Innovative olive tree leaves shredder prototype for the valorization of wasted leaves: An application to high-quality compost production
Extra virgin olive oil is considered worldwide as one of the most important products, a standard bearer of the Mediterranean diet. Despite this, the production chain of extra virgin olive oil generates four times more waste than quantity of oil. For this reason, the disposal of olive mill wastes represents a significant environmental problem in all the Mediterranean countries. In this direction, several innovations and improvement strategies were proposed in the literature to correctly manage these by-products and, in some cases, to valorize them by the recovery of polyphenols and other interesting substances. However, innovations and improvement strategies for the valorization of olive tree leaves are definitely neglected in the literature, thus motivating this work. The aims of this work are as follow: firstly, to develop and test an innovative olive tree leaves shredder prototype to help olive oil millers in the management of waste leaves deriving from pruning and olive oil production; secondly, to test the combination of the minced olive tree leaves with other by-products in the production of high-quality compost. The results showed the effectiveness of the tested olive tree leaves shredder in significantly reducing the volume occupied by the leaves by 40%. Moreover, the mixing of the minced olive tree leaves with other by-products lead to a high-quality compost which, in accordance with Italian legislation, could even be certified and labeled with this denomination. Future research will investigate the quantification of the benefits in terms of environmental impacts using life cycle assessment

Speckle-tracking global longitudinal strain as an early predictor of cardiotoxicity in breast carcinoma
Purpose: Recent development in anticancer therapies for breast carcinoma allowed an improvement in patients\u2019 survival, notwithstanding a parallel increase of cardiovascular morbidity. Cardiotoxicity of anticancer therapies represents a relevant problem due to its insidious onset and potentially irreversible cardiac damage. The aim of the present study was to test whether 2D speckle tracking analysis can help in predicting overt systolic dysfunction. Methods: A \u201creal world\u201d cohort of 69 patients with breast carcinoma undergoing adjuvant and/or neo-adjuvant chemotherapy was tested 2D-speckle tracking analysis before the beginning of chemotherapy and every 3 months for 1 year. Clinical data, 12-lead ECGs, and lab tests were collected according to the same visit protocol. Results: Over 1-year follow-up, 19 patients (27 %) developed cardiac dysfunction according to the CREC criteria, with an average onset time from enrolment of 6.8 months. A global longitudinal strain (GLS) threshold 65 1216 % at 3 months from chemotherapy was able to predict subsequent systolic dysfunction development with high sensitivity (80 %) and specificity (90 %) and a negative predictive value of 92 %. After the introduction of cardioprotective drugs, left ventricular ejection fraction (LVEF) progressively recovered, while alterations of GLS persisted at 1-year follow-up. Conclusions: Strain imaging with 2D speckle tracking allows the identification of patients at low-risk for chemotherapy-related systolic dysfunction and can help optimizing resources allocations and improving follow-up quality. GLS can also provide a more accurate prognostic index of resolved systolic dysfunction when compared to standard LVEF
Poorly differentiated thyroid carcinoma: single centre experience and review of the literature
There is controversy in the literature regarding a distinct subset of thyroid carcinoma whose histologically classification falls between well-differentiated and anaplastic carcinomas, previously identified as ‘poorly differentiated thyroid carcinoma’ (PDTC), or ‘insular carcinoma’, in view of the peculiar morphological characteristics of the cell groupings. The correct diagnosis and treatment of this entity have important prognostic and therapeutic significance. In this review, we describe the epidemiology, diagnosis, and management of PDTC and report our single centre experience to add to the limited evidence existing in the literature
Preoperative Ultrasonography in the Evaluation of Suspected Familial Non-Medullary Thyroid Cancer: Are We Able to Predict Multifocality and Extrathyroidal Extension?
Family history of thyroid cancer increases the risk of harboring thyroid malignancies that end up having extrathyroidal extension (ETE) and multifocality on histology; some authors suggest a more aggressive surgical approach. Their pre-operative identification could allow more conservative surgical procedures if none of these features are suspected. Our aim was to assess if neck ultrasonography could identify or exclude multifocality or ETE in these patients to tailor the extent of surgery. This retrospective study included patients with previous thyroid surgery, ≥1 first-grade relative with thyroid cancer, and who had undergone pre-surgical ultrasound. ETE was suspected in the case of thyroid border interruption or gross invasion of perithyroidal tissues. Multiple suspicious nodules were defined as suspicion of multifocal cancer. The cohort consisted of 45 patients (median age 49 years, 40 with thyroid cancer, 30 females). The positive predictive value of ultrasonography in predicting multifocality and ETE was 57.14% (25.25–84.03) and 41.67% (21.5–65.1%), respectively, while the negative predictive values were 63.2% (56.4–69.4%) and 72.7% (63.3–80.5%). Pre-operative ultrasound examination is unable to reliably identify or exclude multifocal disease or extrathyroidal extension. In patients scheduled for surgery and with a first-degree relative affected by DTC, a “negative” pre-operative US report does not exclude the potential finding of multifocality and ETE at final histopathology
