    Fear of Fatness Among Young Irish Adolescents : Association with Obesity, Reported Iron and Folate Intake, and Dental Health

    OBJECTIVE: To establish the prevalence of fear of fatness (the desire to be thinner) and associated factors, including obesity, reported energy, iron and folate intakes. The difficulties in defining overweight among Irish children and an assessment of dietary and socio-economic factors related to dental caries and also reported in the group. METHODS: A one-year follow up study of 251 healthy schoolchildren at baseline (119 boys and 132 girls; mean age 11 years) attending 7 fee-paying (6 single- and 1-mixed sex) and 8 non-fee-paying (4 single- and 4 mixed-sex) schools in Dublin city center. Assessment of body image perceptions and satisfaction (using figure line drawings) and toothbrushing habits were reported in questionnaires specifically designed for this study. Nutrient intakes and the amount and frequency of cariogenic foods were estimated from two-day food diaries kept on two occasions over a three-week period. Weight, height, triceps and subscapular skinfolds were measured and used in several definitions of overweight, which include Body Mass Index and Actual Relative Weight cut-offs. RESULTS: Significantly more girls than boys (39% vs. 17%) and more overweight children were affected by fear of fatness. Girls were twice as likely as boys to report trying to lose weight (42% vs. 20% at baseline, and 41% vs. 21% at follow up). Overweight (BMI≥26kg/m2) has increased 5-fold among boys and 2.5-fold among girls over the last ten years. The prevalence of overweight differed by 9% at baseline and 8% as follow up depending upon which method of weight status was used. A high prevalence of under-reporting energy intake (39% at baseline and 50% at follow up) was found and associated with a desire to be thinner and with being overweight. Further, a substantial number of schoolchildren, especially girls, reported iron and folate intakes below the estimated average requirements but low tissue stores of iron (7% of boys and 8% of girls at baseline; 3% of boys and 15% of girls at follow up and folate (0% of boys and 1% of girls at baseline; nobody at follow up) did not relate to the reported low intakes. Social disadvantage emerged as a more important predictor than dietary factors of low iron stores among girls only and of high dental caries among all children. CONCLUSIONS: Fear of fatness was particularly prevalent among young Irish adolescent girls and was found to be associated with systematic under-reporting of energy intakes. The prevalence of overweight has increased dramatically over the past ten years and a standard method of assessment is urgently needed for evaluation of obesity prevention initiatives among Irish schoolchildren. The discrepancy between apparently low iron and folate intakes and sufficient actual tissue stores highlights the importance of including an independent measure of reported energy validity when assessing the reported diets of adolescents. Nonetheless, those children with low iron stores are likely to have reduced concentration levels adversely affecting academic progress. They are also at risk of iron deficiency anaemia, thus representing a serious public health concern

    Superficial Soft-Tissue Sarcomas Rarely Require Advanced Soft-Tissue Reconstruction Following Resection

    Objective: Soft-tissue sarcomas are most frequently located deep within myofascial compartments. Superficial soft-tissue sarcomas (S-STS) are relatively less common and may be managed differently than deep sarcomas because generous resection margins are often possible without sacrificing critical structures. We sought to investigate the frequency and types of soft-tissue reconstructive procedures that are required following excision of S-STS. Methods: We reviewed 457 consecutively treated patients with S-STS with a minimum 2-year follow-up from our prospectively maintained database between 1989 and 2009. Results: Mean follow-up was 10.5 years (range, 2–23). Four hundred twenty-one tumors (91%) were excised with negative margins, 38 (8.3%) had microscopically positive margins, and three (0.7%) had grossly positive margins. One patient required an amputation. In 271 (58%) patients, the wounds were closed primarily. In comparison, 93 patients (20%) required a rotation flap, 70 (15%) required a split-thickness skin graft, and 23 (5%) underwent a free tissue transfer (ie, advanced reconstructive procedure). The overall complication rate was 12%, although 43% of patients undergoing free tissue transfer developed complications (P = 0.04). An unplanned excision before referral to our center was a risk factor for local recurrence (P = 0.03) when residual tumor was recovered in the reexcision specimen pathologically. Conclusions: Although concern about the morbidity associated with a free tissue transfer (ie, advanced reconstructive procedure) may potentially limit the adequacy of resection in some patients with S-STS, the results of this study showed that the majority of patients had complete excisions with negative margins and primary closure. Obtaining a negative margin when excising a known or suspected S-STS rarely requires an advanced reconstructive procedure and almost never results in loss of limb

    Self-Cleaning Boudouard Reactor for Full Oxygen Recovery from Carbon Dioxide

    Oxygen recovery from respiratory carbon dioxide is an important aspect of human spaceflight. Methods exist to sequester the carbon dioxide, but production of oxygen needs further development. The current International Space Station Carbon Dioxide Reduction System (CRS) uses the Sabatier reaction to produce water (and ultimately breathing air). Oxygen recovery is limited to 50% because half of the hydrogen used in the Sabatier reactor is lost as methane, which is vented overboard. The Bosch reaction, which converts carbon dioxide to oxygen and solid carbon is capable of recovering all the oxygen from carbon dioxide, and is the only real alternative to the Sabatier reaction. However, the last reaction in the cycle, the Boudouard reaction, produces solid carbon and the resulting carbon buildup will eventually foul the nickel or iron catalyst, reducing reactor life and increasing consumables. To minimize this fouling and increase efficiency, a number of self-cleaning catalyst designs have been created. This paper will describe recent results evaluating one of the designs

