Reação de polimerização em cadeia (PCR) em etapa única para diagnóstico de Leishmania do subgênero (Viannia)
No Brasil, o principal agente etiológico da leishmaniose, apresentando freqüentemente comprometimento das mucosas, pertence ao subgênero (Viannia). A conduta terapêutica no tratamento da leishmaniose depende de seu diagnóstico parasitológico e os métodos clássicos restringem sua identificação. Neste trabalho, descrevemos uma reação de PCR, utilizando primers desenhados a partir de seqüências repetitivas de mini-exons, que amplificam um fragmento de 177pb e que são capazes de distinguir o subgênero (Viannia) do subgênero (Leishmania), tornando-se uma ferramenta útil no diagnóstico desta doença.In Brazil, the main etiologic agent of Leishmaniasis that frequently presents with mucosal involvement belongs to the Viannia subgenus. The therapeutic conduct in this disease depends on the parasitological diagnosis, and classical methods are restricted in identifying the agent. Ageing impairs protective immunity and promotes susceptibility to murine visceral leishmaniasis
It is well accepted that the impact of diseases is generally more detrimental in elderly individuals than in younger ones. Changes in the immune system due to ageing can directly affect the ability to respond effectively to infections and may contribute to the higher morbidities and mortalities in the elderly population. Leishmaniasis is a complex of clinically unique diseases caused by obligate intracellular protozoa belonging to genus Leishmania, wherein visceral leishmaniasis (VL) is the most severe form and is fatal if left untreated. In this study, aged mice (72 weeks old) presented increased susceptibility to L. infantum infection compared to younger mice (4–6-week-old), with notable parasitism in both the spleen and liver, as well as exhibiting hepatosplenomegaly. A pronounced inflammatory profile was observed in the aged-infected mice, with excessive production of TNF-α and nitrite, along with diminished IFN-γ production and reduced proliferative capacity of T cells (assessed by expression of the Ki67 marker). Additionally, both CD4+ and CD8+ T cells from the aged-infected mice presented increased expression of the inhibitory receptors PD-1 and KLRG1 that strongly correlated with the parasitism found in the liver and spleen of this group. Overall, the data reported in this study suggests for the first time that ageing may negatively impact the VL outcome and provides a perspective for new therapeutic strategies involving manipulation of immunosenescence features against Leishmania infection
A case of paracoccidioidomycosis due to Paracoccidioides lutzii presenting sarcoid-like form
Paracoccidioidomycosis (PCM) is a fungal disease caused by Paracoccidioides spp., which can cause a systemic granulomatous infection with tegumentary and visceral involvement. Sarcoid-like skin lesions are uncommon and can be misdiagnosed due to similarities with other granulomatous diseases. Transcriptomic landscape of skin lesions in cutaneous leishmaniasis reveals a strong CD8(+) T cell immunosenescence signature linked to immunopathology
The severity of lesions that develop in patients infected by Leishmania braziliensis is mainly associated with a highly cytotoxic and inflammatory cutaneous environment. Recently, we demonstrated that senescent T and NK cells play a role in the establishment and maintenance of this tissue inflammation. Here, we extended those findings using transcriptomic analyses that demonstrate a strong co-induction of senescence and pro-inflammatory gene signatures in cutaneous leishmaniasis (CL) lesions. The senescence-associated signature was characterized by marked expression of key genes such as ATM, Sestrin 2, p16, p21 and p38. The cell type identification from deconvolution of bulk sequencing data showed that the senescence signature was linked with CD8+ effector memory and TEMRA subsets and also senescent NK cells. A key observation was that the senescence markers in the skin lesions are age-independent of patients and were correlated with lesion size. Moreover, a striking expression of the senescence-associated secretory phenotype (SASP), pro-inflammatory cytokine and chemokines genes was found within lesions that were most strongly associated with the senescent CD8 TEMRA subset. Collectively, our results confirm that there is a senescence transcriptomic signature in CL lesions and supports the hypothesis that lesional senescent cells have a major role in mediating immunopathology of the disease
Foco de leishmaniose tegumentar americana nos municípios de Viana e Cariacica, Estado do Espirito Santo, Brasil
A mucocutaneous leishmaniasis focus occuring in four village situated in Viana and Cariacica counties, located near the sea-coast of Espírito Santo State, Brazil, is described. Seventy one cases of the disease were diagnosed between May 1978 and December 1982. The laboratorial confirmation was obtained by the Montenegro test in 66 patients and by biopsy in 15. The 0 to 10 year age-group was the most affected. The strains of the parasites isolated from 3 patients and 27 dogs were classified as Leishmania braziliensis braziliensis. Among 257 wild animals captured the Leishmania was isolated in two Proechimys iheringi only, and identified as L. mexicana aristedesi. The most numerous sandfly specimens (Lutzomyia intermedia, L. fischeri and L. migonei) were captured with greater frequency both inside and outside residences. The possibility of the transmission's occuring both inside and outside residences was noteworthy.Descreve-se um foco de leishmaniose tegumentar americana abrangendo quatro localidades dos municípios de Viana e Cariacica situados próximo ao litoral do Estado do Espírito Santo, Brasil. Foram diagnosticados 71 casos no período de maio de 1978 a dezembro de 1982. A confirmação laboratorial foi obtida pela intradermorreação de Montenegro em 66 pacientes e pelo encontro do parasita em 15. A faixa etária mais acometida foi a de 0-10 anos. Amostras do parasita isoladas de 3 pacientes e de 27 cães foram classificadas como Leishmania braziliensis braziliensis. De apenas dois Proechimys iheringi, entre 257 mamíferos silvestres estudados, isolou-se Leishmania, que foi identificada como Leishmania mexicana aristedesi. As espécies de flebotomíneos mais numerosas (Lutzomyia intermedia, L. fischeri e L. migonei) apresentaram um elevado grau de adaptação aos ambientes intra e peri-domibiliar. Levantou-se a hipótese da transmissão estar ocorrendo no intra e peri-domicílio
Dengue: clinical forms and risk groups in a high incidence city in the southeastern region of Brazil
Differentiation of mucosal lesions in mucocutaneous leishmaniasis and paracoccidioidomycosis.
Mucocutaneous leishmaniasis and paracoccidioidomycosis are infectious diseases with similar epidemiological and clinical aspects. Cases of both diseases may manifest similar lesions in the mucosa. Therefore, the determination of distinguishing characteristics for the purpose of differential diagnosis is critical for better management of the diseases. The present study evaluated factors that assist in the differentiation of mucosal lesions between these diseases. This cross-sectional study included data from medical records of 122 cases of mucocutaneous leishmaniasis and 83 cases of paracoccidioidomycosis attended at the university hospital Cassiano Antonio Moraes, located in Vitória, Espírito Santo State, Brazil. Comparison between the diseases included the following variables: sex, age, time of disease evolution, location of the lesion and symptoms. Adults and males were affected by both diseases at higher rates. Lesions in the nasal region (95.1%; p-value = 0.000) and the pharynx (20.5%; p-value = 0.009) and nasal obstruction (34.4%; p-value = 0.000) were associated with leishmaniasis. Paracoccidioidomycosis was associated with lesions in the oral region (90.4%; p-value = 0.000), oral pain (16.9%; p-value = 0.000), and hoarseness (14.5%; p-value = 0.008). In leishmaniasis, lesions in oral regions were not associated with oral pain and were frequently located close to the nasal area. The manifestations cited above could improve the differential diagnosis of leishmaniasis and paracoccidioidomycosis, and thereby potentially aid in the choice of appropriate confirmatory diagnostic testing
