    __Abstract__ Accurate diagnosis of young children’s problem behavior is of great importance for understanding the early development of psychopathology and for early and effective interventions. Children, even young children, may offer a valuable complement to information from the traditional adult informants. Yet, in the Netherlands, no validated instrument was available to systematically assess young children’s perspective on their feelings and behavior. To address this gap, the Berkeley Puppet Interview (BPI) was translated into Dutch and introduced in the Generation R Study, a large epidemiological study. The series of studies described in this thesis examined whether information about emotional, behavioral and peer relationship problems, obtained by the Dutch translation of the BPI, provides reliable, valid and valuable information that can be used in research settings. The studies also examined the value of young children’s reports if implemented along with other outcomes in a multi-informant approach

    How competent are healthcare professionals in working according to a bio-psycho-social model in healthcare? : the current status and validation of a scale

    Background : Over the past decades, there has been a paradigm shift from a purely biomedical towards a bio-psycho-social (BPS) conception of disability and illness, which has led to a change in contemporary healthcare. However, there seems to be a gap between the rhetoric and reality of working within a BPS model. It is not clear whether healthcare professionals show the necessary skills and competencies to act according to the BPS model. Objective : The aim of this study was (1) to develop a scale to monitor the BPS competencies of healthcare professionals, (2) to define its factor-structure, (3) to check internal consistency, (4) test-retest reliability and (5) feasibility. Design and Setting : Item derivation for the BPS scale was based on qualitative research with seven multidisciplinary focus groups (n = 58) of both patients and professionals. In a cross-sectional study design, 368 healthcare professionals completed the BPS scale through a digital platform. An exploratory factor analysis was performed to determine underlying dimensions. Statistical coherence was expressed in item-total correlations and in Cronbach's alpha coefficient. An intra-class-correlation coefficient was used to rate the test-retest reliability. Results : The qualitative study revealed 45 items. The exploratory factor analysis showed five underlying dimensions labelled as: (1) networking, (2) using the expertise of the client, (3) assessment and reporting, (4) professional knowledge and skills and (5) using the environment. The results show a good to strong homogeneity (item-total ranged from 0.59 to 0.79) and a strong internal consistency (Cronbach's alpha ranged from 0.75 to 0.82). ICC ranged between 0.82 and 0.93. Conclusion : The BPS scale appeared to be a valid and reliable measure to rate the BPS competencies of the healthcare professionals and offers opportunities for an improvement in the healthcare delivery. Further research is necessary to test the construct validity and to detect whether the scale is responsive and able to detect changes over time

    Novel metaheuristic hybrid spiral-dynamic bacteria-chemotaxis algorithms for global optimisation

This paper presents hybrid spiral-dynamic bacteria-chemotaxis algorithms for global optimisation and their application to control of a flexible manipulator system. Spiral dynamic algorithm (SDA) has faster convergence speed and good exploitation strategy. However, the incorporation of constant radius and angular displacement in its spiral model causes the exploration strategy to be less effective hence resulting in low accurate solution. Bacteria chemotaxis on the other hand, is the most prominent strategy in bacterial foraging algorithm. However, the incorporation of a constant step-size for the bacteria movement affects the algorithm performance. Defining a large step-size results in faster convergence speed but produces low accuracy while de.ning a small step-size gives high accuracy but produces slower convergence speed. The hybrid algorithms proposed in this paper synergise SDA and bacteria chemotaxis and thus introduce more effective exploration strategy leading to higher accuracy, faster convergence speed and low computation time. The proposed algorithms are tested with several benchmark functions and statistically analysed via nonparametric Friedman and Wilcoxon signed rank tests as well as parametric t-test in comparison to their predecessor algorithms. Moreover, they are used to optimise hybrid Proportional-Derivative-like fuzzy-logic controller for position tracking of a flexible manipulator system. The results show that the proposed algorithms significantly improve both convergence speed as well as fitness accuracy and result in better system response in controlling the flexible manipulator

    The Importance of Evaluating Primary Midwifery Care for Improving the Health of Women and Infants

    In most countries, maternal and newborn care is fragmented and focused on identification and treatment of pathology that affects only the minority of women and babies. Recently, a Framework for Quality Maternal and Newborn Care (QMNC) was developed, which encourages a system-level shift to provide skilled care for all. This care includes preventive and supportive care that works to strengthen women’s capabilities and focuses on promotion of normal reproductive processes while ensuring access to emergency treatment when needed. Midwifery care is pivotal in this framework which contains several elements that resonate with the main dimensions of primary care. Primary health care is the first level of contact with the health system where most of the population’s curative and preventive health needs can be fulfilled as close as possible to where people live and work. In this paper, we argue that midwifery as described in the framework requires the application of a primary care philosophy for all childbearing women and infants. Evaluation of the implementation of the framework should therefore include tools to monitor the performance of primary midwifery care

