Social Support Seeking And Early Adolescent Depression And Anxiety Symptoms: The Moderating Role Of Rumination
This study examined how social support seeking and rumination interacted to predict depression and anxiety symptoms 6 months later in early adolescents (N = 118; 11-14 years at baseline). We expected social support seeking would be more helpful for adolescents engaging in low rather than high levels of rumination. Adolescents self-reported on all measures at baseline, and on depression and anxiety symptoms 6 months later. Social support seeking predicted fewer symptoms of depression and anxiety at low rumination levels but was not associated with benefits as rumination increased. For depression symptoms, social support seeking predicted more symptoms at high rumination levels. Results were stronger for emotion-focused than problem-focused support seeking and for depression compared with anxiety symptoms. These findings suggest that cognitive risk factors like rumination may explain some inconsistencies in previous social support literature, and highlight the importance of a nuanced approach to studying social support seeking
Parent Predictors Of Adolescents’ Explanatory Style
The current study tested the prospective relations (6-month lag) between three aspects of the parent-child relationship at Time 1 (T1) and adolescents’ explanatory styles at Time 2 (T2): caregiving behaviors, parents’ explanatory style for their own negative events, and parents’ explanatory style for their children’s negative events. The sample included 129 adolescents aged 11 to 14 years at baseline and their parents. Adolescents reported on their own explanatory style and their parents’ caregiving behaviors; parents self-reported on their caregiving behaviors and their explanatory style for their own and their children’s events. Regression analyses identified maternal acceptance as a significant predictor of T2 adolescents’ explanatory style. Marginal effects emerged for fathers’ psychological control and fathers’ explanatory style for their children’s events. Findings suggest that the ways parents—especially mothers—interact with their children may play a role in adolescents’ cognitive vulnerability to depression
Safety of Intradialytic Bamlanivimab/ Etesevimab Administration in Two COVID-19 Dialysis Outpatients
Chronic hemodialysis patients are at high risk of severe COVID-19 disease and death related to the infection. Anti-spike monoclonal antibodies administration reduces risk of disease progression and hospitalization in high-risk subjects but no clear data on end-stage renal disease are available. We report 2 cases of Bamlanivimab/Etesevimab administration to two not hospitalized chronic hemodialysis patients with SARS-CoV2 infection. Since they are large molecules (human immunoglobulin G1) with molecular weight of 146,000 Da, administration was conducted during the second hour of the dialysis session with no adverse reaction. Conclusions: Intradialytic administration of Bamlanivimab/Etesevimab could be considered safe and may allow adequate clinical observation time without hospital-stay prolongation
Dietary polyphenols and non-alcoholic fatty liver disease
Non-alcoholic fatty liver disease (NAFLD), which is emerging as a major public health issue worldwide, is characterized by a wide spectrum of liver disorders, ranging from simple fat accumulation in hepatocytes, also known as steatosis, to non-alcoholic steatohepatitis (NASH) and cirrhosis. At present, the pharmacological treatment of NAFLD is still debated and dietary strategies for the prevention and the treatment of this condition are strongly considered. Polyphenols are a group of plant-derived compounds whose anti-inflammatory and antioxidant properties are associated with a low prevalence of metabolic diseases, including obesity, hypertension, and insulin resistance. Since inflammation and oxidative stress are the main risk factors involved in the pathogenesis of NAFLD, recent studies suggest that the consumption of polyphenol-rich diets is involved in the prevention and treatment of NAFLD. However, few clinical trials are available on human subjects with NAFLD. Here, we reviewed the emerging existing evidence on the potential use of polyphenols to treat NAFLD. After introducing the physiopathology of NAFLD, we focused on the most investigated phenolic compounds in the setting of NAFLD and described their potential benefits, starting from basic science studies to animal models and human trials
Role of Estimated Glomerular Filtration Rate in Clinical Research: The Never-Ending Matter
Background: Chronic kidney disease (CKD) burden is crucial both on a global scale and at individual patient level, affecting morbidity and mortality directly and through its effect on both cardiovascular damage and CKD progression to end-stage-kidney-disease (ESKD). Unfortunately, the awareness of CKD is poor, with few CKD patients conscious of the severity of their health status. The principal biomarker of kidney function is estimated glomerular filtration rate (eGFR). Methods: We searched the literature and present a review article with the aim of summarizing the role of eGFR in clinical research. In particular, we report the eGFR role as a prognostic, enrichment and endpoint biomarker and its role in the early detection of CKD. Results: eGFR has a major role as a biomarker in clinical research. As a prognostic marker, eGFR reduction is associated with cardiovascular events, ESKD and mortality. As an enrichment biomarker, eGFR values are pivotal for selecting patients to be included in randomized and observational studies; it helps to test a pre-defined drug in early CKD or in more advanced CKD allowing also to avoid screening failures and to shorten the duration of clinical trials. Moreover, eGFR decline (expressed as a percentage of reduction from baseline or continuous slope) can be considered a good endpoint in clinic trials overcoming delays whilst waiting for hard endpoints to develop. Conclusions: eGFR is a strong clinical measure for both observational and intervention studies. It is also helpful in screening the general population for kidney disease and, in particular, to increase awareness of CKD
Estimated glomerular filtration rate in observational and interventional studies in chronic kidney disease
