    To consider e-learning in dental and medical education in Japan

    All of Japanese medical and dental schools have constructed information-technology network systems. But only about 10 percent medical, dental, pharmaceutical, and health professional schools are using e-learning, and 60 percent are negative for planning to introduce e-learning systems. The IT or e-learning systems in medical and dental education in Japan are seen in the on line self-learning media for dissection for gross anatomy and media for simulated training systems for clinical diagnosis, treatments, and skills and the on line problem-based learning systems constructed through the electronic health records in the university hospital to approach data of the clinical examinations and clinical images such as X-ray and MRI images. And this paper introduce the author's experience to build up an atlas of histology, explanation movies for histology practice using images collected for broad casting TV programs for lifelong-learning of citizen of public, and e-learning constructed students in a subject to study medical research tecnique

    Significance of MDR1-Gene and P-Glycoprotein (P-gp) Expressions in the Lesional Skin of Psoriasis Vulgaris

    We have examined P-gp (P-glycoprotein) and multi-drug resistance 1 (MDR1) gene expressions in the lesional skin of psoriasis vulgaris and atopic dermatitis some of those showed a decreased clinical response to topical corticosteroid ointment during the clinical course. The patients were subdivided into four groups; S (-): responder to topical steroid without steroid ointment for one month at the time of biopsy, S (+): responder to topical steroid and under steroid therapy at the time of biopsy, R (-): low or non- responder to topical steroid without steroid therapy for one month at the time of biopsy, R (+): low-or non-responder to topical steroid and under steroid therapy at the time of biopsy. P-gp was mainly expressed in the cytoplasm of some epidermal keratinocytes and most of infiltrating cells in the dermis of the lesional skin of psoriasis or atopic dermatitis. Scores of P-gp protein-expression was significantly higher in the patients under steroid ointment both in psoriasis vulgaris and atopic dermatitis. While R (+) group showed much more intense expression of P-gp than S (+) group in psoriasis vulgaris but this was not the case for atopic dermatitis. MDR1 gene was expressed in the lesional skin of R (+) psoriasis but not in S (+) psoriasis or normal skin. These results suggest that steroid-resistance occasionally experienced in psoriasis vulgaris might be related to the overexpression of P-gp which is possibly induced after topical steroid ointment. This might provide a new insight for the mechanism of steroid -insensitivity in inflammatory skin disorders especially in psoriasis vulgaris

    Effects of Erbium-Doped: Yttrium Aluminum Garnet (Er: YAG) Laser on Bacteremia due to Scaling and Root Planing

    INTRODUCTION: The aims of the present study were: [1] to evaluate whether Erbium-Doped: Yttrium, Aluminum and Garnet (Er: YAG) laser could be a prophylactic methods against transient bacteremia during scaling and root planing (SRP) [2] to confirm the efficacies of SRP with Er: YAG laser by clinical and microbiologic evaluations.METHODS: Twenty chronic periodontitis subjects were randomly treated for quadrant SRP with either conventional hand instrument (n=10) or Er: YAG laser (n=10) monotherapy. Peripheral blood samples were drawn at baseline and 6 minutes after initiation of SRP, and were cultured for the analysis of bacteremia. Clinical measurements of full mouth plaque control record (PCR), probing pocket depth (PPD), clinical attachment level (CAL) and bleeding on probing (BOP) were made at baseline and 1 month after SRP. In addition, microbiologic analyses of subgingival samples were also performed at baseline and 1 month after SRP using the polymerase chain reaction Invader method.RESULTS: The incidence of bacteremia during SRP was 0% (0/10) in the Er: YAG laser group and 80% (8/10) in the hand instrument group, which was significantly different (P = 0.0003). All isolates from blood were facultative or obligate anaerobes and more than half of the microorganisms were species of streptococci. Intragroup comparison revealed that the mean PPD and CAL significantly decreased compared to baseline in both groups (P < 0.05). However, the percentage of BOP positive significantly decreased only in Er: YAG laser group (P = 0.005). In addition, the reductions of the subgingival bacterial counts between baseline and 1 month after SRP were also comparable between the two groups.CONCLUSION: The present study demonstrated that SRP with Er: YAG laser could not only be an alternative treatment for improvement of clinical and microorganisms situations but also a novel prophylactic method against transient bacteremia

