La muestra estuvo constituida por 109 pacientes mujeres y 91 pacientes varones. El rango de edades de los pacientes fue entre 14 y 40 a帽os, con una media de 24.20 y una desviaci贸n est谩ndar de 9.88. La sensibilidad de la IgE total para el diagn贸stico de rinitis al茅rgica fue de 95.5% (IC 95% = 92.38-98.62); la sensibilidad del recuento de eosin贸filos en sangre fue de 41.5% (IC 95% = 34.42-48.58). Los 谩caros del polvo de casa (Dermatophagoides pteronyssinus, Dermatophagoides farinae y Dermatophagoides microceras) fueron los principales aeroal茅rgenos causantes de sensibilizaci贸n en los pacientes con diagn贸stico de rinitis al茅rgica. Se concluye que la determinaci贸n de la IgE total parece ser una prueba diagn贸stica de utilidad para el diagn贸stico de rinitis al茅rgica, lo que no ocurre con el recuento de eosin贸filos en sangre.Trabajo acad茅mic

    Introduction: Pharmacological desensitization induces a temporary hyposensitivity to the causative drug in patients with Immunoglobulin E-mediated drug reactions. Case of report: We present the case of a patient with respiratory exacerbation of cystic fibrosis who presented immediate hypersensitivity to colistin, an essential antibiotic for its treatment鈥攂oth due to the sensitivity of the microorganism and the lack of availability of other drugs. The procedure was successfully performed in the Intensive Care Unit, and the patient completed the treatment schedule. Conclusion: Currently, the patient is undergoing specific treatment for his underlying disease and has not presented any new episodes of exacerbation or pneumonia.Introducci贸n: La desensibilizaci贸n farmacol贸gica induce un estado temporal de hiposensibilidad a un medicamento que se realiza a pacientes con reacciones mediadas por Inmunoglobulina E. Reporte de caso: Presentamos el caso de un paciente con exacerbaci贸n respiratoria de fibrosis qu铆stica que present贸 hipersensibilidad inmediata a colistina, un antibi贸tico esencial para su tratamiento, tanto por la sensibilidad del microorganismo, como la falta de disponibilidad de otros medicamentos. Se realiz贸 el procedimiento en Unidad de Cuidados Intensivos de forma exitosa y el paciente complet贸 el esquema del tratamiento.     Genetic errors of immunity distinguish pediatric non-malignant lymphoproliferative disorders

    Background Pediatric non-malignant lymphoproliferative disorders (PLPD) are clinically and genetically heterogeneous. Long-standing immune dysregulation and lymphoproliferation in children may be life-threatening, and a paucity of data exists to guide evaluation and treatment of children with PLPD. Objective The primary objective of this study was to ascertain the spectrum of genomic immunologic defects in PLPD. Secondary objectives included characterization of clinical outcomes and associations between genetic diagnoses and those outcomes. Methods PLPD was defined by persistent lymphadenopathy, lymph organ involvement, or lymphocytic infiltration for more than 3 months, with or without chronic or significant EBV infection. Fifty-one subjects from 47 different families with PLPD were analyzed using whole exome sequencing (WES). Results WES identified likely genetic errors of immunity in 51% to 62% of families (53% to 65% of affected children). Presence of a genetic etiology was associated with younger age and hemophagocytic lymphohistiocytosis. Ten-year survival for the cohort was 72.4%, and patients with viable genetic diagnoses had a higher survival rate (82%) compared to children without a genetic explanation (48%, p = 0.03). Survival outcomes for individuals with EBV-associated disease and no genetic explanation were particularly worse than outcomes for subjects with EBV-associated disease and a genetic explanation (17% vs. 90%; p = 0.002). Ascertainment of a molecular diagnosis provided targetable treatment options for up to 18 individuals and led to active management changes for 12 patients. Conclusion PLPD therefore defines children with high risk for mortality, and WES informs clinical risks and therapeutic opportunities for this diagnosis
