Effect of high-vacuum setting on phacoemulsification efficiency
Purpose
To evaluate the effect of a high-vacuum setting versus a low-vacuum setting on the efficiency of phacoemulsification.

Design
Prospective clinical trial.
Methods
Consecutive patients having cataract surgery in 2014 were recruited. Cataract surgery was performed by 2 experienced surgeons using a phacoemulsification machine with monitored forced infusion. The cataractous lens was split into 2 heminuclei using the stop-and-chop technique; in 1 heminucleus, phacoemulsification and aspiration used a high-vacuum setting (600 mm Hg; treatment group) and in the other heminucleus, a low-vacuum setting (350 mm Hg; control group). The high and low settings were alternated by case per the operating list to reduce surgeon bias. The main outcome measures were cumulative dissipated energy (CDE) and active heminucleus removal time.
Results
One hundred sixty patients (160 eyes) were enrolled in the study, and 158 were included in the analysis. The CDE per heminucleus was significantly lower with the high-vacuum setting than with the low-vacuum setting (mean 2.81 percent-seconds; 95% confidence interval (CI), 2.44-3.21 versus 3.81 percent-seconds; 95% CI, 3.38-4.20; P < .001). The active heminucleus removal time was significantly shorter in the high-vacuum group than the low-vacuum group (mean 27.77 seconds; 95% CI, 25.26-30.19 versus 33.59 seconds; 95% CI, 31.07-35.92; P < .001). The observed differences were independent of the surgeon, patient age and sex, incision size, and nucleus density. No intraoperative complications were observed in either group.

On Distributive Subalgebras of Qualitative Spatial and Temporal Calculi
Qualitative calculi play a central role in representing and reasoning about
qualitative spatial and temporal knowledge. This paper studies distributive
subalgebras of qualitative calculi, which are subalgebras in which (weak)
composition distributives over nonempty intersections. It has been proven for
RCC5 and RCC8 that path consistent constraint network over a distributive
subalgebra is always minimal and globally consistent (in the sense of strong
-consistency) in a qualitative sense. The well-known subclass of convex
interval relations provides one such an example of distributive subalgebras.
This paper first gives a characterisation of distributive subalgebras, which
states that the intersection of a set of relations in the subalgebra
is nonempty if and only if the intersection of every two of these relations is
nonempty. We further compute and generate all maximal distributive subalgebras
for Point Algebra, Interval Algebra, RCC5 and RCC8, Cardinal Relation Algebra,
and Rectangle Algebra. Lastly, we establish two nice properties which will play
an important role in efficient reasoning with constraint networks involving a

