    Grado de conocimiento, tratamiento y control de la hipertensión arterial, hipercolesterolemia y diabetes mellitus en la población general adulta

    ObjetivosEstimar la prevalencia de los principales factores de riesgo cardiovascular (FRCV) tras la confirmación diagnóstica con el criterio clínico. Analizar el grado de su conocimiento y control, y evaluar la asociación de los FRCV entre sí, estimando qué población es susceptible de intervención.DiseñoEstudio observacional, descriptivo,transversal.EmplazamientoEstudio poblacional realizado en el Área de Salud número 20 de la Comunidad Valenciana (comarca de la Vega Baja del río Segura)PacientesPersonas con ≥ 20 años residentes en dicha área. Muestreo aleatorio polietápico proporcional con definición de cuotas muestrales según el tipo de residencia. Tamaño muestral: 2.550 personas (falta de respuesta en la primera fase en el 26%). Segunda fase del estudio: se realiza la confirmación clínica de los 374 casos detectados de hipertensión arterial (HTA), los 126 de hipercolesterolemia (HCOL) y los 33 de diabetes mellitus (DM).MedicionesSe realiza una entrevista a los pacientes detectados para las tomas de presión arterial a los hipertensos y para practicar la extracción sanguínea a hipercolesterolémicos y diabéticos. Se han utilizado los criterios diagnósticos preconizados por los últimos consensos.ResultadosParticipan 1.886 personas (78,1%). Prevalencias halladas: HTA total, 42 ± 2,2%; diagnosticada, 11,7 ± 1,4%; HCOL total, 26,6 ± 2%; DM total, 9,5 ± 1,3%; tabaquismo: 33,6 ± 2,1% y obesidad: 31,6 ± 2,1%. Grado de control: HTA, 8,6%; HCOL, 21,7%; DM, 56,1%. Asociación de FRCV: el 25,5% no presenta hipertensión, hipercolesterolemia ni fuma.ConclusionesElevada prevalencia FRCV en nuestra área frente a los resultados publicados de ámbito nacional. El tabaquismo resulta menos frecuente que en el resto del país. El grado de control es escaso y la asociación de los FRCV principales para cardiopatía isquémica reflejan que cerca del 75% de la población debe ser intervenida.ObjectivesTo calculate the prevalence of the main cardiovascular risk factors (CVRF) after diagnostic confirmation using clinical criteria. Patients. People who had lived in this Area for ³20 years. Proportional, multi-stage randomised sampling with definition of sample quotas according to type of residence. Sample size: 2550 people (no reply in first stage: 26%). The second stage involved the clinical confirmation of those identified as suffering from hypertension (HT) (374), hypercholesterolaemia (HCOL) (126), and diabetes (DM) (33).MeasurementsPatients identified were interviewed for taking blood pressure in the case of those with HT, and blood samples in the case of those with HCOL and DM. The diagnostic criteria recommended by the latest consensus were used.Results1886 people (78.1%) took part. Prevalences found: total HT 42±2.2%, diagnosed: 11.7±1.4; total HCOL 26.6±2%; total DM 9.5±1.3%.Tobacco dependency was 33.6±2.1% and Obesity: 31.6±2.1%. Degree of control: HT, 8.6%; HCOL, 21.7%; DM, 56.1%. Association of CVR factors with each other: 25.5% had neither HT nor HCOL and did not smoke.ConclusionsHigh prevalence of CVRF in our area, compared with published national findings.Tobacco dependency is less common than at national level. The degree of control is very low and the association of the main CVRF factors for ischaemic cardiopathy reflect that nearly 75% of the population requires a preventive intervention

    Effect of diacutaneous fibrolysis on the muscular properties of gastrocnemius muscle

    Diacutaneous fibrolysis is a noninvasive technique that has been shown to be effective in the treatment of musculoskeletal disorders such as shoulder pain, lateral epicondylalgia, patellofemoral pain syndrome and carpal tunnel syndrome. However, while diacutaneous fibrolysis is applied to soft tissue, its effects on muscular properties are unknown. The purpose of the present study was to evaluate the effects of diacutaneous fibrolysis on muscle properties as measured by tensiomyography and myotonometry in asymptomatic subjects. An analytical descriptive study was performed. A single session of diacutaneous fibrolysis on the gastrocnemius muscle was applied to one limb (treated limb group) and the other limb was the control (control limb group). Subjects were assessed with tensiomyography and myotonometry before treatment (T0), after treatment (T1) and 30 minutes later (T2). The primary outcomes were tensiomyography and myotonometry variables. The treated limb group showed a statistically significant increase (p<0.05) in tensiomyography parameters. A decrease in rigidity and increase in relaxation was also observed on myotonometry at T1, with some of the effects being maintained at T2. Rigidity and relaxation at T1 were statistically significant between groups (p<0.05). A single session of diacutaneous fibrolysis to the gastrocnemius muscle of asymptomatic subjects produced immediate changes in muscle properties. These changes were maintained 30 minutes after the application of the technique