    Exploring the relation of harsh parental discipline with child emotional and behavioral problems by using multiple informants. The generation R study

    Parental harsh disciplining, like corporal punishment, has consistently been associated with adverse mental health outcomes in children. It remains a challenge to accurately assess the consequences of harsh discipline, as researchers and clinicians generally rely on parent report of young children's problem behaviors. If parents rate their parenting styles and their child's behavior this may bias results. The use of child self-report on problem behaviors is not common but may provide extra information about the relation of harsh parental discipline and problem behavior. We examined the independent contribution of young children's self-report above parental report of emotional and behavioral problems in a study of maternal and paternal harsh discipline in a birth cohort. Maternal and paternal harsh discipline predicted both parent reported behavioral and parent reported emotional problems, but only child reported behavioral problems. Associations were not explained by pre-existing behavioral problems at age 3. Importantly, the association with child reported outcomes was independent from parent reported problem behavior.     A novel hybrid bacteria-chemotaxis spiral-dynamic algorithm with application to modelling of flexible systems

    This paper presents a novel hybrid optimisation algorithm namely HBCSD, which synergises a bacterial foraging algorithm (BFA) and spiral dynamics algorithm (SDA). The main objective of this strategy is to develop an algorithm that is capable to reach a global optimum point at the end of the final solution with a faster convergence speed compared to its predecessor algorithms. The BFA is incorporated into the algorithm to act as a global search or exploration phase. The solutions from the exploration phase then feed into SDA, which acts as a local search or exploitation phase. The proposed algorithm is used in dynamic modelling of two types of flexible systems, namely a flexible robot manipulator and a twin rotor system. The results obtained show that the proposed algorithm outperforms its predecessor algorithms in terms of fitness accuracy, convergence speed, and time-domain and frequency-domain dynamic characterisation of the two flexible systems. © 2014 Elsevier Ltd

    Parental depression and child well-being: Young children's self-reports helped addressing biases in parent reports

    Objectives Effects of maternal and paternal depression on child development are typically evaluated using parental reports of child problems. Yet, parental reports may be biased. Methods In a population-based cohort, parents reported lifetime depression (N = 3,178) and depressive symptoms (N = 3,131). Child emotional and behavioral problems were assessed at age 6 years by child self-report using the Berkeley Puppet Interview, by mother report using the Child Behavior Checklist (CBCL), and at age 3 years by father and mother reported CBCLs. Results Both maternal and paternal depression was associated with more child problems. Associations were of similar strength if child problems were obtained by self-reports. However, if parents reported about their own depression or depressive symptoms and about their child's problems, estimates were generally stronger for associations with the reporting parent's depression as the determinant. For instance, if mothers reported child emotional problems, associations were stronger for maternal (B = 0.27; 95% confidence interval (CI) = 0.19, 0.35) than for paternal lifetime depression (B = 0.12; 95% CI = 0.02, 0.21; P-value for difference = 0.02). Conclusion Depression of mothers and fathers affects young children's well-being. However, if parents reported about their own depression and about child problems, associations were inflated. To accurately estimate effects of parental depression, multiple-source data including young children's perspectives must be considered

    Brain morphology of childhood aggressive behavior: A multi-informant study in school-age children

    Objective: Few studies have focused on the neuroanatomy of aggressive behavior in children younger than 10 years. Here, we explored the neuroanatomical correlates of aggression in a population-based sample of 6- to 9-year-old children using a multiple-informant approach. Methods: Magnetic resonance (MR) scans were acquired from 566 children from the Generation R study who participated in the Berkeley Puppet Interview and whose parents had completed the Child Behavior Checklist. Linear regression analyses were used to examine associations between aggression and amygdala and hippocampal volume. We performed surface-based analyses to study the association between aggression and cortical thickness, surface area, and gyrification. Results: Aggressive behavior was associated with smaller amygdala (p <.05) but not hippocampal volume. Aggression was associated with a thinner cortex in the left precentral cortex (p <.01) and in a cluster including the right inferior parietal, supramarginal, and postcentral cortex (p <.001). Gender moderated the association between aggression and cortical thickness in the right medial posterior cortex (p =.001) and the right prefrontal cortex (p <.001). Aggression was associated with decreased gyrification in a large cluster including the right precentral, postcentral, frontal, and parietal cortex (p =.01). Moreover, aggression was associated with decreased gyrification in the right occipital and parietal cortex (p =.02). Conclusion: We found novel evidence that childhood aggressive behavior is related to decreased amygdala volume, decreased sensorimotor cortical thickness, and decreased global right hemisphere gyrification. Aggression is related to cortical thickness in regions associated with the default mode network, with negative associations in boys and positive associations in girls