Estimated glomerular filtration rate is considered the principal measure of kidney function and, together with albuminuria, is a relevant prognostic factor for the development of end-stage kidney disease. Due to the strong association between estimated glomerular filtration rate and clinical events, such as commencement of dialysis, cardiovascular outcomes and all-cause death, estimated glomerular filtration rate is crucial for clinical decision-making in terms of scheduling follow-up and pharmacological interventions, and planning renal replacement therapies in advanced chronic kidney disease. In this review we discuss the available methods for measuring glomerular filtration rate and for estimating it through mathematical equations developed over the last few decades. We summarize the prognostic association of different percentages of estimated glomerular filtration rate decline and the main clinical outcomes, and how treatments modify estimated glomerular filtration rate decline and the risk of future endpoints. We also examine the role of pre-clinical trial slope and that of estimated glomerular filtration rate as a useful biomarker when evaluating patients for inclusion into both observational and interventional studies
The assessment and the within-plant variation of the morpho-physiological traits and VOCs profile in endemic and rare Salvia ceratophylloides Ard. (Lamiaceae)
Salvia ceratophylloides (Ard.) is an endemic and rare plant species recently rediscovered as very few individuals at two different Southern Italy sites. The study of within-plant variation is fundamental to understand the plant adaptation to the local conditions, especially in rare species, and consequently to preserve plant biodiversity. Here, we reported the variation of the morpho-ecophysiological and metabolic traits between the sessile and petiolate leaf of S. ceratophylloides plants at two different sites for understanding the adaptation strategies for surviving in these habitats. The S. ceratophylloides individuals exhibited different net photosynthetic rate, maximum quantum yield, light intensity for the saturation of the photosynthetic machinery, stomatal conductance, transpiration rate, leaf area, fractal dimension, and some volatile organic compounds (VOCs) between the different leaf types. This within-plant morpho-physiological and metabolic variation was dependent on the site. These results provide empirical evidence of sharply within-plant variation of the morpho-physiological traits and VOCs profiles in S. ceratophylloides, explaining the adaptation to the local conditions
From pre-and probiotics to post-biotics: A narrative review
Background and aims: Gut microbiota (GM) is a complex ecosystem containing bacteria, viruses, fungi, and yeasts. It has several functions in the human body ranging from immunomodulation to metabolic. GM derangement is called dysbiosis and is involved in several host diseases. Pre-, probiotics, and symbiotics (PRE-PRO-SYMB) have been extensively developed and studied for GM re-modulation. Herein, we review the literature data regarding the new concept of postbiotics, starting from PRE-PRO-SYMB. Methods: We conducted a search on the main medical databases for original articles, reviews, meta-analyses, randomized clinical trials, and case series using the following keywords and acronyms and their associations: Gut microbiota, prebiotics, probiotics, symbiotic, and postbiotics. Results: Postbiotics account for PRO components and metabolic products able to beneficially affect host health and GM. The deeper the knowledge about them, the greater their possible uses: The prevention and treatment of atopic, respiratory tract, and inflammatory bowel diseases. Conclusions: Better knowledge about postbiotics can be useful for the prevention and treatment of several human body diseases, alone or as an add-on to PRE-PRO-SYMB
Impact of patients nutritional status on major surgery outcome
Abstract. – OBJECTIVE: Surgery is a major
stress factor that activates several inflammatory
and catabolic pathways in man. An appropriate
nutritional status allows the body to react properly
to this stressor and recover in a faster and
more efficient manner. On the other hand, malnutrition
is related to a worse surgery outcome
and to a higher prevalence of comorbidities and
mortality.
The aims of this study were to evaluate the nutritional
status of patients undergoing major surgery
and investigate the potential correlation between
malnutrition and surgical outcomes.
PATIENTS AND METHODS: Mini Nutritional
Assessment (MNA) and global clinical examination
(including biochemical parameters and
comorbidities existence) were undertaken in 50
consecutive patients undergoing major surgery.
Patients’ clinical conditions were re-evaluated at
3 and 6 days after surgery, recording biochemical
parameters and systemic and/or wound-related
complications.
RESULTS: A compromised nutritional status
was present in more than half (54%) of patients
(malnutrition in 10% and risk of malnutrition
in 44% of patients, respectively). Females
were slightly more at risk of malnutrition (48%
vs. 41%, p=NS, females vs. males) and clearly
malnourished (14% vs. 7%, p<0.05, females
vs. males). Age was an independent risk factor
for malnutrition and within the elders’ group (>
80 years old) 16.70% of patients was diagnosed
with malnutrition and 58.3% was at risk of malnutrition.
Systemic complications were registered
in all patients both at 3 and 6 days after surgery.
However, well-nourished and at-risk of malnutrition
patients had earlier complications that only
partially resolved within six days after the operation.
Malnourished patients showed fewer complications
at the 3rd post-surgery follow-up day
but had a worse outcome six days after surgery.
CONCLUSIONS: Older age and but not female
sex are independent risk factors for malnutrition
development in patients undergoing major
surgery. More interestingly, more than half
of patients with an impaired nutritional status
presented a less appropriated stress response
to surgery. These data suggest that nutritional
status assessment may be important to recognize
patients at potential risk of surgical complications
and that early nutritional interventions
must be promptly arranged