    Boreal forest floor greenhouse gas emissions across a Pleurozium schreberi-dominated, wildfire-disturbed chronosequence

    The boreal forest is a globally critical biome for carbon cycling. Its forests are shaped by wildfire events that affect ecosystem properties and climate feedbacks including greenhouse gas (GHG) emissions. Improved understanding of boreal forest floor processes is needed to predict the impacts of anticipated increases in fire frequency, severity, and extent. In this study, we examined relationships between time since last wildfire (TSF), forest floor soil properties, and GHG emissions (CO2, CH4, N2O) along a Pleurozium schreberi-dominated chronosequence in mid- to late succession located in northern Sweden. Over three growing seasons in 2012–2014, GHG flux measurements were made in situ and samples were collected for laboratory analyses. We predicted that P. schreberi-covered forest floor GHG fluxes would be related to distinct trends in the soil properties and microbial community along the wildfire chronosequence. Although we found no overall effect of TSF on GHG emissions, there was evidence that soil C/N, one of the few properties to show a trend with time, was inversely linked to ecosystem respiration. We also found that local microclimatic conditions and site-dependent properties were better predictors of GHG fluxes than TSF. This shows that site-dependent co-variables (that is, forest floor climate and plant-soil properties) need to be considered as well as TSF to predict GHG emissions as wildfires become more frequent, extensive and severe

    Revealing the transfer pathways of cyanobacterial-fixed N into the boreal forest through the feather-moss microbiome

    INTRODUCTION: Biological N(2) fixation in feather-mosses is one of the largest inputs of new nitrogen (N) to boreal forest ecosystems; however, revealing the fate of newly fixed N within the bryosphere (i.e. bryophytes and their associated organisms) remains uncertain. METHODS: Herein, we combined (15)N tracers, high resolution secondary ion mass-spectrometry (NanoSIMS) and a molecular survey of bacterial, fungal and diazotrophic communities, to determine the origin and transfer pathways of newly fixed N(2) within feather-moss (Pleurozium schreberi) and its associated microbiome. RESULTS: NanoSIMS images reveal that newly fixed N(2), derived from cyanobacteria, is incorporated into moss tissues and associated bacteria, fungi and micro-algae. DISCUSSION: These images demonstrate that previous assumptions that newly fixed N(2) is sequestered into moss tissue and only released by decomposition are not correct. We provide the first empirical evidence of new pathways for N(2) fixed in feather-mosses to enter the boreal forest ecosystem (i.e. through its microbiome) and discuss the implications for wider ecosystem function

    The Art of Dying: Mira de Amescua’s La hija de Carlos V and Ximénez de Enciso’s La mayor hazaña de Carlos V

    Charles V was a popular figure on the early modern stage; plays offered nostalgic glimpses of a warrior king who conquered foreign lands and stifled the spread of heresy. This article examines two plays that dramatize Charles V’s life: Mira de Amescua’s La hija de Carlos V and Ximénez de Enciso’s La mayor hazaña de Carlos V. Instead of serving audiences the normal rations of a swashbuckling Charles, these plays push aside the battles and the pageantry in order to emphasize the inner conflict that troubled the emperor during his final years. I analyse how these two plays use stage directions and performative cues to depict the emperor’s abdication and retirement to Yuste, finding that the plays engage with vanitas, the still-life visual reminders of human mortality, and the ars moriendi literary tradition in order to promote the concept of a good death. Carlos V era un personaje popular en los corrales de comedias; las obras ofrecían una imagen nostálgica de un rey guerrero que conquistaba tierras lejanas y evitaba la difusión de la herejía. Este artículo propone examinar dos obras sobre la vida de Carlos V: La hija de Carlos V de Mira de Amescua y La mayor hazaña de Carlos V de Ximénez de Enciso. En vez de alabar el lado militar del emperador, las obras se alejan de las batallas y la pompa para enfocarse en el conflicto interior que le preocupaba durante sus años postreros. Analizo las acotaciones y otras indicaciones de la puesta en escena relacionadas con la abdicación del emperador a Yuste, y concluyo que las obras dialogan con vanitas, las pinturas de naturaleza muerta que recuerdan la mortalidad humana, y la tradición literaria de las ars moriendi para promover la idea del bien morir
