    Beyond the Noise, Behind the Cotton Wool. Reflections on truth, silence, and the outward purposelessness of art

    At age four, I used to secretly squeeze through the cracks in our backyard fence to go and explore the world on the other side. One step at a time, a little further each time, those childhood escapades soon grew into what would become the quest of a lifetime: a relentless and passionate chase for "what lies beyond". Beyond the backyard fence, beyond sight, beyond the human scope - a chase, one might say, for the true essence of the world. So I wandered and wondered, from one country to the next and from mathematics to glass to art. A somewhat winding journey I admit, yet one consistently marked by introspection and punctuated by moments of unexpected stillness, of monumental silence. Could it be, I sometimes wonder, that silence is the expression of the essence of the world? It is with that question stirring at the back of my mind that I initiated my thesis work about a year ago. The essence of the world, however, is not ours to touch, nor is pure silence ours to experience; both forever elude human perception. Yet contemplating the horizon one can still gather a sense of infinity. Hands anchored on matter, stretching my mind to philosophy and literature, I thus attempted to expand my own horizons and achieve a broader understanding of the kinship between truth, silence, and art. The present thesis, alongside my recent artistic work, comes as a result of this investigation. Combining my personal views with the ideas of various philosophers and artists, and inspired in particular by the writings of Virginia Woolf, it explores the nature of art, the human longing for being, and the tenuous threads of silence that span the gap between the two

    Le territoire, « matrice » de culture : Analyse des mémoires déposés à la commission Coulombe par les premières nations du Québec

    Cet article présente une analyse des mémoires déposés par les premières nations du Québec à la Commission d’étude sur la gestion de la forêt publique québécoise (2004). Les mémoires et les présentations révèlent une pluralité d’approches au sein des communautés autochtones et entre elles. Cependant, par delà cette pluralité on peut déceler une vision commune de la forêt qui est érigée en matrice de civilisation, comme l’avaient été lors de la crise hydroélectrique des années 1975-1980 les lacs et les rivières. Ce changement d’ancrage physique de la culture – de l’eau vers la forêt – s’accompagne d’une nouvelle attitude face aux autres acteurs du développement, notamment l’État et l’industrie. En effet, face aux développements forestiers, les premières nations du Québec demandent une plus grande autonomie et le respect de leurs droits ancestraux mais elles ne réclament plus systématiquement l’arrêt du développement industriel de leur territoire, demandant plutôt à être intégrées de plein droit dans la gouvernance de celui-ci.In this article we present an analysis of the briefs submitted by Québec’s First Nations at the Commission for the study of public forest management in Québec (2004). The briefs and presentations made by them reveal the multiplicity of approaches within and between First Nations. However, beyond this multiplicity we can detect a common vision of the forest which is erected as a civilisation matrix, as were the lakes and rivers at the time of the hydroelectric crisis in 1975-1980. This change in the culture’s physical anchorage – from water to forest – is accompanied by a new attitude towards other development actors, notably the State and the industry. Indeed, in the face of forest development, Québec’s First Nations ask for a greater autonomy and the respect of their ancestral rights, but they do not systematically demand the halt of their territory’s industrial development, but rather ask to be rightfully integrated in its governance

    Supporting work practices through telehealth: impact on nurses in peripheral regions

    <p>Abstract</p> <p>Background</p> <p>In Canada, workforce shortages in the health care sector constrain the ability of the health care system to meet the needs of its population and of its health care professionals. This issue is of particular importance in peripheral regions of Quebec, where significant inequalities in workforce distribution between regions has lead to acute nursing shortages and increased workloads. Information and communication technologies (ICTs) are innovative solutions that can be used to develop strategies to optimise the use of available resources and to design new nursing work practices. However, current knowledge is still limited about the real impact of ICTs on nursing recruitment and retention. Our aim is to better understand how work practice reorganization, supported by ICTs, and particularly by telehealth, may influence professional, educational, and organizational factors relating to Quebec nurses, notably those working in peripheral regions.</p> <p>Methods/Design</p> <p>First, we will conduct a descriptive study on the issue of nursing recruitment. Stratified sampling will be used to select approximately twenty innovative projects relating to the reorganization of work practices based upon ICTs. Semi-structured interviews with key informants will determine professional, educational, and organizational recruitment factors. The results will be used to create a questionnaire which, using a convenience sampling method, will be mailed to 600 third year students and recent graduates of two Quebec university nursing faculties. Descriptive, correlation, and hierarchical regression analyses will be performed to identify factors influencing nursing graduates' intentions to practice in peripheral regions. Secondly, we will conduct five case studies pertaining to the issue of nursing retention. Five ICT projects in semi-urban, rural, and isolated regions have been identified. Qualitative data will be collected through field observation and approximately fifty semi-structured interviews with key stakeholders.</p> <p>Discussion</p> <p>Data from both parts of this research project will be jointly analysed using triangulation of researchers, theoretical approaches, methods, and results. Continuous exchanges with decision makers and periodic knowledge transfer activities are planned to facilitate the dissemination and utilization of research results in policies regarding the nursing recruitment and retention.</p

