    Efficient Method of Removing the Noise using High Dynamic Range Image

    Various tone mapping methods have been proposed to make the image better concurrent to human visual observation. In general, tone mapping can also be carried in nearby and/or world features. In this work, a progressive tone mapping framework such as wavelet filter and spongy thresholding is proposed to diminish the noise this 4% quicker than may shrink process. It's one of a kind curve centered universal tone mapping methods that increase the bright and darkish regions. Peak Signal Noise Ratio (PSNR) value is calculated to know the enrichment value.     Use of Cone-Beam Computed Tomography in the Diagnosis and Treatment of an Unusual Canine Abnormality

    Diagnosis and treatment planning are important for successful endodontic treatment. We report a 24-year old male who presented to the Government Dental College in Kozhikode, Kerala, India, in 2015 with pain in his right upper canine. A digital periapical radiograph indicated the presence of a supernumerary tooth superimposing the root of the canine. However, cone-beam computed tomography (CBCT) confirmed that the supernumerary tooth was an illusion and that the canine root had a sharp invagination involving the labial and pulpal dentin surfaces, with evidence of periapical bone destruction. A blunt resection was performed at the level of the invagination and the resected end was filled with a dentin substitute. At a one-year follow-up, the patient was asymptomatic and the periapical region appeared to be healing well. This report highlights the importance of CBCT in visualising abnormal canine morphology, thus allowing appropriate endodontic treatment

    Best chirplet chain: near-optimal detection of gravitational wave chirps

    The list of putative sources of gravitational waves possibly detected by the ongoing worldwide network of large scale interferometers has been continuously growing in the last years. For some of them, the detection is made difficult by the lack of a complete information about the expected signal. We concentrate on the case where the expected GW is a quasi-periodic frequency modulated signal i.e., a chirp. In this article, we address the question of detecting an a priori unknown GW chirp. We introduce a general chirp model and claim that it includes all physically realistic GW chirps. We produce a finite grid of template waveforms which samples the resulting set of possible chirps. If we follow the classical approach (used for the detection of inspiralling binary chirps, for instance), we would build a bank of quadrature matched filters comparing the data to each of the templates of this grid. The detection would then be achieved by thresholding the output, the maximum giving the individual which best fits the data. In the present case, this exhaustive search is not tractable because of the very large number of templates in the grid. We show that the exhaustive search can be reformulated (using approximations) as a pattern search in the time-frequency plane. This motivates an approximate but feasible alternative solution which is clearly linked to the optimal one. [abridged version of the abstract]Comment: 23 pages, 9 figures.     Knowledge and perception about generic drugs in patients coming to OPD of tertiary care centre

    Background: In an era of rapidly rising health care costs generic medicines provide a less expensive alternative to branded medicines. In addition to reducing the overall health care expenditure, it has been shown to improve adherence. Objective was to study knowledge and perception about generic drugs among patients coming to outpatient department of tertiary care centre.Methods: After ethical approval a cross sectional questionnaire based study was conducted. Patients (n=71) were interviewed according to questionnaire in vernacular language by investigator to fill questionnaire.Results: About 28% people think that price of generic drug is less than a branded drug while nearly 61% of people don’t know of it. Only 18.85% participants had taken generic medicine. Trusting efficacy of generic drugs only 30 participants were in favour it. Even they have not seen or heard publicity of generic drugs (61.97%). They (60.56%) opined that generic drugs never prescribe in our country.Conclusions: Limitation in knowledge and perception about generic medicines has been seen among participants

    Assessing intravenous midazolam for conscious sedation during cataract surgery

    Background: A large number of geriatric populations above the age of 50 worldwide suffer from cataract. Cataract starts with short-sightedness and gradually worsens resulting in blurring of vision and inability to visualize and distinguish fine details. Surgery is the only available treatment for cataract. Anaesthesia is essential during cataract surgery to minimize pain caused during surgical procedure and to achieve favourable surgical outcome. The current investigation was aimed towards assessing the performance of intravenous midazolam used for conscious sedation during cataract surgery along with retrobulbar block.Methods: Current study is a randomized double blinded study performed for duration of 6 months on 60 patients undergoing cataract surgery at Terna medical college and hospital. Patients were divided in two groups; group M received 0.02 mg/kg midazolam diluted to 5 ml, group N received 5 ml normal saline before cataract surgery. All vital hemodynamic parameters were observed after 5 minutes of sedation, immediately after block administration and after every 15 minutes till the end of the surgery to assess the effect of sedation. Patients and surgeons satisfaction levels were also documented post-surgery.Results: Patients who were sedated with midazolam prior to cataract surgery along with block exhibited a significant decrease in hemodynamic parameters like SAP, DAP and heart rate which indicated effective sedation. Anxiety level also significantly decreased in the patients who received midazolam. No major adverse or intra-operative events were observed in the patients who received midazolam.Conclusions: Sedation with midazolam provides haemodynamic conditions favourable for cataract surgery along with high level of patient and surgeon satisfaction

