    Incidence of Opportunistic Infections Among Adult HIV Positive People Receiving Co-trimoxazole Prophylaxis

    In Ethiopia, Co-trimoxazole prophylaxis therapy (CPT) used to prevent opportunistic infections among people living with HIV is the standard of practice; however incidence of opportunistic infection and their predictors are rarely documented in the country. This was a retrospective follow up study to describe the incidence and predictors of opportunistic infections among 244 adults receiving CPT. Participants were followed for a median time of 72 weeks. During a study period a total of 53opportunistic infections were recorded; making the overall incidence rate 23.9/100 person-years. High incidence of opportunistic infections is likely to occur if: the clients were married (adjusted hazard ratio (AHR) 1.965;(95% CI: 1.109, 3.451), had history of tuberculosis treatment (AHR: 2.34(95% CI:1.05, 5.24)), patients who are indicated for CPT because of both clinical and WHO clinical staging criteria(AHR 2.418 (95% CI:1.02, 5.72 ),and had poor adherence to CPT (AHR, 2.11 (95% CI: 1.19-3.72)). Eventhough adherence is non-substitutable strategy to prevent opportunistic infection, the cohort of HIV patients failed to adhere to CPT, which in turn resulthigh incidence of opportunistic infections among them, therefore improving adherence as guideline should be a priority to prevent OIs among people living with HIV in the study region

    Prevalence of bovine trypanosomosis in Wemberma district of West Gojjam zone, North West Ethiopia

    A cross-sectional survey of bovine trypanosomosis was carried out in Wemberma district of west Gojjam zone, North West Ethiopia. From three peasant associations in the district (one from the midland and two from lowland), 384 cattle were randomly selected and examined for trypanosomosis. The prevalence of the disease as determined by buffy coat techinque was 7.81(95% CI = 5.11- 10.5%). Trypanosoma vivax and T. congolense were detected from buffy coat positive samples. Among the total of 30 cases of trypanosome infections detected 24(80%) of the infections were due to T.vivax and the rest 6(20 %) were due to T. congolense. No statistically significant associations(P>0.05) were observed between the disease and potential risk factors like age, sex and agroecology. However, when the different species of trypanosomes were considered, T. congolense infections were found only in the lowland. A significant association was observed (P<0.05) between the disease positivity and body condition score. When the mean packed cell volume of trypanosome infected animals was compared with that of non infected animals, it was significantly lower (P<0.05) in the infected animals, and the reduction was significantly lower (p<0.05) for T. congolense infection as compared with T. vivax infection. In conclusion, trypanosomsis was found to be important disease in the study area, and T. vivax was the more prevalent species and T. congolense with more negative impact in mean packed cell volume of affected animals

    Improvement of sheep skin quality after treatment with diazinon against cockle

    Cockle, otherwise known as ekek locally, has been economically the most important skin defect among the tanneries in Ethiopia for the last three decades. The disease has been experimented on since 1996 when FAO sponsored the trials on sheep and goat skin improvement trial (TCP/ETH/4558&6712) and found that diazinon improved the skin quality of treated sheep at least by 1 grade. However, the trials had not been sustainable to find out what the skin quality of the lambs of the dams that had been treated with diazinon would look like. The trial carried out at Debre Birhan Agriculture Research Centre filled this gap and showed repeatedly that sheep treated against lice and keds with diazinon improved the skin quality of the adult by 1 grade and those of their lambs by 3-5 grades. It started with 367 Menz and 265 Horro sheep in October 2009 and completed in January 2011. Lice and keds count of the trial sheep was carried out before and after treatment with diazinon. After the treatment, the parasite count dropped to 0 and there was a significant difference between the skin grades of the sheep before and after treatment with diazinon at 95% confidence level. The dramatic skin quality improvement appeared in the lambs which, compared to the adult sheep skins, had improved by 89% to 90%

