharyngeal (NP) carriage in children under 2 years of age from a rural population in Sindh, Pakistan. The study was carried out in two union councils of Matiari - Khyber and Shah Alam Shah Jee Wasi (Latitude 25.680298 / Longitude 68.502711). Data was collected on socio-demographics, clinical characteristics and vaccination status using android phone-based application. NP samples were collected using standard World Health Organisation (WHO) techniques, culture and serotyping was done using sequential Multiplex PCR described by Centre for Disease Control, USA. We looked at the carriage rate of vaccine type (VT) and non-vaccine type (NVT) serotypes over time in vaccinated and unvaccinated children. We additionally looked at the predictors for pneumococcal carriage.     Utilisation of an operative difficulty grading scale for laparoscopic cholecystectomy

    Background A reliable system for grading operative difficulty of laparoscopic cholecystectomy would standardise description of findings and reporting of outcomes. The aim of this study was to validate a difficulty grading system (Nassar scale), testing its applicability and consistency in two large prospective datasets. Methods Patient and disease-related variables and 30-day outcomes were identified in two prospective cholecystectomy databases: the multi-centre prospective cohort of 8820 patients from the recent CholeS Study and the single-surgeon series containing 4089 patients. Operative data and patient outcomes were correlated with Nassar operative difficultly scale, using Kendall’s tau for dichotomous variables, or Jonckheere–Terpstra tests for continuous variables. A ROC curve analysis was performed, to quantify the predictive accuracy of the scale for each outcome, with continuous outcomes dichotomised, prior to analysis. Results A higher operative difficulty grade was consistently associated with worse outcomes for the patients in both the reference and CholeS cohorts. The median length of stay increased from 0 to 4 days, and the 30-day complication rate from 7.6 to 24.4% as the difficulty grade increased from 1 to 4/5 (both p < 0.001). In the CholeS cohort, a higher difficulty grade was found to be most strongly associated with conversion to open and 30-day mortality (AUROC = 0.903, 0.822, respectively). On multivariable analysis, the Nassar operative difficultly scale was found to be a significant independent predictor of operative duration, conversion to open surgery, 30-day complications and 30-day reintervention (all p < 0.001). Conclusion We have shown that an operative difficulty scale can standardise the description of operative findings by multiple grades of surgeons to facilitate audit, training assessment and research. It provides a tool for reporting operative findings, disease severity and technical difficulty and can be utilised in future research to reliably compare outcomes according to case mix and intra-operative difficulty

    Global, regional, and national burden of diabetes from 1990 to 2021, with projections of prevalence to 2050: a systematic analysis for the Global Burden of Disease Study 2021

    Background Diabetes is one of the leading causes of death and disability worldwide, and affects people regardless of country, age group, or sex. Using the most recent evidentiary and analytical framework from the Global Burden of Diseases, Injuries, and Risk Factors Study (GBD), we produced location-specific, age-specific, and sex-specific estimates of diabetes prevalence and burden from 1990 to 2021, the proportion of type 1 and type 2 diabetes in 2021, the proportion of the type 2 diabetes burden attributable to selected risk factors, and projections of diabetes prevalence through 2050. Methods Estimates of diabetes prevalence and burden were computed in 204 countries and territories, across 25 age groups, for males and females separately and combined; these estimates comprised lost years of healthy life, measured in disability-adjusted life-years (DALYs; defined as the sum of years of life lost [YLLs] and years lived with disability [YLDs]). We used the Cause of Death Ensemble model (CODEm) approach to estimate deaths due to diabetes, incorporating 25 666 location-years of data from vital registration and verbal autopsy reports in separate total (including both type 1 and type 2 diabetes) and type-specific models. Other forms of diabetes, including gestational and monogenic diabetes, were not explicitly modelled. Total and type 1 diabetes prevalence was estimated by use of a Bayesian meta-regression modelling tool, DisMod-MR 2.1, to analyse 1527 location-years of data from the scientific literature, survey microdata, and insurance claims; type 2 diabetes estimates were computed by subtracting type 1 diabetes from total estimates. Mortality and prevalence estimates, along with standard life expectancy and disability weights, were used to calculate YLLs, YLDs, and DALYs. When appropriate, we extrapolated estimates to a hypothetical population with a standardised age structure to allow comparison in populations with different age structures. We used the comparative risk assessment framework to estimate the risk-attributable type 2 diabetes burden for 16 risk factors falling under risk categories including environmental and occupational factors, tobacco use, high alcohol use, high body-mass index (BMI), dietary factors, and low physical activity. Using a regression framework, we forecast type 1 and type 2 diabetes prevalence through 2050 with Socio-demographic Index (SDI) and high BMI as predictors, respectively. Findings In 2021, there were 529 million (95% uncertainty interval [UI] 500–564) people living with diabetes worldwide, and the global age-standardised total diabetes prevalence was 6·1% (5·8–6·5). At the super-region level, the highest age-standardised rates were observed in north Africa and the Middle East (9·3% [8·7–9·9]) and, at the regional level, in Oceania (12·3% [11·5–13·0]). Nationally, Qatar had the world’s highest age-specific prevalence of diabetes, at 76·1% (73·1–79·5) in individuals aged 75–79 years. Total diabetes prevalence—especially among older adults—primarily reflects type 2 diabetes, which in 2021 accounted for 96·0% (95·1–96·8) of diabetes cases and 95·4% (94·9–95·9) of diabetes DALYs worldwide. In 2021, 52·2% (25·5–71·8) of global type 2 diabetes DALYs were attributable to high BMI. The contribution of high BMI to type 2 diabetes DALYs rose by 24·3% (18·5–30·4) worldwide between 1990 and 2021. By 2050, more than 1·31 billion (1·22–1·39) people are projected to have diabetes, with expected age-standardised total diabetes prevalence rates greater than 10% in two super-regions: 16·8% (16·1–17·6) in north Africa and the Middle East and 11·3% (10·8–11·9) in Latin America and Caribbean.
