    The Changing Epidemiology of Malaria in Ifakara Town, Southern Tanzania.

    Between 1995 and 2000 there were marked changes in the epidemiology of malaria in Ifakara, southern Tanzania. We documented these changes using parasitological and clinical data from a series of community- and hospital-based studies involving children up to the age of 5 years. There was a right shift and lowering in the age-specific parasite prevalence in the community-based cohort studies. The incidence of clinical malaria in placebo-receiving infants in additional study cohorts dropped from 0.8 in 1995 to 0.43 episodes per infant per year in 2000, an incidence rate ratio of 0.53 (95% confidence interval: 0.404, 0.70, P<0.0001). At the same time, there was an increase in the total number of malaria admissions and a marked right shift in the age pattern of these admissions (median age in 1995 1.55 years vs. 2.33 in 2000, P<0.0001). However, the burden of malaria deaths remained in infants. We discuss how these dramatic changes in the epidemiology of malaria may have arisen from the use of currently available malaria control tools. Caution is required in the interpretation of hospital-based data as it is likely to underestimate the impact of anaemia on mortality in the community, where most paediatric deaths occur. Even in low/moderate malaria transmission settings, where older children suffer most malaria episodes, targeting effective malaria control at infants may produce important reductions in infant mortality caused by malaria

    Electrical properties and strain gauge factor of Cr/SiOx cermet films with compositions 50/50 and 70/30 wt% were investigated in order to evaluate their use in strain gauge devices. The films were deposited by flash evaporation. The microstructures and resulting phases were characterized by electron diffraction and electron microscopy. The influence of the thickness and deposition rate on the sheet resistance, the temperature coefficient of resistance and the gauge factor were investigated. The results are consistent with a mixed conduction mechanism with metallic and a thermally activated tunneling components, between interconnected and discrete conductive phases, respectively.Se investigaron las propiedades eléctricas y el factor extensométrico de películas del cermet- Cr/SiOx en composiciones 50/50 y 70/30 % en peso, para evaluar su uso en dispositivos "strain gauge". Las películas fueron depositadas por evaporación "flash". Las estructuras y fases resultantes fueron caracterizadas por microscopía y difracción de electrones. Se estudió la influencia del espesor y la velocidad del depósito sobre la resistencia laminar, el coeficiente térmico de resistencia y el factor extensométrico. Los resultados son consistentes con un mecanismo de conducción mixto, con una componente metálica y otra por efecto túnel térmicamente activado, entre fases conductoras interconectadas y discretas, respectivamente

    The symmetric-Toeplitz linear system problem in parallel

    [EN] Many algorithms exist that exploit the special structure of Toeplitz matrices for solving linear systems. Nevertheless, these algorithms are difficult to parallelize due to its lower computational cost and the great dependency of the operations involved that produces a great communication cost. The foundation of the parallel algorithm presented in this paper consists of transforming the Toeplitz matrix into a another structured matrix called Cauchy¿like. The particular properties of Cauchy¿like matrices are exploited in order to obtain two levels of parallelism that makes possible to highly reduce the execution time.     Cluster-randomized study of intermittent preventive treatment for malaria in infants (IPTi) in southern Tanzania: evaluation of impact on survival.

    BACKGROUND\ud \ud Intermittent Preventive Treatment for malaria control in infants (IPTi) consists of the administration of a treatment dose of an anti-malarial drug, usually sulphadoxine-pyrimethamine, at scheduled intervals, regardless of the presence of Plasmodium falciparum infection. A pooled analysis of individually randomized trials reported that IPTi reduced clinical episodes by 30%. This study evaluated the effect of IPTi on child survival in the context of a five-district implementation project in southern Tanzania. [Trial registration: clinical trials.gov NCT00152204].\ud \ud METHODS\ud \ud After baseline household and health facility surveys in 2004, five districts comprising 24 divisions were randomly assigned either to receive IPTi (n = 12) or not (n = 12). Implementation started in March 2005, led by routine health services with support from the research team. In 2007, a large household survey was undertaken to assess the impact of IPTi on survival in infants aged two-11 months through birth history interviews with all women aged 13-49 years. The analysis is based on an "intention-to-treat" ecological design, with survival outcomes analysed according to the cluster in which the mothers lived.\ud \ud RESULTS\ud \ud Survival in infants aged two-11 months was comparable in IPTi and comparison areas at baseline. In intervention areas in 2007, 48% of children aged 12-23 months had documented evidence of receiving three doses of IPTi, compared to 2% in comparison areas (P < 0.0001). Over the three years of the study there was a marked improvement in survival in both groups. Between 2001-4 and 2005-7, mortality rates in two-11 month olds fell from 34.1 to 23.6 per 1,000 person-years in intervention areas and from 32.3 to 20.7 in comparison areas. In 2007, divisions implementing IPTi had a 14% (95% CI -12%, 49%) higher mortality rate in two-11 month olds in comparison with non-implementing divisions (P = 0.31).\ud \ud CONCLUSION\ud \ud The lack of evidence of an effect of IPTi on survival could be a false negative result due to a lack of power or imbalance of unmeasured confounders. Alternatively, there could be no mortality impact of IPTi due to low coverage, late administration, drug resistance, decreased malaria transmission or improvements in vector control and case management. This study raises important questions for programme evaluation design

    An overview of the first 5 years of the ENIGMA obsessive–compulsive disorder working group: The power of worldwide collaboration

    Abstract Neuroimaging has played an important part in advancing our understanding of the neurobiology of obsessive?compulsive disorder (OCD). At the same time, neuroimaging studies of OCD have had notable limitations, including reliance on relatively small samples. International collaborative efforts to increase statistical power by combining samples from across sites have been bolstered by the ENIGMA consortium; this provides specific technical expertise for conducting multi-site analyses, as well as access to a collaborative community of neuroimaging scientists. In this article, we outline the background to, development of, and initial findings from ENIGMA's OCD working group, which currently consists of 47 samples from 34 institutes in 15 countries on 5 continents, with a total sample of 2,323 OCD patients and 2,325 healthy controls. Initial work has focused on studies of cortical thickness and subcortical volumes, structural connectivity, and brain lateralization in children, adolescents and adults with OCD, also including the study on the commonalities and distinctions across different neurodevelopment disorders. Additional work is ongoing, employing machine learning techniques. Findings to date have contributed to the development of neurobiological models of OCD, have provided an important model of global scientific collaboration, and have had a number of clinical implications. Importantly, our work has shed new light on questions about whether structural and functional alterations found in OCD reflect neurodevelopmental changes, effects of the disease process, or medication impacts. We conclude with a summary of ongoing work by ENIGMA-OCD, and a consideration of future directions for neuroimaging research on OCD within and beyond ENIGMA

    AbstractFe-Al based alloys exhibit excellent properties but suffer metal dusting in carburizing atmospheres. Surface composition can be a determinant factor in the solution of this problem. We calculate in this work the C adsorption energies in the L21 Fe2AlX (X=Ti,V,Nb) structures and we study the influence of surface covering. Our results show the beneficial effect of Ti, suggesting that there could exist an activation energy to promote the incorporation of C in the subsurface layers when the surface is covered enoug

    In 2007 Cortés and Peña introduced a pivoting strategy for the Neville elimination of nonsingular sign regular matrices and called it two-determinant pivoting. Neville elimination has been very useful for obtaining theoretical and practical properties for totally positive (negative) matrices and other related types of matrices. A real matrix is said to be almost strictly sign regular if all its nontrivial minors of the same order have the same strict sign.
