New approach for analysing dynamical processes on the surface of photospheric vortex tubes

The majority of studies on multi-scale vortex motions employ a two-dimensional geometry by using a variety of observational and numerical data. This approach limits the understanding the nature of physical processes responsible for vortex dynamics. Here we develop a new methodology to extract essential information from the boundary surface of vortex tubes. 3D high-resolution magnetoconvection MURaM numerical data has been used to analyse photospheric intergranular velocity vortices. The Lagrangian Averaged Vorticity Deviation (LAVD) technique was applied to define the centers of vortex structures and their boundary surfaces based on the advection of fluid elements. These surfaces were mapped onto a constructed envelope grid that allows the study of the key plasma parameters as functions of space and time. Quantities that help in understanding the dynamics of the plasma, e.g. Lorentz force, pressure force, plasma-$\beta$ were also determined. Our results suggest that, while density and pressure have a rather global behaviour, the other physical quantities undergo local changes, with their magnitude and orientation changing in space and time. At the surface, the mixing in the horizontal direction is not efficient, leading to appearance of localized regions with higher/colder temperatures. In addition, the analysis of the MHD Poynting flux confirms that the majority of the energy is directed in the horizontal direction. Our findings also indicate that the pressure and magnetic forces that drive the dynamics of the plasma on vortex surfaces are unbalanced and therefore the vortices do not rotate as a rigid body

A prospective antibiotic point prevalence survey in two primary referral hospitals during and after pilgrims stay in Madinah, Saudi Arabia

Purpose: To assess current patterns of antibiotic use by carrying out two point-prevalence surveys (PPS) in Madinah after the return of hajj pilgrims from Makkah and when Madinah is free from pilgrims. Methods: In September 2016 and November 2016, a prospective PPS was conducted on two separate dates (during the hajj pilgrims stay in Madinah and after they leave). Data on antibiotics use were generated during these two periods. This involved an audit from all the departments of two referral hospitals (King Fahad Hospital (KFH) - 425 beds, and Al Ansar Hospital - 100 beds) of inpatients records. Data were collected using standard forms adapted from the European Centre for Disease Control (ECDC). Results: A total of 675 inpatients were included in PPS; among them, 332 (49.18 %) patients were receiving antibiotic therapy. In September 2016, 168 patients were treated with antibiotics, with a prevalence rate of 50.60 %, whereas, in November 2016, the prevalence rate was 49.40 %. Overall, 198 patients were identified in surgical wards, of which 132 patients (66.6 %) were receiving antibiotic therapy; 121 patients in ICU of which 70 patients (57.8 %) received antibiotics; 13 patients in other wards of which 6 (46.1 %) received antibiotic treatment; and 343 patients in medical wards of which 126 patients (36.7 %) were treated with antibiotics. There was no significant difference in prevalence of antibiotic prescribing between the two surveys (Pearson Chi-square test, p = 0.56) and with regards to patient age between the two surveys (Mann-Whitney U-test, p = 0.32). Conclusion: The results demonstrate that antibiotic use with adherence to hospital guidelines and PPS helps in identifying targets for quality improvement. Moreover, to escalate the prudent use of antibiotics in hospitals, PPS provides a useful tool. Furthermore, this survey provides a background to evaluate antibiotic use by a standardized methodology. Re-infection with a different SARS-CoV-2 clade and prolonged viral shedding in a hematopoietic stem cell transplantation patient

Immunocompromised patients who have a severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection pose many clinical and public health challenges. We describe the case of a hematopoietic stem cell transplantation patient with lymphoma who had a protracted illness requiring three consecutive hospital admissions. Whole genome sequencing confirmed two different SARS-CoV-2 clades. Clinical management issues and the unanswered questions arising from this case are discussed

Ipsilateral versus contralateral static endurance- balance abilities among healthy college students

الملخص: أهداف البحث: سعت هذه الدراسه ألي تحديد الأرتباط والفرق، إن وجد، بين الجانب المماثل وبين الجانبين المتقابلين أثناء تمرين التوازن والتحمل الثابت. طرق البحث: شارك ١١٢ طالب جامعي يتمتع بصحه جيده (٥٥ إناث، ٥٧ ذكور) في هذه الدراسه. أجري كل مشارك تمرين Quadruped bird dog ( رفع ذراع واحده والساق في الجانب المقابل) والحفاظ علي التوازن في حاله ثابته. كما أجري كل مشارك تمرين ال side bridge من وضع الأستلقاء علي الجانب ورفع الجذع والوركين والحفاظ علي التوازن لأطول فتره ممكنه. النتائج: أظهر أختبار Wilcoxon فرق ذو دلاله أحصائيه لتمرين Quadruped bird dog وليس لتمرين Side bridge. كما أظهرت الدراسه معامل أرتباط قوي مقداره 0.85 و 0.75 لتمرين Quadruped bird dog وتمرين Side bridge علي التوالي. الأستنتاجات: قد تكون هذه النتائج مفيده لأخصائي التأهيل للتقييم الموضوعي لقدرات التوازن والتحمل الثابت ومتابعة التقدم في العلاج. كما أنه يساعد في تحديد التمرين المناسب والمتدرج لثبات الجذع. Abstract: Objectives: This study sought to determine the association and the difference, if any, between the levels of the contralateral and between the levels of the ipsilateral sides during static endurance-balance exercise. Methods: One hundred twelve healthy active- college students (55 females and 57 males) participated in this cross-sectional study. Each participant performed the contralateral (raising one arm and opposite-side leg) quadruped bird dog exercise and balanced in static condition. Side bridge exercise was performed from lying on your side then engaging your core muscles and lifting your upper body and hips off the ground, maintaining a straight line and holding this position as long as tolerated. Results: Wilcoxon signed rank test showed significant difference (p = 0.004) between the contralateral right and left quadruped bird dog but insignificant difference (p = 0.059) between the ipsilateral right and left side bridge endurance-balance exercises. Mann–Whitney U test showed that the holding time was significant across gender for the contralateral but was insignificant for the ipsilateral endurance-balance exercise. Mann–Whitney U test was insignificant (p > 0.05) between those being recreationally active or inactive. Kruskal–Wallis test revealed insignificant difference between body mass index categories. Spearman's rho correlation coefficient showed strong positive correlation equals 0.85 and 0.75 (p < 0.001) of the contralateral quadruped bird dog and the ipsilateral side bridge exercises respectively. Conclusions: A significant difference was observed for contralateral, while no significant difference was found for ipsilateral endurance-balance abilities. Therefore, clinicians and rehab specialist should consider these findings when assessing the endurance-balance abilities to properly devise appropriate exercise progression of different trunk stabilizers