Examining the Relationships among the Coaching Climate, Life Skills Development and Well-Being in Sport
Using Benson and Saito's framework for positive youth development, we investigated the relationships between the coaching climate, young people's perceived life skills development within sport, and their psychological well-being. British youth sport participants (N=326, Mage=13.81, range=11â18 years) completed a survey assessing the coaching climate, participants' perceived life skills development (teamwork, goal setting, time management, emotional skills, interpersonal communication, social skills, leadership, and problem solving and decision making) and psychological well-being (self-esteem, positive affect, and satisfaction with life). In all analyses, the coaching climate was positively related to young peoples' perceived development of life skills within sport and their psychological well-being. Total life skills development (a summative score of all eight life skills scores) was positively related to all three psychological well-being indicators â providing support for the "pile-up" effect â and partially mediated the relationships between the coaching climate and participants' psychological well-being. Interpretation of the results indicated that coaches should foster the development of multiple life skills in youth sport participants, as they are associated with participants' psychological well-being. Whither Capitalism? Financial externalities and crisis
As with global warming, so with financial crises â externalities have a lot to answer for. We
look at three of them. First the financial accelerator due to âfire salesâ of collateral assets -- a
form of pecuniary externality that leads to liquidity being undervalued. Second the ârisk-
shiftingâ behaviour of highly-levered financial institutions who keep the upside of risky
investment while passing the downside to others thanks to limited liability. Finally, the
network externality where the structure of the financial industry helps propagate shocks
around the system unless this is checked by some form of circuit breaker, or âring-fenceâ.
The contrast between crisis-induced Great Recession and its aftermath of slow growth in the
West and the rapid - and (so far) sustained - growth in the East suggests that successful
economic progress may depend on how well these externalities are managed
Metal-insulator transition in vanadium dioxide nanobeams: probing sub-domain properties of strongly correlated materials
Many strongly correlated electronic materials, including high-temperature
superconductors, colossal magnetoresistance and metal-insulator-transition
(MIT) materials, are inhomogeneous on a microscopic scale as a result of domain
structure or compositional variations. An important potential advantage of
nanoscale samples is that they exhibit the homogeneous properties, which can
differ greatly from those of the bulk. We demonstrate this principle using
vanadium dioxide, which has domain structure associated with its dramatic MIT
at 68 degrees C. Our studies of single-domain vanadium dioxide nanobeams reveal
new aspects of this famous MIT, including supercooling of the metallic phase by
50 degrees C; an activation energy in the insulating phase consistent with the
optical gap; and a connection between the transition and the equilibrium
carrier density in the insulating phase. Our devices also provide a
nanomechanical method of determining the transition temperature, enable
measurements on individual metal-insulator interphase walls, and allow general

TGF-beta 1 induces human alveolar epithelial to mesenchymal cell transition (EMT)
Background: Fibroblastic foci are characteristic features in lung parenchyma of patients with idiopathic pulmonary fibrosis (IPF). They comprise aggregates of mesenchymal cells which underlie sites of unresolved epithelial injury and are associated with progression of fibrosis. However, the cellular origins of these mesenchymal phenotypes remain unclear. We examined whether the potent fibrogenic cytokine TGF-β1 could induce epithelial mesenchymal transition (EMT) in the human alveolar epithelial cell line, A549, and investigated the signaling pathway of TGF-β1-mediated EMT.
Methods: A549 cells were examined for evidence of EMT after treatment with TGF-β1. EMT was assessed by: morphology under phase-contrast microscopy; Western analysis of cell lysates for expression of mesenchymal phenotypic markers including fibronectin EDA (Fn-EDA), and expression of epithelial phenotypic markers including E-cadherin (E-cad). Markers of fibrogenesis, including collagens and connective tissue growth factor (CTGF) were also evaluated by measuring mRNA level using RT-PCR, and protein by immunofluorescence or Western blotting. Signaling pathways for EMT were characterized by Western analysis of cell lysates using monoclonal antibodies to detect phosphorylated Erk1/2 and Smad2 after TGF-β1 treatment in the presence or absence of MEK inhibitors. The role of Smad2 in TGF-β1-mediated EMT was investigated using siRNA.
Results: The data showed that TGF-β1, but not TNF-ι or IL-1β, induced A549 cells with an alveolar epithelial type II cell phenotype to undergo EMT in a time-and concentration-dependent manner. The process of EMT was accompanied by morphological alteration and expression of the fibroblast phenotypic markers Fn-EDA and vimentin, concomitant with a downregulation of the epithelial phenotype marker E-cad. Furthermore, cells that had undergone EMT showed enhanced expression of markers of fibrogenesis including collagens type I and III and CTGF. MMP-2 expression was also evidenced. TGF-β1-induced EMT occurred through phosphorylation of Smad2 and was inhibited by Smad2 gene silencing; MEK inhibitors failed to attenuate either EMT-associated Smad2 phosphorylation or the observed phenotypic changes.
Conclusion: Our study shows that TGF-β1 induces A549 alveolar epithelial cells to undergo EMT via Smad2 activation. Our data support the concept of EMT in lung epithelial cells, and suggest the need for further studies to investigate the phenomenon
Toward Understanding Massive Star Formation
Although fundamental for astrophysics, the processes that produce massive
stars are not well understood. Large distances, high extinction, and short
timescales of critical evolutionary phases make observations of these processes
challenging. Lacking good observational guidance, theoretical models have
remained controversial. This review offers a basic description of the collapse
of a massive molecular core and a critical discussion of the three competing
concepts of massive star formation:
- monolithic collapse in isolated cores
- competitive accretion in a protocluster environment
- stellar collisions and mergers in very dense systems
We also review the observed outflows, multiplicity, and clustering properties
of massive stars, the upper initial mass function and the upper mass limit. We
conclude that high-mass star formation is not merely a scaled-up version of
low-mass star formation with higher accretion rates, but partly a mechanism of
its own, primarily owing to the role of stellar mass and radiation pressure in

