Viscoelastic testing reveals normalization of the coagulation profile 12Â weeks after severe COVID-19
COVID 19 is associated with a hypercoagulable state and frequent thromboembolic complications. For how long this acquired abnormality lasts potentially requiring preventive measures, such as anticoagulation remains to be delineated. We used viscoelastic rotational thrombelastometry (ROTEM) in a single center cohort of 13 critical ill patients and performed follow up examinations three months after discharge from ICU. We found clear signs of a hypercoagulable state due to severe hypofibrinolysis and a high rate of thromboembolic complications during the phase of acute illness. Three month follow up revealed normalization of the initial coagulation abnormality and no evidence of venous thrombosis in all thirteen patients. In our cohort the coagulation profile was completely normalized three months after COVID-19. Based on these findings, discontinuation of anticoagulation can be discussed in patients with complete venous reperfusion
Semen amyloids participate in spermatozoa selection and clearance
Unlike other human biological fluids, semen contains multiple types of amyloid fibrils in the absence of disease. These fibrils enhance HIV infection by promoting viral fusion to cellular targets, but their natural function remained unknown. The similarities shared between HIV fusion to host cell and sperm fusion to oocyte led us to examine whether these fibrils promote fertilization. Surprisingly, the fibrils inhibited fertilization by immobilizing sperm. Interestingly, however, this immobilization facilitated uptake and clearance of sperm by macrophages, which are known to infiltrate the female reproductive tract (FRT) following semen exposure. In the presence of semen fibrils, damaged and apoptotic sperm were more rapidly phagocytosed than healthy ones, suggesting that deposition of semen fibrils in the lower FRT facilitates clearance of poor-quality sperm. Our findings suggest that amyloid fibrils in semen may play a role in reproduction by participating in sperm selection and facilitating the rapid removal of sperm antigens

Structure, function and antagonism of semen amyloids
Amyloid fibrils are linear polypeptide aggregates with a cross-β structure. These fibrils are best known for their association with neurodegenerative diseases, such as Alzheimer's or Parkinson's, but they may also be used by living organisms as functional units, e.g. in the synthesis of melanin or in the formation of bacterial biofilms. About a decade ago, in a search for semen factors that modulate infection by HIV-1 (a sexually transmitted virus and the causative agent of the acquired immune deficiency syndrome (AIDS)), it was demonstrated that semen harbors amyloid fibrils capable of markedly increasing HIV infection rates. This discovery not only created novel opportunities to prevent sexual HIV-1 transmission but also stimulated research to unravel the natural role of these factors. We discuss here the identification of these intriguing structures, their molecular properties, and their effects on both sexually transmitted diseases and reproductive health. Moreover, we review strategies to antagonize semen amyloid to prevent sexual transmission of viruses
A scalable process for the generation of durable easy-to-clean coatings on foils
The cleaning of decorative surfaces, e.g., on furniture or in automotive industry, is a time-consuming process with high maintenance costs that partly requires special cleaning chemicals as well as a laborious manual effort. To ease the cleaning process, easy-to-clean coatings are intensively discussed and an enormous demand of this surface functionalization in different fields of applications is forecasted by all industrial branches. Therefore, a novel and durable easy-to-clean coating based on a UV-curable clearcoat with incorporated SiO2 particles and a thin functional polymer coating based on a zwitterionic phosphorylcholine was developed. The coating can be applied on PVC foils as well as PET foils and subsequently transferred to different decorative surfaces, e.g., via lamination or bonding. The developed three-step application process consists of a lacquering step with UV curing, a plasma surface modification, and the deposition of a thin functional polymer layer. It can be easily integrated in industrial printing or roll-to-roll processes, which are normally used for foil functionalization. With the presented coating, it is possible to remove oil impurities from the functionalized foil by pure water without the necessity of cleaning surfactants. Furthermore, the thermal, chemical, and mechanical durability of the coating will be discussed
Thromboembolic complications in critically ill COVID-19 patients are associated with impaired fibrinolysis
Background: There is emerging evidence for enhanced blood coagulation in coronavirus 2019 (COVID-19) patients, with thromboembolic complications contributing to morbidity and mortality. The mechanisms underlying this prothrombotic state remain enigmatic. Further data to guide anticoagulation strategies are urgently required.