    Knowledge attitude and practice of haemovigilance among doctors in tertiary care hospital in Nagpur, Maharashtra, India

    Background: Haemovigilance program of India was launched on December 2012 and is a comprehensive, and a well-structured approach to collect, collate, and analyse data to address the issues of adverse reactions associated with blood transfusion. The information thus collected would facilitate corrective and preventive actions to be taken to minimize the potential risk associated blood collection processing and transfusion to patients. Considering the deep concern over the under-reporting of transfusion reactions prevailing among the doctors, the present study was done to know the knowledge attitude and practice (KAP) of haemovigilance among doctors.Methods: The present study was a cross sectional questionnaire based study, which included doctors of a tertiary care hospital. The purpose of the questionnaire was to know awareness of doctors regarding haemovigilance programme of India. With this study we also tried to   identify the factors responsible for underreporting of transfusion reactions and to find out the possible ways to improve reporting of transfusion reactions.Results: (38.88%) and (30%) of the responders were aware of the haemovigilance programme and transfusion reaction reporting centre respectively. Reporting of transfusion reaction was poor 22.22% among the respondents. According to respondents creating awareness about haemovigilance by conducting continuing medical education (CMEs), and   training to healthcare professionals would lead to improvement in reporting of transfusion reactions. Complacency and ignorance were the main factors which discouraged transfusion reaction reporting by doctors.Conclusions: Increasing awareness of haemovigilance among doctors and training on reporting transfusion reactions would likely improve spontaneous reporting and help to strengthen the blood transfusion system

    Relating field-induced shift in transition temperature to the kinetics of coexisting phases in magnetic shape memory alloys

    In a magnetic shape memory alloy system, we vary composition following phenomenological arguments to tune macroscopic properties. We achieve significantly higher shift in austenite to martensitic phase transition temperature with magnetic field. This enhancement is accompanied by significant broadening of the transition and by field-induced arrest of kinetics, both of which are related to the dynamics of the coexisting phases. This reveals hitherto unknown interrelationship between different length-scales.     Vitamin D: prescription audit in tertiary health care centre

    Background: Vitamin-D is critically important for development, growth and maintenance of health of human being. Many evidences show association between vit-D deficiency and several serious health conditions. Data collection on use of drugs is being obtained with the aim of optimizing drug therapy. So far till date only few studies about prescription pattern of vit-D have been found. Hence it is very important to do audit of prescriptions of vit-D. The aim of the study is to provide concise and updated information about the use of vit-D in tertiary care hospital and to record demographic details of patients.Methods: After ethical approval, this cross sectional study was conducted at tertiary health centre. Patient and drug data (name of drug, dose, dosage form and route of administration) was collected from the patient’s prescriptions in OPD.Results: Highest prescriptions of vit-D were found in orthopedic department (22% of total prescriptions of that department). Prescribing percentage of vit-D in medicine department was 4.6%, dermatology 1.5% and in psychiatry 0.8% of total prescriptions. Prescriptions of vit-D in combination with calcium were found in orthopedics (52%), medicine (7%) and obstetrics and gynecology (10%) departments.Conclusions: Highest prescriptions of vit-D alone and with calcium found in orthopedic department. Periodic therapeutic audit is necessary to rationalise the use of vit D

    Hierarchical Approach for Total Variation Digital Image Inpainting

    Abstract The art of recovering an image from damage in an undetectable form is known as inpainting. The manual work of inpainting is most often a very time consuming process. Due to digitalization of this technique, it is automatic and faster. In this paper, after the user selects the regions to be reconstructed, the algorithm automatically reconstruct the lost regions with the help of the information surrounding them. The existing methods perform very well when the region to be reconstructed is very small, but fails in proper reconstruction as the area increases. This paper describes a Hierarchical method by which the area to be inpainted is reduced in multiple levels and Total Variation(TV) method is used to inpaint in each level.
