    Trends in the leading causes of childhood mortality from 2004 to 2016 in Qatar

    Introduction: Childhood mortality is an important health indicator that reflects the overall health status of a population. Despite the decrease in global childhood mortality rates over the past decades, it still remains an important public health issue in Qatar.Methods: The data from 2004-2016 were extracted from the Qatar Ministry of Public Health Birth and Death Database. International Classification of Diseases (ICD-10) was used for coding the causes of death. The childhood mortality rate was defined as the probability of a child dying between the first and the fifth birthday, expressed as the number of deaths per 1,000 children surviving to 12 months of age. The sex ratio was calculated by dividing the mortality rate of males by that of females. Mann-Kendall trend test was performed to examine time trends. Relative risks were calculated to examine differences by nationality (Qatari and non-Qatari) and sex.Results: A significant decrease in mortality rate of children aged one to five was observed from 1.76 to 1.05 per 1000 children between 2004 and 2016 (Kendall tau=-0.6, p=0.004). Three prominent causes of mortality were motor vehicle accidents, congenital malformations of the circulatory system, and accidental drowning/submersion. A statistically non-significant decrease in childhood mortality from motor vehicle accidents was oberved for all nationalities (total (Kendall tau=-0.03), Qatari (Kendall tau=-0.14), and non-Qatari (Kendall tau=-0.12)). A significant decrease was seen for total accidental drowning and submersion (Kendall tau=-0.54, p=0.012), while no statistically significant decrease was seen for total congenital malformations of the circulatory system (Kendall tau=-0.36, NS). The Qatari population did have a significant decrease in childhood mortality due to congenital malformations of the circulatory system (Kendall tau=-0.67, p=0.003) and accidental drowning and submersion (Kendall tau=-0.55, p=0.016).Conclusion: The study is a first attempt to evaluate childhood mortality statistics from Qatar and could be useful in supporting Qatarā€™s ongoing national health strategy programs

    TOD'S ROLE IN ENHANCING URBAN QUALITY OF LIFE AND PRESERVING BUILT HERITAGE: THE CASE OF MSHEIREB DOWNTOWN DOHA

    The term 'Transit-Oriented Development' or TOD was firstly used by the American planner Peter Calthorpe in 1993. To address the urban sprawling caused by extensive car use and the disappearance of historical community centers, the strategy for TOD is based on planning land use on a regional scale integrated to public transport systems. TOD became part of the movement known broadly as 'new urbanism'. Lately, Doha faced rapid growth in social, economic, and built environmental aspects that shaped its current and future urban planning. TOD is seen as an efficient and energetic approach to resolve the existing challenges in urban planning of the city through a sustainable strategy in urban developmental planning. Msheireb Downtown, the heritage district of Doha, is the selected case study under investigation. The research study focuses on modeling a new pattern of sustainable traditional urban developmental planning, which supports integration of transportation planning, land use and cultural/identity heritage. The study intends to explore "new urbanism" theories, which are the basis for TOD, while investigating the role of TOD in enhancing urban quality of life by following a number of principles that address physical, social, economic, environmental and mobility concerns. The research study adopts a qualitative approach. Oral and visual data are collected via (i) interviews with urban planners and consultants from Msheireb Properties, the Ministry of Municipality and Environment and Qatar Rail; (ii) walkthrough studies; (iii) site visits and observations about the spatial form of buildings, streets and open spaces; (iv) on-site interviews; and (v) a survey conducted during the walk-through studies. The study aims at revealing the extent to which the TOD principles can be implemented in the selected study area, by exploring (i) its conditions [site analysis] and (ii) how the urban quality of life in the vernacular urbanism is achieved in modern times

    Evaluation of Lipid Profile and Oxidative Stress of Workers in Fast Food Restaurants and The Risk of Atherosclerosis

