    An Interprofessional Approach to Teaching Evidence-based Practice

    Purpose: The purpose of this mixed methods study was to identify students’ engagement and implementation of EBP competencies after exposure to EBP content in an interprofessional online research course. In this research, a diverse, convenience sample of nursing, respiratory care, and social work students were selected. The results aim to assist educators in preparing the next generation for their first professional role in providing care using the best evidence. The research questions that guided the study were: (1) What are the levels of practice, attitude, knowledge and skill regarding EBP among the sample? (2) What are the barriers and facilitators to acquiring EBP competency? (3) What are the differences in EBP attitudes, knowledge, and skills between the diverse disciplines in the sample? Methods: This descriptive mixed methods study employed a cross sectional design that allowed comparisons of programs and the different levels within the program. Quantitative data was collected using the S-EBPQ survey, allowing participants’ the opportunity to self-report on the practice, attitude, and knowledge/skills. Qualitative data was collected through two focus groups of approximately six to ten students from the three programs. Results: Data showed that students within the three selected disciplines had a positive attitude towards EBP. Theme analysis supported this view as participants noted EBP impacts patient safety and quality care. Barriers identified included, “lack of resources, use of research language interchangeably with EBP, quick exposure to EBP and confusion among EBP practices in the clinical setting.” Participants had an awareness of EBP and recognized the value of EBP. However, there may be ways to improve the implementation of EBP throughout the learning process while students are in undergraduate studies. Conclusion: Using an interprofessional online research course to introduce students to EBP was successful. Providing an interprofessional approach to teaching EBP will provide support the implementation of EBP competencies. Further development of additional interprofessional EBP courses or activities should be considered. This study found that if interprofessional care teams’ work together and use the EBP process in their courses it will improve student perceived value of EBP

    Creating the Capacity to Screen Deaf Women for Perinatal Depression [poster]

    There are approximately 1 million Deaf women in the U.S. who depend on American Sign Language (ASL) for communication. Although Deaf women become pregnant and enter motherhood at rates similar to hearing women, Deaf women attend fewer prenatal appointments, receive less information from their physicians, are less satisfied with physician concern and quality of communication, and are less satisfied with their prenatal care. These barriers persist after childbirth, leaving Deaf mothers with little professional support for struggles with postnatal healthcare, breastfeeding, and childcare. Combined with pre-existing mental health disparities observed among members of the Deaf community, such barriers leave Deaf women especially vulnerable to development or exacerbation of depression during the perinatal period (i.e., during pregnancy or within one year postpartum). Expert groups recommend depression screening as a standard of perinatal care - the first critical step to direct women to treatment. Yet, available screening tools are not accessible to Deaf women due to documented low levels of English literacy and health literacy. It is, therefore, critical to develop and validate tools to screen for depression among Deaf perinatal women so they may access the same standard of care as other perinatal women. To address these barriers, our team is conducting a one-year, community-engaged pilot study to develop and perform preliminary psychometric analyses on an ASL translation of the Edinburgh Postnatal Depression Scale (EPDS).     Project ACCCES: A Collaboration to develop Capacity for Community-Engaged research in Springfield

    Background: Residents of Springfield, Massachusetts experience disproportionately poorer health compared to the rest of the state. Springfield\u27s community of color population (39.5% Hispanic/Latino; 21.6% Black; 2% Asian) experiences high rates of poverty, unemployment, and low educational attainment leading to large health disparities. Objective: The overall objective of the PCORI-funded Project ACCCES is to develop capacity to conduct community engaged research (CEnR) in Springfield that is based on patient and community needs, can be more easily translated, and improves community health. This engagement framework will be applied to development of patient-centered comparative effectiveness research proposals. Methods: In the project\u27s first phase, we conducted focus groups with patients, community organization leaders, and physicians to better understand the most pressing health needs of the community, past research experiences, and important considerations for building community-academic partnerships. We then convened a multi-stakeholder Advisory Board to inform all Project ACCCES activities. In the second phase, we developed and implemented two half-day workshops to give participants the knowledge and tools to conduct CEnR and to begin to build community-academic partnerships in the areas of perinatal/maternal-child health, mental health/substance use, chronic disease/nutrition, and respiratory health. The third phase is currently underway and is focused on convening research partnership action groups that will seek to address Springfield residents\u27 health needs through culturally appropriate interventions. The final phases will entail dissemination of both the process used to develop new collaborations to conduct CEnR in an underserved community and the workshop template, and development of structures to foster sustainability of partnerships

    Formulation of a killed whole cell pneumococcus vaccine - effect of aluminum adjuvants on the antibody and IL-17 response

