The Macroeconomic and Institutional Context of the 2012 Budget
In my paper, I look at the 2012 budget from two distinct points of view. Approaching from the perspective of the economic environment, I describe the most important lessons and relevant empirical experiences of budgetary adjustments as well as the topical macroeconomic and budgetary processes providing the foundation for next year’s budget. The second half of the paper discusses the institutional environment of the adoption of the budget. The paper presents the tasks related to the preparation and adoption of the 2012 budget, providing assistance for the legislative work of Parliament adopted by the Fiscal Council and which also follow from the new Constitution adopted in April. Zero and Minimal Fluoroscopic Approaches During Ablation of Supraventricular Tachycardias : A Systematic Review and Meta-Analysis
Catheter ablations for cardiac arrhythmias are conventionally performed under fluoroscopic guidance. To guide these procedures, zero/minimal fluoroscopy (Z/MF) approaches have become available, using three-dimensional electroanatomical mapping systems. Our aim was to conduct a meta-analysis comparing these two different methods for the treatment of paroxysmal supraventricular tachycardia (SVT).Electronic databases were searched and systematically reviewed for studies comparing procedural parameters and outcomes of conventional, fluoroscopy-guided vs. Z/MF approaches in patients undergoing electrophysiology (EP) procedures for SVTs. The random-effects model was used to derive mean difference (MD) and risk ratios (RRs) with 95% confidence interval (CI).Twenty-four studies involving 9,074 patients met our inclusion criteria. There was no difference between the groups in terms of acute success rate (RR = 1.00, 95% CI, 0.99-1.01; p = 0.97) and long-term success rate (RR: 1.01, 95% CI, 1.00-1.03; p = 0.13). Compared to the conventional method, zero-and-minimal fluoroscopy (Z/MF) ablation significantly reduced fluoroscopic time [MD: -1.58 min (95% CI, -2.21 to -0.96 min; p < 0.01)] and ablation time [MD: -25.23 s (95% CI: -42.04 to -8.43 s; p < 0.01)]. No difference could be detected between the two groups in terms of the procedure time [MD: 3.06 min (95% CI: -0.97 to 7.08; p = 0.14)] and the number of ablation applications [MD: 0.13 (95% CI: -0.86 to 1.11; p = 0.80)]. The complication rate was 1.59% in the entire study population and did not differ among the groups (RR: 0.68, 95% CI: 0.45-1.05; p = 0.08).The Z/MF approach for the catheter ablation of SVTs is a feasible method that reduces radiation exposure and ablation time without compromising the acute and long-term success or complication rates
A kronotípus és az alvásminõség szerepe a mentális megbetegedések tünettanában és prognózisában
napszaki preferenciák kognitív és affektív folyamatokkal
való összefüggésének vizsgálata az elmúlt években a
pszichiátriai kutatások terén is egyre nagyobb hangsúllyal
jelentkezik. Mindez nem meglepõ, hiszen a napszaki
preferenciák (kronotípus) az aktivitás és
passzivitás ritmusának meghatározása mellett a pszichés
egyensúlyban, vagy éppen egyensúlyvesztésben
szerepet játszó faktorokkal is szoros összefüggést mutatnak.
Az elmúlt években számos vizsgálat kimutatta,
hogy az esti kronotípus összefügg a pszichés sérülékenység
holdudvarába tartozó személyiségjellemzõkkel
(neuroticizmus, újdonságkeresés), valamint egyes
maladaptív viselkedésformák (pl. egészségkárosító magatartások)
fokozott kockázatával. Az esti
kronotípussal jellemezhetõ személyek ráadásul gyakrabban
számolnak be különbözõ alvászavarokról, amelyek
szintén növelhetik a pszichés problémák
kialakulásának kockázatát. Habár a kronotípus hátterében
egy rendkívül bonyolult idegrendszeri apparátus
mûködése áll, a napszaki preferenciák mégis könnyen
mérhetõk kérdõíves eszközökkel is. Korábbi vizsgálatok
alapján a kronotípus kérdõíveken alapuló mérése
hitelesen tükrözi a napszaki ritmussal kapcsolatban
álló biológiai folyamatok mûködését (pl. egyes hormonok
szekréciójának ritmusát). Mivel a kronotípus jelentõs
szerepet játszik a kognitív teljesítmény és a
hangulat napi ingadozásában, a napszaki preferenciák
figyelembevétele a rutin pszichiátriai gyakorlatban,
például a mentális funkciók vizsgálata során is fontos
szempont lehet. Újabb kutatások pedig arra hívják fel a
figyelmet, hogy az esti kronotípus, a pszichiátriai kórképekben
gyakorta tapasztalható napszaki ritmuseltolódás
és alváspanaszok fokozzák a relapszus
kockázatát, így e faktorok figyelembevétele az optimális
beavatkozás kiválasztását is elõsegítheti. A fentiekben
vázoltak mellett az elõadásban bemutatjuk, hogy
milyen szerepet tölthet be a kronotípus a hangulat- és
szorongásos zavarok tünettanában, majd röviden ismertetjük
a kronotípus mérésére alkalmas kérdõívvel,
a Morningness-Eveningness Questionnaire hazai
adaptációjával (MEQ-H) kapcsolatos tapasztalatainkat,
valamint a Nyírõ/OPAI II. Pszichiátriai Osztályán
induló kutatásunk jelenlegi állását
The Interaction of Political, Fiscal and Financial Stability: Lessons from the Crisis
