    We determined the relationship between plasma and red blood cell concentrations of quinine in children with uncomplicated falciparum malaria from an endemic area of Amazonian region. Quinine was determined by high performance liquid chromatography with ultraviolet detection. In the steady state the ratio between plasma and red blood cell quinine concentration was 1.89 ± 1.25 ranging from 1.05 to 2.34. This result demonstrated that quinine do not concentrate in red blood cell of Brazilian children and characterize the absence of interracial difference in this relationship.Neste estudo foi determinada a relação entre as concentrações plasmáticas e eritrocitárias de quinina em crianças com malária falciparum não complicada, oriundas de área endêmica da Região Amazônica. A quinina foi detrminada por cromatografia líquida de alta eficiência. No estado de equilíbrio, a relação foi 1,89 ± 1,25 variando de 1,05 a 2,34. Estes resultados demonstraram que a quinina não se concentra nos eritrócitos das crianças e caracterizaram a ausência de diferença racial nesta relação

    Dimensão Institucional da Sustentabilidade e Gestão Ambiental no município de Moju, Pará:: uma aplicação do Barômetro da Sustentabilidade

    Few analytical tools have been applied in sustainability studies in Amazonia, especially in theState of Pará. Municipalities in Pará included in the Centers for Biodiesel Production program ofthe Brazilian government, particularly Moju, are undergoing a phase of intensive agroindustrialinvestment for the production of Oil Palm, and of firm insertion of family agriculture in such aprocess. In this context, questions have arisen about the ability of Moju in absorbing such a publicpolicy, represented by new territorial dynamics and which require strong local capacity ofenvironmental management. This research aims to evaluate the institutional dimension ofsustainability of the municipality of Moju by verifying the conditions presented by the city councilvis-à-vis environmental management. We used the Barometer of Sustainability (BS) for thisanalysis, taking into account five themes (institutional structure, operational structure, terms ofconduct adjustment, rural environmental register and landholding tenure) and 33 indicators,complemented by interviews with local administrators and entrepreneurs. Results revealed thatMoju is indeed equipped for full environmental management, but also that this operationalstructure is still inadequate, which should certainly bring out problems if this policy is put intoeffect.Poucas ferramentas de análise de sustentabilidade têm sido aplicadas na Amazônia, em particular no Estado do Pará. Os municípios paraenses inseridos no Polo Biodiesel, com destaque para Moju, vivem uma fase de intensos investimentos agroindustriais com vistas à produção de palma de óleo (óleo de dendê), e de firme inserção da agricultura familiar nesse processo. Neste contexto, surgem questionamentos acerca da capacidade deste município em receber uma política pública como essa, que se configura em novas dinâmicas territoriais e que requerem uma forte capacidade de gestão ambiental municipal para direcionar as atividades produtivas e sociais em direção à sustentabilidade. Este trabalho visa avaliar a dimensão institucional da sustentabilidade em Moju, verificando as condições apresentadas pelo município quanto à gestão ambiental. Utilizaram-se para esta análise o Barômetro da Sustentabilidade (BS), considerando cinco temas (estrutura institucional, estrutura operacional, termo de ajuste de conduta, cadastro ambiental rural e regularização fundiária) e 33 indicadores, e também entrevistas com gestores e empreendedores locais. Os resultados mostram que este município está habilitado para exercer a gestão ambiental plena, mas também que sua estrutura operacional ainda é deficiente, o que certamente trará problemas para a condução de tal política

    Características clínicas, demográficas e epidemiológicas dos pacientes com hepatite B em seguimento em hospital universitário no sudeste do Brasil: predominância de casos HBeAg negativos