Methods: We used viscoelastic rotational thromboelastometry (ROTEM) in a single-center cohort of 40 critically ill COVID-19 patients.
Results: Clear signs of a hypercoagulable state due to severe hypofibrinolysis were found. Maximum lysis, especially following stimulation of the extrinsic coagulation system, was inversely associated with an enhanced risk of thromboembolic complications. Combining values for maximum lysis with D-dimer concentrations revealed high sensitivity and specificity of thromboembolic risk prediction.
Conclusions: The study identifies a reduction in fibrinolysis as an important mechanism in COVID-19-associated coagulopathy. The combination of ROTEM and D-dimer concentrations may prove valuable in identifying patients requiring higher intensity anticoagulation
Erhöhte Reaktivität Herzschlag-evozierter Potenziale während der Aufmerksamkeits- fokussierung auf den eigenen Herzschlag und erhöhte kardiovaskuläre Aktivität bei Personen mit hoher somatoformer Symptombelastung
Die psychobiologischen Mechanismen der Entstehung körperlicher Symptome bei somatoformen Störungen sind weitgehend
unklar. Im Perception-Filter-Modell wird postuliert, dass (1.) verstärkte Signalübermittlung von Körperorganen
und (2.) verringerte Filterfunktion im ZNS zu (3.) verstärkter Wahrnehmung von Symptomen führt. Bislang existiert jedoch
keine Evidenz, die diesen Zusammenhang an einer von somatoformen Symptomen belasteten Population unterstĂĽtzt.
Um diesen Sachverhalt aufzudecken, wurden zunächst 486 Personen aus der Allgemeinbevölkerung anhand einer Online-
Version des Screenings für Somatoforme Störungen (SOMS-2) anhand des oberen und unteren Centils in Personen mit
hoher (HSB) und niedriger somatoformer Symptombelastung (NSB) geteilt. Jeweils 29 NSB- (21 Frauen; 26,0 J.) und HSBPersonen
(27 Frauen; 28,4 J.) wurden im Labor anhand (1.) verstärkter Körpersignale hinsichtlich kardiovaskulärer Aktivität
und Cortisol-Aufwachreaktion, sowie (2.) Filterfunktionen im ZNS hinsichtlich Herzschlag-evozierter Potentiale (HEPs)
während der Durchführung eines Herzschlagwahrnehmungstests und während Ruhe untersucht. HEPs reflektieren die
Repräsentation kardio-afferenter Signale im ZNS. Die HSB-Gruppe zeigte eine höhere Ruhe-Herzrate (72,7 vs. 66,8 bpm)
und eine geringere Herzratenvariabilität (RMSSD: 36,8 vs. 52,5 ms) als die LSB-Gruppe. Außerdem zeigten die HSB-Personen
bei der Aufmerksamkeitsfokussierung auf den eigenen Herzschlag eine höhere Reaktivität der HEP-Amplitude als
die LSB-Personen. Es gab keine Gruppenunterschiede hinsichtlich der Cortisol-Aufwachreaktion. Die Ergebnisse belegen
sowohl (1.) die verstärkte Signalübermittlung von Körperorganen, als auch (2.) verringerte Filterfunktionen bei Personen
mit hoher somatoformer Symptombelastung. Dies spricht für die Gültigkeit des Perception-Filter-Modells. Die erhöhte
HEP-Reaktivität bezüglich der Aufmerksamkeitsfokussierung auf Körperprozesse könnte einen Biomarker für Personen
mit hohem Risiko für die Entwicklung von somatoformen Störungen darstellen
Kardiale Modulation der Schreckreaktion bei hoher gegenüber niedriger Symptombelastung: afferente Signalübermittlung auf der Hirn-Körper-Achse beein usst frühe Stimulus-Verarbeitung bei hoher Symptombelastung