    Protocol for Genome Editing to Produce Multiple Mutants in Wheat

    Here, we describe a protocol for producing multiple recessive mutants via genome editing in hexaploid wheat (Triticum aestivum) cv. Fielder. Using Agrobacterium-delivered CRISPR/Cas9 and three sub-genome-specific primer sets, all possible combinations of single, double, and triple transgene-free mutants can be generated. The technique for acceleration of generation advancement with embryo culture reduces time for mutant production. The mutants produced by this protocol can be used for the analysis of gene function and crop improvement. For complete details on the use and execution of this protocol, please refer to Abe et al.     Evaluation of Rheumatoid Arthritis Using a Scoring System Devised from Magnetic Resonance Imaging of Rheumatoid Knees

    We studied the magnetic resonance imaging (MRI) of 120 knees in 86 rheumatoid arthritis (RA) patients and of 14 unaffected knees in 12 control cases. We also developed a scoring system as a quantitative analysis method. We divided the MRI into 10 items, and classified the severity of the symptoms into 4 grades (score 0 to 3). The average total score increased according to the radiographic grade. Soft tissue lesions were clearly detected, even in the early stages of RA. Items such as synovial proliferation showed a high score even in the early stages, suggesting that it was the initial symptom of RA. The score also showed a correlation with the inflammatory signs. These results suggest that this scoring system is very sensitive and yields a good reflection of RA activity. We demonstrated that this system is simple and convenient for routine diagnostic use. We further demonstrated that it is useful for following the advancement of RA and for evaluating the response to treatment.</p

    The Health Sciences University of Hokkaido established School of Psychological Sciences consisting of Department of Communication Disorders and Department of Clinical Psychology at 2002. The Department of Communication Disorders was the 10th Japanese higher educational institution of a 4 year-course to speech therapists (ST), the medical profession by the national license, corresponding to the speech language pathologists and audiologists in United States. The higher education to ST has been underdeveloped compared with medical education to medical, dental, pharmaceutical, nursing and other health professions. During the first 4 years, our departments reformed the curriculum, including various problem-based small group learning classes, anatomy and physiology class including dissection of gross anatomy using animal organs and physiological experiments, objective structured clinical examination (OSCE), integrated classes for various skills for ST and clinical experiences using problem-based learning, based on the model core-curriculum for ST education which we proposed at first in Japan. In addition we started the graduate school for ST at 2006 ; this is the first school to aim to develop the higher professional capability of ST including clinical ability in Japan

    Fibroblast growth factor 23 (FGF23), a hormone, mainly produced by osteocytes, regulates phosphate and vitamin D metabolism. By contrast, 1,25-dihydroxyvitamin D3, the active form of vitamin D, has been shown to enhance FGF23 production. While it is likely that osteocytes are heterogenous in terms of gene expression profiles, specific subpopulations of Fgf23-expressing osteocytes have not been identified. Single-cell RNA sequencing (scRNA-seq) technology can characterize the transcriptome of an individual cell. Recently, scRNA-seq has been used for bone tissue analysis. However, owing to technical difficulties associated with isolation of osteocytes, studies using scRNA-seq analysis to characterize FGF23-producing osteocytes are lacking. In this study, we characterized osteocytes secreting FGF23 from murine femurs in response to calcitriol (1,25-dihydroxyvitamin D3) using scRNA-seq. We first detected Dmp1, Mepe, and Phex expression in murine osteocytes by in situ hybridization and used these as marker genes of osteocytes. After decalcification, enzyme digestion, and removal of CD45+ cells, femoral bone cells were subjected to scRNA-seq. We identified cell clusters containing osteocytes using marker gene expression. While Fgf23 expression was observed in some osteocytes isolated from femurs of calcitriol-injected mice, no Fgf23 expression was detected in untreated mice. In addition, the expression of several genes which are known to be changed after 1,25-dihydroxyvitamin D3 treatment such as Ccnd2, Fn1, Igfbp7, Pdgfa, and Timp1 was also affected by calcitriol treatment in Fgf23-expressing osteocytes, but not in those lacking Fgf23 expression, even after calcitriol administration. Furthermore, box-and-whisker plots indicated that Fgf23 expression was observed in osteocytes with higher expression levels of the Fam20c, Dmp1, and Phex genes, whose inactivating mutations have been shown to cause FGF23-related hypophosphatemic diseases. These results indicate that osteocytes are heterogeneous with respect to their responsiveness to 1,25-dihydroxyvitamin D3, and sensitivity to 1,25-dihydroxyvitamin D3 is one of the characteristics of osteocytes with Fgf23 expression. It is likely that there is a subpopulation of osteocytes expressing several genes, including Fgf23, involved in phosphate metabolism