    Programmatic correlates of maternal healthcare seeking behaviors in Ethiopia

    Background: Considerable improvement in maternal healthcare use has been observed since the inception of the health extension program (HEP) in Ethiopia in 2003.Objective: This paper evaluates the influence of HEP outreach strategies on maternal healthcare use.Method: Cross-sectional survey of 2,916 women with children 0 to 11 months from Amhara, Oromiya, Southern Nations, Nationalities and People’s, and Tigray regions, obtained between December 2008 and January 2009, were analyzed using regression models to assess the impacts of HEP strategies on maternal health outcomes.Result: The analyses found that communities (i.e., kebeles) with relatively high prevalence of model families, higher rate of household visits by health extension workers, and higher rate of household visits by voluntary community health workers were associated with improved antenatal care use, tetanus toxoid vaccination coverage, and receiving postnatal care visits; but the strategies were not associated with deliveries attended by health professionals.Conclusion: Although the impacts of HEP strategies on maternal healthcare use were statistically significant, they were not optimum to reach the maternal mortality reduction targets of the government of Ethiopia.     Maternal and perinatal mortality and morbidity of uterine rupture and its association with prolonged duration of operation in Ethiopia: A systematic review and meta-analysis.

    BACKGROUND: Uterine rupture is the leading cause of maternal and perinatal morbidity and it accounts for 36% of the maternal mortality in Ethiopia. The maternal and perinatal outcomes of uterine rupture were inconclusive for the country. Therefore, this systematic review and meta-analysis aimed to estimate the pooled maternal and perinatal mortality and morbidity of uterine rupture and its association with prolonged duration of operation. METHODS: The Preferred Reporting Items for Systematic Reviews and Meta-Analyses checklist was used for this systematic review and meta-analysis. We systematically used PubMed, Cochrane Library, and African Journals online databases for searching. The Newcastle- Ottawa quality assessment scale was used for critical appraisal. Egger's test and I2 statistic used to assess the check for publication bias and heterogeneity. The random-effect model was used to estimate the pooled prevalence and odds ratios with 95% confidence interval (CI). RESULTS: The pooled maternal mortality and morbidity due to uterine rupture in Ethiopia was 7.75% (95% CI: 4.14, 11.36) and 37.1% (95% CI: 8.44, 65.8), respectively. The highest maternal mortality occurred in Southern region (8.91%) and shock was the commonest maternal morbidity (24.43%) due to uterine rupture. The pooled perinatal death associated with uterine rupture was 86.1% (95% CI: 83.4, 89.9). The highest prevalence of perinatal death was observed in Amhara region (91.36%) and the lowest occurred in Tigray region (78.25%). Prolonged duration of operation was a significant predictor of maternal morbidity (OR = 1.39; 95% CI: 1.06, 1.81). CONCLUSIONS: The percentage of maternal and perinatal deaths due to uterine rupture was high in Ethiopia. Uterine rupture was associated with maternal morbidity and prolonged duration of the operation was found to be associated with maternal morbidities. Therefore, birth preparedness and complication readiness plan, early referral and improving the duration of operation are recommended to improve maternal and perinatal outcomes of uterine rupture

    Male Partner's Involvement in HIV Counselling and Testing and Associated Factors among Partners of Pregnant Women in Gondar Town, Northwest Ethiopia

    Background. Despite the existence of several programmes promoting male involvement in HIV counselling and testing during their wife's pregnancy as a part of PMTCT, few men have heeded the call. The aim of this study was to assess male partner's involvement in HCT and its associated factors. Methods. This study was based on institution based cross-sectional study design that used systematic random sampling technique. A total of 416 partners were interviewed in the data collection. Multivariable logistic regression model was fitted to identify the independent predictors. Result. In this study, the prevalence of male involvement in HCT was found to be 40.1% (95% CI: 35.3%-44.7%). The independent predictors of male involvement were partners who were younger, were cohabitant, were with multigravida wives, were knowledgeable on route of mother-to-child transmission, and discussed HCT. Conclusion. The prevalence of male involvement in HCT was found to be suboptimal compared to similar studies in Ethiopia. There is a need of interventions on partners who are older, separated, and with lower gravidity wife. Awareness creation campaign should also be created on the route of mother-to-child transmission of HIV and on the importance of discussion with wife