Somatische Belastungsstörungen (SBS) sind oftmals durch andauernde medizinisch-
unerklärte Symptome gekennzeichnet, deren Entstehung größtenteils
ungeklärt ist. Diese Studie hatte zum Ziel, die empirisch bislang unbeantwortete
Frage zu klären, ob Symptomentstehung auf veränderte Signalübermittlung
auf der Hirn-Körper-Achse zurückzuführen ist. Zunächst wurden 486
Personen aus der Allgemeinbevölkerung anhand des SOMS-2 in Personen
mit hoher Symptombelastung (HSB; unterstes Perzentil) und niedriger Symptombelastung
(NSB; oberstes Perzentil) unterteilt. Personen mit HSB stellen
eine besondere Risikogruppe fĂĽr SBS dar. 28 HSB- und 31 NSB-Personen
durchliefen ein Paradigma der kardialen Modulation der Schreckreaktion
(CMS), ein Verfahren, das kardio-afferente Signalübermittlung prä-attentiv
abbilden kann. Ihnen wurden je zehn akustische Schreckreize (105 dB) in
sechs Zeitpunkten nach der kardialen R-Zacke (0, 100, 200, 300, 400, 500
ms) präsentiert. Als Indikator für die Schreckreaktion wurden die N1- und
P2-Amplitude der auditorisch-evozierten Potenziale ĂĽber Cz gemessen, da
der Effekt der Hirn-Körper-Signalübermittlung im Kortex abgebildet werden
sollte. Es zeigten sich geringere N1-Amplituden auf die Schreckreize,
die während der systolischen Phase (200, 300 ms) im Vergleich zur diastolischen
Phase (0 ms) präsentiert wurden (p=.0002), was für das Vorliegen eines
CMS-Effekts spricht. Die HSB-Gruppe zeigte höhere P2-Amplituden als die
NSB-Gruppe. Es zeigte sich auĂźerdem, dass der CMS-Effekt bezĂĽglich der
N1-Komponente in der HSB-Gruppe geringer ist (p=.035), jedoch bezĂĽglich
der P2-Komponente stärker ist als in der NSB-Gruppe (p=.031). Afferente
Signalübermittlung auf der Hirn-Körper-Achse könnte bei Personen mit HSB
bereits frühe, automatischeWahrnehmungsprozesse verändern, die durch späte,
aufmerksamkeits-gesteuerte Prozesse kompensiert werden. Diese prä-attentive
Beeinflussung der Stimulus-Verarbeitung könnte ein Mechanismus der Symptomentstehung
bei Personen mit HSB und SBS sein
Storage-Dependent Generation of Potent Anti-ZIKV Activity in Human Breast Milk
Zika virus (ZIKV) causes congenital neurologic birth defects, notably microcephaly, and has been associated with other serious complications in adults. The virus has been detected in human breast milk and possible transmissions via breastfeeding have been reported. Breast milk is rich in nutrients and bio-active substances that might directly affect viral infectivity. Thus, here, we analyzed the effect of human breast milk on ZIKV infection. We observed that fresh human breast milk had no effect on ZIKV, but found that upon storage, milk effectively suppressed infection. The antiviral activity is present in the fat-containing cream fraction of milk and results in the destruction of the structural integrity of viral particles, thereby abrogating infectivity. The release of the factor is time dependent but varies with donors and incubation temperatures. The viral titer of milk that was spiked with ZIKV decreased considerably upon storage at 37 °C for 8 h, was lost entirely after 2 days of 4 °C storage, but was not affected at −20 °C. This suggests that cold storage of milk inactivates ZIKV and that the antiviral factor in milk may also be generated upon breastfeeding and limit this transmission route of ZIKV
