The roles, resources and competencies of employee lay judges: A cross-national study of Germany, France and Great Britain
This research project analysed and compared the roles, resources and competencies of lay judges in Germany, France and Great Britain, where lay judges take up their role through nationally distinctive routes: nomination essentially by the social partners in Germany, self-nomination in Great Britain and election in France. The primary research consisted of qualitative data collected through interviews, set against contextual information on national institutional arrangements, industrial relations, This research project analysed and compared the roles, resources and competencies of lay judges in Germany, France and Great Britain, where lay judges take up their role through nationally distinctive routes: nomination essentially by the social partners in Germany, self-nomination in Great Britain and election in France. The primary research consisted of qualitative data collected through interviews, set against contextual information on national institutional arrangements, industrial relations, and court procedures. The key findings are as follows:
âą The dominant influence on lay judgeâs reported perception of their role is their experience of the prevailing industrial relations system in each country, mediated by the labour court structure. Routes to nomination not only reflect national systems but may reinforce them and have a bearing on employee lay judgesâ sense of organisational allegiance.
âą While acknowledging distinct employer and employee perspectives, there was an aspiration to be impartial and a commitment to fairness. This was most unambiguously expressed in Germany and Great Britain. In France, deliberations were sometimes reported as resembling a negotiation between employee and employer lay judges, but one that had to culminate in a legally correct judgment. Very few employee lay judge interviewees reported that they experienced enduring dissonance between sitting on the employee side in the court and their role as a lay judge, although this was noted by several at the outset.
âą Our interview findings from lay and professional judges indicated that lay judges bring distinctive knowledge. Some of this knowledge is explicit and often specific. Lay knowledge is often tacit, however, and acquired through long exposure to workplace events. Such knowledge was valued by many professional judge interviewees as adding an extra dimension to decision-making and was seen by near-ly all our interviewees as the main contribution of lay judges to the judicial process. Crucially tacit knowledge is a form of understanding that needs to be elicited in the process of deliberations, rather than as evidence provided by an expert witness. As well as bringing knowledge to the court, lay judges also reported that they could enhance their representational and personal-professional skills by transferring knowledge and experience acquired in court back to the work-place.
âą Gender played some role in the motivation to become a lay judge in Great Britain and in how lay judges assessed their contribution in Germany. Some female interviewees in Britain reported that work-place problems they had personally experienced had contributed to their motivation to become a lay judge. In Germany, there were some differences between men and women in their views of the nature of their contribution: whereas men tended to emphasise the specialist knowledge they could bring to bear in deliberations, women highlighted a âsocial perspectiveâ
Vitamin D Deficiency in Adult Patients with Schizophreniform and Autism Spectrum Syndromes: A One-Year Cohort Study at a German Tertiary Care Hospital
Introduction: Vitamin D has many immunomodulatory, anti-inflammatory, and neuroprotective functions, and previous studies have demonstrated an association between vitamin D deficiency and neuropsychiatric disease. The aim of our study was to analyze the prevalence of vitamin D deficiency in a one-year cohort of adult inpatients with schizophreniform and autism-spectrum syndromes in a naturalistic in-patient setting in Germany. Participants and methods: Our study was comprised of 60 adult schizophreniform and 23 adult high-functioning autism spectrum patients who were hospitalized Page: 2between January and December of 2015. We compared our findings with a historical German reference cohort of 3,917 adults using Pearsonâs two-sided chi-squared test. The laboratory measurements of 25-hydroxyvitamin D2/3 (25(OH)vitamin D) were obtained using a chemiluminescence immunoassay. Results: In the schizophreniform group, we found decreased ( 30 ng/ml were observed in only 5% of the schizophreniform patients, 8.7% of the autism spectrum patients, and 21.9% of the healthy controls. Discussion: We found very high rates of 25(OH)vitamin D deficiency in both patient groups, and have discussed whether our findings might be related to alterations in the immunological mechanisms. Irrespective of the possible pathophysiological links between vitamin D deficiency and schizophrenia or autism spectrum disorders, a more frequent measurement of vitamin D levels seems to be justified in these patient groups. Further prospective, controlled, blinded, and randomized research should be conducted to analyze the effectiveness of vitamin D supplementation on the improvement of psychiatric symptoms
Development of an in-vitro model for extracorporeal blood pumps to study the effects of artificial pulsatility on human blood