    Genetic diversity and population structure among sorghum (Sorghum bicolor, L.) germplasm collections from Western Ethiopia

    The Western Ethiopian region harbors a unique set of sorghum germplasm adapted to conditions not conventional to sorghums grown in other parts of the world. Accessions from the region possess unique resistance to multiple leaf and grain diseases. This study is aimed at exploring the extent of genetic variation and population structure among accessions of this region. A total of 123 accessions comprising 111 from Western Ethiopia (62 from Asosa and 49 from Pawe) and 12 U.S. adapted lines were genotyped using 30 sorghum simple sequence repeat markers (SSR). Genetic diversity and population structure were analyzed using PowerMarker and STRUCTURE software, respectively, based on 23 polymorphic SSR markers. Principal component analysis (PCA) was performed to view the variability in multi-dimensional space. Population structure analysis revealed considerable admixtures between Pawe and Asosa accessions, while the PowerMarker analysis grouped the accessions into three distinct clusters largely based on collection regions. The PCA did not clearly differentiate Asosa and Pawe accessions, but U.S. adapted lines were clearly separated from the rest. The study indicated the presence of marked genetic variability among accessions from Western Ethiopia and also provided clues on shared genetic events among accessions adapted to the two areas in Western Ethiopia.Keywords: Sorghum, genetic diversity, population structure, SSR, Ethiopi

    Trends and determinants of underweight and overweight/obesity among urban Ethiopian women from 2000 to 2016

    Background: Nutritional, epidemiological and demographic transitions have been associated with the emergence of the double burden of malnutrition globally. In Ethiopia, there has been no nationally representative investigation of trends and determinants of both underweight and overweight/obesity among urban women. This study examined the trends and determinants of underweight and overweight/obesity in urban Ethiopian women from 2000 to 2016. Methods: Trends in the prevalence of underweight and overweight/obesity were investigated based on a series of the Ethiopia Demographic and Health Survey (EDHS) data for the years 2000 (n = 2559), 2005 (n = 1112), 2011 (n = 3569), and 2016 (n = 3106). Multivariable multinomial logistic regression was used to investigate the association between socioeconomic, demographic, behavioural, and community-level factors with underweight and overweight/obesity. Results: The prevalence of underweight in urban Ethiopian women reduced significantly from 23.2% (95% confidence interval [CI]: 20.3, 26.3%) in 2000 to 14.8% (95% CI: 13.1, 16.7%) in 2016, while overweight/obesity increased significantly from 10.9% (95% CI: 9.1, 13.0%) in 2000 to 21.4% (95% CI: 18.2, 25.1%) in 2016. Urban women from rich households and those who had never married were less likely to be underweight. Urban women who were from wealthy households and those who attained at least secondary education were more likely to be overweight/obese. Women who were informally employed and listened to the radio were less likely to be overweight/obese compared to those who were unemployed and did not listen to the radio, respectively. Conclusion: The prevalence of overweight/obesity increased from 2000 to 2016, with a concurrent reduction in the prevalence of underweight. Interventions aiming to reduce overweight and obesity should target urban women with higher education, those who resided in wealthier households and those who watched the television

Unfortunately these devices remain plagued by thromboembolic complications associated with chronic platelet activation – mandating complex, lifelong anticoagulation therapy. To address the unmet need for enhancing the thromboresistance of these devices to extend their long term use, we developed a universal predictive methodology entitled Device Thrombogenicity Emulation (DTE) that facilitates optimizing the thrombogenic performance of any MCS device – ideally to a level that may obviate the need for mandatory anticoagulation