    INTRODUCTION: Hepatitis B is common in Brazil, although there are regional differences regarding the degree of endemicity, the most frequent forms of transmission and the presence of different evolutive stages of chronic disease. The present study aimed to determine the clinical, demographic and epidemiological characteristics of patients chronically infected with hepatitis B virus (HBV) residing in the Ribeirão Preto region, southeastern Brazil. METHODS: A total of 529 medical records of individuals with HBV monoinfection were reviewed. RESULTS: More than 60% of the subjects were males, with a mean age of 38 years-old. The HBeAg-negative serological pattern was verified in 84.4% of the patients, among whom the risk of vertical/intrafamily transmission was 43.2% (p = 0.02). The consumption of alcohol in amounts exceeding 20g a day was observed in 21.3% of the subjects and was more frequent among men (33%) (p < 0.001). Among patients with cirrhosis, 54.1% were alcohol abusers (p = 0.04), all of them males. The presence of cirrhosis was more frequent in the HBeAg-positive group (24.4%) than in the HBeAg-negative group (10.2%) (p < 0.001). CONCLUSIONS: High proportions of HBV-infected subjects with an HBeAg-negative pattern were observed, with a higher risk of vertical/intrafamily transmission. Alcohol abuse was associated with male subjects and with cirrhosis of the liver in this group. A tendency toward an increase in the number of HBeAg-negative cases was observed over time.INTRODUÇÃO: No Brasil, a hepatite B é comum. No entanto, há diferenças regionais no que diz respeito ao grau de endemicidade, as formas de transmissão mais encontradas e a presença dos diferentes estágios evolutivos da doença crônica. O objetivo deste trabalho foi o de conhecer características clínicas, demográficas e epidemiológicas de pacientes cronicamente infectados pelo vírus da hepatite B (HBV), residentes na região de Ribeirão Preto, no sudeste do Brasil. MÉTODOS: Foi realizada a análise retrospectiva de 529 prontuários de indivíduos com monoinfecção pelo HBV. RESULTADOS: Mais de 60% eram masculinos, a média de idade foi de 38 anos. O padrão sorológico HBeAg negativo foi encontrado em 84,4% dos pacientes, entre os quais o risco para transmissão vertical/intrafamiliar foi de 43,2% (p = 0,02). Verificou-se uso de álcool em quantidades maiores que 20g ao dia em 21,3% dos indivíduos, sendo mais frequente entre os homens (33%) (p < 0,001). Entre os pacientes com cirrose, 54,1% faziam uso abusivo de bebidas alcoólicas (p = 0,04), sendo todos estes do gênero masculino. A presença de cirrose foi maior no grupo HBeAg positivo (24,4%) que no grupo HBeAg negativo (10,2%) (p < 0,001). CONCLUSÕES: Observaram-se elevadas proporções de indivíduos com infecção pelo HBV com padrão sorológico HBeAg negativo, entre os quais houve maior risco para a transmissão vertical/intrafamiliar. O uso abusivo de álcool esteve associado a indivíduos do sexo masculino e, neste grupo, à cirrose hepática.     A esclerose lateral amiotrófica (ELA) é uma doença neurodegenerativa que afeta as células nervosas responsáveis pelo controle dos músculos voluntários, resultando em fraqueza muscular e atrofia. Nesse contexto, as intervenções imunomoduladoras têm como objetivo modular a resposta imune do organismo, ao reduzir a inflamação e possibilitar a neuroproteção, a partir do uso de anticorpos monoclonais, inibidores de citocinas e moduladores do sistema imunológico. No entanto, a eficácia de tais intervenções no tratamento da ELA ainda é incerta. Nesse sentido, o presente estudo tem como objetivo analisar a eficácia de intervenções imunomoduladoras para o tratamento da esclerose lateral amiotrófica. Para isso, foram selecionados cinco artigos que abordavam sobre a sua eficácia, por meio de uma estratégia de busca com recorte temporal entre 2017 e 2023, nas bases de dados PubMed (Medline), Cochrane Library e Embase. As intervenções imunomoduladoras, como o uso de inibidores de citocinas, têm demonstrado eficácia no tratamento da esclerose lateral amiotrófica (ELA). Além disso, há evidências de que a inflamação crônica pode estar envolvida em sua patogênese, o que sugere que a modulação do sistema imunológico pode ser uma abordagem terapêutica promissora. Em estudos clínicos recentes, a terapia com inibidores de citocinas mostrou-se capaz de reduzir a progressão da doença e melhorar a qualidade de vida dos pacientes com ELA. Ademais, há evidências que o uso de células-tronco pode melhorar o status funcional em pacientes com a doença. Entretanto, são necessários mais estudos, como ensaios clínicos randomizados e revisões sistemáticas com meta-análises, a fim de ratificar a eficácia das estratégias imunomoduladoras para a patologia