    The use of HaloTag-based technology in flow and laser scanning cytometry analysis of live and fixed cells

    <p>Abstract</p> <p>Background</p> <p>Combining the technologies of protein tag labeling and optical microscopy allows sensitive analysis of protein function in cells.</p> <p>Findings</p> <p>Here, we describe development of applications using protein tag technology (HaloTag (HT)-based) for flow and laser scanning cytometry (LSC). Cell lines, expressing recombinant surface β1-integrin-HT and HT-p65 fusion protein, and a CD4 T cell line (Jurkat) infected with human immunodeficiency virus type 1 (HIV-1) reporter virus expressing the unfused HT (HIV-1<sub>Lai-Halo</sub>), were stained with different HT ligands and successfully detected by flow cytometers equipped with 488 and 561 nm lasers as well as a laser scanning cytometer (equipped with 488 and 405 nm lasers) alone or combined with cell cycle and viability markers.</p> <p>Conclusions</p> <p>Use of HT technology for cytometric applications has advantages over its use in microscopy as it allows for the statistical measurement of protein expression levels in individual cells within a heterogeneous cell population in combination with cell cycle analysis. Another advantage is the ability of the HaloTag to withstand long fixation and high concentration of fixative, which can be useful in research of infectious agents like HIV and/or mycobacteria.</p

    Nox4 reprograms cardiac substrate metabolism via protein O-GlcNAcylation to enhance stress adaptation.

    Cardiac hypertrophic remodeling during chronic hemodynamic stress is associated with a switch in preferred energy substrate from fatty acids to glucose, usually considered to be energetically favorable. The mechanistic interrelationship between altered energy metabolism, remodeling, and function remains unclear. The ROS-generating NADPH oxidase-4 (Nox4) is upregulated in the overloaded heart, where it ameliorates adverse remodeling. Here, we show that Nox4 redirects glucose metabolism away from oxidation but increases fatty acid oxidation, thereby maintaining cardiac energetics during acute or chronic stresses. The changes in glucose and fatty acid metabolism are interlinked via a Nox4-ATF4-dependent increase in the hexosamine biosynthetic pathway, which mediates the attachment of O-linked N-acetylglucosamine (O-GlcNAcylation) to the fatty acid transporter CD36 and enhances fatty acid utilization. These data uncover a potentially novel redox pathway that regulates protein O-GlcNAcylation and reprograms cardiac substrate metabolism to favorably modify adaptation to chronic stress. Our results also suggest that increased fatty acid oxidation in the chronically stressed heart may be beneficial

    Cytosine-5 RNA methylation links protein synthesis to cell metabolism.

    Posttranscriptional modifications in transfer RNA (tRNA) are often critical for normal development because they adapt protein synthesis rates to a dynamically changing microenvironment. However, the precise cellular mechanisms linking the extrinsic stimulus to the intrinsic RNA modification pathways remain largely unclear. Here, we identified the cytosine-5 RNA methyltransferase NSUN2 as a sensor for external stress stimuli. Exposure to oxidative stress efficiently repressed NSUN2, causing a reduction of methylation at specific tRNA sites. Using metabolic profiling, we showed that loss of tRNA methylation captured cells in a distinct catabolic state. Mechanistically, loss of NSUN2 altered the biogenesis of tRNA-derived noncoding fragments (tRFs) in response to stress, leading to impaired regulation of protein synthesis. The intracellular accumulation of a specific subset of tRFs correlated with the dynamic repression of global protein synthesis. Finally, NSUN2-driven RNA methylation was functionally required to adapt cell cycle progression to the early stress response.     The HAND-Q : Psychometrics of a New Patient-reported Outcome Measure for Clinical and Research Applications

    Background: The perspective of the patient in measuring the outcome of their hand treatment is of key importance. We developed a hand-specific patient-reported outcome measure to provide a means to measure outcomes and experiences of care from the patient perspective, that is, HAND-Q. Methods: Data were collected from people with a broad range of hand conditions in hand clinics in six countries between April 2018 and January 2021. Rasch measurement theory analysis was used to perform item reduction and to examine reliability and validity of each HAND-Q scale. Results: A sample of 1277 patients was recruited. Participants ranged in age from 16 to 89 years, 54% were women, and a broad range of congenital and acquired hand conditions were represented. Rasch measurement theory analysis led to the refinement of 14 independently functioning scales that measure hand appearance, health-related quality of life, experience of care, and treatment outcome. Each scale evidenced reliability and validity. Examination of differential item functioning by age, gender, language, and type of hand condition (ie, nontraumatic versus traumatic) confirmed that a common scoring algorithm for each scale could be implemented. Conclusions: The HAND-Q was developed following robust psychometric methods to provide a comprehensive modular independently functioning set of scales. HAND-Q scales can be used to assess and compare evidence-based outcomes in patients with any type of hand condition.Peer reviewe
