    A cluster-randomised controlled trial to test the efficacy of facemasks in preventing respiratory viral infection among Hajj pilgrims

    BACKGROUND: Cost-effective interventions are needed to control the transmission of viral respiratory tract infections (RTIs) in mass gatherings. Facemasks are a promising preventive measure, however, previous studies on the efficacy of facemasks have been inconclusive. This study proposes a large-scale facemask trial during the Hajj pilgrimage in Saudi Arabia and presents this protocol to illustrate its feasibility and to promote both collaboration with other research groups and additional relevant studies. METHODS/DESIGN: A cluster-randomised controlled trial is being conducted to test the efficacy of standard facemasks in preventing symptomatic and proven viral RTIs among pilgrims during the Hajj season in Mina, Mecca, Saudi Arabia. The trial will compare the 'supervised use of facemasks' versus 'standard measures' among pilgrims over several Hajj seasons. Cluster-randomisation will be done by accommodation tents with a 1:1 ratio. For the intervention tents, free facemasks will be provided to be worn consistently for 7days. Data on flu-like symptoms and mask use will be recorded in diaries. Nasal samples will be collected from symptomatic recruits and tested for nucleic acid of respiratory viruses. Data obtained from questionnaires, diaries and laboratory tests will be analysed to examine whether mask use significantly reduces the frequency of laboratory-confirmed respiratory viral infection and syndromic RTI as primary outcomes. CONCLUSIONS: This trial will provide valuable evidence on the efficacy of standard facemask use in preventing viral respiratory tract infections at mass gatherings.     Genetic identification and symbiotic efficiency of Sinorhizobium meliloti indigenous to Saudi Arabian soils

    Soil bacteria Sinorhizobium meliloti are of enormous agricultural value, because of their ability to fix atmospheric nitrogen in symbiosis with an important forage crop legume-alfalfa. The main aim of this study was (i) to isolate indigenous S. meliloti strains from different field sites in Saudi Arabia, (ii) to assess genetic diversity and genetic relationships amongst strains of natural populations and (iii) to provide information about nodulation and symbiotic efficiency of indigenous S. melitloti strains. Nineteen strains isolated from alfalfa nodules collected from different field sites and one reference strain were analyzed. Genetic characterization by rep-PCR and RAPD-PCR was applied to study thestatus of S. melitloti populations inhabiting nodules of alfalfa. Cluster analysis of rep-PCR profiles showed significant differences among S. meliloti isolates. Both methods resulted in almost identicalgrouping of strains. Among indigenous strains two divergent groups could be determined. The biggest differences were detected among the reference strains along with two local isolates and all fieldisolates. Quantitative expression of symbiotic efficiency was evaluated by measurement of total nitrogen in plants and dry matter yield of plants. All strains nodulated alfalfa cultivar CAF 101 but withdifferent efficiency. Significant differences in dry mater yield of alfalfa as well as total nitrogen content were determined depending on the strain used. The results indicate that sixty one percent of indigenous S. meliloti strains can be characterized as low efficient strains

    A study of resident duty hours and burnout in a sample of Saudi residents

    Abstract Background Work hour restrictions in residency programs have been implemented over the last several decades in Europe, USA, and Canada. To best of our knowledge, there is no study of resident duty hours in the Kingdom of Saudi Arabia. In addition, few studies have looked at the prevalence of burnout amongst Saudi residents. The present study explored resident duty hours and burnout amongst residents in Saudi Arabia. Methods A paper-based questionnaire was designed to survey resident duty hours in Saudi Arabia and was administered along with the Maslach Burnout Inventory. The questionnaires were administered to residents in medical and surgical residency programs at King Abdulaziz Medical City-Riyadh and two hospitals in Buraidah, Qassim Province. Results A total of 181 residents from the three hospitals participated in the survey. In terms of average number of work hours per week, 50% of all residents reported working 60–79 h while 30% reported working 80 or more hours per week. The prevalence of burnout was 81%. There was no association between higher number of working hours and the prevalence of burnout. Conclusion This was the first study describing resident duty hours and exploring the relationship between duty hours and burnout in Saudi Arabia. Our main findings were that the majority of residents work 60 or more hours per week, and there was a very high degree of burnout amongst residents. A larger multi-centre study of resident duty hours and its effect on patient safety and resident well-being is needed to develop work hour regulations in Saudi Arabia. In addition, there is an urgent need to develop programs that address resident burnout

    Camel Milk Beneficial Effects on Treating Gentamicin Induced Alterations in Rats

    The potential effect of camel milk (CM) against gentamicin (GM) induced biochemical changes in the rat serum was evaluated. Four groups of six albino rats were used for control, CM fed, injected with GM(i.p.), and then fed and injected with GM. The results showed that the administration of GM significantly altered the levels of aspartate aminotransferase (AST), alanine aminotransferase (ALT), alkaline phosphatase (ALP), and lactate dehydrogenase (LDH) activity in rat serum. CM restored these parameters to almost their normal range in group IV. Additionally, the present study showed that injection of rats with gentamicin caused an increase in malondialdehyde (MDA) and myeloperoxidase (MPO) activity while the antioxidant enzymes like superoxide dismutase (SOD) and glutathione s-transferase (GST) activity decreased significantly (P≤0.05). Administration of CM significantly (P≤0.05) inhibited the formation of MDA and activity of MPO and upregulated the antioxidant enzymes (SOD and GST) activity. The overall findings of this study demonstrated that pretreatment with CM gave protection against GM induced hepatic damage possibly by inhibiting oxidative stress and inflammation, and hence camel milk can be identified as a new therapeutic agent