    Cooking fumes and biomass smoke pose significant threats to air degradation and human health, especially to people who work in this field on a daily basis, due to the large amounts of hazardous pollutants released from grilling activities. Chronic exposure to these fumes is associated with a risk of inflammation and oxidative stress, which can increase the risk of atherosclerosis. In this study, the lipid profile Glutathion (GSH) and Malondialdehyde (MDA) of workers in fast food restaurants and patients with atherosclerosis were evaluated. The study included 40 Atherosclerosis patients, 40 fast food workers, and 40 healthy individuals as a control group, the ages ranges of (20-40) years. The obtained results indicated a significant increase in cholesterol, triglycerides, LDL, and VLDL and a decrease in HDL (p < 0.01) in patients and fast-food workers compared to control group. The results also indicated a significant increase in the level of MDA, and a significant decrease in the level of GSH for patients and fast-food workers compared to the control. In conclusion, it appears that fast food workers experience oxidative stress and disruption in lipid metabolism that may increase the risk of atherosclerosis. Therefore, safety and quality control measures must be strengthened to reduce the occupational risks of fast-food restaurant workers. Ā  Ā  Ā 

    A Retrospective Study of Non-Communicable Diseases amongst Blue-Collar Migrant Workers in Qatar

    BACKGROUND: South Asian workers have a greater predisposition to non-communicable diseases (NCDs) that is exacerbated by migration and length of residence in host countries. Aims: To examine the association between length of residence in Qatar with diagnosis of NCDs in male blue-collar workers. METHODS: A retrospective investigation of the electronic health records (EHRs) of 119,581 clinical visits by 58,342 patients was conducted. Data included age, nationality and confirmed ICD-10 diagnosis. Based on duration of residence, the population was divided into groups: ā‰¤6 months, 6ā€“12 months, 1ā€“ā‰¤2 years, 2ā€“ā‰¤5 years, 5ā€“ā‰¤6 years, >6 years. It was assumed that the group that had been resident in Qatar for ā‰¤6 months represented diseases that had been acquired in their countries of origin. Results: South Asian (90%) patients presented with NCDs at a younger (mean Ā± SD age of 34.8 Ā± 9.0 years) age. Diabetes and hypertension were higher in those who had just arrived (<6 monthsā€™ group), compared to the other durations of residence groups. Conversely, acute respiratory infections, as well as dermatitis and eczema, all increased, perhaps a consequence of shared living/working facilities. Only patients with diabetes and hypertension visited the clinic multiple times, and the cost of medication for these NCDs was affordable, relative to earnings. DISCUSSION/CONCLUSIONS: Blue-collar workers were predominantly South Asian, from lower socioeconomic classes, with early onset chronic NCDs. Notably, residence in Qatar gave them better access to affordable, significantly subsidized healthcare, leading to effective management of these chronic conditions

    Improving influenza vaccination rate among primary healthcareworkers in Qatar

    The purpose of this study was to improve influenza vaccination, and determine factors influencing vaccine declination among health care workers (HCW) in Qatar. We launched an influenza vaccination campaign to vaccinate around 4700 HCW in 22 Primary Health Care Corporation (PHCC) centers in Qatar between 1st and 15th of November, 2015. Our target was to vaccinate 60% of all HCW. Vaccine was offered free of charge at all centers, and information about the campaign and the importance of influenza vaccination was provided to employees through direct communication, emails, and social media networks. Staff were reported as vaccinated or non-vaccinated using a declination form that included their occupation, place of work and reasons for declining the vaccine. Survey responses were summarized as proportional outcomes. We exceeded our goal, and vaccinated 77% of the target population. Only 9% declined to take the vaccine, and the remaining 14% were either on leave or had already been vaccinated. Vaccine uptake was highest among aides (98.1%), followed by technicians (95.2%), and was lowest amongst pharmacists (73.2%), preceded by physicians (84%). Of those that declined the vaccine, 34% provided no reason, 18% declined it due to behavioral issues, and 21% declined it due to medical reasons. Uptake of influenza vaccine significantly increased during the 2015 immunization campaign. This is attributed to good planning, preparation, a high level of communication, and providing awareness and training to HCW with proper supervision and monitoring. 1 2017 by the authors.     Early Versus Late Intramedullary Nailing for Traumatic Femur Fracture Management: Meta-Analysis

