    Improvement of visual acuity based on optical coherence tomography patterns following intravitreal bevacizumab treatment in patients with diabetic macular edema

    <b>AIM:</b>To report the visual outcome based on various patterns of optical coherence tomography (OCT) morphology in diabetic macular edema (DME), following treatment with anti-VEGF intravitreal bevacizumab (IVB) injection.<b>METHODS:</b>Sixty-seven consecutive subjects with centre involving DME underwent intravitreal injection of Bevacizumab (1.25 mg/0.05 mL) in this retrospective, comparative, non randomized study. The DME was classified into one of four categories:focal, diffuse, focal cystoid and neurosensory detachment based on OCT. Best corrected visual acuity (BCVA), macular appearance, and OCT findings were used to decide whether the subject should have a repeat injection of intravitreal bevacizumab. Outcome measures were a change in mean BCVA (Snellen converted to logMAR) and central macular thickness (CMT) in each group during the six month follow-up period.<b>RESULTS:</b>The mean BCVA improved to logMAR 0.23 at final follow-up from a baseline of 0.32 logMAR (<i>P=</i>0.040) in the focal group, logMAR 0.80 at final follow-up from a baseline of 0.82 logMAR (<i>P=</i>0.838) in the diffuse group, worsened to logMAR 0.53 at final follow-up from a baseline of 0.43 logMAR (<i>P=</i>0.276) in the focal cystoid group, and improved to logMAR 0.79 at final follow-up from a baseline of 0.93 logMAR (<i>P=</i>0.490) in the neurosensory detachment group. The mean CMT before treatment were 298.8±25.03 μm in the focal group, 310.8±40.6 μm in the diffuse group, 397.15±31.05 μm in the focal cystoid group and 401.03±75.1 μm in the neurosensory detachment group. A mean of 2.05 (range:1-5) injections in the focal group, 1.32 (range:1-2) in the diffuse group, 2.6 (range:1-6) in the focal cystoid group and 2.6 (range:1-6) in the neurosensory detachment group were performed during the six month follow-up period. Following intravitreal bevacizumab treatment, vision improved, remained unchanged or worsened in 11, 7 and 2 subjects in focal group; 11, 9 and 8 in diffuse group; 0, 2 and 4 in focal cystoid group and 5, 5 and 3 subjects respectively in neurosensory detachment group.<b>CONCLUSION:</b>OCT morpholgy patterns in DME may predict the effects of intravitreal bevacizumab treatment, and patients with focal DME are most likely to benefit from the improvent of visual acuity from this treatment

    The application of dynamic self-organised multilayer network inspired by the Immune Algorithm for weather signals forecast

    Neural network architecture called Dynamic Self-organised Multilayer Network Inspired by the Immune Algorithm is proposed for the prediction of weather signals. Two sets of experiments have been implemented. The simulation results showed slight improvement achieved by the proposed network when using the average results of 30 simulations. For the second set of experiments, the simulation results indicated that there is no significant improvement over the first set of experiments. Since clustering methods have been widely used in different applications of data mining, the adaption of unsupervised learning in the proposed network might serve these different applications, for example, medical diagnostics and pattern recognition for big data. The structure of the proposed network can be modified for clustering tasks by changing the back-propagation algorithm in the output layer. This can extend the application of the proposed network to scientifically analyse different types of big data

    Potentiometric and conductometric studies of malonyl bis(salicyloylhydrazone) and divalent metal complexes

    AbstractA series of complexes of divalent transition metal ions with malonyl bis(salicyloylhydrazone) (H4MSH) have been prepared and characterized with the help of conductometric, potentiometric methods. The proton–ligand and metal–ligand stability constants were obtained pH-metrically. The electrical conductivity of solid complexes was measured at 289K. The low molar conductance values observed for these complexes indicate that, they are non-electrolytes. They are soluble to a limited extent in DMF and DMSO. The elemental analyses of the complexes indicate that the complexes have 1:1 and 2:1 (M:L) stoichiometry with the existence of water, chloride, acetone molecules inside the coordination sphere as evidence from the IR spectral studies. Further, the complexes have been formulated by comparing C, H, N & metal analysis data, and UV–visible spectra of the complexes have been discussed. The protonation constants of the ligand and the stability constants of their metal complexes will be evaluated potentiometrically. The stoichiometric ratios of the complexes formed in solution will be evaluated applying the molar ratio (spectrophotometric) method and confirmed conductometrically

    Rapidly progressive periodontal disease associated with human immunodeficiency virus