Impact of inhaled corticosteroids on growth in children with asthma: systematic review and meta-analysis
Background: Long-term inhaled corticosteroids (ICS) may reduce growth velocity and final height of children with asthma. We aimed to evaluate the association between ICS use of >12 months and growth. Methods: We initially searched MEDLINE and EMBASE in July 2013, followed by a PubMed search updated to December 2014. We selected RCTs and controlled observational studies of ICS use in patients with asthma. We conducted random effects meta-analysis of mean differences in growth velocity (cm/year) or final height (cm) between groups. Heterogeneity was assessed using the I2 statistic. Results: We found 23 relevant studies (twenty RCTs and three observational studies) after screening 1882 hits. Meta-analysis of 16 RCTs showed that ICS use significantly reduced growth velocity at one year follow-up (mean difference -0.48 cm/year (95% CI -0.66 to -0.29)). There was evidence of a dose-response effect in three RCTs. Final adult height showed a mean reduction of -1.20 cm (95% CI -1.90 cm to -0.50 cm) with budesonide versus placebo in a high quality RCT. Meta-analysis of two lower quality observational studies revealed uncertainty in the association between ICS use and final adult height, pooled mean difference -0.85 cm (95% CI -3.35 to 1.65). Conclusion: Use of ICS for >12 months in children with asthma has a limited impact on annual growth velocity. In ICS users, there is a slight reduction of about a centimeter in final adult height, which when interpreted in the context of average adult height in England (175 cm for men and 161 cm for women), represents a 0.7% reduction compared to non-ICS users
Exposure assessment of radon in the drinking water supplies: a descriptive study in Palestine
<p>Abstract</p> <p>Background</p> <p>Radon gas is considered as a main risk factor for lung cancer and found naturally in rock, soil, and water. The objective of this study was to determine the radon level in the drinking water sources in Nablus city in order to set up a sound policy on water management in Palestine.</p> <p>Methods</p> <p>This was a descriptive study carried out in two phases with a random sampling technique in the second phase. Primarily, samples were taken from 4 wells and 5 springs that supplied Nablus city residents. For each source, 3 samples were taken and each was analyzed in 4 cycles by RAD 7 device manufactured by Durridge Company. Secondly, from the seven regions of the Nablus city, three samples were taken from the residential tap water of each region. Regarding the old city, ten samples were taken. Finally, the mean radon concentration value for each source was calculated.</p> <p>Results</p> <p>The mean (range) concentration of radon in the main sources were 6.9 (1.5-23.4) Becquerel/liter (Bq/L). Separately, springs and wells' means were 4.6 Bq/L and 9.5 Bq/L; respectively. For the residential tap water in the 7 regions, the results of the mean (range) concentration values were found to be 1.0 (0.9-1.3) Bq/L. For the old city, the mean (range) concentration values were 2.3 (0.9-3.9) Bq/L.</p> <p>Conclusions</p> <p>Except for Al-Badan well, radon concentrations in the wells and springs were below the United State Environmental Protection Agency maximum contaminated level (U.S EPA MCL). The level was much lower for tap water. Although the concentration of radon in the tap water of old city were below the MCL, it was higher than other regions in the city. Preventive measures and population awareness on radon's exposure are recommended.</p
