    Obliteration of Dentinal Tubules by Desensitizing Agents Based on Silver Fluoride/Potassium Iodide or Pre-Reacted Glass Particles: An in Vitro Study

    Objective: To evaluate the efficacy of desensitizing agents for the obliteration of dentinal tubules subjected or not to a simulated oral environment. Material and Methods: Dentinal discs (n=8) treated with Riva-Star (RS) or PRG-Barrier-Coat (PRG) were submitted (cycled) or not submitted (control) to erosive-abrasive-thermal cycles and evaluated using scanning electron microscopy/energy dispersive spectroscopic analysis. The variables analyzed were tubule obliteration and dentin surface chemical composition. Data were analyzed by non-parametric tests (p<0.05). Results: The cycled and control groups did not differ significantly for the responses in each material. The PRG control and cycled groups had fewer visible tubules and a higher proportion of totally obliterated tubules than the RS groups. The percentages of silver coverage were higher in the RS-control than in the RS-cycled. There was a significant inverse correlation between the presence of silver and non-obliterated tubules (R=-0.791; p<0.001). The percentages of carbon, aluminum, strontium, and potassium were significantly higher in the PRG-control and PRG-cycled compared to the RS control. The percentages of calcium, phosphorus, and silver were significantly higher in the RS compared to the PRG groups. PRG-control showed a higher percentage of boron than RS-control. Conclusion: PRG promoted greater tubule obliteration than SR. Simulated stress did not affect the obliterating effect of each agent. Greater silver coverage corresponded to a lower proportion of non-obliterated tubules in RS. Carbon, aluminum, strontium, boron, and potassium predominated in the dentin surface treated with PRG, while calcium, phosphorus, and silver prevailed in RS groups

    Association between Dentin Hypersensitivity and Health/Oral Health-related Quality of Life: A Systematic Review and Meta-Analysis

    Objective: To investigate the association between DH and Health (HRQoL) or Oral Health-Related Quality of Life (OHRQoL). Material and Methods: PubMed, Web of Science, Scopus, EMBASE, Cochrane, Scielo, LILACS/BBO, Biblioteca Digital de Teses e Dissertações (BDTD), Open Grey, and Google Scholar databases were screened in September 2019 (updated in October 2022). Observational studies were selected to compare HRQoL/OHRQoL(outcome) according to DH(exposure) or evaluate the association among these variables. Standardized Joanna Briggs Institute critical appraisal tool for analytical cross-sectional studies was used to analyze the risk of bias. A random-effects meta-analysis was conducted to synthesize evidence for the association between DH and OHRQoL. Results: 10 papers met inclusion criteria and were evaluated. In most studies, presenting or having a greater intensity of DH was associated with a negative impact on one\u27s quality of life. However, most of these studies showed a moderate to high risk of methodological bias. The consistent finding from studies with a low risk of bias suggests a significant association between DH and OHRQoL. Meta-analysis was feasible for three studies with substantial heterogeneity. The pooled Odds Ratio was 2.14 (95%CI 1.15-3.99; I2=57,44%).Conclusion: Many studies presented a high risk of bias; therefore, the actual effect of DH on one\u27s quality of life remains uncertain

    Effect of supragingival plaque control on subgingival microflora and periodontal tissues