    INTRODUCTION: There is no consensus yet on the impact of timing of femur fracture (FF) internal fixation on the patient outcomes. This meta-analysis was conducted to evaluate the contemporary data in patients with traumatic FF undergoing intramedullary nail fixation (IMN). METHODS: English language literature was searched with publication limits set from 1994 to 2016 using PubMed, Scopus, MEDLINE (OVID), EMBASE (OVID), Web of Science, and Cochrane Central Register of Controlled Trials (CENTRAL). Studies included randomized controlled trials (RCTs), prospective observational or retrospective cohort studies, and case-control studies comparing early versus late femoral shaft fractures IMN fixation. Variable times were used across studies to distinguish between early and late IMN, but 24 h was the most frequently used cutoff. The quality assessment of the reviewed studies was performed with two instruments. Observational studies were assessed with the Newcastle-Ottawa Quality Assessment Scale. RCTs were assessed with the Cochrane Risk of Bias Tool. RESULTS: We have searched 1151 references. Screening of titles and abstracts eliminated 1098 references. We retrieved 53 articles for full-text screening, 15 of which met study eligibility criteria. CONCLUSIONS: This meta-analysis addresses the utility of IMN in patients with FF based on the current evidence; however, the modality and timing to intervene remain controversial. While we find large pooled effects in favor of early IMN, for reasons discussed, we have little confidence in the effect estimate. Moreover, the available data do not fill all the gaps in this regard; therefore, a tailored algorithm for management of FF would be of value especially in polytrauma patients

    Arab female and male perceptions of factors facilitating and inhibiting their physical activity: Findings from a qualitative study in the Middle East

    Objectives: Physical inactivity is a leading global risk to health by contributing to obesity and other chronic diseases. Many chronic non-communicable diseases, such as cancer, diabetes, and cardiovascular diseases (CVDs), can be prevented and controlled by modifying lifestyle behaviors such as physical activity [PA]. However, prevalence of insufficient physical activity and obesity is high in the Middle East Region. In Qatar, the incidence rates of CVDs, diabetes, colon, and breast cancer have been rising rapidly. The purpose of this study was to explore facilitators and barriers influencing PA of adult Arab men and women living in Qatar and to understand what they think would be helpful to increase PA. The goal of the research is to identify culturally appropriate and effective interventions that improve the health of Arab population. Design: Using the socioecological model as the theoretical framework, we conducted an exploratory qualitative study with 128 Arab adult men and women living in Qatar. We utilized focus group interviews to collect the data and performed thematic analysis to generate themes. Results: At the individual level, perceived benefits of PA, presence of diseases, personā€™s will, motivation and goals, and time to exercise influenced the individualā€™s PA. At the sociocultural level, religious teachings of Islam, cultural, attitude, beliefs, and practices, and informal support influenced the participantsā€™ PA. At the organizational and political level, physical environment to exercise, accessibility of facilities, organizational support, and health information about PA influenced their PA. Conclusion: Arab men and women are aware of the importance and benefits of PA. They have the motivation to be physically active, but in the absence of supportive environment, their knowledge might not translate into action. Creating supportive environments at multiple levels that are conducive to PA is warranted

    Medical education and research environment in Qatar: a new epoch for translational research in the Middle East

    Recent advances in medical technology and key discoveries in biomedical research have the potential to improve human health in an unprecedented fashion. As a result, many of the Arab Gulf countries, particularly Qatar are devoting increasing resources toward establishing centers of excellence in biomedical research. However, there are challenges that must be overcome. The low profile of private medical institutions and their negligible endowments in the region are examples of such challenges. Business-type government controlled universities are not the solution for overcoming the challenges facing higher education and research programs in the Middle East