IntroductionThe application of extracorporeal circulation (ECC) systems is known to be associated with several implications regarding hemolysis, inflammation, and coagulation. In the last years, systems with pulsatile blood flow are increasingly used with the intention to improve hemodynamics in reperfusion. However, their implications on the aforementioned aspects remain largely unknown. To investigate the effects of pulsatility, this ex-vivo study was initiated.MethodsTest circuits (primed with human whole blood) were set up in accordance with the recommendations of international standards for in-vitro evaluation of new components and systems of ECC. Diagonal pumps were either set up with non-pulsatile (nâ=â5, NPG) or pulsatile (nâ=â5, PG) pump settings and evaluated for 6 h. All analyses were conducted with human whole blood. Blood samples were repeatedly drawn from the test circuits and analyzed regarding free hemoglobin, interleukin 8 (IL-8), platelet aggregation and acquired von Willebrand syndrome (AVWS).ResultsAfter 1 h of circulation, a significant coagulation impairment (impaired platelet function and AVWS) was observed in both groups. After 6 h of circulation, increased IL-8 concentrations were measured in both groups (NPG: 0.05â±â0.03âpg./mL, PG: 0.03â±â0.01âpg./mL, pâ=â0.48). Pulsatile pump flow resulted in significantly increased hemolysis after 6 h of circulation (NPG: 37.3â±â12.4âmg/100âL; PG: 59.6â±â14.5âmg/100âL; pâ<â0.05).ConclusionOur results indicate that the coagulative impairment takes place in the early phase of ECC. Pulsatility did not affect the occurrence of AVWS ex-vivo. Prolonged durations of pulsatile pump flow led to increased hemolysis and therefore, its prolonged use should be employed cautiously in clinical practice with appropriate monitoring
Goettingen Minipigs (GMP): Comparison of Two Different Models for Inducing Diabetes
Purpose: Preclinical experiments on large animals are indispensable for evaluating the effectiveness of diabetes therapies. Miniature swine are well suited for such studies due to their physiological and pathophysiological responses. Methods: We compare two methods for inducing diabetes in Goettingen minipigs (GMP), in five with the beta cell toxin streptozotocin (STZ) and in five other GMP by total pancreatectomy (PE). Glucose homeostasis was assessed with the intravenous glucose-tolerance test (IVGTT) and continual monitoring of interstitial glucose levels. At conclusion of the observation period, the pancreata were examined histologically. Three non-diabetic GMP served as control group. Results: The IVGTT revealed markedly diabetic profiles in both GMP groups. STZ-GMP were found to harbor residual C-peptides and scattered insulin-positive cells in the pancreas. PE-GMP survived the total pancreatectomy only with intensive postoperative care. Conclusions: Although both methods reliably induced diabetes in GMP, the PE-GMP clearly had more health problems and required a greater expenditure of time and resources. The PE-GMP model, however, was better at eliminating endogenous insulin and C-peptide than the STZ-GMP model

The Use of Corneoscleral Grafts to Maintain Tectonic Stability in Severe Keratolysis
Severe corneal ulcerations, causing major keratolysis with large perforation of the cornea or extending to the limbal region, are an ophthalmic emergency. In these cases, a larger corneoscleral graft can be transplanted to restore tectonic integrity, alleviate pain, save vision, and prevent loss of the eye. Chart review of 34 patients with a corneoscleral graft â„9.5 mm was conducted. Primary endpoints of the study were tectonic stability defined as no need for another keratoplasty or enucleation. In addition, visual acuity, postoperative complications, and secondary procedures were analyzed. In total, 12 patients (35%) were female. The mean age at transplantation was 65 ± 19 years. The underlying disease was a perforated infectious corneal ulcer in 30 cases (88%). Mean follow up was 675 ± 789 days. Tectonic stability at the end of the follow-up was maintained with a probability of 56% in a KaplanâMeier analysis. Another penetrating keratoplasty was necessary in six cases (17%) and enucleation in five cases (15%). A corneoscleral transplant remains a viable treatment option to prevent enucleation in severe keratolysis. In our study, this was possible in about half of the cases. Postoperative complications, secondary surgeries, and markedly reduced visual acuity put the advantages into perspective

Objectives: Assessment of children's laboratory test results requires consideration of the extensive changes that occur during physiological development and result in pronounced sex- and age-specific dynamics in many biochemical analytes. Pediatric reference intervals have to account for these dynamics, but ethical and practical challenges limit the availability of appropriate pediatric reference intervals that cover children from birth to adulthood. We have therefore initiated the multi-center data-driven PEDREF project Next-Generation Pediatric Reference Intervals) to create pediatric reference intervals using data from laboratory information systems. Methods: We analyzed laboratory test results from 638,683 patients (217,883-982,548 samples per analyte, a median of 603,745 test results per analyte, and 10,298,067 test results in total) performed during patient care in 13 German centers. Test results from children with repeat measurements were discarded, and we estimated the distribution of physiological test results using a validated statistical approach (kosmic). Results: We report continuous pediatric reference intervals and percentile charts for alanine transaminase, aspartate transaminase, lactate dehydrogenase, alkaline phosphatase, y-glutamyl-transferase, total protein, albumin, creatinine, urea, sodium, potassium, calcium, chloride, anorganic phosphate, and magnesium. Reference intervals are provided as tables and fractional polynomial functions (i.e., mathematical equations) that can be integrated into laboratory information systems. Additionally, Z-scores and percentiles enable the normalization of test results by age and sex to facilitate their interpretation across age groups. Conclusions: The provided reference intervals and percentile charts enable precise assessment of laboratory test results in children from birth to adulthood. Our findings highlight the pronounced dynamics in many biochemical analytes in neonates, which require particular consideration in reference intervals to support clinical decision making most effectively