    Os autores relatam caso de Criptococcose cutânea primária, causada pelo Cryptococcus neoformans, em paciente imunocompetente, fazendeiro que desenvolveu extensivas lesões, no antebraço, após injúria provocada por galináceo, quando fazia limpeza de seu celeiro. Tratamento oral com fluconazol resultou em cura total. A literatura relata raridade de criptococcose cutânea primária em imunocompetentes e sua relativa frequência em imunodeprimidos.<br>The authors report a primary cutaneous cryptococcosis, caused by Cryptococcus neoformans in immunocompetent patient, a farmer who developed extensive lesions at the site of an injury caused by one of the chickens on his right forearm, while he was cleaning out his barn. Oral treatment with fluconazole was totally successful. A review of the literature showed the rarity of cutaneous cryptococcosis in immunocompetent patients and in contrast, that skin lesions frequently occur in immunocompromised patients

    Buffy Coat Transcriptomic Analysis Reveals Alterations in Host Cell Protein Synthesis and Cell Cycle in Severe COVID-19 Patients

    Transcriptome studies have reported the dysregulation of cell cycle-related genes and the global inhibition of host mRNA translation in COVID-19 cases. However, the key genes and cellular mechanisms that are most affected by the severe outcome of this disease remain unclear. For this work, the RNA-seq approach was used to study the differential expression in buffy coat cells of two groups of people infected with SARS-CoV-2: (a) Mild, with mild symptoms; and (b) SARS (Severe Acute Respiratory Syndrome), who were admitted to the intensive care unit with the severe COVID-19 outcome. Transcriptomic analysis revealed 1009 up-regulated and 501 down-regulated genes in the SARS group, with 10% of both being composed of long non-coding RNA. Ribosome and cell cycle pathways were enriched among down-regulated genes. The most connected proteins among the differentially expressed genes involved transport dysregulation, proteasome degradation, interferon response, cytokinesis failure, and host translation inhibition. Furthermore, interactome analysis showed Fibrillarin to be one of the key genes affected by SARS-CoV-2. This protein interacts directly with the N protein and long non-coding RNAs affecting transcription, translation, and ribosomal processes. This work reveals a group of dysregulated processes, including translation and cell cycle, as key pathways altered in severe COVID-19 outcomes

    Clinical Profile and Risk Factors for Severe COVID-19 in Hospitalized Patients from Rio de Janeiro, Brazil: Comparison between the First and Second Pandemic Waves

    Since COVID-19 was declared a pandemic, Brazil has become one of the countries most affected by this disease. A year into the pandemic, a second wave of COVID-19 emerged, with a rapid spread of a new SARS-CoV-2 lineage of concern. Several vaccines have been granted emergency-use authorization, leading to a decrease in mortality and severe cases in many countries. However, the emergence of SARS-CoV-2 variants raises the alert for potential new waves of transmission and an increase in pathogenicity. We compared the demographic and clinical data of critically ill patients infected with COVID-19 hospitalized in Rio de Janeiro during the first and second waves between July 2020 and October 2021. In total, 106 participants were included in this study; among them, 88% had at least one comorbidity, and 37% developed severe disease. Disease severity was associated with older age, pre-existing neurological comorbidities, higher viral load, and dyspnea. Laboratory biomarkers related to white blood cells, coagulation, cellular injury, inflammation, renal, and liver injuries were significantly associated with severe COVID-19. During the second wave of the pandemic, the necessity of invasive respiratory support was higher, and more individuals with COVID-19 developed acute hepatitis, suggesting that the progression of the second wave resulted in an increase in severe cases. These results can contribute to understanding the behavior of the COVID-19 pandemic in Brazil and may be helpful in predicting disease severity, which is a pivotal for guiding clinical care, improving patient outcomes, and defining public policies
