Symmetry of paraspinal muscle denervation in clinical lumbar spinal stenosis: Support for a hypothesis of posterior primary ramus stretching?
Introduction Denervation of the paraspinal muscles in spinal disorders is frequently attributed to radiculopathy. Therefore, patients with lumbar spinal stenosis causing asymmetrical symptoms should have asymmetrical paraspinal denervation. Methods Seventy‐three patients with clinical lumbar spinal stenosis, aged 55–85 years, completed a pain drawing and underwent masked electrodiagnostic testing, including bilateral paraspinal mapping and testing of 6 muscles on the most symptomatic (or randomly chosen) limb. Results With the exception of 10 subjects with unilateral thigh pain ( P = 0.043), there was no relationship between side of pain and paraspinal mapping score for any subgroups (symmetrical pain, pain into 1 calf only). Among those with positive limb EMG (tested on 1 side), no relationship between side of pain and paraspinal EMG score was found. Conclusion Evidence suggests that paraspinal denervation in spinal stenosis may not be due to radiculopathy, but rather due to stretch or damage to the posterior primary ramus. The Influence of Military Service Experiences on Current and Daily Drinking
Traumatic military service experience can lead to increased alcohol consumption among veterans, who may use alcohol as a form of self-medication. Veterans with a psychiatric disorder or traumatic brain injury are more likely than nonveterans to be daily drinkers. Non-combat and combat veterans without a PD or TBI are less likely than nonveterans to be daily drinkers

Development of an open-source, flexible framework for complex inter-institutional disparate data sharing and collaboration
Clinical information, “-omic” datasets, and tissue samples are difficult to harmonize and manage for data mining. We have developed a platform for storing clinical research data while providing access to associated data from other information stores. Data on 34 metrics from 11,000 neuroblastoma patients were instantiated into a database. The Django web framework was used to create a model for rapid development of tools and views with a front-end interface for generating complex queries. Working with Nationwide Children’s Hospital, we can now consume their tissue inventory data through an API. The end-user sees the number of patients who both match their search and have tissue available. Since initial implementation, the current tasks revolve around developing a governance structure and the necessary data use agreements. Efforts now are to (1) update the data with 5000 more patients, and (2) link to genomic data stores, facilitating disparate data acquisition for research studies
Intradialytic versus home based exercise training in hemodialysis patients: a randomised controlled trial
Background: Exercise training in hemodialysis patients improves fitness, physical function, quality of life and markers of cardiovascular disease such as arterial stiffness. The majority of trials investigating this area have used supervised exercise training during dialysis (intradialytic), which may not be feasible for some renal units. The aim of this trial is to compare the effects of supervised intradialytic with unsupervised home-based exercise training on physical function and arterial stiffness
The impact of research on policy: a case of qualifications reform
The relationship between research and policymaking has been discussed repeatedly. However, the debate tends to be in general, abstract terms or from a macro-economic perspective with any examples described in a fairly cursory way. Despite the inherent complexity of the research-policy interface, analyses tend to homogenize ‘research’ and ‘policy’ as coherent entities with discussions often focusing on products (research and policies) rather than on the relationships between producers (researchers and policy makers). Here we take one piece of research on qualifications that has influenced policy rhetoric over the last 5 years. We trace the career of the research from its production in the late 1990s in order to understand the conditions of its dormancy, reemergence and use over the ensuing years. The paper serves to document the case, which is important in its own right, but also proposes a typology of ways in which research gets adopted and adapted into policy

Identifying limitations when deriving probabilistic views of North Atlantic hurricane hazard from counterfactual ensemble NWP re-forecasts
Downward counterfactual analysis – or quantitatively estimating how our observed history could have been worse – is increasingly being used by the re/insurance industry to identify, quantify, and mitigate against as-yet-unrealised “grey-swan” catastrophic events. While useful for informing site-specific adaptation strategies, the extraction of probabilistic information remains intangible from such downside-only focused analytics. We hypothesise that combined upward and downward counterfactual analysis (i.e., how history could have been either better or worse) may allow us to obtain probabilistic information from counterfactual research if it can be applied objectively and without bias.
Here we test this concept of objective counterfactual analysis by investigating how initial-condition-driven track variability of events in our North Atlantic Hurricane (NAHU) record may affect present-day probabilistic views of US landfall risk. To do this, we create 10,000 counterfactual NAHU histories from NCEP GEFS v2 initial-condition ensemble reforecast data for the period 1985-2016 and compare the statistics of these counterfactual histories to a model-based version of our single observational history.
While the methodology presented herein attempts to produce the histories as objectively as possible, there is clear – and, ultimately, intuitively understandable – systematic underprediction of US NAHU landfall frequency in the counterfactual histories. This limits the ability to use the data in real-world applications at present. However, even with this systematic under-prediction, it is interesting to note both the magnitude of volatility and spatial variability in hurricane landfalls in single cities and wider regions along the US coastline, which speaks to the potential value of objective counterfactual analysis once methods have evolved
Charged AdS Black Holes and Catastrophic Holography
We compute the properties of a class of charged black holes in anti-de Sitter
space-time, in diverse dimensions. These black holes are solutions of
consistent Einstein-Maxwell truncations of gauged supergravities, which are
shown to arise from the inclusion of rotation in the transverse space. We
uncover rich thermodynamic phase structures for these systems, which display
classic critical phenomena, including structures isomorphic to the van der
Waals-Maxwell liquid-gas system. In that case, the phases are controlled by the
universal `cusp' and `swallowtail' shapes familiar from catastrophe theory. All
of the thermodynamics is consistent with field theory interpretations via
holography, where the dual field theories can sometimes be found on the world