    Background Streptococcus pneumoniae causes widespread morbidity and mortality. Current vaccines contain free polysaccharides or protein-polysaccharide conjugates, and do not induce protection against serotypes that are not included in the vaccines. An affordable and broadly protective vaccine is very desirable. The goal of this study was to determine the optimal formulation of a killed whole cell pneumococcal vaccine with aluminum-containing adjuvants for intramuscular injection. Methods Four aluminium-containing adjuvants were prepared with different levels of surface phosphate groups resulting in different adsorptive capacities and affinities for the vaccine antigens. Mice were immunized three times and the antigen-specific antibody titers and IL-17 responses in blood were analyzed. Results Although all adjuvants induced significantly higher antibody titers than antigen without adjuvant, the vaccine containing aluminum phosphate adjuvant (AP) produced the highest antibody response when low doses of antigen were used. Aluminum hydroxide adjuvant (AH) induced an equal or better antibody response at high doses compared with AP. Vaccines formulated with AH, but not with AP, induced an IL-17 response. The vaccine formulated with AH was stable and retained full immunogenicity when stored at 4°C for 4 months. Conclusions Antibodies are important for protection against systemic streptococcal disease and IL-17 is critical in the prevention of nasopharyngeal colonization by S. pneumoniae in the mouse model. The formulation of the whole killed bacterial cells with AH resulted in a stable vaccine that induced both antibodies and an IL-17 response. These experiments underscore the importance of formulation studies with aluminium containing adjuvants for the development of stable and effective vaccines

    Cetacean sightings around the Republic of the Maldives, April 1998

    In April 1998, as part of a project to collect biopsy samples of putative pygmy blue whales (Balaenoptera musculus brevicauda) in the waters around the Republic of the Maldives, Indian Ocean, incidental sightings of cetaceans encountered were recorded. Using modified line-transect methods and handheld binoculars, a total of 267 sightings of 16 species of whales and dolphins were recorded during 20 at-sea days in the northeastern part of the atoll. Significant results include the following: (1) cetaceans were abundant and species diversity was high, including nearly every pantropical species of pelagic cetacean; (2) the spinner dolphin (Stenella longirostris) was by far the most common species encountered (56 sightings) and also had the largest mean school size ( = 50.3 individuals); (3) blue whales were rare; only four individuals were sighted; (4) a large concentration of Bryde’s whales (28 sightings in two days) was apparently feeding in nearshore waters; (5) this paper reports the first records for the Maldives of Cuvier’s beaked whale (Ziphius cavirostris), Blainville’s beaked whale (Mesoplodon densirostris) and the dwarf sperm whale (Kogia sima): the latter was particularly common (17 sightings); (6) the spotted dolphin (Stenella attenuata) was rare and almost always associated with yellowfin tuna (Thunnus albacares), spinner dolphin, or seabirds, as has been reported in the eastern Pacific and western Indian oceans

    Structure-based evaluation of C5 derivatives in the catechol diether series targeting HIV-1 reverse transcriptase

    Using a computationally driven approach, a class of inhibitors with picomolar potency known as the catechol diethers were developed targeting the non-nucleoside-binding pocket of HIV-1 reverse transcriptase. Computational studies suggested that halogen-bonding interactions between the C5 substituent of the inhibitor and backbone carbonyl of conserved residue Pro95 might be important. While the recently reported crystal structures of the reverse transcriptase complexes confirmed the interactions with the non-nucleoside-binding pocket, they revealed the lack of a halogen-bonding interaction with Pro95. To understand the effects of substituents at the C5 position, we determined additional crystal structures with 5-Br and 5-H derivatives. Using comparative structural analysis, we identified several conformations of the ethoxy uracil dependent on the strength of a van der Waals interaction with the CÎł of Pro95 and the C5 substitution. The 5-Cl and 5-F derivatives position the ethoxy uracil to make more hydrogen bonds, whereas the larger 5-Br and smaller 5-H position the ethoxy uracil to make fewer hydrogen bonds. EC50 values correlate with the trends observed in the crystal structures. The influence of C5 substitutions on the ethoxy uracil conformation may have strategic value, as future derivatives can possibly be modulated to gain additional hydrogen-bonding interactions with resistant variants of reverse transcriptase.Fil: Frey, Kathleen M..     New Measure to Screen Deaf Women for Perinatal Depression

    Approximately 1 million women in the U.S. have profound hearing loss and use American Sign Language (ASL) as their primary language. Many providers are unfamiliar with the unique linguistic and cultural needs of the Deaf community, therefore Deaf women experience major obstacles to receiving effective physical and mental healthcare. For example, failure to provide ASL interpreters or translations from written English is a common communication barrier that prevents Deaf women from receiving health-related treatment and information. In 2017, Drs. Melissa Anderson, Kelly Wolf Craig, and Nancy Byatt were awarded a 1-year pilot project grant for their Creating the Capacity to Screen Deaf Women for Perinatal Depression project. The primary goal of this project was to translate the Edinburgh Postnatal Depression Scale (EPDS) from written English to American Sign Language (ASL). Using the new ASL EPDS, the team aimed to recruit 50 Deaf perinatal women from across the United States to conduct depression screening interviews.