    Severe periodontal inflammation with generalized dental plaque accumulation, spontaneous and severe gingival bleeding, fungal infection, and interdental papillae necrosis are presented in a patient infected with human immunodeficiency virus (HIV). Bite-wing radiographs revealed a generalized horizontal alveolar bone loss of 7-8 millimetres in both arches. Erythematous patches were noted on the gingival mucosa in both jaws. DNA testing was performed to indentify the periodontopathogens. The patient had no signs or symptoms of acquired immunodeficiency syndrome. This case-report presents the massive periodontal destruction that occurred in a patient infected with HIV. Therefore, it is highly recommended that patients infected with HIV should be regularly monitored to aid in early detection and to provide proper management of periodontal inflammatory conditions to minimize its destruction

    Pupil Localisation and Eye Centre Estimation using Machine Learning and Computer Vision

    Various methods have been used to estimate the pupil location within an image or a real-time video frame in many fields. However, these methods lack the performance specifically in low-resolution images and varying background conditions. We propose a coarse-to-fine pupil localisation method using a composite of machine learning and image processing algorithms. First, a pre-trained model is employed for the facial landmark identification to extract the desired eye-frames within the input image. We then use multi-stage convolution to find the optimal horizontal and vertical coordinates of the pupil within the identified eye-frames. For this purpose, we define an adaptive kernel to deal with the varying resolution and size of input images. Furthermore, a dynamic threshold is calculated recursively for reliable identification of the best-matched candidate. We evaluated our method using various statistical and standard metrics along-with a standardized distance metric we introduce first time in this study. Proposed method outperforms previous works in terms of accuracy and reliability when benchmarked on multiple standard datasets. The work has diverse artificial intelligence and industrial applications including human computer interfaces, emotion recognition, psychological profiling, healthcare and automated deception detection

    Tissue Reactions to Various Suture Materials Used in Oral Surgical Interventions

    A variety of suture materials are available for primary wound closure following oral surgical procedures. The aim was to review the tissue reactions to the various suture materials used in oral surgical interventions. Databases were searched using the following keywords: cotton, nylon, polyglecaprone 25, polytetrafluoroethylene (ePTFE), Polyglactin 910, polyglycolic acid (PGA), polylactic acid, silk, surgery, suture, and tissue reaction. Articles published only in English language were included. Seventeen studies were included. Two studies reported that polyglecaprone 25 had positive effects on wound-healing as compared to silk. Six studies reported that silk elicits more intense tissue inflammatory response and delayed wound healing as compared to other suture materials (including ePTFE, polyglecaprone-25, PGA, and nylon). Polyglactin 910 sutures were associated with the development of stitch abscess in one clinical study. Eight studies reported that tissue reactions are minimal with nylon sutures. Tissue reactions to suture materials used for oral surgical interventions may vary depending on the surface properties and bacterial adherence properties of the material

    In this work 5-methylene-yl - (2-methy –oxazole-4-one) (1H) imidazole (1) were synthesized from the reaction of L-Histidine with acetic anhydride and which converted to the of 5-methylene-yl-(2-methyl 3-amino imidazole-4-one)-1H-imidazole (2) by reaction with hydrazine hydrate. Schiff bases (3-6) were synthesized from the reaction of compound (2) with different aromatic aldehyde. Reaction of compounds (3-6) with chloroacetyl chloride gives azetidinone one derivatives (7-10). These compounds were characterized by FT-IR and some of them with 1H-NMR and 13C-NMR spectroscopy

    Advanced Artificial Neural Network Classification for Detecting Preterm Births Using EHG Records

    Globally, the rate of preterm births are increasing, thus resulting in significant health, development and economic problems. Current methods for the early detection of such births are inadequate. Nevertheless, there has been some evidence that the analysis of uterine electrical signals, collected from the abdominal surface, could provide an independent and easier way to diagnose true labour and detect when preterm delivery is about to occur. Using advanced machine learning algorithms, in conjunction with Electrohysterography signal processing, numerous studies have focused on detecting true labour several days prior to the event. However, in this paper, the Electrohysterography signals have been used to detect preterm births. This has been achieved using an open dataset, which contains 262 records for women who delivered at term and 38 who delivered prematurely. Several new features from Electromyography studies have been utilized, as well as feature-ranking techniques. Features are ranked to determine their discriminative capabilities in detecting term and preterm records. Seven different artificial neural networks were then used to identify these records. The results illustrate that the Radial Basis Function Neural Network classifier performed the best, with 85% sensitivity, 80% specificity, 90% area under the curve and a 17% mean error rate