    O objetivo deste trabalho foi estudar, clínica e microbiologicamente, 44 sítios em 11 pacientes com periodontite crônica generalizada. IP, IG, SS, PS e NI foram registrados. Amostras de placa subgengival foram colhidas nos mesmos sítios para cultivo de bactérias anaeróbias e determinação dos morfotipos microbianos por MCE. Os registros clínicos e estudos microbiológicos foram tomados no "baseline" e 4 semanas após a incorporação em um programa de controle de placa e cálculo supragengival. A análise microbiológica categorizou o grau de desenvolvimento em: 0 - não detectado, 1 - escasso, 2 - moderado e 3 - abundante. Os registros clínicos no "baseline" e dia 28 foram: IP - 1,73 ± 0,10 e 0,30 ± 0,08, IG - 1,73 ± 0,08 e 1,41 ± 0,08, SS - 0,91 ± 0,04 e 0,59 ± 0,07, PS - 6,43 ± 0,20 e 5,77 ± 0,25, NI - 6,86 ± 0,32 e 6,52 ± 0,34, respectivamente. A redução do IP, IG, SS e PS foi significativa. Não foram registradas diferenças significativas no NI. As proporções relativas dos morfotipos bacterianos observados por MCE no "baseline" e dia 28 foram: células cocóideas - 21,16 ± 3,77 e 36,00 ± 4,66, bacilos móveis - 44,86 ± 2,65 e 39,50 ± 2,64, treponemas totais - 24,66 ± 3,08 e 19,25 ± 2,75. No "baseline" e no dia 28 foi observado: Pi/n - 1,36 ± 0,18 e 0,43 ± 0,11, Pg - 0,48 ± 0,16 e 0,32 ± 0,13, Aa - 0,23 ± 0,09 e 0,23 ± 0,10, Fusobacterium nucleatum - 0,32 ± 0,14 e 0,41 ± 0,13 e peptostreptococos - 0,82 ± 0,19 e 0,54 ± 0,16, respectivamente. Houve um aumento significativo das células cocóideas, diminuição de treponemas e de Pi/n.The aim of this study was to investigate, clinically and microbiologically, forty-four sites in 11 patients presenting with generalized chronic periodontitis. Plaque Index (PI), Gingival Index (GI), Probing Bleeding (PB), Probing Depth (PD) and Insertion Level (IL) were registered. Samples of subgingival plaque were collected in the same sites for cultivation of anaerobic bacteria and determination of microbiological morphotypes using dark field microscopy. Clinical and microbiological data were recorded on the baseline and 4 weeks after the adoption of a program to control supragingival plaque and calculus. The microbiological analysis categorized the degree of development as follows: 0 - not detected, 1 - scarce, 2 - moderate and 3 - abundant. The clinical results at the baseline and on the 28th day were, respectively: PI - 1.73 ± 0.10 and 0.30 ± 0.08; GI - 1.73 ± 0.08 and 1.41 ± 0.08; PB - 0.91 ± 0.04 and 0.59 ± 0.07; PD - 6.43 ± 0.20 and 5.77 ± 0.25; and IL - 6.86 ± 0.32 and 6.52 ± 0.34. There was significant decrease in PI, GI, PB and PD. However, the difference in IL was not significant. The relative proportions of the microbial morphotypes observed under dark field microscopy at the baseline and on the 28th daywere, respectively: coccoid cells - 21.16 ± 3.77 and 36.00 ± 4.66; mobile bacillus - 44.86 ± 2.65 and 39.50 ± 2.64; and total treponemes - 24.66 ± 3.08 and 19.25 ± 2.75 . The cultures presented, at the baseline and on the 28th day, respectively: Prevotella intermedia/nigrescens (Pi/n) - 1.36 ± 0.18 and 0.43 ± 0.11; Porphyromonas gingivalis - 0.48 ± 0.16 and 0.32 ± 0.13; Actinobacillus actinomycetemcomitans - 0.23 ± 0.09 and 0.23 ± 0.10; Fusobacterium nucleatum - 0.32 ± 0.14 and 0.41 ± 0.13; and peptostreptococci - 0.82 ± 0.19 and 0.54 ± 0.16.     Analysis of Bone-implant Interfacial Stress during Disocclusion in Complete Implant-supported Prosthesis: A Finite Element Study

    Aim: Little is known about the biomechanical behavior of disocclusion patterns in implant-supported prostheses. Thus, the aim of the present study was to analyze the stresses generated at bone-implant interface during different patterns of disocclusion in an implant-supported lower complete denture without free distal ends using the three-dimensional (3-D) finite element method. Study Design: Finite element method. Methods: A 3-D model of a complete denture supported by five inter-foraminal implants and two distal was developed (CAE software Abaqus). A canine guide disocclusion (CG) was simulated applying a nodal load of 15 N at an angle of 45° on the canine tooth prosthesis, while to a bilateral balanced occlusion (BBO) a similar load pattern was applied at four distinct points, bilaterally. Linear elastic static analysis was used to compare the magnitude of maximum and minimum principal stresses at bone-implant interface for each simulation. Results: The disocclusion pattern generated during CG exhibited a greater stress concentration at the bone-implant interface of the distal implant on the working side. BBO showed a more homogeneous stress distribution pattern at the bone-implant interface of the two distal implants. Anterior implants showed lower stress concentration. Conclusions: Bilateral balanced occlusion (BBO) resulted in a more favorable stress distribution in this complete implant-supported prosthesis model

    Implications of Edentulism on Quality of Life among Elderly

    This study aimed was to test the association between quality of life and edentulism among elderly individuals in a city in southeastern Brazil. This cross-sectional study was carried out with 163 individuals aged 60 years or older, functionally independent and non-institutionalized. Data were collected with a questionnaire and oral examination. The edentulism was the dependent variable. The independent variables were sex, age, household income and quality of life (WHOQOL-Old) and their scores. To assess the association between the dependent variable and independent variables was used bivariate analysis (p < 0.10). Poisson regression model was performed, adjusting for age and sex. The average age of participants was 69 years (± 6.1), 68.7% were female and 52.8% were diagnosed as completely edentulous (90% CI: 0.33–1.24). When the independent variables were associated to the prevalence of edentulism, statistically significant associations were found for age (p = 0.03) and social participation dimension of the WHOQOL-Old (p = 0.08). In the Poisson regression, social participation remained statistically associated to edentulism {RP = 2.12 [90% CI (1.10–4.00)]}. The social participation proved to have a significant association to edentulism, thereby attesting to the negative effect of this condition on social aspects