    Integrative and comparative genomic analyses identify clinically relevant pulmonary carcinoid groups and unveil the supra-carcinoids

    International audienceThe worldwide incidence of pulmonary carcinoids is increasing, but little is known about their molecular characteristics. Through machine learning and multi-omics factor analysis, we compare and contrast the genomic profiles of 116 pulmonary carcinoids (including 35 atypical), 75 large-cell neuroendocrine carcinomas (LCNEC), and 66 small-cell lung cancers. Here we report that the integrative analyses on 257 lung neuroendocrine neoplasms stratify atypical carcinoids into two prognostic groups with a 10-year overall survival of 88% and 27%, respectively. We identify therapeutically relevant molecular groups of pulmonary car-cinoids, suggesting DLL3 and the immune system as candidate therapeutic targets; we confirm the value of OTP expression levels for the prognosis and diagnosis of these diseases, and we unveil the group of supra-carcinoids. This group comprises samples with carcinoid-like morphology yet the molecular and clinical features of the deadly LCNEC, further supporting the previously proposed molecular link between the low-and high-grade lung neuroendocrine neoplasms

    Effect of angiotensin-converting enzyme inhibitor and angiotensin receptor blocker initiation on organ support-free days in patients hospitalized with COVID-19

    IMPORTANCE Overactivation of the renin-angiotensin system (RAS) may contribute to poor clinical outcomes in patients with COVID-19. Objective To determine whether angiotensin-converting enzyme (ACE) inhibitor or angiotensin receptor blocker (ARB) initiation improves outcomes in patients hospitalized for COVID-19. DESIGN, SETTING, AND PARTICIPANTS In an ongoing, adaptive platform randomized clinical trial, 721 critically ill and 58 non–critically ill hospitalized adults were randomized to receive an RAS inhibitor or control between March 16, 2021, and February 25, 2022, at 69 sites in 7 countries (final follow-up on June 1, 2022). INTERVENTIONS Patients were randomized to receive open-label initiation of an ACE inhibitor (n = 257), ARB (n = 248), ARB in combination with DMX-200 (a chemokine receptor-2 inhibitor; n = 10), or no RAS inhibitor (control; n = 264) for up to 10 days. MAIN OUTCOMES AND MEASURES The primary outcome was organ support–free days, a composite of hospital survival and days alive without cardiovascular or respiratory organ support through 21 days. The primary analysis was a bayesian cumulative logistic model. Odds ratios (ORs) greater than 1 represent improved outcomes. RESULTS On February 25, 2022, enrollment was discontinued due to safety concerns. Among 679 critically ill patients with available primary outcome data, the median age was 56 years and 239 participants (35.2%) were women. Median (IQR) organ support–free days among critically ill patients was 10 (–1 to 16) in the ACE inhibitor group (n = 231), 8 (–1 to 17) in the ARB group (n = 217), and 12 (0 to 17) in the control group (n = 231) (median adjusted odds ratios of 0.77 [95% bayesian credible interval, 0.58-1.06] for improvement for ACE inhibitor and 0.76 [95% credible interval, 0.56-1.05] for ARB compared with control). The posterior probabilities that ACE inhibitors and ARBs worsened organ support–free days compared with control were 94.9% and 95.4%, respectively. Hospital survival occurred in 166 of 231 critically ill participants (71.9%) in the ACE inhibitor group, 152 of 217 (70.0%) in the ARB group, and 182 of 231 (78.8%) in the control group (posterior probabilities that ACE inhibitor and ARB worsened hospital survival compared with control were 95.3% and 98.1%, respectively). CONCLUSIONS AND RELEVANCE In this trial, among critically ill adults with COVID-19, initiation of an ACE inhibitor or ARB did not improve, and likely worsened, clinical outcomes.