The current financial, economic and fiscal crisis is among other things characterised by complex interrelations between financial, fiscal, macroeconomic and political instability. One instability breeds another, with feedback loops generating self-reinforcing adverse cycles: The financial crisis triggered the ‘Great Recession’. Countermeasures by governments – to save banks and bolster up aggregate demand – ultimately jeopardized fiscal sustainability and bred the fiscal crisis. The latter in turn destabilised sovereign bond markets and banking systems in several countries. Political instability resulted from the substantial fiscal consolidations forced upon governments in the light of threatening or actual loss of access to financial market financing, and the accompanying deep recessions and sharp increase in unemployment. Political instability in turn further erodes economic and financial market confidence, thus worsening short and long-term economic and fiscal prospects, and further aggravating financial instability. In the EU and more specifically the Euro Area, multiple channels of spillovers and contagion turn the problems from purely national phenomena to ones of EU-wide and ultimately even global scope. Thus, apart from national political processes, Euro Area and EU-wide economic governance has been criticized for not addressing reform needs decisively, thus prolonging and deepening the cycle of instability
A napszaki ritmus és a tüneti kép súlyosságának összefüggései Obszesszív-Kompluzív zavarban (OCD) szenvedő pszichiátriai betegek körében
Az utóbbi években számos kutatás eredménye utalt arra, hogy egészséges személyek körében
az esti napszaki preferencia a pszichopatológiai tényezők, kockázati faktorok tekintetében
kedvezőtlen profillal társul. Pszichiátriai betegek körében a kronotípus és pszichopatológia
tünetek súlyosságának összefüggését kevesebb tanulmány vizsgálta, ugyanakkor ezek
hasonlóképpen az esti preferencia és a súlyosabb tüneti kép, valamint a kedvezőtlenebb
gyógyulási esély összefüggésére utaltak. Az OCD jellemzői a visszatérő, intruzív, a beteg
akarata ellenére jelentkező nyugtalanító gondolatok és impulzusok, valamint az ismétlődő,
ritualisztikus cselekedetek. Az elmúlt évtizedek intenzív katatásai ellenére az OCD-ben
tapasztalt klinikai tünetek hátterében meghúzódó neurokognitív deficit jellege még nem
tisztázott.
Egyes vizsgálatok arra utalnak, hogy OCD-ben specifikus eltérések mutatkoznak a napszaki
ritmus és az alvásmintázat tekintetében, ugyanakkor tisztázatlan, hogy ezek a jegyek milyen
összefüggést mutatnak az OCD-ben szenvedő betegek pszichés állapotával. Célunk ezért az
OCD-s betegek napszaki preferenciája, alváspanaszai és tüneti profilja közti összefüggések
vizsgálata volt.
Tanulmányunkban OCD-s és vegyes pszichiátriai betegségben szenvedő betegeket
hasonlítottunk össze a Morningness-eveningness (Napszaki Ritmusok) kérdőív, a rövidített
Beck depresszió skála, a STAI Vonásszorongás skála valamint a Pozitív és Negatív
Affektivitás kérdőívek segítségével. Az OCD-s tüneteket a Yale-Brown Obszesszív-
Kompulzív Skálával mértük. A BNO diagnózist klinikus (szakorvos vagy szakpszichológus)
végezte. Az adatok elemzése még zajlik, azonban kezdeti eredményeink arra utalnak, hogy az
OCD csoportban az esti napszaki ritmus valamint az alváspanaszok kedvezőtlenebb tüneti
képpel járnak együtt.

Brain protein expression changes in WAG/Rij rats, a genetic rat model of absence epilepsy after peripheral lipopolysaccharide treatment
Peripheral injection of bacterial lipopolysaccharide (LPS) facilitates 8-10Hz spike-wave discharges (SWD) characterizing absence epilepsy in WAG/Rij rats. It is unknown however, whether peripherally administered LPS is able to alter the generator areas of epileptic activity at the molecular level. We injected 1mg/kg dose of LPS intraperitoneally into WAG/Rij rats, recorded the body temperature and EEG, and examined the protein expression changes of the proteome 12h after injection in the fronto-parietal cortex and thalamus. We used fluorescent two-dimensional differential gel electrophoresis to investigate the expression profile. We found 16 differentially expressed proteins in the fronto-parietal cortex and 35 proteins in the thalamus. It is known that SWD genesis correlates with the transitional state of sleep-wake cycle thus we performed meta-analysis of the altered proteins in relation to inflammation, epilepsy as well as sleep. The analysis revealed that all categories are highly represented by the altered proteins and these protein-sets have considerable overlap. Protein network modeling suggested that the alterations in the proteome were largely induced by the immune response, which invokes the NFkB signaling pathway. The proteomics and computational analysis verified the known functional interplay between inflammation, epilepsy and sleep and highlighted proteins that are involved in their common synaptic mechanisms. Our physiological findings support the phenomenon that high dose of peripheral LPS injection increases SWD-number, modifies its duration as well as the sleep-wake stages and decreases body temperature
