    Surgical aortic valve replacement and patient-prosthesis mismatch a meta-analysis of 108 182 patients

    OBJECTIVES: This study sought to evaluate the impact of patient–prosthesis mismatch (PPM) on the risk of perioperative, early-, mid- and long-term mortality rates after surgical aortic valve replacement. METHODS: Databases were searched for studies published until March 2018. The main outcomes of interest were perioperative mortality, 1-year mortality, 5-year mortality and 10-year mortality. RESULTS: The search yielded 3761 studies for inclusion. Of these, 70 articles were analysed, and their data were extracted. The total num- ber of patients included was 108 182 who underwent surgical aortic valve replacement. The incidence of PPM after surgical aortic valve re- placement was 53.7% (58 116 with PPM and 50 066 without PPM). Perioperative mortality [odds ratio (OR) 1.491, 95% confidence interval (CI) 1.302–1.707; P < 0.001], 1-year mortality (OR 1.465, 95% CI 1.277–1.681; P < 0.001), 5-year mortality (OR 1.358, 95% CI 1.218–1.515; P < 0.001) and 10-year mortality (OR 1.534, 95% CI 1.290–1.825; P < 0.001) were increased in patients with PPM. Both severe PPM and moderate PPM were associated with increased risk of perioperative mortality, 1-year mortality, 5-year mortality and 10-year mortality when analysed together and separately, although we observed a higher risk in the group with severe PPM. CONCLUSIONS: Moderate/severe PPM increases perioperative, early-, mid- and long-term mortality rates proportionally to its severity. The findings of this study support the implementation of surgical strategies to prevent PPM in order to decrease mortality rates

    AVALIAÇÃO DO RISCO DE QUEDAS E QUALIDADE DE VIDA DOS IDOSOS COM ACIDENTE VASCULAR ENCEFÁLICO

    http://dx.doi.org/10.5902/2236583411720O presente estudo caracteriza-se por ser randomizado, sendo avaliados, risco de quedas, capacidade funcional, equilíbrio corporal e qualidade de vida, através da escala de Dawton, escala de Medida de Independência Funcional, escala de Berg, The Medical Outcomes Study 36-item Short-Form Health Survey (SF-36), respectivamente. Foram avaliados os idosos com Acidente Vascular Encefálico, sendo 10 idosos institucionalizados (grupo I) e 10 idosos não institucionalizados (grupo II). Ao avaliar o equilíbrio e risco de quedas em ambos os grupos obteve-se diferença estatisticamente significativa, sendo que as médias para ambos foram favoráveis para os idosos do grupo II demonstrando que os idosos do grupo I estão mais propensos a quedas. Os resultados para análise da capacidade funcional também apresentaram diferença estatisticamente significativa, sendo que os idosos do grupo I obtiveram menor capacidade funcional. Conclui-se que idosos com sequela de Acidente Vascular Encefálico do grupo I apresentaram alto índice de quedas, além de menor capacidade funcional

    Brazilian recommendations on the safety and effectiveness of the yellow fever vaccination in patients with chronic immune-mediated inflammatory diseases

    Background: In Brazil, we are facing an alarming epidemic scenario of Yellow fever (YF), which is reaching the most populous areas of the country in unvaccinated people. Vaccination is the only effective tool to prevent YF. In special situations, such as patients with chronic immune-mediated inflammatory diseases (CIMID), undergoing immunosuppressive therapy, as a higher risk of severe adverse events may occur, assessment of the risk-benefit ratio of the yellow fever vaccine (YFV) should be performed on an individual level. Main body of the abstract: Faced with the scarcity of specific orientation on YFV for this special group of patients, the Brazilian Rheumatology Society (BRS) endorsed a project aiming the development of individualized YFV recommendations for patients with CIMID, guided by questions addressed by both medical professionals and patients, followed an internationally validated methodology (GIN-McMaster Guideline Development). Firstly, a systematic review was carried out and an expert panel formed to take part of the decision process, comprising BRS clinical practitioners, as well as individuals from the Brazilian Dermatology Society (BDS), Brazilian Inflammatory Bowel Diseases Study Group (GEDIIB), and specialists on infectious diseases and vaccination (from Tropical Medicine, Infectious Diseases and Immunizations National Societies); in addition, two representatives of patient groups were included as members of the panel. When the quality of the evidence was low or there was a lack of evidence to determine the recommendations, the decisions were based on the expert opinion panel and a Delphi approach was performed. A recommendation was accepted upon achieving ≥80% agreement among the panel, including the patient representatives. As a result, eight recommendations were developed regarding the safety of YFV in patients with CIMID, considering the immunosuppression degree conferred by the treatment used. It was not possible to establish recommendations on the effectiveness of YFV in these patients as there is no consistent evidence to support these recommendations. Conclusion: This paper approaches a real need, assessed by clinicians and patient care groups, to address specific questions on the management of YFV in patients with CIMID living or traveling to YF endemic areas, involving specialists from many areas together with patients, and might have global applicability, contributing to and supporting vaccination practices. Based on an extensive review of scientific articles, theses and relevant studies, this literature review aims to deepen the understanding of the complex interactions between autism and associated psychiatric disorders. By exploring the available evidence, we highlight the need for a critical approach to understanding the coexistence of autism with psychiatric conditions such as anxiety, depression and obsessive-compulsive disorder. The systematic review offers a synthesis of the most recent findings, addressing not only the diagnostic challenges, but also the clinical and therapeutic impacts of these comorbidities. Furthermore, this review emphasizes the importance of holistic intervention strategies, considering both the specific aspects of autism and the needs associated with psychiatric disorders. The study highlights gaps in the existing literature, pointing to areas in need of future research to improve understanding and clinical management of these complex conditions.
