Success rates of re-excision after positive margins for invasive lobular carcinoma of the breast.
Rates of positive margins after surgical resection of invasive lobular carcinoma (ILC) are high (ranging from 18 to 60%), yet the efficacy of re-excision lumpReceptor subtypeectomy for clearing positive margins is unknown. Concerns about the diffuse nature of ILC may drive increased rates of completion mastectomy to treat positive margins, thus lowering breast conservation rates. We therefore determined the success rate of re-excision lumpectomy in women with ILC and positive margins after surgical resection. We identified 314 cases of stage I-III ILC treated with breast conserving surgery (BCS) at the University of California, San Francisco. Surgical procedures, pathology reports, and outcomes were analyzed using univariate and multivariate statistics and Cox-proportional hazards models. We evaluated outcomes before and after the year 2014, when new margin management consensus guidelines were published. Positive initial margins occurred in 118 (37.6%) cases. Of these, 62 (52.5%) underwent re-excision lumpectomy, which cleared the margin in 74.2%. On multivariate analysis, node negativity was significantly associated with successful re-excision (odds ratio [OR] 3.99, 95% CI 1.15-13.81, p = 0.029). After 2014, we saw fewer initial positive margins (42.7% versus 25.5%, p = 0.009), second surgeries (54.6% versus 20.2%, p < 0.001), and completion mastectomies (27.7% versus 4.5%, p < 0.001). In this large cohort of women with ILC, re-excision lumpectomy was highly successful at clearing positive margins. Additionally, positive margins and completion mastectomy rates significantly decreased over time. These findings highlight improvements in management of ILC, and suggest that completion mastectomy may not be required for those with positive margins after initial BCS

Independencia - 2016”, donde la población está conformada por 63 personas que
es el total de trabajadores de la organización. Así mismo, los datos se recolectaron
mediante la técnica de la encuesta usando como instrumento al cuestionario,
conformado por 24 preguntas en la escala de Likert. Una vez recolectados los
datos estos fueron procesados mediante el SPSS V. 22. Teniendo como resultado

Nivel de actividad física en escolares del Colegio Santa Ana de los Jardines de San Martín de Porres-2019
La presente investigación titulada “Nivel de actividad física en escolares del colegio Santa Ana de los Jardines de San Martín de Porres-2019” tiene por objetivo determinar el nivel de actividad física que se presenta en los alumnos de 8 a 14 años de dicha institución educativa. Es un estudio con enfoque cuantitativo, cuyo diseño es no experimental, de naturaleza descriptiva y de corte transversal. Se utilizó como instrumento el cuestionario de actividad física PAQ-C, cuya aplicación ayudo a determinar los resultados en una muestra de 150 escolares. En los resultados se halló que, del total de los escolares participantes, el 60.7%(n=91) es considerado sedentario, el 1.3%(n=2) es considerado muy sedentario, el 29.3%(n=44) es considerado como moderadamente activos, en tanto, el 8% (n=12) es considerados como
activo, mientras que solo un 0.7%(n=1) es considerado muy activo. Entonces, se concluye que la gran mayoría de escolares participantes en el presente estudio presentan un nivel bajo de actividad física a pesar que se dedican a realizar actividades recreativas activas y/o a practicar algún deporte en específico
CALIDAD DE VIDA Y DEPRESIÓN EN ADULTOS MAYORES DE UN CENTRO DE SALUD- CHICLAYO
Esta investigación de tipo no experimental y diseño descriptivo correlacional, planteó hallar la relación existente entre calidad de vida y depresión en adultos mayores de un centro de salud de Chiclayo, para lo cual se consideró un total de 130 adultos mayores que cumplieron con los criterios de inclusión, a quienes se aplicó el test de Calidad de vida de y la escala de depresión geriátrica de Yesavage, ambos test fueron validados en poblaciones con características similares a las de la muestra, dentro de los resultados se encontró correlación significativa entre las variables generales con un valor p < 0.05, lo que determina la asociación entre calidad de vida y depresión en adultos mayores, concluyendo que ciertos aspectos del estado de ánimo como el optimismo y la felicidad se verán afectados, viéndose reflejado en la manifestación de sentimientos de tristeza.Tesi
SISTEMAS ADHESIVOS EN EL TRATAMIENTO RESTAURADOR DE LESIONES DE CARIES DENTAL
Los adhesivos son materiales resinosos que han sido modificados a través del tiempo, contribuyendo a la unión entre la resina compuesta y los tejidos dentales. Existen sistemas adhesivos que son diferenciados por su contenido y la forma en la que actúan como los de grabado total que promueven la eliminación del barrillo dentinario con el uso de ácido fosfórico, y los de autograbado que contienen un primer ácido que disuelve y penetra el barrillo dentinario. En esta monografía se tiene como objetivo sintetizar la información actualizada relacionada a los sistemas adhesivos utilizados en el tratamiento restaurador de lesiones de caries dental. El conocimiento de las propiedades y su relación con el desempeño clínico, permite al odontólogo tener un mejor criterio de elección de los sistemas adhesivos para cada tratamiento. Se concluye que los sistemas adhesivos han atravesado diferentes etapas, logrando traspasar las perspectivas de cada una de ellas, uniendo componentes, reduciendo pasos operatorios y brindándonos seguridad en la calidad del tratamiento.Trabajo de suficiencia profesiona

Rural Hispanic-Serving Institutional Context on the Development of Hispanic-Serving Initiatives and Strategies
The focus of this single-site case study was to explore how rural dispersed Hispanic-serving community colleges in the Pacific Northwest support the development of Hispanic-Serving initiatives and strategies. The conceptual frameworks of Latino critical theory and the multicultural organizational development model were used to analyze and interpret the data. Interviews were conducted with five faculty members and two staff member with a focus on strategies and initiatives Dalé College had implemented to support Latinx students. Publicly available documents were reviewed to better understand the organizational identity of the institution. The findings include the following themes: (a) change management, (b) Hispanic-serving institution (HSI) identity, (c) community buy-in, and (d) faculty development. Recommendations for policy, practice, and future research are presented
Pain assessment and management in care homes::understanding the internal and external context through a scoping review

Pain is frequently considered an expected part of old age and morbidity, and may often be either under-reported by care home residents, or go unrecognized by care staff. We conducted a systematic scoping review to explore the complexity of pain recognition, assessment and treatment for residents living in care homes, and to understand the contexts that might influence its management. Methods: Scoping review using the methodological framework of Levac and colleagues. Articles were included if they examined pain assessment and/or management, for care or nursing home residents. We searched Medline, CINAHL, ASSIA, PsycINFO, EMBASE, Cochrane Library, and Google Scholar; reference lists were also screened, and website searches carried out of key organisations. Conversations with 16 local care home managers were included to gain an understanding of their perspective. Results: Inclusion criteria were met by 109 studies. Three overarching themes were identified: Staff factors and beliefs - in relation to pain assessment and management (e.g. experience, qualifications) and beliefs and perceptions relating to pain. Pain assessment – including use of pain assessment tools and assessment/management for residents with cognitive impairment. Interventions - including efficacy/effects (pharmaceutical/non pharmaceutical), and pain training interventions and their outcomes. Overall findings from the review indicated a lack of training and staff confidence in relation to pain assessment and management. This was particularly the case for residents with dementia. Conclusions: Further training and detailed guidelines for the appropriate assessment and treatment of pain are required by care home staff. Professionals external to the care home environment need to be aware of the issues facing care homes staff and residents in order to target their input in the most appropriate way.CSO Catalytic Research Grant: CGA/19/15.21pubpu
