    Expanding the clinical and immunological phenotypes of PAX1-deficient SCID and CID patients

    Paired box 1 (PAX1) deficiency has been reported in a small number of patients diagnosed with otofaciocervical syndrome type 2 (OFCS2). We described six new patients who demonstrated variable clinical penetrance. Reduced transcriptional activity of pathogenic variants confirmed partial or complete PAX1 deficiency. Thymic aplasia and hypoplasia were associated with impaired T cell immunity. Corrective treatment was required in 4/6 patients. Hematopoietic stem cell transplantation resulted in poor immune reconstitution with absent naïve T cells, contrasting with the superior recovery of T cell immunity after thymus transplantation. Normal ex vivo differentiation of PAX1-deficient CD34+ cells into mature T cells demonstrated the absence of a hematopoietic cell-intrinsic defect. New overlapping features with DiGeorge syndrome included primary hypoparathyroidism (n = 5) and congenital heart defects (n = 2), in line with PAX1 expression during early embryogenesis. Our results highlight new features of PAX1 deficiency, which are relevant to improving early diagnosis and identifying patients requiring corrective treatment

    Juvenile Myelomonocytic Leukemia in Turkey: A Retrospective Analysis of Sixty-five Patients

    Objective: This study aimed to define the status of juvenile myelomonocytic leukemia (JMML) patients in Turkey in terms of time of diagnosis, clinical characteristics, mutational studies, clinical course, and treatment strategies.Materials and Methods: Data including clinical and laboratory characteristics and treatment strategies of JMML patients were collected retrospectively from pediatric hematology-oncology centers in Turkey. Results: Sixty-five children with JMML diagnosed between 2002 and 2016 in 18 institutions throughout Turkey were enrolled in the study. the median age at diagnosis was 17 months (min-max: 2-117 months). Splenomegaly was present in 92% of patients at the time of diagnosis. the median white blood cell, monocyte, and platelet counts were 32.9x109/L, 5.4x109/L, and 58.3x109/L, respectively. Monosomy 7 was present in 18% of patients. JMML mutational analysis was performed in 32 of 65 patients (49%) and PTPN11 was the most common mutation. Hematopoietic stem cell transplantation (HSCT) could only be performed in 28 patients (44%), the majority being after the year 2012. the most frequent reason for not performing HSCT was the inability to find a suitable donor. the median time from diagnosis to HSCT was 9 months (min-max: 2-63 months). the 5-year cumulative survival rate was 33% and median estimated survival time was 30±17.4 months (95% CI: 0-64.1) for all patients. Survival time was significantly better in the HSCT group (log-rank p=0.019). Older age at diagnosis (>2 years), platelet count of less than 40x109/L, and PTPN11 mutation were the factors significantly associated with shorter survival time.     Plasma Exchange in Critically Ill Children: A Single-center Experience

    Introduction: The aim of this study was to identify demographic and clinical characteristics of patients who were subjected to plasma exchange (PE) at our unit, and to investigate the effect of these factors on treatment outcome and patient prognosis. Methods: Demographic, clinical and laboratory data of patients who were subjected to PE between January 2012 and August 2015 were obtained from the hospital information system, medical records and the records of apheresis unit. Results: Plasma exchange was performed in 40 patients for 168 times throughout the study. The median age of the patients was 9.4 (range: 1.5-17.3) years, with a male/female ratio of 1.35. Of the patients, 47.5% had an underlying disease. The most common comorbidity was malignancy. The most common indication for PE was sepsis-related multiple organ failure (n=19, 47.5%). The mortality rate was higher in patients with an underlying chronic disease, compared to those without (25% and 7.5%, respectively). No life-threatening complication associated with the apheresis procedure was observed. Conclusion: Our results suggest that PE can be safely performed in children. It seems that indication for PE and the presence of underlying diseases are affecting the mortality rate

    Quantum mechanical calculations of ground state energy, vibration wavenumbers, and electronic absorption wavelengths of N '-[(Z)-(4-methylphenyl)methylidene]-4-nitrobenzohydrazide with C15H13N3O3 empirical formula was performed by using Gaussian 09 program. Becke's three-parameter exchange functional in conjunction with the Lee-Yang-Parr correlation functional and Heyd-Scuseria-Ernzerhof functional levels of density functional theory (DFT) with the 6-311++G(d,p) basis set were used in the performing of above mentioned calculations. The highest occupied and lowest unoccupied molecular orbital (HOMO and LUMO) energies have been also calculated at the same levels. Stability of the molecule arising from hyperconjugative interactions and charge delocalization has been analyzed using natural bond orbital (NBO) analysis. Nonlinear optical (NLO) behavior of the title molecule has been examined by the determining of electric dipole moment (mu), polarizability (alpha), and static first-order hyperpolarizability (beta). Finally, molecular electrostatic potential (MEP) surface as well as Mulliken and NBO atomic charges were calculated by using Gaussian 09 program

    Primer İzole Total Serebellar Agenezi: Yetişkin bir Hastanın Olgu Sunumu ve Literatür Taraması

    Serebellar agenezi oldukça nadir görülür. Yenidoğan dönemde ölüme ve ciddi paraliziye sebep olabileceği gibi, yetişkin hayatta asemptomatik olarak da görülebilir. Radyolojik görüntüleme yöntemlerinin gelişmesiyle birlikte tanı olasılığı artmıştır. Serebellar agenezili hastalarda motor bozukluklar, mental durum bozukluğu serebellar koordinasyon bozukluğu bulunabildiği gibi, bunlardan hiçbiri de bulunmayabilir. Ek konjenital patolojilerin eşlik etmediği serebellar agenezide normal yaşam mümkündür. Serebellumdaki konjenital bozukluklar sıklıkla Dandy-Walker malformasyonu ve Chiari Malformasyonu şeklinde görülür. İleri tanı ve tedavilere gerek kalıp kalmayacağı açısından bu hastaların ayırıcı tanısının önemli olduğunu düşünmekteyiz. Radyolojik görüntüleme ile baş ağrısı yakınmasıyla başvuran 32 yaşındaki erkek hastamızda tespit ettiğimiz primer izole serebellar agenezi’yi literatür eşliğinde değerlendirdik
